1.The study of portal vein hemodynamics changes after autologous bone marrow stem cell transplantation

Objective To study portal vein hemodynamics changes after autoiogous bone marrow stem cell transplantation in patients with liver cirrhosis. Methods The hemodynamic parameters, including portal vein diameter, average portal blood flow velocity and spleen size, were determined by Colour Doppler Ultrasonography in 50 patients after autologous bone marrow stem cell transplantation and all cases were followed for up to 6 months. Results (1) After autologous marrow stem cell transplantation, the portal vein diameter reduced significantly at different timepoints compared to before the treatment ([1.41 ± 0. 15] cm, [1. 38 ± 0. 11]cm,[1.36±0. 17] cm vs. [1. 53 ±0. 18] cm,t = 1. 987,1.994,1. 976, Ps < 0.05), but we found no significant difference in the comparisons among different timepoints (F = 2. 35 ,P >0.05). (2) After autologous marrow stem cell transplantation, the average portal blood flow velocity increased significantly at different timepoints compared to before the treatment ([15. 7 ± 3. 6] cm/s, [16. 1 ± 2.4] cm/s, [15. 9 ± 3.0] cm/s vs.[11.4 ± 3. 3] cm/s ,t = 2. 345, 2.460,2. 381, Ps < 0. 01) , but we found no significant differences in the comparisons among different timepoints (F = 2. 14, P > 0.05) . (3) After autologous marrow stem cell transplantation,the spleen size reduced significantly at different timepoints compared to before the treatment ([4.8±0.3]cm,[4.7±0.6]cm,[4.8±0.5]cm vs. [5. 2 ±0. 7]cm,t =2. 289,2. 390,2.425,Ps <0.01), but we found no significant differences in the comparisons among different timepoints (F = 2. 26, P > 0.05) .Conclusion The autologous bone marrow stem cell transplantation can effectively improve the portal vein blood flow,reduce the spleen size,alleviate portal hypertension in patients with liver cirrhosis.

3.THE MORPHOLOGICAL CHARACTER ON LYMPHOCYTE HOMING AND THE EXPRESSION OF LFA-1,PECAM-1 IN THE RAT MESENTERIC LYMPH NODE

Objective To study the morphological character of the high endothelial venules(HEVs) and the expression of lymphocyte function-associated antigen-1(LFA-1),platelet endothelium cell adhesion molecular-1(PECAM-1) in the rat mesenteric lymph node,inquire into their morphological representation when lymphocytes pass through the HEVs and the regulative action of LFA-l and PECAM-1. Methods To observe their morphological representation when lymphocyte pass through the HEVs wall and analyze the expression of LFA-1,PECAM-1 with the light microscope,electron microscope and immunohistochemistry method in the rat mesenteric lymph node. Results Lymphocytes contacted the endothelial cells by protuberances or membrane protuberances like welding spot,lymphocytes located in and between the endothelial cells or in the junction space of them in HEVs,there were lymphocytes in the lucid layer between inner and outer layers of basement membranes;LFA-1 mainly expressed at lymphocytes that adhered to endothelial cell in HEVs;PECAM-1 mainly expressed at the endothelial cells and the migrating lymphocytes.Conclusion 1.Lymphocytes contacted the endothelial cells by protuberances,then they got across the endothelial cells into the cellular junction space by the intraendothelial and interendothelial,and at last they emigrated from the basement membranes into the lymph tissue;2.LFA-1 involved in firm adheresion between lymphocytes and the endothelial cells;3.PECAM-1 probably had a relationship with that lymphocytes migrated endothelial cells.
5.Effects of sevoflurane postconditioning on intestinal ischemia-reperfusion injury in rats

Objective To investigate the effects of sevoflurane postconditioning on intestinal ischemiareperfusion (I/R) injury in rats.Methods Thirty-six adult male Sprague-Dawley rats,weighing 200-220 g,were randomly divided into 4 groups (n =9 each):sham operation group (group Sham),group I/R,ischemic postconditioning group (group Ipo) and sevoflurane postconditioning group (group Sevo).Intestinal I/R was induced by clamping the superior mesenteric artery (SMA) for 60 min followed by 120 min of reperfusion in groups I/R,Ipo and Sevo.In group Ipo the animals were subjected to 3 cycles of 30 min reperfusion-30 min ischemia starting from the beginning of reperfusion.The animals inhaled 1.15% sevoflurane for 30 min starting from the beginning of reperfusion in group Sevo.The animals were sacrificed at 120 min of reperfusion and then the small intestines were removed for determination of malondialdehyde (MDA) content and superoxide dismutase (SOD) activity (by colorimetric method) and caspase-3 protein expression in intestinal tissues (by Western blot).The density of apoptotic cells was calculated by TUNEL.Results Compared with group Sham,the intestinal injury score,density of apoptotic cells and MDA content were significantly increased,SOD activity was decreased,and caspase-3 protein expression was up-regulated in groups I/R,Ipo and Sevo (P < 0.05).Compared with group l/R,the intestinal injury score,density of apoptotic cells and MDA content were significantly decreased,SOD activity was increased,and caspase-3 protein expression was down-regulated in groups Ipo and Sevo (P < 0.05).There was no significant difference in the intestinal injury score,density of apoptotic cells,SOD activity,MDA content and caspase-3 protein expression between Sevo and Ipo groups (P > 0.05).Conclusion Sevoflurane postconditioning can attenuate intestinal I/R injury through reducing lipid peroxidation and cell apoptosis in rats,and the protective effect is similar to that of ischemic post-conditioning.
8.Laparoscopic versus conventional high ligation for treating inguinal hernia

Objective To compare surgical outcomes between laparoscopic high ligation and conventional procedure in the treatment of inguinal hernia.Methods High ligation of the hernial sac was performed in 94 patients with indirect inguinal hernia and communicating hydrocele between July 2002 and December 2004.Forty-four patients were treated laparoscopically by using a self-made hooked sledge-shaped needle(Laparoscopic Group),whereas conventional open procedure was conducted in 50 patients(Conventional Group).Results As compared with the Conventional Group,the Laparoscopic Group presented shorter operating time(18?5 min vs 27?8 min,t=-6.436,P=0.000),less postoperative analgesic requirement(5 patients vs 14 patient,?2 =4.017,P=0.045),shorter hospitalization length(3.2?1.2 d vs 7.3?1.9 d,t=-12.311,P=0.000),and higher total expenditure(2965.5?516.7 yuan vs 2389.7?372.3 yuan,t=-6.251,P=0.000).Conclusions Compared to conventional procedure,laparoscopic high ligation of hernial sac has advantages of minimal invasion,less pain,quicker recovery,shorter hospitalization,and better cosmetic results.It is an ideal procedure for treating pediatric indirect inguinal hernia and communicating hydrocele.
10.Analysis of high risk factors on femoral head necrosis after femoral neck surgery in adult patients

Objective To evaluate the potential high risk factors of femoral head necrosis after femoral neck surgery in adult patients. Methods From January 2009 to October 2014,390 patients with femoral neck fractures in our hospital were treated with multiple hollow compression screws. All patients were followed up for 12 to 60 months. Then retrospectively analyzed the incidence rates of femoral head nec-rosis and the clinical data about age,gender,fracture side,Garden index,fracture shift condition,internal medicine complications,whether to do lower limb preoperative traction,restoration method,postoperative load time and whether take out hollow compression screw internal fixa-tion,so as to explore the higher risk factors for femoral head necrosis. Results Among the 390 cases,352 cases got followed up. There were 45 cases of them ended with nonunionand 49 cases ended with femoral head necrosis. The univariate regression analysis results showed that the age,fracture shift condition,internal medicine complications,postoperative load time and Garden index,whether take out internal fixation were risk factors for femoral head necrosis (P<0. 05). The multi-factor results showed that aged from 40 to 60 years old,transfered fracture, seriously complications,time of loading less than 3 mouths,Ⅲ and Ⅳ Garden index,take out internal fixation were the high risk factors for femoral head necrosis (P<0. 05). Conclusion Patients who aged from 40 to 60 years old and having transfered fracture,seriously complica-tion,early weight bearing,high Garden index and internal fixation take-out would increase the risk of femoral head necrosis.
