1.Plasmakinetic enucleation of the prostate: the complication and outcome according to the prostate size

Objective To evaluate surgical complications and outcomes based on prostate size in patients with benign prostatic hyperplasia (BPH) treated with plasmakinetic enucleation of prostate (PKEP).Methods A retrospective review was conducted of PKEP performed in 326 patients with BPH.According to the prostate size on preoperative transrectal ultrasonography measurement,patients were divided into three groups:groupl:<40ml (n =92),group2:40~80ml (n =155),and group3:>80ml (n =79).Intraoperative and perioperative parameters were evaluated.Patient perioperative data and postoperative outcomes were compared.Patients were followed up at 1,3,6 and 12 months after surgery.Early and late complications were recorded.Results There were significant differences among three groups regarding the mean operative time (P < 0.01) and the mean resected tissue weight (P < 0.01).However,enucleation efficiency (P < 0.01) in gm tissue per minute increased significantly as prostate size increased.Mean hemoglobin decrease (P >0.05),mean serum sodium decrease (P >0.05),mean postoperative irrigation time (P > 0.05),mean catheter time (P > 0.05) and mean hospital stay (P >0.05) did not differ significantly among three groups.Three groups had a similar and significant postoperative improvement in international prostate symptom score (IPSS),quality of life (QOL),maximum urine rate (Qmax),and post-void residual urine volume(PVR) independent of prostate size (P < 0.01),but no significant difference was found among three groups during 1,3,6 and 12-month follow-up (P > 0.05).Perioperative and postoperative complications did not depend on prostate size (P > 0.05).Conchsions Although patients with a larger BPH required significantly longer operation time in PKEP,prostate size did not affect perioperative and postoperative complications or micturition improvement.
2.Flow cytometric detection of peripheral blood T lymphocyte subpopulations in patients with early syphilis

Objective To detect the lymphocyte subpopulations in patients with early syphilis ,so as to know their immunity con‐dition and guide clinical treatment .Methods Flow cytometry was used to detect CD3+ ,CD4+ and CD8+ lymphocyte subpopula‐tions in peripheral blood from 59 volunteers ,including 20 normal individuals (Cont group) ,10 patients with primary syphilis (PS group) ,15 patients with secondary syphilis (SS group) and 14 patients with early latent syphilis (ELS group) .Results CD3+ , CD4+ and CD8+ lymphocyte subpopulations in each syphilis groups were higher than Cont group .SS group had the highest CD3+level ,second was PS group ,and third was ELS group .There was significant difference among three groups (P<0 .05) .The CD3+level of SS group was significantly higher than that of Cont group (P<0 .05) .ELS group had the highest CD4+ level ,second was SS group ,third was PS group .The difference of CD4+ level was significant among three groups (P<0 .05) .PS group had the high‐est CD8+ level ,second was SS group ,third was ELS group .However ,the difference of CD8+ level among three groups was not sig‐nificant (P>0 .05) .There was no significant difference of CD4+ /CD8+ among three groups(P>0 .05) .Conclusion Cytoimmunity in early syphilis enhances ,which could be benefit to eliminate Treponema Pallidum .
3.Clinical observation of the changes of serum helper T-lymphocytes (TH) cytokines in patients with HIV-infection and opportunistic infection.

OBJECTIVETo observe the changes of serum helper T-Lymphocyte (Th) cytokines at each stage in patients with human immunodeficiency virus (HIV) infection and with opportunistic infection.
METHODSSeventeen normal subjects were studied as controls. Among the 85 patients with HIV-infection studied, 31 had opportunistic infection. The study was divided into stage A, B, and C according to the standards set forth by The US Centers for Disease Control and Prevention (CDC). 17, 29, and 39 subjects were respectively at stage A, B, and C. The levels of the CD4+ T cells and the CD8+ T cells were measured by flow cytometry (FCM), while the levels of interleukin-2 (IL-2), interferon-gamma (IFN-gamma), interleukin-6 (IL-6), and interleukin-10 (IL-10) were measured using enzyme-linked immunosorbent assay (ELISA). All data were analyzed with statistic software SPSS11.0.
RESULTSThe level of the CD4+ T cells was (361.85 +/- 230.61) 10(6)/L in the experimental group, lower than that of the control group (772.41 +/- 161.56) 10(6)/L (t = 6.992, P < 0. 01). The level of IL-2 was (61.82 +/- 63.59) pg/ml, lower than that of the control group (111.25 +/- 66.14) pg/ml (t = 2.907, P < 0.01). In the experimental group, the level of the CD8+ T cells was 713.36 +/- 317.59 10(6)/L, higher than that of the control group (583.24 +/- 96.28) 10(6)/L (t = 3.127, P < 0.01), the level of IL-10 was (1362.70 +/- 869.49) pg/ml, higher than that of the control group (818.54 +/- 276.22) pg/ml (t = 4.704, P < 0.01), and the level of IL-6 was (1883.14 +/- 1058.61) pg/m, higher than that of the control group [(1208.52 +/-745.36) pg/ml] (t = 2.502, P < 0.05). Along with the progression of the disease, the level of IL-2 in the experimental group was decreasing gradually, reaching (51.72 +/- 62.28) pg/ml at stage C and (69.02 +/- 62.77) pg/ml at stage B, both of which were lower than those of the control group. The levels of IL-6 and IL-10 rose gradually and were (2040.27 +/- 1078.95) pg/ml and (1472.10 +/-982.03 ) pg/ml respectively at stage C, higher than those of the control group. At stage B, the level of IL-10 was (1347.35 +/- 780.95) pg/ml, higher than that of the control group (818.54 +/- 276.22) pg/ml. The level of IL-6 was (2236.24 +/- 1052.42) pg/ml in patients with opportunistic infection, higher than that in those without opportunistic infection (1680.43 +/- 1017.05) pg/ml (t = 2. 395, P < 0. 05).
CONCLUSIONDynamical measure of the levels of the serum IL-2, IL-6 and IL-10 in patients with HIV infection is a must. Therefore, the progression of AIDS can be controlled by increasing the level of IL-2, decreasing the levels of IL-6 and IL-10, and adjusting the balance of TH1/TH2 cells.

4.Clinical manifestation and laboratory examinations of invasive Scedosporium infection

Objective To investigate the clinical manifestation and the characteristics of laboratory examinations of invasive Scedocporium infection.Methods The clinical data of 8 patients infected with Scedosporium from January 2011 to April 2017 were collected and retrospective analysis combined with related literatures was performed.Results Among the 8 patients,6 strains of S.apiospermum,1 strains of Peudallescheria boydii and 1 strains of S.prolificans were detectable.The predisposing factors of Scedosporium infection were trauma,environmental exposure and hypoimmunity.The septahypha in specimens could be direcdy observed under microscopic examination with positive rate 100%.The growth speed of cultured colony was relatively fast and the invasiveness was strong.The colony of Scedosporium displayed various forms from white cashmere to black yeast sample.The color was gradually become dark from the center of colony with lengthening time of cultivation.Scedosporium could be identified by microscopic morphology combining culture technique.Conclusion The course of invasive Scedosporium infection may progress rapidly with serious and dangerous illness state.The most common infection of Scedosporium should be induced by S.apiospernum.The knowledge and understanding for Scedosporium infection should be strengthened to improve the level of diagnosis and treatment.
5.Interventional treatment of dural arteriovenous fistula

Objective To explore the clinical skills and curative efficacy of endovascular embolization on dural arteriovenous fistula (DAVF). Methods A total of 50 patients with DAVF,admitted to our hospital from 2000 to 2009, underwent endovascular embolization with different materials through a microcatheter under DSA. After embolization, imaging examination was applied and clinical manifestations were noted during the clinical follow-up to evaluate the clinical efficacies.Results Immediate postoperative angiography indicated that the fistulas were totally occluded in 43patients, occluded evidently in 6 and occluded partially in 1. The 6-84 months follow-up found that clinical symptoms of 34 patients disappeared, 6 alleviated evidently, 7 relapsed, and 3 worsened; no complications were noted. Conclusion The endovascular embolization for DAVF is effective; and choosing the right embolic approach and material and improving the technology are the keys of successful treatment.
6.Correlated factors of bleeding in patients with cerebral arteriovenous malformation

Objective To explore the influencing factors of bleeding in patients with cerebral arteriovenous malformation (AVM) through observing the angioarchitecture of AVM with cerebral DSA.Methods The clinical and imaging data of 199 patients with AVM, including 107 with bleeding and 92without bleeding, were retrospectively analyzed. The relationships between bleeding and such factors as the site and size of masses, the type and site of supplying arteries, the number and pathway of venous drainage, having vascular tumor-like changes or not, the relation between AVM and aneurysm in terms of location, and the number and size of concurrent aneurysms were determined by univariate analyses and Binary logistic regression with SPSS13.0. Results Univariate analysis indicated that such factors as micro-sized (0.5-1.0 cra) and small-sized (1.0-2.0 cra) masses ofAVM, AVM underdeep location,having exclusively deep draining veins, fewer draining veins and deep feeding arteries, concurrent small aneurysms (<5 mm), aneurysms at the end of the blood-supply artery were positively correlated to hemorrhagic presentation. The number of feeding arteries and the number of concurrent aneurysms were not correlated with hemorrhagic presentation. Having vascular tumor-like changes can decreased the risk of bleeding. When stepwise multiple logistic regression analysis was employed, only micro-sized and small-sized masses of AVM, having deep feeding arteries, and aneurysms at the end of the blood-supply artery were dependent predictors of hemorrhagic presentation. Conclusion Micro-sized and small-sized masses of AVM, having deep draining veins and aneurysms at the end of the blood-supply artery are the most powerful risk predictors for hemorrhagic AVM presentation.
7.The anti-HIV activity of three 2-alkylsulfanyl-6-benzyl-3, 4-dihydropyrimidin-4 (3H)-one derivatives acting as non-nucleoside reverse transcriptase inhibitor in vitro.

It was recently shown that several new synthetic 2-alkylsulfanyl-6-benzyl-3, 4-dihydropyrimidin-4(3H)-one (S-DABO) derivatives demonstrated anti-HIV-1 activity. Three of the derivatives namely RZK-4, RZK-5 and RZK-6 were used in this study to explore their inhibitory effects on a variety of HIV strains. These compounds at a concentration of 200 microg mL(-1) almost completely inhibited the activity of recombinant HIV-1 reverse transcriptase. All of the three compounds reduced replication of HIV-1 laboratory-derived strains, low-passage clinical isolated strain, and the drug resistant strain. In particular RZK-6 showed potent activity against the HIV-1 drug resistant strain. In general, the antiviral activities are similar in magnitude to nevirapine (NVP), which is a non-nucleoside reverse transcriptase inhibitor approved by FDA. The therapeutic indexes of these compounds were remarkable, ranging from 3704 to 38462 indicating extremely low cytotoxicity. These results suggest that the three S-DABO derivatives in this study have good potential for further development in anti-HIV-1 therapy. It may be particularly useful to target at the non-nucleoside reverse transcriptase inhibitors resistant HIV-1 strain.

8.Tyrosine kinase inhibitory activity of dehydroabietylamine derivatives tested by homogeneous time-resolved fluorescence based high throughput screening model.

Protein tyrosine kinases (PTKs) are attractive targets in searching for therapeutic agents against many diseases. In this study, a series of dehydroabietylamine derivatives were first determined to show PTK inhibitory activity using a high-throughput screening (HTS) method based on homogeneous time-resolved fluorescence (HTRF) technology. The structure-activity relationships of the dehydroabietylamine derivatives were established, and it was found that the compounds with a nitrogen-containing side chain had better inhibitory activity. Further studies showed that the compounds substituted with halogen in the phenyl ring resulted in higher inhibitory activity on the epidermal growth factor receptor (EGFR), and can be a guide to modify the structure of dehydroabietylamine derivatives. Dehydroabietylamine derivatives might be a new class of multi-targeted and effective PTK inhibitors with structure modifications.

9.Study on endpoint determination method of tablet coating based on 3σ criteria and logic regression.

This paper explores the statistical distribution characteristics of coating film thickness, so as to present a new method for determining coating endpoint based on 3σ criterion and logic regression. Firstly, the spectrum and thickness of 4 batch samples were collected. Secondly, the spectral range of normal products was obtained by 3σ criterion, with the spectral feature NI as the number of test spectrum in the above range. Then, the model based on 3σ criterion and logic regression was built according to the best condition in K-fold cross-validation and the determined threshold of qualified rate in the coating endpoint. Finally, the qualified rate of test set samples at different time points was calculated by the above model, and the above change trend and the threshold value were combined to determine the coating endpoint. The results of KS analysis showed the distribution of thickness of the qualified products followed the normal distribution(P=0.081>0.05). The accuracy of the coating endpoint determination was as high as 100% by the model based on 3σ criterion and logic regression when the determined threshold of qualified rate was 90%. Therefore, the 3σ criterion was feasible to the research of coating eligibility. This paper reveals certain random phenomena in the coating process, and the method features a high accuracy, quick analysis and a good interpretability, which provides a reference for online detection and qualify evaluation in future.

Objective To investigate the risk factors for bronchopulmonary dysplasia(BPD)in twin preterm infants with a gestational age of<34 weeks,and to provide a basis for early identification of BPD in twin preterm infants in clinical practice.Methods A retrospective analysis was performed for the twin preterm infants with a gestational age of<34 weeks who were admitted to 22 hospitals nationwide from January 2018 to December 2020.According to their conditions,they were divided into group A(both twins had BPD),group B(only one twin had BPD),and group C(neither twin had BPD).The risk factors for BPD in twin preterm infants were analyzed.Further analysis was conducted on group B to investigate the postnatal risk factors for BPD within twins.Results A total of 904 pairs of twins with a gestational age of<34 weeks were included in this study.The multivariate logistic regression analysis showed that compared with group C,birth weight discordance of>25%between the twins was an independent risk factor for BPD in one of the twins(OR=3.370,95%CI:1.500-7.568,P<0.05),and high gestational age at birth was a protective factor against BPD(P<0.05).The conditional logistic regression analysis of group B showed that small-for-gestational-age(SGA)birth was an independent risk factor for BPD in individual twins(OR=5.017,95%CI:1.040-24.190,P<0.05).Conclusions The development of BPD in twin preterm infants is associated with gestational age,birth weight discordance between the twins,and SGA birth.
