1.Analysis of the procedure for prolapse and hemorrhoids

Objective To assess the clinical value of procedure for prolapse and hemorrhoids(PPH).Methods 32 patients withⅢandⅣhemorrhoids underwent PPH.Pain,bleeding prolapse and edema,1 day,7 days and 30 days after the procedure were investigated.Results Of the 32 patients,the average operating time was 8.20min.On the first day of the procedure,pain occurred in 9.3% and bleeding in 5.7%.But 30 days later,both pain and bleed- ing were relieved.The mean hospital stay after the procedure was(3.4?1.2)days,and the mean hospital cost was (7342.6?486.3)yuan.Retention of urine(68.8%)was the frequent complication,whose percentage was as high as 68.8%.Conclusion PPH is effective,safe,minimally invasive,and is the first choice to deal with severe hemor- rhoids.
2.Comparitive analysis between hemiarthroplasty and dynamic hip screws in femoral intertrochanteric fracture

[Objective]Through a comparitive analysis between hemiarthroplaty and dynamic hip screws in femoral intertrochanteric fracture,to discuss the differences and similaries between two groups of patients.[Method]Using the methods of retrospective review,the author collected 63 cases of femoral intertrochanteric fracture,treated by hemiarthroplasty and dynamic hip screws respectively.Followed for a minimum period of 6 months,the author compared the postoperative complications,cumulative mortality rate and loss of ambulatory grades between two groups.[Result]There was no siganificant difference in operative time and introperative bleeding and no evident distinctions showed in posteroperative general conditions.But in local symptoms,DHS group appeared the cut-out from femoral head and coax vara resulting from the collapse of medial cortex,hemiarthroplaty group emerged the pain in hip without proof of loosening and infection of prosthesis.Hip scores decreased by 8.6% in DHS group and 9.2% in hemiarthroplaty group.In the opinion of the function convalescence,there was no definition to be worth recommending between two groups.[Conclusion]To eldly patients with unstable intertrochanteric fracture,fine function should be expected equal to preoperation after operation,but the chief motive is to provide the conditions which permit the patients earlier activity.So hemiarthroplaty is a reasonable alternative to a sliding screw device to treat intertrochanteric fractures.
3.Arthroscopy-assisted Treatment for Popliteal Cyst

Objective To discuss the techniques and efficacy of arthroscopy-assisted treatment for popliteal cyst. Methods From June 2004 to January 2006, 14 patients with popliteal cyst were diagnosed and treated with arthroscopy. During the operation, the popliteal cyst was removed, and then the hernial opening was repaired by suturing the medial head of the gastrocnemius tendon. The outcomes of the operation were evaluated by using Rauschning and Lindgren Grading. Results Arthroscopic examinations showed evident pathological changes in the knee joints of the 14 patients, including 6 cases of posterior horn medial meniscus tear, 1 case of chondromalacia of patella, 2 patellofemoral malalignment, 4 osteoarthritis, and 1 discoid lateral meniscal tear. The patients were followed up for 5-18 months (mean 11 months). According to the Rauschning and Lindgren Grading, 8 patients were grade Ⅱ and 6 were grade Ⅲ before the operation. After the operation, 9 patients were grade 0, 4 were grade Ⅰ, and 1 was grade Ⅱ. There was a significant difference between the pre-and postoperative grading (Z=-4.498, P=0.000). No patient had recurrence during the follow-up. Conclusions Popliteal cyst is usually induced by knee joint disorders. It was difficult to eliminate the cyst by simple resection. Arthroscopy-assisted treatment can achieve good outcomes.
4.Effects of gingival fiberotomy on periodontical tissues

5.Prospective analysis of radiofrequency versus mechanical debridement in treatment of knee osteoarthritis

Objective To analyse the differences between radiofrequency and mechanical debridement in treatment of knee osteoarthritis. Methods Randomized control and prospective case study were designed, with 24 cases in the radiofrequency treatment group and 36 cases in the mechanical debridement group (the control). All the cases were followed up for an average of 12.4 months (ranging 6 to 24months) according to Lysholm knee function evaluation score. Results The Lysholm scores for the treatment group were superior to those in the control group in cases of Outbridge grades Ⅱ and Ⅱ-Ⅲ chondromalacia (P0.05). Conclusion Radiofrequency is the first choice in treatment of chondromalacic lesions of no more than Outbridge grade Ⅲ.
7.Case-control study on manipulation following arthroscopic capsular release for the treatment of frozen shoulder.

OBJECTIVETo compare therapeutic effects between the normal rehabilitation and combined with manipulative method after arthroscopic capsular release for the treatment of severe frozen shoulder, and to evaluate the application value of manipulationp.
METHODSFrom March 2007 to July 2010,arthroscopic capsular release was performed in 48 cases (48 shoulders, 23 left side, 25 right side). All the patients were divided into two groups: control group (11 males and 15 females) and manipulation group (9 males and 13 females). The patients in the control group were treated with conventional rehabilitation procedure, and the patients in the manipulation group were treated with additional manipulation procedure. From the 2nd day after operation, the manipulation was performed for 20 minutes every time, twice daily, and it continued for 10 days. All the cases were followed up and the scale of American Shoulder and Elbow Surgeons Standardized Assessment Form (ASES self-report section) and the range of motion (ROM) were recorded.
RESULTSThe mean follow-up period was (12.54 +/- 5.78) months (ranging from 4 to 25 months). Both ASES scores and ROM in the manipulation group were better than those in the control group at the 1st month after operation, and the difference between the ASES scores and flexion of the shoulder were significant. However, there was no significan difference at the latest follow-up.
CONCLUSIONCompared with the conventional rehabilitative procedure, manipulation following arthroscopic capsular release could promote the process of joint rehabilitation and help the patient back to normal life earlier, but there is no evidence of long term advantage.

8.THE ASSESSMENT OF LOCALITIVE METHODS IN INSULINOMAS

Objective To detect all methods of localization in the insulinomas,and promote the rate of localization in insulinomas.Methods Between April 1990 and April 1999,28 patients of insulinomas that confirmed by operation and pathology.US,CT,MRI and DSA were used for preoperative diagnosis,IOUS,PVS and touching were used for intraoperative diagnosis,and analyzed the results of operation.Results The rates of localization were 28.6% with US,32.2% with CT,41.7% with MRI,50.0% with DSA,82.1% with touching,and 92.3% with IOUS and PVS.The rates of preoperative were higher than that intraoperative.Conclusion The rates of localization were so lower in preoperative diagnosis that did not be detected repeatedly,but intraoperative diagnosis was very vital.IOUS not only to promote the localizative sensitivity,but also to discrease the rates of operative complication.
9.Homing ability of the SCA-1+/CD45+/CD31+ subgroup of bone marrow mesenchymal stem cells

BACKGROUND:Since the FDA was the first to approve autologous bone marrow stem cel transplantation for treatment of myocardial infarction in 2003, there has a large number of clinical and basic research reports. However, their conclusions are different and stem cel homing is a key point. OBJECTIVE:To explore the homing abilities of different subgroups of mouse bone marrow mesenchymal stem cels in myocardial regeneration. METHODS:After mouse bone marrow mesenchymal stem cels were detected using a mouse cardiac stem cel surface differentiation antigen, four cel subgroups were separated on the basis of CD45 and CD31. The homing abilities of the four subgroups were assayed in a Transwel chamberin vitro. The different cel subgroups were injected into the model mice suffering from myocardial infarction for 48 hours. The mice were sacrificed at 48 hours, 96 hours, and 7 days after injection; the hearts were taken and analyzed through whole-body imaging and fluorescence intensity detection. RESULTS AND CONCLUSION:The SCA-1+/CD45+/CD31+ subgroup exhibited the strongest homing ability. The whole-body imaging indicated that the fluorescence intensity of SCA-1+/CD45+/CD31+ subgroup was higher than that of the other subgroups at 48 hours, 96 hours and 7 days after stem cel injection. The migration rate of SCA-1+/CD45+/CD31+ subgroup was also the highest. These findings indicate that the homing ability of the SCA-1+/CD45+/CD31+ subgroup of mouse bone marrow mesenchymal stem cels exhibit a homing trend to the damaged myocardial tissue.
