1.Therapeutic Observation of Acupuncture-moxibustion for Postsurgical Gastroparesis and Effect on Gastrin and Motilin

Objective To observe the clinical efficacy of acupuncture-moxibustion in treating postsurgical gastroparesis syndrome (PGS) and the effect on gastrin (GAS) and motilin (MTL).Method Fifty PGS patients were randomized into a treatment group and a control group, 25 cases in each group. The control group was intervened by conventional medication, including effective gastrointestinal decompression, nutrition support, and psychotherapy; the treatment group was by acupuncture-moxibustion therapy in addition to the intervention given to the control group. The time of restoring gastric motility, the distention and fullness grading score, and serum GAS and MTL levels were observed, and the clinical efficacies were also compared.Result The total effective rate was 100.0% in the treatment group versus 92.0% in the control group, and the between-group difference was statistically insignificant (P>0.05). According to statistical analysis, there was a significant difference in comparing the time of restoring gastric motility between the two groups (P<0.01). The distention and fullness grading scores, GAS and MTL levels were significantly changed after intervention in both groups (P<0.01). The distention and fullness grading score, GAS and MTL levels of the treatment group were significantly different from that of the control group after intervention (P<0.01).Conclusion Acupuncture- moxibustion plus medication is an effective approach in treating PGS, and it can up-regulate the GAS and MTL levels.
2.Effects of health education and stove improvement in preventing coal-burning-borne endemic fluorosis in Pingba County of Guizhou Province

Objective To observe the effect of health education and installation of improved stove in control of coal-burning-borne endemic fluorosis in Pingba County of Guizhou Province.Methods Two towns(Shizi and Qibo) of Pingba County that had implemented the health education and installed the improved stoves for control of coal-burning-borne endemic fluorosis were selected as the investigation sites.Two villages in each selected town and 30 householders as well as all students of grade 4-6 were investigated.Questionnaires were made by survey of the fluorosis control knowledge,household investigations were used to know the use of household stoves,corn and peppers drying method,and the dental fluorosis of children aged 8-12 were examined by Dean methods before the intervention(2006) and after implementation(2009) of health education and improvement of stoves.Results The awareness of fluorosis control among students and households after the intervention was 97.36% (405/416) and 86.46%(415/480),respectively,and the difference was statistically significant(x2 =658.86,163.10,all P < 0.01) compared with that before the intervention [32.19%(1083/3364),43.33%(130/300)].Correct using rate of the household stoves was 95.42% (229/240) after the intervention and 45.85% (3976/8672) before the intervention,the difference was statistically significant(x2 =230.25,P < 0.01).The correct rates of drying corn and chili were 91.67% (110/120) and 97.50% (117/120) before the intervention,and 20.00% (12/60),26.67% (16/60) after the intervention,the difference was statistically significant(x2 =94.07,104.02,all P < 0.01).The incidences of dental fluorosis of children aged 8-12 were 31.21%(1305/4182) after the intervention and 87.71%(2856/3256) before the intervention,the difference was statistically significant(x2 =2371.91,P < 0.01).Conclusions The measures of health education and intervention of stove improvement in Pingba County of Guizhou Province for prevention and control of coal-burning-borne endemic fluorosis is effective.

4.Moving cupping at Hechelu combined with rubbing method for depression of diabetes mellitus.

OBJECTIVETo compare the efficacy between moving cupping at Hechelu combined with rubbing method and western medication for depression of diabetes mellitus (DM).
METHODSTwo hundred and sixteen patients were randomly divided into an observation group and a control group, 108 cases in each group. Patients in the observation group were treated with moving cupping at Hechelu combined with rubbing method, once every: other day; six treatments were considered as one course, and totally two courses were given with an interval of: 4 days between courses. Patients in the control group were treated with oral administration of fluoxetine hydrochloride capsules, once a day for consecutive 4 weeks. The Hamilton depression scale (HAMD), self-rating depression scale (SDS) and TCM symptom score were measured before treatment, after the treatment and in follow-up visit one and a half months after treatment. The fasting blood glucose was tested before and after treatment. The glycosylated hemoglobin (HbA1c) was tested in the follow-up visit.
RESULTSThe total effective rate was 90.9% (90/99) in the observation group, which was superior to 73.7% (70/95) in the control group (P < 0.05). After the treatment, HAMD, SDS and TCM symptom scores were all reduced apparently in the observation group and the control group (all P < 0.05). After the treatment and the follow-up visit, the TCM symptom score in the observation group was lower than that in the control group (P < 0.05). The levels of HbA1c and GLU were stable in the observation group, and were decreased compared with those before treatment; but the difference between the, two groups was not significant (P > 0.05).
CONCLUSIONThe Hechelu theory-based TCM treatment has better: efficacy for depression of diabetes mellitus than fluoxetine hydrochloride capsule, which has less adverse effects.

5.Standardized management of hypertension in communities

Objective To confirm whether community management of hypertension could improve blood pressure control in Chongqing.Methods Cluster sampling method was used to select 5283 adults from 20 community healthcare centers in Chongqing.Matched t test was used to analyze the changes of blood pressure before and after the intervention.x2 test analysis was performed to compare the rate of normal blood pressure.Results The average age of 5283 participants was (60.5 ± 11.0) years old.Systolic blood pressure (SBP) and diastolic blood pressure (DBP) were significantly decreased after intervention (total population:t values were 16.98 and 13.80,respectively; male:t values were 12.58 and 10.66,respectively; female:t values were 11.60 and 9.10,respectively; all P < 0.05).The most significant decrease in SBP was found in 50-59 y age group (t =15.29,P <0.05),followed by 40-49 y age group (t =9.22,P <0.05).The control rate of hypertension was increased by 5.3% after 1 year's intervention (x2 =134.5,P<0.05),except for 60-69 y age group and ≥70 y age group (x2 values were 2.5 and 1.7,respectively ; both P > 0.05).Conclusion Our results show that standardized management of hypertension in communities can decrease the level of blood pressure and increase the control rate of hypertension.
6.Clinical analysis of 87 cases of bronchial asthma combined with allergic rhinitis by azelastine hydrochloride nasal spray

Objective To observe the effect of bronchial asthma combined with allergic rhinitis by azelastine hydrochloride nasal spray.Methods Eighty-seven cases of bronchial asthma combined with allergic rhinitis patients were divided into treatment group(45 cases)and control group (42 cases) by random number table method,both groups were given the treatment of bronchial asthma according to the guidelines for regulating,treatment group on the basis was combined with azelastine hydrochloride 1 spray per side,2times per day,for 3 months.Observe two groups of asthma symptoms within 3 months control situation,the number of antibiotics and hospitalization for acute exacerbation.Results Treatment group total effective rate,asthma control test score,number of antibiotics and due to the number of exacerbations hospitalization were superior to control group [93.33%(42/45) vs.76.19%(32/42),(23.80 ± 2.15)scores vs.(22.05 ±3.16) scores,13.3% (6/45) vs.35.7% (15/42),11.1% (5/45) vs.33.3% (14/42)] (P < 0.05 or < 0.01).Conclusion Combined by azelastine hydrochloride nasal spray may make a good control,reduce the use of antibiotics for acute attack and times of hospitalization,worthy of clinical application.
7.Distribution and drug resistance of 2547 Enterococci strains

Objective To investigate the characteristics of distribution and the changes in drug resistance of Enterococci,providing reference for clinical treatment.Methods The distribution and drug resistance in 2547 clinical isolates of Enterococci from 2001 to 2007 in Huzhou Central Hospital and the First People's Hospital of Huzhou were retrospectively analyzed.Results A total of 2547 strains of Enterococci were isolated from 94876 clinical specimens(2.68%),in which Enterococcus faecalis wag primary and Enterococcus faecium Wag ranking the second.The positive rate of Enterococci in urine specimens was the highest,and that in sputum specimens was increasing recently.Enterococci showed higg resistance to erythromycin,rifampin,ciprofloxacin and levofloxacin;Enterococcus faecalis Wag more sensitive to penicillin,ampiciilin and ndtrofurantoin than Enterococcus faecium,while Euterococcus faecium was more sensitive to chloramphended and tetracycline than Enterococcus faecalis;both of them were sensitive to vaneomycin and teicoplanin.Conclusions Enterococcus faecalis and Enterococcus faecium are the common pathogen in Enterococci infections.Enterococci show high drug resistance,so the clinical use of antibiotics should be based on the resuhs of drug sensitivity test.
8.Biofeedback therapy for chronic prostatitis: Application and consideration.

Studies of biological feedback (BF) for the treatment of chronic prostatitis (CP) are occasionally reported have exhibited some related problems. This article presents an evaluation of the published literature on the BF treatment of CP at home and abroad in the aspects of instrument, method, application, effect, function, and mechanism. UROSTYMTM and MyoTrac are often employed and their operating paths are basically the same. NIH prostate symptom scores, urinary function, pain, sexual function, immune function, prostate fluid, and other indicators are generally used for the analysis of the effects of BF alone or in combination with other therapies on CP and its related symptoms. Either BF alone or BF combined with other therapies can promote urination, reduce pain, improve the quality of life, attenuate inflammation, improve sexual function, adjust immunity, and lessen physical and chemical stimulation. However, the relevant literature is of low quantity and quality, the reported studies are not standardized, and exploration of the action mechanisms is neglected.

9.Standard operation procedure and cost of primary public healthcare services

Objective To explore cost of standard operation procedure of primary public healthcare services.Methods Standard operation procedure of primary public healthcare services was put forward according to national basic public healthcare service standards (2011 edition) in 2012.Random sampling method was used to choose participants from two community sanitary service centers,two township heahhcare centers and one maternity and child heahhcare hospital.Service standard operation procedure was used to measure human cost and supportive cost of public healthcare services.Results Management of 10 thousand patients who had different diseases needed various numbers of medical staff (MS),such as health profile needed 3.4 MS,hypertension management needed 10.8 MS,diabetes management needed 10.6 MS,elderly people care needed 9.2 MS,child care needed 4.6 MS,maternal care needed 24.3 MS,psychosis management needed 13.3 MS,and planned immunity for children needed 4.6 MS.Besides,the people whole covered service projects need 2.4 MS per 100 thousand people.The research showed that managing 1 sample of different kind people needed different human cost,such as health profile needed 22.67 yuan,hypertension management needed 72.69 yuan,planned immunity for children needed 30.68 yuan,diabetes management needed 71.34 yuan,old people management needed 61.50 yuan,child care needed 30.88 yuan,maternal care needed 157.15 yuan,psychosis management needed 74.25 yuan.Besides,the people whole covered service projects needed 124.9 thousand yuan per 100 thousand people.Conclusion For primary public healthcare service project,it should be critical to modify manning regulation and labor costs.
10.Clinical early intervention of Tongxia Huayu Decoction on pancreatic microcirculatory disturbance in severe acute pancreatitis

OBJECTIVE: To study the mechanisms of Tongxia Huayu Decoction (a Chinese herbal decoction for purgation and removing blood stasis) in prognostic improvement for severe acute pancreatitis by early intervention on pancreatic microcirculatory disturbance. METHODS: Fifty-three patients with severe acute pancreatitis were divided randomly into treatment group (n=28) and control group (n=25). Tongxia Huayu Decoction was given to the patients in treatment group in addition to the normal treatment in control group for one week. The clinical symptoms and signs, hemodiastase, urinary amylase, C-reactive protein (CRP) and endothelin (ET) of the patients in the two groups before and after treatment were observed and detected. RESULTS: The total response rate of the treatment group was 98.4%, while that of the control group was 80%, with significant difference between them (P<0.05). There was no significant difference of the contents of hemodiastase, urinary amylase, CRP and ET between the two groups before treatment, while they were significantly decreased after treatment (P<0.01) with more obvious change in treatment group (P<0.01). CONCLUSION: Tongxia Huayu Decoction brings satisfied therapeutic effect on severe acute pancreatitis. The mechanisms may associate with its reducing function on ET releasing so as to inhibit the pancreatic microcirculatory disturbance and acinar cell injury induced by ET.
