1.Influence of catharsis and cognitive therapy on the psychology of patients with lung cancer

BACKGROUND: The negative emotion can directly attenuate the humoral and cell mediated immunity, which contributing to the onset of lung cancer,therefore psychological therapy is crucial for the prevention of lung cancer.OBJECTIVE: To investigate the effect of catharsis and cognitive therapy on the psychology of patients with lung cancer, as well as the short-term clinical therapeutic effect, so as to exploring the importance of psychological interventionDESIGN: Non-randomized same period control, 1:1 matched observation SETTING: Cadres Therapeutic Ward of Affiliated Second Hospital of China Medical University and Cadres Therapeutic Ward of General Hospital of Shenyang Military Area Command of Chinese PLA.PARTICIPANTS: Totally 124 patients with lung cancer, received treatment at Cadres Therapeutic Ward of Affiliated Second Hospital in China Medical University and General Hospital of Shenyang Military Area Commaud of Chinese PLA from February 1999 to December 2002, volunteered into this study, were randomly divided into treatment group and control group, all data was paired by 1:1 according to age, gender, UICC-TUM stage and pathological type, totally 42 pair data were obtained.METHODS: All participant received the same chemotherapy and radiotherapy (etoposide carboplatin), the patients in treatment group were given additional emotional catharsis and cognitive therapy at the beginning of treatment once every five days for 1-3 times; And then patients were informed of related cancer knowledge once or twice a week for totally 6 weeks. Before treatment, SCL-90 (including 9 symptomatic factors that divide into five grades: 0 score meant never, 1 score was gentle, 2 scores were medium, 3 scores were quite severe and 4 scores were severer) was used to evaluate the psychology of patients; moreover the therapeutic effect was assessed according to WHO standards for lung cancer (focus disappeared for more than 4 weeks is believed of complete relieve; Focus area reduced by above 50% and maintained for 4 weeks is believed of part relieve) MAIN OUTCOME MEASURES: Scores for SCL-90 and therapeutic effect of two groups before and after treatment RESULTS: Totally 42 pairs (84 cases) were remained in the final result duced after treatment in two groups (P < 0.05), but the scores for somatzation, individual sensitivity, depression anxiety and psychosis in treatment group were found lower than corresponding control group after treatment (1.64±0.41, 1.76±0.34; 1.31±0.30, 1.67±0.35; 1.40±0.29, 1.58±0.41;Therapeutic effect: The total relieve rate in treatment group was higher than control group, but difference was of no statistical significance (68%,52%, x2=2.381, P > 0.05).CONCLUSION: Although the therapeutic effect of catharsis and cognitive therapy is s unobvious short-term in patients with lung cancer, but it can attenuate the negative emotion, such as depression and anxiety, which can improve their psychology and immune-regulating function.
2.Usefulness and safety of transtfaoracic echocardiogra phyguiding transcatheter closure of atrial septal defect

Objective To evaluate the efficiency, safety and complications of transcatheter closure of atrial septal defect (ASD) using Amplatzer and sideris occluder under the guidance of transthoracic echocardiography and fluoroscopy. Methods Fourteen patients of ASD, 6 male and 8 female, age ranged from 2.6 to 43 years old, were treated by transcatheter closure of ASD with Sideris or Amplatzer occluders. All cases were guided by thansthoracic echocardiography and fluoroscopy. Results The diameter of ASD (balloon stretched diameter) was 14-34 mm (24.16 ? 5.16)mm. The Amplatzer devices were used in 9 cases, all of them were successfully occluded with satisfactory results. Of 5 patients treated by Sideris buttoned devices, 4 had success procedure and one failed, in which unbuttoning occurred and retrieval of the device was performed by emergency surgery. Conclusion Transcatheter closure of ASD is a simple and convenient method with a high success rate and reliable curative effect. The indication of Amplatzer method is much wider and the instant success rate after the procedure is much higher than those of Sideris device. Thansthoracic echocardiography plays a key role for guiding transcatheter closure of ASD in this study.

4.Recombinant activated factor Ⅶ for coagulopathy in patients with severe multiple injuries

Objective To evaluate the effect of recombinant activated factor Ⅶ (rⅦa) in treatment of hemorrhagic shock after severe multiple injuries with coagulopathy.Methods Sixteen cases of coagulopathy after severe multiple injuries administered with rⅦa between July 2011 and June 2012 were reviewed.The requirements of blood product and coagulogram variation were comparatively studied before and after rⅦa therapy.Results After rⅦa therapy,bleeding was brought to a halt in 24 hours for nine cases and in 72 hours for seven cases.In the end,13 out of the 16 cases survived in the absence of myocardial infarct,cerebrovascular accident or deep vein thrombosis.Requirements of red blood cells,fresh frozen plasma,cryoprecipitate and platelet (PLT) were decreased at 48 hours after the final therapy as compared with those at 48 hours prior to the primary therapy,but statistical significance only existed in the reduction of fresh frozen plasma and cryoprecipitate (P < 0.05).The coagulogram indices including prothrombin time (PT) and activated partial thromboplastin time (APTT) at 4 hours after the final therapy presented statistical differences from those prior to the primary therapy (P < 0.05).Conclusion rⅦa is an important,effective and safe auxiliary means for surgical hemostasis of coagulopathy after severe multiple injuries.

6.The relationships between serum interleukin-18 and resistin for predicting intraamniotic infection

Objective To study the relationships between serum intedeukin-18(IL-18)and resistin for predicting intraamniotic infection.Method Serum levels of IL-18 and resistin were measured in 43 pregnant women with intraamniotic infection (infection group)and 40 normal pregnant women(control group).Results The level of IL-18 in infection group[(38.7±10.4)μg/L]was higher than that in control group [(23.5±5.6)μg/L],there was significant difference between two groups (P<0.01).The level of resistin in infection group[(24.84±5.32)μg/L]was higher than that in control group[(17.3±5.15) μg/L],there was significant difference between two groups(P<0.01).The levels of resistin and IL-18 were correlated in pregnant women with intraamniotic infection(r=0.61,P<0.01).Conclusion Detecting the levels of IL-18 and resistin in pregnant women can identify intramnniotic infection.
7.Relationship between plasma brain natriuretic peptide level and renin-angiotensin-aldosterone system in patients with persistent atrial fibrillation

Objective Through clinical observation and research with persistent atrial fibrillation in patients with blood-brain natriuretic peptide ( BNP) level change detection renin, angiotensin Ⅱ, aldosterone, analyze their data, investigate persistent atrial fibrillation ( PAF ) serum brain natriuretic hormone levels change and the relationship between the renin-angiotensin-aldosterone system (RAAS).Methods 80 patients with coronary artery disease in our hospital from December 2013 to October 2015 were selected, randomly divided into experimental principle PAF group (40 cases) and control group ( patients in sinus rhythm,with 40 cases) , BNP, renin, angiotensinⅡ, aldosterone levels were measured in two groups, and left atrial diameter (LAD) measurements.Results Under the same condition of cardiac function, the blood BNP concentration in PAF group was higher than that in control group (P <0.05).The results of the linear regression analysis showed that the correlation coefficient of blood BNP level and AngII concentration in PAF group was 0.301, the correlation coefficient of blood BNP level and LAD indicators in PAF group was 0.373, the concentrations of BNP and RAAS were positively correlated with LVMI, the differences were statistically significant (P<0.05), the blood BNP level was closely related to the level of RAAS in the PAF group (P<0.05).Conclusion Blood BNP concentration was significantly higher in patients with PAF and blood BNP concentration is closely related with the RAAS, LAD indicators, and a positive correlation.
8.Protective effects as ischemic preconditioning of sasanquasaponin mediated by K_(ATP) channel in the intact rat hearts 1

AIM: To study protective effects as myocardial ischemic preconditioning of sasanquasaponin (SQS) and its relationship with K ATP channel. METHODS: The study adopted the model of myocardial ischemic injury induced by subcutaneous injection of isoproterenol (ISO) in rats, administering specific K ATP channel blocker gliberclamide (GLI 5 mg?kg -1 ). Four groups were set as NS group, I/R group, SQS group ( 0.2 mg?kg -1 ), and GLI group (5 mg?kg -1 ). Prior to injection of ISO, all agents were intraveneously injected into rats for 3 days, one time per day. Subsequently, ISO was subcutaneuously injected into rats by the ways of many different sites, and some indices were measured including ECG, serum creatine kinase (CK) activity, free fatty acid (FFA), and adenosine contents in rats. RESULTS: Preconditioningly intravenous injection of SQS could effectively protect myocardium from ischemic injury induced by ISO. With GLI injected prior to SQS, the cardioprotective effects of SQS were significantly attenuated. CONCLUSION: SQS can protect myocardium from ischemic injury induced by ISO, and the protection may be mediated by K ATP channel.
9.Anti-oxygen free radicals and anti-lipoperoxidation of sasanquasaponin(SQS) to myocardial ischemic rat

AIM In this study, we observe the effects of SQS on contents of myocardial malondialdehyde(MDA)and activities of superoxide dismutase(SOD) and glutathione peroxidase(GSH Px) through adopting the model of myocardial ischemic injury induced by subcutaneous injection of isoprenaline(ISO 4 mg?kg -1 ?d -1 ?2 d) into rats. METHODS Four groups were divided, namely NS, I/R, SQS1 and SQS2 group. The contents of MDA and activities of SOD and GSH Px were examined after administering SQS for 3 days(1 time per day). RESULTS The results show the elevation of S T in ECG, increase of MDA contents and decrease of SOD and GSH Px activities in I/R group. SQS may antagonize the changes of MDA, SOD and GSH Px induced by ISO, revealing the relationship to dose dependence. CONCLUSION SQS is most likely to possess the capabilities of anti oxygen free radicals and anti lipoperoxidation to myocardial ischemic injury induced by ISO.

Object To study the protective effect and pharmacological ischemic preconditioning of sasanquasaponin (SQS) on intact rat heart and its relationship with NO Methods A model of rat myocardial ischemia induced by sc injection of isoproterenol (ISO) was prepared, and after the administration of selective NO synthesis inhibitor methylene blue (MET), the ECG, serum creatine kinase (CK) activity, free fatty acid (FFA) and adenosine contents were determined Results Preconditioning iv injection of SQS can effectively protect the myocardium from ischemia induced by ISO The cardio protective effect was significantly attenuated when the NO synthesis inhibitor MET were injected prior to SQS Conclusion SQS can effectively protect myocardium ischemia induced by ISO and the protection is most likely to be mediated by NO
