1.Clinical observation on hepatocirrhosis of Child-Pugh B treated with Fufangbiejiasan for three years

To observe the therapeutic e ects of Fufangbiejiasan on patients with hepatocirrhosis of Child-Pugh B.Methods:Eighty-eight patients with hepatocirrhosis of Child-Pugh B,Chronic Hepatitis B were enrolled,and divided into two groups,treated with Fufangbiejiasan and Binglian Jianganlin capsule respectively.The therapeutic course was 3 years for both groups.Liver function,HBV.DNA,AFP and CT/MRI were examined at 6th,12th,24th and 36th month after treatment.Results:Levels of blood albumin were markedly increased in Fufangbiejiasan group after treatment at 6th,12th,36th month(38.5?2.1),(39.1 ?3.9),(40.8?3.4)g/L while those in Binglian Jianganlin group were reduced(28.9?1.6),(28.8?1.7),(25.6?1.5)g/L.Level of PTA in Fufangbiejiasan group was signi cantly higher than those in controls;Compared with controls,Fufangbiejiasan treatment was more e ective :34(77.3),41(93.2),42(95.5) and 0(0),0(0),0(0)(P
2.Retrospective Analysis of 2473 ADR Case Reports in the City of Wuxi

OBJECTIVE:To investigate the characteristics of adverse drug reactions(ADR)of the hospitals(Level 2 and above) of Wuxi in order to promote rational use of drugs in the clinic.METHODS:2 473 cases of ADR collected from Jan.2005 to Dec.2006 in 11 hospitals(Level 2 and above)of Wuxi were retrospectively studied.RESULTS:Most patients who had ADR were old patients over 60 years and teenagers(≤10years).Most ADR were caused by anti-infection drugs(60.9%),followed by the preparations of Chinese materia medica(13.7%).The intravenous medication was the main route of medication resulting in serious adverse reactions;Most of the ADR cases were known and mild, 74 cases were severe and scarce(3.0%).CONCLU-SION:More attention should be paid to monitoring clinical ADR and the quality of ADR reports needs to be improved.
3.Investigation of radiation exposure protection of cardiovascular intervention

Objective To investigate the radiation doses in different areas around the operation table during intervention procedure and to study the effect of X-ray protection equipments.Methods A total of 37 procedures(12 diagnostic cardiac angiographies and 25 percutaneous coronary intervention therapy) were investigated.The detection of X-ray was carried out in 2 separate groups.The first group included the operators,assistants and nurses.There were 6 detecting points on each person resulting in a total of 18 detecting points.The second group consisted of 10 detecting points on different areas of the patients and the protecting lead shields around.The measurement of X-ray doses was performed by the specialist from the radiation protection agency.Results The doses of X-ray was directly proportional to the duration of exposure.The X-ray doses varied among the 28 detecting points.The highest amount of radiation were detected on the left arm of the operator and on the left axilla and the back of the patients.Above 90% of radiation could be protected by lead shields and lead clothes.Conclusion Lead protective products available at precent are efficient in protecting radiation exposure during cardiovascular intervention procedures.More attention still need to be paid to certain highly exposed areas.

5.Observation on Regularity and Characteristics of Response to Interferon Treatment for Chronic Hepatitis B

Objective To summarize the regularity and and characteristics of response to interferon treatment for patients with chronic hepatitis B(CHB).Methods 150 cases of CHB were randomly divided into two groups:interferon therapy (observation group,n=90)and routine protecting liver(control group,n=60)By prospective researching programme,HBV markers,including liver function test were continous periodically detected simultaneously,HBV-DNA quantitative blot hybridization,HBV-DNA quantitative PCR determination were performed.Results The typical responsiveness curve of transaminase decreased rapidly,was accompanied with the use of interferon. HBV DNA turns negative earlier than HBeAg doses. In those patients who was response to interferon completely after interferon administration,the HBV DNA and HBeAg turning negative average time was 1 9 and 3 4 months respectively. The level of HBV DNA was continuons decreased in all patients treated with interferon. HBV DNA quantitative PCR provide more accuracy of viral copies in patient's peripheral blood than that in dot hybridization in interferon therapy.conclusions Interferon the treatment for patients with CHB is effectiveness.
6.Comparative study of microendoscopic disecetomy and fenestration discectomy for mono-segment lumbar disc herniation

[Objective]To compare the clinical results,advantages and disadvantages of the mono-segment lateral lumbar disc herniated patients with the treatment of micro-endoscopic discectomy(MED) and fenestration discectomy(FD).[Methods]Sixty-nine cases operated with mono-segment discectomy were analyzed from July 2003 to July 2005,among them,32 cases operated with microendoscopic discectomy and 37 cases operated with fenestration discectomy.The operating time,blood loss,time stay-in-bed,duration of hospitalization after operation were compared.[Results]The rate of excellent and good outcomes was 93.7% in MED group,and 91.9% in FD group.No significant difference was found between them,and the same with the postoperative sciatica relief significant differences ould be observed in the operation time,blood loss,duration of hospitalization after operation and postoperative visual analog scales.[Conclusion]The symptoms of the lumbar disc herniation can be reliefed effectively by both methods,but micro-endoscopic discectomy has more advantages of less trauma,less blood loss,earlier rehabilitation,less postoperative low back pain.

Objective To evaluate the cancer risk from trihalmethanes in the reclaimed water using for road clearing and public garden virescence in Beijing.Methods The reclaimed water samples were collected and the concentration of trihalmethanes was determined,the questionnaires were used to investigate the way and level to exposure to THMs for the occupational workers and general populations respectively when the reclaimed water was used in road clearing and public garden virescence,based on this,health risks assessment method was used to evaluate health affect of THMs in reclaimed water on population quantitatively,in 2003.Results Inbreathe and dermal exposure to THMs in reclaimed water were the main ways of exposure for occupational worker and general populations in road clearing and public garden virescence in Beijing.The major components of THMs in the reclaimed water were TCM,BDCM,DBCM and TBM respectively.Conclusion The cancer risk of the occupational workers and general peoples exposure to THMs is less than 1?10-5,lower than the acceptable level documented by USEPA.

AIM: Although the evidence indicates that extracellular matrix metalloproteinase inducer (EMMPRIN) is closely associated with matrix metalloproteinase (MMP) expression in tumor cells, tumor invasion and metastasis, no direct proof that EMMPRIN regulates MMPs in monocytes, especially in the atherogenic milieu is observed. Here we tested this hypothesis by examining MMP-9 expression in macrophages/foam cells and monocyte migration through EMMPRIN knockdown by siRNA. METHODS: The methods of qPCR and Western blotting were used to detect the suppressions of EMMPRIN mRNA and protein expression in macrophages and foam cells transfected with EMMPRIN-specific siRNA. The protein expression of MMP-9 in macrophages and foam cells was also determined. Monocyte migration after EMMPRIN knockdown was observed by a Transwell assay. RESULTS: EMMPRIN knockdown by siRNA markedly abolished the MMP-9 expression by 50% and 40% in macrophages and foam cells, respectively. Migration induced by chemotactic factor MCP-1 and VEGF was significantly attenuated (P<0.05) in monocytes treated with EMMPRIN-siRNA. CONCLUSION: The protein expression and secretion of MMP-9 are down-regulated by EMMPRIN knockdown during monocyte differentiation into macrophages and foam cells. Moreover, EMMPRIN siRNA treatment also prevents monocyte migration. Thus, EMMPRIN plays a key regulatory role for MMP activity and monocyte migration, making it a potential target for pharmacological intervention of atherosclerosis.
10.Role of NK cells in mouse allogeneic bone marrow transplantation

AIM: To study the effect of natural killer(NK)-cells on graft-versus-host disease(GVHD), graft rejection, engraftment and reconstituting of hematopoiesis in mouse allogeneic bone marrow transplantation. METHODS: Lethally irradiated BALB/c(H-2 d) mice were transplanted with C57/6j(H-2 b) bone marrow containing donor peripheral T cells and/or NK cells. Recipients CD 34 + cell counts and the expression of H-2K b+ cell were detected by flow cytometry, peripheral white blood cell(WBC) was detected by auto-cytometry, and the recipients survival rates, GVHD, engraftment and hematopoiesis recovery were then observed. RESULTS: In the group of transplanted with NK cells infusion, the incidence of GVHD was evidently lessened, the survival rates, WBC and CD 34 + cell counts and the expression of H-2K b+ cell were significantly high than that without NK cells infusion. CONCLUSION: In mouse allogeneic bone marrow transplantation, alloreactivity NK cells prevent GVHD, reduce graft rejection, and promote engraftment and reconstituting of hematopoiesis. [
