1.The changes in the difference between the target and measured plasma concentrations of propofol given by target-controlled infusion during the three phases of orthotopic liver transplantation

Objective To examine the changes in the difference between the target and actually measured plasma concentrations of propofol administered by target-controlled infusion (TCI) during the three phases (preanhepatic, anhepatic, neohepatic) of orthotopic liver transplantation. Methods Ten ASA Ⅲ-Ⅳ patients aged 30-54 yr, weighing 56-79 kg undergoing orthotopic liver transplantation were enrolled in the study. The patients were unpremedicated. Radial artery was cannulatecl and Swan-Ganz catheter was placed via right internal jugular vein. BP, ECG, SpO2 , PCT CO2, PAP, PCWP, body temperature and blood gases, electrolytes and glucose were monitored during operation. Anesthesia was induced with scopolamine 0.6 mg, midazolam 0.05 mg ? kg , etomidate 0.2 mg ? kg-1 , fentanyl 5 ?g ? kg-1 and rocuronium 0.6 mg ? kg -1 . Propofol was given with the TCI system after induction. Target plasma propofol concentration was set at 0.5 ?g ? ml -1 which was maintained during operation. Arterial blood samples were taken after equilibrium between plasma and effect site concentrations had been reached and during the three phases of orthotopic liver transplantation. Plasma propofol concentration was measured by high-performance liquid chromatography with fluorescence detection. Results One patient was excluded from data analysis because TCI propofol was stopped during operation. Objective To investigate the effect of glucose-based peritoneal dialysis fluids and icodextrin-based peritoneal dial-ysis fluids on the expression of TLR2 and TLR4 on huamn peritoneal mesothelial cells .Methods Human peritoneal mesothelial cell line 5 - 10 generations(HMrSV5) was cultured in DMEM /F12 medium supplemented with 10% (v/v) fetal calf serum (FCS) .Cell viability and cell proliferation were assessed using M TT method .The experiment were divided into 5 different groups :group A (control group) ,1 .5% dextrose group ,2 .5% dextrose group ,4 .25% dextrose group and 7 .5% Lcodextrin group .Icodextrin group (aikau dextrin) ,TLR2 and TLR4 expression were detected by Western blot .Results Treatment with different concentrations of glucose-based peritoneal dialysis fluids for 24 h did not affect the expression of TLR2 and TLR4 protein .In addition ,after stimula-tion for 48 h ,1 .5% dextrose ,2 .5% dextrose ,4 .25% dextrose decreased TLR2 expression by (5 .5 ± 2 .8)% ,(31 .4 ± 7 .5)% , (54 .9 ± 1 .9)% respectively ,TLR4 expression by (32 .9 ± 17 .6)% ,(47 .7 ± 13 .5)% ,(66 .4 ± 13 .5)% respectively .Stimulation for 72 h ,decreased TLR2 expression by (29 .4 ± 14 .7)% ,(38 .9 ± 9 .9)% ,(63 .5 ± 16 .5)% respectively ,TLR4 expression by(59 .5 ± 16 .8)% ,(63 .1 ± 9 .5)% ,(79 .2 ± 14 .0)% respectively .There was no significant change in TLR2 and TLR4 protein expression on 7 .5% icodextrin group .Conclusion Glucose-based peritoneal dialysis fluids ,but not icodextrin-based peritoneal dialysis fluids downregulates expression of TLR2 and TLR4 by HM rSV5 .
3.Effects of somatostatin on the apoptosis of colorectal cancer cells

Objective To investigate the effects of somatostatin on the apoptosis of colorectal cancer cells. Methods The expression of somatostatin mRNA in colorectal cancer tissues from 79 patients who had been admired to Yijishan Hospital from January 2004 to October 2006 was detected by nested RT-PCR. The apoptotic index of colorectal cancer cells was detected by TUNEL, and the protein expressions of somatostatin, Fas, FasL, caspase-3 and caspase-8 in colorectal cancer tissues were detected by immunohistochemistry. All data were analyzed by chi-square test, q test and Spearman rank correlation coefficient. Results There was a positive correlation between the mRNA and protein expression of somatostatin (r = 0.98, P < 0.05). The mRNA and protein expression of somatostatin in poorly and moderately differentiated colorectal cancers were significantly lower than that in well differentiated colorectal cancers (χ~2 = 10.78, 11.24, 5.27, 5.24, P < 0.05). The positive expression rates of mRNA and protein of somatostatin in papillary adenocarcinoma were significantly higher than those in mucinous adenocarcinoma and signet ring cell carcinoma and undifferentiated carcinoma (χ~2= 6.56, 6.99, 5.44, 7.39, P < 0.05). The mRNA and protein expression of somatostatin in colorectal cancer in Dukes A and B were significantly higher than that in Dukes C and D (χ~2 =5.17, 4.06, P <0.05). The apoptotic index in high or moderate somatostatin expression group was significantly higher than that in low somatostain expression group (q = 5.66, 4.21, P < 0.05), and the positive expression rates of Fas, caspase-8 and caspase-3 in high or moderate somatostatin expression group were significantly higher than those in low somatostafin expression group (χ~2= 5.48, 5.62, 6.89, 4.32, 4.19, 3.91, P <0.05). Conclusion Somatostatin plays an important role in the regulation of cell apoptosis in colorectal cancer, and the mechanism may be related to the aberrant expression of Fas/FasL.

5.Surgical Treatment of Chronic Pancreatitis with Mass in Head

Objective To summarize and discuss the diagnostic and treating experiences of chronic pancreatitis with mass in the head. Methods Eight patients of chronic pancreatitis with mass in the head who were misdiagnosised as carcinoma of head of pancreas were analyzed retrospectively in the past 10 years. Results All the patients exhibited abdominal pain,5 of whom were with jaundice and 3 with anorexia. All the patients were misdiagnosised as carcinoma of head of pancreas before the operation,but the pathology after operation indicated chronic pancreatitis. The pancreaticoduodenectomy was performed in 5 patients,the choledochojejunostomy in 2 patients,while the exploratory laparotomy in 1 patient. After the operations,the abdominal pain was relieved in 7 patients, while 2 patients who accepted pancreatoduodenectomy suffered from pancreatic fistula,1 of whom died in the end. Conclusion It’s hard to differentiate the chronic pancreatitis with mass in the head from the carcinoma of head of pancreas before operation. If the carcinoma of head of pancreas can’t be excluded during the operation,the pancreatoduodenectomy should be performed,while the duodenum-preserving total resection of the head of the pancreas or any intra-drainage operations should be done if chronic inflammation is found in the whole pancreas with a negative result of the biopsy of the pancreas through the needle aspiration.

7.Transplantation of marrow mesenchymal stem cells improves the ventricular remodeling and heart functions to acute MI rat models

Objective To study the effects of marrow mesenchymal stem cells on heart functions and ventricular remodeling after myocardial infarction in rats. Methods Myocardial infarction model was established by ligation of the left anterior descending coronary artery (LAD) in adult SD rats. 4 and 8 weeks after MMSCs implantation, he-modynamic evaluations, left ventricular weight/body weight ratio and heart weight/body weight ratio were determined. HE staining was performed for counting microvasculars and Van Gieson staining was used for measurements and calculation of the myocardial fibrillar collagen. Then we investigated the migration and evolution of MSCs in vivo by fluorescent microscope. Results HR was significantly decreased in transplantation MMSCs group. Eight weeks after transplantation, body weight in transplantation MMSCs group reached that of control group. At the same time, SBP, DBP and MBP were significantly increased in transplantation MMSCs group. HR was significantly decreased in transplantation MMSCs group. Secondly, left ventricular weight/body weight ratio and heart weight/body weight ratio were significantly decreased 4 weeks after transplantation MMSCs. Then the ratio was significantly decreased 8 weeks after transplantation MMSCs. Thirdly density of microvasculars was increased at the boundary of infarction site in the animals transplanted MMSCs. Finally, total volume of the myocardial fibrillar collagen was reduced in the MMSCs treated groups after MI. Conclusion Transplanting MMSCs can improve the ventricular remodeling and heart functions in acute MI rat models.
8.Clinical significance of NGAL in patients with primary hypertension

Objective To investigate the serum and urine levels of neutrophil gelatinase‐associated lipocalin(NGAL) in patients with primary hypertension and related clinical significance .Methods A total of 107 patients with primary hypertension and 100 healthy subjects were enrolled ,and detected for serum and urine levels of NGAL and urinary micro‐albumin .Results Serum and u‐rine levels of NGAL in hypertension patients were (1 .98 ± 1 .49) and (1 .29 ± 0 .54)μg/L ,higher than the (0 .69 ± 1 .02) and (0 .55 ± 0 .35)μg/L in healthy subjects(P< 0 .05) .Urinary micro‐albumin level in hypertension patients was also higher than healthy subjects (P<0 .05) .Conclusion Serum and urine levels of NGAL in patients with primary hypertension could be signifi‐cantly increased ,which might be with important clinical significance in diagnosis ,treatment and prognosis of hypertensive nephropa‐thy .
9.Mycoplasma infection situation of genital tract and medication countermeasures among gynecological patients in hospital of traditional Chinese medicine

Objective To study the infection rate and sensitivity of genitourinary tract mycoplasma among the patients in the traditional Chinese medicine(TCM) hospital ,and its similarities and differences with the western medicine hospital ,thus to increase the therapeutic effect by combining with the related therapeutic measures of TCM .Methods The mycoplasma identification and drug susceptibility test results in 4 086 female genital tract specimens were performed the retrospective analysis .Then the medica‐tion strategy was investigated by combining with clinic .Results Of the 4 086 specimens ,1 891 cases were Mycoplasma positive with the total positive rate of 46 .3% ;in which the positive rate of ureaplasma urealyticum(UU) was higher(40 .7% ,1663 cases) , the most sensitive drugs were pyostacin and josamycin with the sensitive rates of 99 .11% and 99 .01% respectively ;mycoplasma hominis(M H) was less with the positive rate of 2 .7% (110 cases) ,josamycin and doxycycline were sensitive;the positive rate of UU and M H was 2 .9% (118 cases) ,doxycycline and pyostacin were sensitive with the sensitive rates of 95 .77% and 95 .26% re‐spectively .Conclusion Mycoplasma has higher infection rate in the genitourinary tract among gynecological patients ,the results are similar between the hospital of TCM and Western medicine hospital ;UU is mainly Mycoplasma type;the drug susceptibility test re‐sults reveal that the empirical medication for anti‐mycoplasma infection can select doxycycline and josamycin;it is recommended that the combined therapy with syndrome differentiation of traditional Chinese medicine by combining with the drug sensitivity test re‐sults has better clinical efficacy .
10.Correlation between intraventricular pressure and early phase proto-oncogene expression in volume overload rats

AIM: To investigate the effect of intraventricular pressure change by volume overload (VOL) on expression of proto-oncogene c- fos ,c- jun ,c- myc and egr-1 . METHODS: Left ventricular systolic pressure(LVSP) and left ventricular end diastolic pressure (LVEDP) of rat with VOL induced by aortacaval fistula operation and rats of control group were measured at 30 min, 1, 4, 6, 12 and 48 h after the operation,these mRNAs at the foregoing time points were measured by slot bloting method and quantified with densitometry. RESULTS: Be compared with the control group, VOL rats LVSP decreased ( P 0.05) after the operation.The proto-oncogene expression signals were not detected in the control,negative controls and VOL rats at 30 min after the operation. The c- fos, c- jun and egr-1 mRNA signals appeared earlier,at 1 h, and c- myc mRNA increased later at 4 h.All reached peak value at 4 h and then declined gradually.The c- fos mRNA were not detected at 48 h. The c- myc ,c- 0jun and egr-1 mRNA persisted throught the entire observation period from 1 h to 48 h. CONCLUSIONS: During VOL early phase the overload have effect on expression of the proto-oncogene mRNA,c- fos, c- jun and egr-1 mRNA appear earlier, c- myc later, egr -1,c- jun and c- myc persist longer period, but the expressions do not strengthen with the ventricule wall load increase.This sequential induction pattern may reflect the time course regularity of the proto-oncogenes expression induced by VOL,and indicate the proto-oncogenes expression initiate while the heart load accumulate some extent and duration and the load magnitude may not play a critical role.
