2.Three Cases of Lupus enteritis: Response to Steroid Therapy.

Systemic lupus erythematosus is a systemic disorder which has frequent involvement of gastrointestinal tract. Non specific symptoms such as anorexia, nausea, diarrhea and abdominal pain are well known symptoms when the gastrointestinal tract is involved. The most feared gastrointestinal complication of systemic lupus erythematosus is lupus enteritis. The pathological change in lupus enteritis is usually a result of mesenteric vasculitis. Major complications such as intestinal bleeding and perforation may occur and sometimes result in sugery. Because of high mortality rate in case of major complications, early diagnosis and appropriate treatment is very important. We experienced three patients with lupus enteritis who presented with severe abdominal pain and dirrhea. They were diagnosed by characteristic radiographical findings of small bowel series and barium study. All radiographical findings has been resolved completely with the steroid therapy. Conclusively we can induce complete remission by steroid therapy alone, if we diagnose lupus enteritis in the early period of disease course.

3.Intratumoral Gas Formation of HCC after Trancatheter Arterial Chemoembolization (TACE).

PURPOSE: To determine the factors predisposing to the intratumoral gas formation after TACE in the treatment of HCC. MATERIALS AND METHODS: A total of 176 pathologically or clinically proven patients with HCC on whom CT scan had been performed in 2 weeks to 3months after TACE were reviewed. The patients were classified in two groups; intratumoral gas-forming group(n=18) and non-gas forming group(n=158). Chi-square (X2) test was applied to analyze the factors influencing to gas formation. The radiological factors studied were the morphologic type and location of HCC, presence or absence of tumor capsule on CT, the degree of tumor vascularities, presence or absence of portal vein thrombosis, presence or absence of arterioportal shunt, and TACE catheterization method on angiegraphic examination and the duration of post-TAE fever. RESULT: Intratumoral gas foramtion after TACE occurred significantly in tumors with peripheral location or capsule on CT tumors with less vascularity or peripheral arterioportal shunt on angiegraphic examination, and TACE with selective catheterization method(p<0.05). But the morphologic types of HCC and presence of main portal vein thrombosis were not significantly different between the two groups (p>0.05). The average duration of fever sustained after TACE was longer in gas-forming group than those in non-gas forming group (p<0.05). CONCLUSION: TACE of HCC with weak collaterals or with peripherd prevent recanalization from collaterals has tendency to intratumoral gas form-his and prolonged fever duration.

4.Maternal and Neonatal Effects of Thiopental Sodium-Enflurane-N2O and Propofol-N2O Anesthesia for Cesarean Section.

BACKGROUND: In anesthesia for cesarean section, thiopental sodium is regarded as the standard induction agent. Propofol, 2,6 di-isopropyl phenol, is a relatively new intravenous anesthetic agent and has been used for induction and maintenance of general anesthesia or total intravenous anesthesia. Propofol has properties which suggest that it might be useful alternative to thiopental. METHODS: Forty patients (ASA physical status 1,2) scheduled for cesarean section were randomized to either propofol (n=20) or thiopental group (n=20). In thiopental group anesthesia was induced with thiopental 4-5 mg/kg intravenously and maintained by inhalation of enflurane and nitrous oxide. In propofol group anesthesia was induced with propofol 2 mg/kg intravenously and maintained by continuous infusion of propofol 6-10 mg/kg/hr and inhalation of nitrous oxide. RESULTS: Systolic and mean arterial pressure were increased significantly in both groups at 1 min after intubation, but degree of increase were less in propofol group. There was no significant difference in diastolic pressure in both groups. Heart rate was increased significantly in both groups at afterinduction, but degree of increase were less in propofol group. The Apgar scores of the neonates and blood gas analyses of umbilical vein were not significantly different in both groups. Maternal recovery from anesthesia was quicker in propofol group. CONCLUSIONS: A propofol infusion coupled with nitrous oxide was proved to be clinically satisfactory anesthesia for cesarean section with no adverse effect on both mother and fetus. Conclusively, propofol would be an excellant alternative to thiopental sodium and inhalation anesthetic in general anesthesia for cesarean section.

5.A Case of Primary Mucinous Adenocarcinoma of the Colon in Children.

Carcinomas are malignant neoplasms composed of epithelial cells and epithelial cancers are rare in the young. When it comes to colorectal carcinoma, the incidence of the large bowel has been noted with increasing frequency in the younger population, but this disease has remained very rare in children. Only 1% of all malignant growths of the large intestine arise in persons under 20 years of age. Inspite of these facts, the colon is the most common site of gastrointestinal carcinoma in children. While the prognosis is relatively acceptable in adults, it is very gloomy in children. Of all the histologic categories of colorectal cancer, the mucinous variety, which accounts for approximately 15% of adult colorectal carcinoma, is the predominant histologic variety in patients younger than 20 years of age at the time of diagnosis. Now, only two cases of carcinoma of the colon in children under 15 years of age have been reported in Korea. We report a case of mucinous adenocarcinoma of the colon in a 14-year-old boy with a brief review of the cases on record.

6.A Case of Congenital Gastric Outlet Obstruction with Serosal Fibrous Band in Prematurity.

Most of the gastric outlet obstruction symptoms like vomiting and abdominal distension were caused by congenital anatomical abnormality in a neonate. Abnormal structures associated with congenital gastric outlet obstruction have been categorized by its site and extent of obstruction. We report one case of persisting vomiting in a premature infant caused by serosal fibrous band in gastric outlet lesion, excluded from the category of congenital gastric outlet obstruction.

7.A Study on the Dietary Habits and Health Consciousness of High School Students in Seoul.

This study was conducted to examine the dietary behaviors with 510 high school students (260 males and 250 females) residing in Seoul. The average age of male and female students were 16.3 and 16.4 years old, respectively. Mean height and weight of male students were 174.1 cm and 65.9 kg and those of female students were 161.6 cm and 52.5 kg. The body mass index (BMI) of male and female students were 21.7 kg/m2 and 20.1 kg/m2, respectively, which were within the normal range. Students skipped breakfast more than once a week were 41.1% of males and 44.4% of females. More than 40% of the male and female students skipped breakfast frequently. The mean mini dietary assessment index score of male and female students were 30.6 and 29.6 out of total 50 index scores, respectively. Female students perceived significantly less healthy and fatter than male students. Furthermore, body image satisfaction of the female students was lower than that of male students. Female students were more significantly concerned with body image and desired a thin body shape. Only 22.7% of male students and 6.4% of female students exercised regularly, and the frequency, time, and intensity of exercise in the male students were significantly higher than those of female students.

8.Delayed-Onset Complete Atrioventricular Block in a Patient with Murine Typhus Myocarditis.

Murine typhus is a flea-borne infectious disease caused by Rickettsia typhi, of which myocarditis is a rare complication in the acute disseminating phase. A 62-year-old female presented with a fever and was diagnosed with murine typhus. She was treated with doxycycline and discharged after complete resolution of the fever. However, recurrent presyncope and exertional dyspnea developed 6-8 weeks later. Complete atrioventricular (AV) block with a wide QRS escape rhythm and a left bundle branch block configuration was documented. Subacute myocarditis was diagnosed based on persistent cardiac troponin-I elevation and typical cardiac magnetic resonance imaging findings. A permanent pacemaker was implanted for symptomatic complete AV block. Few reports of myocarditis in murine typhus have been published. We report a case of murine typhus myocarditis complicated by complete AV block in the late convalescence phase.

9.Risperidone Monotherapy in Children and Adolescents with Autism Spectrum Disorders: A Naturalistic Study.

OBJECTIVES: We retrospectively investigated the efficacy and tolerability of risperidone monotherapy in subjects with autism spectrum disorder (ASD). In addition, we did mixed effect model analysis of the effects of risperidone in patients with ASDs naturalistically treated in a routine clinical setting to determine whether the clinical effects were maintained and the side effects were tolerable. METHODS: This retrospective study assessed children and adolescents with ASD, who were on risperidone monotherapy from July 2010 to July 2011 at the Child and Adolescent ASD Clinic at Seoul National Hospital. Outcome measures included the Clinical Global Impression-Severity of Illness (CGI-S) and the CGI-Improvement (CGI-I) scales along with other clinical indices: dosage, target symptoms, and side effects. RESULTS: The mean dose of risperidone in 47 children and adolescents with ASD (40 males, 7 females; age range 5-19 years) who were on risperidone monotherapy was 1.6+/-0.8 mg/day, and the mean duration of the treatment period was 20.2+/-17.3 months. Aggressive behavior, stereotypic behavior, irritability, and self-injurious behavior were the most frequent target symptoms of risperidone. The most common side effects were weight gain followed by somnolence and extrapyramidal symptoms. In a mixed effects model analysis of CGI-I scores, the mean CGI-I score at the 1 month follow-up was significantly different from the mean CGI-I score of the 3-month follow-up (p=.046), and the CGI-I scores were equally maintained over 3 to 48 months [F(6, 28.9)=4.393, p=.003]. Of the 47 patients, 33 patients (70.2%) were identified as the response group, showing an end point CGI-I rating of 3 or under and having continued risperidone treatment for at least 6 months. The baseline CGI-S score showed significant association with clinical response to risperidone (p=.005), the mean baseline CGI-S was higher in the response group compared to the non-response group. CONCLUSION: In this study, clinical improvement of risperidone stabilized around 3 months and was equally maintained up to 48 months with tolerable side effects, supporting maintenance of risperidone treatment in children and adolescents with ASDs.

10.A Case of Neurofibromatosis with Invasion of Bladder.

Neurofibromatosis is a rare systemic disease, and genitourinary tract involvement is especially uncommon. Bladder is the most frequently involved organ in the genitourinary tract. Bladder neurofibromatosis may present as a diffuse infiltrative process or an isolated neurofibroma. The symptoms vary, ranging from urinary incontinence to retention. Treatment is usually conservative. The patient should be worked up to rule out other manifestation of tumor enlargement and followed to evaluate the development of new lesion. We report a case of the development of invasion of bladder in a patient with neurofibromatosis.
