2.Curative effect of Novolin 70/30 and Gliclazide Sustained-release Tablets on insulin therapy for poorly controlled obese patients with type 2 diabetes mellitus

Objective To explore the clinical efficacy of combination therapy with promethrin and gliclazide sustained-release tablets in the treatment of obese patients with type 2 diabetes mellitus with poor blood glucose control. Methods Selected from December 2015 to March 2017 in our hospital treated 96 cases of insulin treatment of poor blood glucose control in obese patients with type 2 diabetes, were randomly divided into study group and control group, 48 cases in each group.The study group were treated with Novolin 70/30(Novolin 30R)combined with Gliclazide Sustained-release Tablets,while the patients in control group were treated with Novolin 30R combined with repaglinide. Results The decrease of FBG, 2h PG, HbAlc, HOMA-IR and the increase of HOMA-IS in the study group were better than those in the control group (P<0.05).There was no significant difference in BMI between the two groups after treatment and BMI in the control group was significantly higher than that before treatment (P<0.05),CRP was not statistically significant before treatment.Conclusion The application of Novolin 30R and Gliclazide Sustained-release Tablets after treatment can make the use of insulin alone poor glycemic control in obese patients with type 2 diabetes have more desirable clinical efficacy,is conducive to ensuring the safety and effectiveness of drugs.
3.Efficacy of Shaaban and low molecular weight heparin in prevention and treatment of deep venous thrombosis after knee replacement

Objective To compare the efficacy of Shaaban and low molecular weight heparin in the prevention and treatment of deep venous thrombosis of lower extremity after knee replacement. Methods January 2011 to 2013 year in January in our hospital 120 cases of knee joint replacement patients as the research object, divided into observation group and control group, the observation group were treated by oral anticoagulant treatment of Lee Shaaban, the control group by subcutaneous injection of low molecular weight heparin treatment than patients were compared between the two groups after deep vein thrombosis the incidence of 14d after treatment, adverse reactions and blood coagulation indexes in patients with drug. Results The observation group of patients with deep vein thrombosis rate was 1.67%, significantly lower than the control group 13.34%, the difference was statistically significant (P<0.05); no statistical significance in two groups of patients after the treatment of 14d PLT, APTT and PT level difference; the observation group of patients with drug adverse reaction incidence rate was 5%. The 13.34% was significantly lower than the control group, the difference was statistically significant (P<0.05). Conclusion Compared with low molecular weight heparin, the incidence of deep vein thrombosis is lower, the adverse reactions are less, and the curative effect is more obvious after applying Shaaban's knee replacement.
4.Expression and clinical significance of Id1 and p53 in human breast cancer

Objective To detect the expression of Id1 and p53 in human breast cancer and study their relationship. Methods To choose 80 cases of breast cancer from the surgical department of Tangshan people's hospital. All patients didn't get radiotherapy and chemotherapy before surgery. The expression of Idl and p53 of breast cancer were studied by using SABC immunohistochemistry. Results The expressions of Id1 were found in cytoplasm, while the expression of p53 was found in cell nucleus. The positive rates of Id1 and p53 were 87.5 %(70/80)and 90.0%(72/80) respectively in breast cancer tissues. The expression of Id1 and p53 were significantly higher in breast cancer tissue than those in adjacent non-tumor breast tissues,benign tumor tissues and normal breast tissues(P <0.05). The positive rates and expression intensity of Id1 in breast cancer had significant correlation with lymph node metastasis and pTNM stage of the tumor, but had no significant correlation with age, histological differentiation and tumor size. The positive rates and expression intensity of p53 in breast cancer had significant correlation with lymph node metastasis of the tumor, but had no significant correlation with age, histological differentiation,tumor size and pTNM stage. Conclusion Expression of Id1 in human breast cancer is up-regulated. Its expression is associated with lymph node metastasis and TNM stage and represents an independent marker for prognosis of breast cancer. To reduce the expression of Id1 will become the strategy of curing breast cancer. Expression of Id1 and p53 in human breast cancer have the same clinic significance. Each of them can represent an independent marker for prognosis of breast cancer.
5.Progress on individual therapy of colorectal cancer

Chemotherapy plays an important role in advanced colorectal cancer. Patients may show various clinical response to selection different drugs. The importance of individual therapy based on pharmacogenomics has been emphasized in cancer therapy. To explore the individual therapy of colorectal cancer, the progress of molecular markers which influence the efficacy of anti-tumor drugs containing fluorouracil, oxaliplatin, CPT-11, monoclonal antibody drugs is reviewed in this article.
6.Comparison of the clinical effect of different treatment methods for the patients with great saphenous vein varicosity

Objective To compare the clinical effect of different treatment methods for the patients with great saphenous vein varicosity.Methods 140 patients with great saphenous varicose veins were selected,and according to random number table method they were divided into observation group and control group,70 cases in each group.The control group was given conventional stripped surgery,the observation group adopted high ligation joint int-racavitary laser treatment.The clinical effects of the two groups were compared.Results The operating time,intraop-erative blood loss,postoperative length of hospital stay,postoperative recurrence rate of the observation group were (51.32 ±4.97)min,(40.03 ±9.67)mL,(6.77 ±1.14)days,4.29%,respectively,which of the control group were (72.45 ±8.79)min,(108.68 ±17.75)mL,(11.69 ±2.68)days,20.00%,there were statistically significant differ-ences between the two groups(t =17.507,28.415,14.134,χ2 =8.101,all P <0.05).The incidence rate of postop-erative complications of the observation group was 4.29%,which of the control group was 14.29%,there was statisti-cally significant difference between the two groups(χ2 =4.155,P <0.05).The intervention pain score,the quality of life score of the observation group were (1.46 ±0.54)points,(90.04 ±9.77)points,which of the control group were (2.67 ±1.15)points,(78.61 ±6.82)points,there were statistically significant differences between the two groups (t =7.968,8.026,all P <0.05).Conclusion High ligation joint intracavitary laser in the treatment of great saphe-nous varicose veins is superior to the traditional operation method in operation time,intraoperative hemorrhage,length of hospital stay,and complications,and it can effectively relieve patients'pain and improve the quality of life.
7.The surgical therapy of recurrent retroperitoneal malignant fibrous histiocytoma:an analysis of 29 cases

Objective To sum up the clinical experience in the diagnosis and treatment of recurrent retroperitoneal malignant fibrous histiocytoma (RMFH). Methods[WT5”BZ] Clinical data of 29 recurrent RMFH from 1990 to 2002 were retrospectively analyzed. ResultsTwelve (41.4%) cases underwent total tumor resection after the first recurrence. Seven cases underwent multiple viscera resection,of which two did after the first recurrence,three after second and the other two after third recurrence. Twenty-five patients were followed up for 6 months to 8 years,The 1,3 and 5 year survival rate was 73.2%,46.7% and 19.8% respectively. [WT5”HZ]ConclusionTotal tumor resection is an effective way to treat RMFH,and multiple viscera combined resection increases the success rate of total tumor resection.
8.Comparison of Cost-minimization Analyses between Metformin and Acarbose Combined with Insulin in Treating Type2Diabetes Mellitus

OBJECTIVE:To compare the effects and costs between metformin and acarbose when combined with insulin in treating type2diabetes mellitus.METHODS:Cost-minimization analysis in medical economy was applied to analyze the therapeutic effects,costs and cost related factors with the respect for the patients.RESULTS:There was no statistically sig?nificant difference in effects of the2therapeutic regimens.Total cost and drug cost per head in metformin group were1941.46yuan and138.86yuan respectively as compared with2308.37yuan and229.82yuan respectively in acarbose group.The analysis on cost related factors showed no statistical significance.CONCLUSION:Metformin combined with insulin is a preferable therapeutic regimen.
9.Clinical study advance of orthostatic hypertension in children

Orthostatic hypertension (OHT)in children was first proposed by Chinese scholars in 2012.The diagnostic criterion was revised in 2015.OHT in children was not rare in clinical practice.Children and adolescents are susceptible to OHT when they suddenly change posture or standing for a long time.The mechanisms of OHT were not clear.Dizziness,headache,nausea and vomiting were the main manifestations of OHT.Severe symptom in childhood OHT includes syncope.The prognosis of OHT was good,but the abnormal blood pressure will last to adults.Further study in OHT will help to predict the risk of cardiovascular and cerebrovascular diseases and help to evaluate target or-gan injury.The treatment of OHT has been explored,and non -medical treatment is the main option.Study in medical treatment for OHT has not been conducted.Now,the diagnostic criteria in children,epidemiologic status,clinical fea-tures and treatment of OHT were focused to deepen the understanding of OHT in children and improve the clinical standard of the disease.
10.Standardized management of thalassemia

Thalassemia is inherited hemolytic anemia caused by globin production disorder,clinically known as beta thalassemia and alpha thalassemia resulted from beta and alpha chain synthesis disorder.Significant anemic clinical manifestations are observed in intermediate -level and severe -level patients with the 2 types of anemia, affecting greatly the growth and life quality of children patients who need blood transfusion to stay alive.The incidence rate of thalassemia is as high as 7% -20% in South China,so the prevention and standaridized treatment of thalasse-mia and the management over target population draw more attention.This paper is a review on the latest development of blood transfusion,chelation therapy,health education,birth intervention for thalassemia.
