1.Clinical analysis of 5 cases with hemangiopericytoma in nasal cavity and paranasal sinuses.

OBJECTIVE:
To improve the accuracy of diagnosis and treatment efficacy of hemangiopericytoma in nasal cavity and paranasal sinuses.
METHOD:
The clinical and pathological data of 5 cases with hemangiopericytoma in nasal cavity and paranasal sinuses verified by pathology were analyzed retrospectively.
RESULT:
Computed tomography scan revealed vascular in 5 cases. On CT scan, hemangiopericytoma generally appeared to be a uniform high density mass with obvious enhancement upon injection of contrast material. In pathological examination, there were plentiful capillaries which were like tree branch. The normal endocytes were in the inner wall of the vessel. The round, oval and spear-like pericytes scattered around the vessel. The split phase of the nucleus could be found in the tumor cell. All cases underwent surgical resection and were proved by pathological examination. The clinical data showed that the prognosis of sinonasal hemangiopericytoma was closely related to its histological grade. The recur rate in highly malignant hemangiopericytoma was obviously higher than that in middle and low malignant tumor. The rate of misdiagnosis was 80%.
CONCLUSION
Hemangiopericytoma is a potentially malignant tumor. Medical imaging can help to demonstrate the site, configuration, and characteristics of the tumors and contribute to the treatment. But there are not characteristic medical features. The final diagnosis must depend on the closely related to its pathological grade. The doctor should pay attention to the description of histological pathology. All the patients must be followed up carefully.

2.Diagnostic value of digital subtraction CT angiography in small intracranial aneurysms

Objective To evaluate the value of digital subtraction computed tomography angiography(DSCTA)in the diagnosis of intracranial small aneurysms.Methods 92 patients with intracranial aneurysms confirmed by digital subtraction angiography(DSA) or surgery were selected as the subjects of this study.Their clinical and CT imaging data were analyzed retrospectively.The diagnos-tic accuracy of DSCTA and conventional CTA for intracranial aneurysms was compared.Results A total of 102 aneurysms were con-firmed in the 92 patients,100 of them were detected by DSCTA and 89 by conventional CTA (χ2 =8.707,P =0.003).Two small aneurysms of less than 5.0 mm located respectively at the cavernous sinus and beneath the clinoid segments were missed by DSCTA, while 13 by conventional CTA.When aneurysms of less than 3.0 mm and 3.0-5.0 mm were combined with calculation,the sensi-tivity of DSCTA in detecting aneurysms of less than 5.0 mm was significantly higher than that of conventional CTA (χ2 =8.393,P =0.004).Conclusion DSCTA has more advantages than conventional CTA in the diagnosis of intracranial aneurysms,especially for the small aneurysms adjacent to the skull base.DSCTA can be used as a preferred technique in the screening and diagnosis of in-tracranial aneurysms.

4.Loin-term outcomes and quality of Life after percutaneous lunlbar discectomy for lumbar disc herniation

Objective To assess the long-term outcomes as well as the living quality of the patients with lumbar disc herniation (LDH) after the treatment of percutaneous lumbar discectomy (PLD), and to discuss the influential factors related to the long-term effectiveness. Methods During the period of January 2000 to March 2002, PLD was performed in 129 patients with LDH. By using self-evaluation questionnaires of Oswestry disability index (ODI), Short Form-36 (SF-36) and Japanese Orthopaedic Association (JOA) through letter or telephone interviews as well as the patients' initial medical records, the related clinical data were collected. Statistical analysis was conducted by using Wilcoxon's rank sum test, Chi-square test. Results One hundred and eight patients (83.7%) were able to be followed up and 104 effective ques-tionnaires were collected. The mean follow-up time was (6.64 ± 0.67) years, the excellent rate (ODI score, 0 ~ 20%) was 71.15%. The average scores of the JOA and SF-36 was 23.66 ± 5.72 and 75.88 ± 25.57, respectively. The scores of quality of life were obviously improved in all follow-up subscales. Conventional operations were carried out subsequently in 9 patients as they failed to respond to PLD. No complications related to PLD occurred in this study. The age, course of the disease and the patient's condition at the time of discharge might bear a relationship to long-term effectiveness. Conclusion PLD is a safe and minimally-invasive technique for the treatment of LDH with quick and reliable effect, PLD can dramatically improve the quality of life. Many factors, such as the age, course of the disease and the patient's condition at the time of discharge, can affect the long-term outcomes.

6.The inhibiting effect on rat C Ⅱ TA and MHC Ⅱ gene expression by plasmid vector-based RNAi technulogy

Objective To investigate the inhibitory effect on rat MHC Ⅱ transactivator (C Ⅱ TA) and MHC Ⅱ gene expression by plasmid vector-based RNAi technology. Methods According to C Ⅱ TA ge-netic information, three short hairpin RNA (shRNA) sequences were designed and the corresponding plas- mid vectors were constructed. Rat dendritic cell (DC) was transfected in vitro and rat spleen was transfected in vivo. Real time RT-PCR and flow cytometry were used to detect the expression of C Ⅱ TA and MHC Ⅱ in DC and spleen after transfection. Results C Ⅱ TA-shRNA plasmids were successfully constructed. Com-pared with control groups, the mRNA transcription levels of C Ⅱ TA and MHC Ⅱ and the expression level of MHC H were significantly inhibited in all three pC Ⅱ TA-shRNA experimental groups ( P < 0.01 ). There was positive correlation between the expression of C Ⅱ TA and MHC Ⅱ. Among the three shRNA groups, the first one showed the strongest inhibitory effect. Conclusion The expression of rat C Ⅱ TA and MHC Ⅱ can be obviously inhibited by plasmid vector of shRNA targeting C Ⅱ TA gene, which can be used for further investi-gation of gene therapy.
7.Analysis of Volatile Oils and Ethanol-Extracts from Different Parts of Radix Fici Hirtae

To investigate the quality of different parts of Radix Fici Hirtae. The content and components of volatile oils and ethanol-extracts from different parts of Radix Fici Hirtae were analyzed by the combination of gas chromatography and mass spectrometry (GC-MS). The content of volatile oils in the cortex was higher than that in the xylem and the chemical components either in the cortex or in the xylem are similar. The main components in volatile oils are hexadecanoic acid, oleic acid+linoleic acid, linoleic acid amide, palmitic acid amide,stearic acid amide and dibutyl phthalate. The main components in ethanol-extracts are psoralen, hexadecanoic acid, hexadecanoic acid ethyl ester, bergapten, oleic acid and linoleic acid. The content of psoralen in the cortex was higher than that in the xylem.[Conclusion]The chemical components in different parts of Radix Fici Hirtae are similar but the contents of volatile components and psoralen are higher in the cortex than those in the xylem.
8.Expressions of miRNA-21 and PTEN and the clinical significance in colorectal carcinoma

Objective To explore the correlations between expression of miRNA-21 and PTEN and the invasion, metastasis of colorectal carcinoma.Methods The mRNA level of miRNA-21 and mRNA and protein levels of PTEN were assayed by real-time quantitative PCR and immunohistochemical methods respectively in 65 colorectal carcinoma specimen.Results The expression of miRNA-21 mRNA in cancer tissues was significantly higher than that in paracancerous tissues (t =3.50, P < 0.05).Both mRNA and protein expressions of PTEN in tumor tissue were significantly lower than that in paracancerous tissues (t =7.35,t =12.23;P < 0.05).Expression of miRNA-21 mRNA, PTEN mRNA and protein were obviously related with depth of invasion(F =18.36 ,F =17.26 ,F =12.83;P < 0.05), Dukes stage (F =31.25, F =24.43, F =57.12;P < 0.05) and lymph node metastasis (t =5.85, t =2.18, t =4.05;P < 0.05), while it was not related to tumor differenciation and tumor location (F =7.39, t =4.62;F =7.78, t =1.29;F =5.14,t =1.37;P > 0.05).Positive relation between miRNA-21 and PTEN in colorectal carcinoma was identified (r =-0.994, r =-0.927;P < 0.05).Conclusion High expression of miRNA-21 and low expression of PTEN are both closely associated with invasion and metastasis of colorectal carcinoma.
9.Development of PEG-PLGA nanoparticles as drug carrier with tumor-targeted therapy

In recent years, tumor is a refractory disease occurring frequently which is the main cause of death. Surgery, radiotherapy and chemotherapy are the usual therapeutic tools. However,radiotherapy and chemotherapy have serious side-effects and surgery can not be used effectively when metastasis happened. Therefore, tumor-targeted therapy has developed as a better way to cure tumor. Development of research on the use of PEG-PLGA nanoparticles as drug carriers are reviewed in this article, furthermore, problems about that are analysed.
10.Role of Leukocyte Activation and Inflammatory Reaction in Chronic Venous Insufficiency

Objective To discuss the role of leukocyte activation and inflammatory processes in the disease of chronic venous insufficiency(CVI).Methods The relevant literatures about the role of leukocyte activation and in- flammatory reaction in CVI were reviewed.Results The role of inflammatory reaction in occurrence and develop- ment of venous diseases has been studied a lot in recent years.It was found that the leukocyte activation and inflam- matory reaction are involved in the structural remodeling of venous valves and walls,leading to valvular incompe- tence and formation of varicose veins.Conclusion Leukocyte activation and inflammatory processes take important roles in the occurrence and progression of CVI.
