1.Prevention and Surgical Treatment of Afferent Loop Syndrome Following Subtotal Gastrectomy

Objective To investigate the cause, the prevention, the clinical manifestation,the diagnosis and the treatment of afferent loop syndrome following Billroth-Ⅱ-subtotal gastrectomy.Methods 10 cases of afferent loop syndrome whose history of operation, clinical manifestation, imaging examinations, and treatment were analyzed.Results Laparotomy was done again for all the 10 patients. Different extent of dilated afferent loop were seen intraoperatively. 6 patients underwent the Roux-en-Y anastomosis and the other 4 patients underwent the Braun anastomosis. No patients were died perioperatively. No recurrence was observed in followed up for 2 to 5 years after the operation.Conclusions Occurrence of afferent loop syndrome is associated with the incorrect operation technique. Improving the operation technique should be emphasized for preventing afferent loop syndrome. Once the definite diagnosis of afferent loop syndrome was worked out, the laparotomy shoud be done again as soon as possible. The optional operation can choose the Billroth-Ⅰoperation, the Roux-en-Y anastomosis, the Braun anastomosis or jejunal interposition,respectively.
2.The Efficacy of Prostaglandin E1 in the Treatment of COPD with Pulmonary Artery Hypertension

Objective To investigate the effects of prostaglandin E 1 (PGE 1) on the chronic obstructive pulmonary diseases (COPD) with pulmonary artery hypertension in the elderly people. Methods 60 elderly patients of COPD with pulmonary artery hypertension were randomly divided into therapy group and control group. The patients in therapy group received routine therapy plus intravenous 10?g PGE 1 once a day for 15 days. The patients in control group only received routine therapy. Before and after therapy, the mean pulmonary artery pressure (MPAP), artery O 2 pressure (PaO 2) and artery CO 2 pressure (PaCO 2), whole blood viscosity ratio, plasma viscosity ratio, hematocrit, platelet adhesive ratio and erythrocyte sedimentation rate were measured. Aim To investigate change in Cl- channels of hypertensive rats aortic smooth muscle cells. Methods 2 kidney-2 clip renovascular hypertensive rats(RVHR) model was established. In thoracic aorta smooth muscles from the hypertensive rats 1~12 weeks after operation,the changes of tension of aortic rings were recorded in vitro. Effects of Cl- channel blookers,DIDS(4,4-diisothiocyanato-stilbene-2,2'disulphonate)and NPPB[5-nitro-2-(3-phenylpropy-lamino) benzonic acid], in different concentration on contractile response of hypertensive rats aorta smooth muscle induced by 10 ?mol?L -1 phenylephrine were observed.Results The difference of inhibitory effects of 300 ?mol?L -1 DIDS and 100 ?mol?L -1 NPPB on Phe-induced contractile response between RVHR and sham-operated rats was not evident.Inhibitory effects of 300 ?mol?L -1 DIDS and 100 ?mol?L -1 NPPB on contractile response of hypertensive rats aorta smooth muscle induced by 10 ?mol?L -1 phenylephrine were lower than those of sham-operated rats in 8 weeks and 12 weeks after operation. With extension of time after operation and gradual increase of blood pressure in RVHR,inhibitory effects of DIDS and NPPB on Phe-induced contractile response gradually reduced.Conclusion Action of DIDS-sensitive and NPPB-sensitive Cl- channels in hypertensive rats aortic vascular smooth muscle cells changes. DIDS-sensitive and NPPB-sensitive Cl- channels play an important role in development and maintenance of hypertension.
4.Clinical analysis of high intraocular pressure after trabeculectomy

AIM: To analyze the common causes and treatments methods of high intraocular pressure (>21mmHg) after trabeculectomy.
METHODS: Twenty-four eyes of glaucoma patients with high intraocular pressure after trabeculectomy in our hospital from July 2010 to September 2014 were retrospectively studied. The common cause and processing methods were analyzed.
RESULTS:The causes which lead to early postoperative high intraocular pressure included high filtering bleb scar in 11 eyes ( 46%) , filtration obstruction in 4 eyes ( 17%) , malignant glaucoma 3 eyes ( 12%) , iris resection of malignant glaucoma unreasonable 2 eyes ( 8%) , hyphema 2 eyes ( 8%) , wrap cystic filtering bleb 2 eyes (8%), etc. After proper treatments, intraocular pressures of all patients were bellowed 21mmHg.
CONCLUSION: High intraocular pressure with surgery for glaucoma is caused by multiple factors, preoperative and intraoperative avoid as far as possible, postoperative early detection for symptomatic treatment is the key to successful operation.
5.Relationship between D2S140 gene polymorphism and type 2 diabetes mellitus

Objective To determine the relationship between the polymorphism of D2S140 gene and type 2 diabetes mellitus.Methods D2S140 region polymorphism were analyzed with the polymerase chain reaction and short tandem repeat methods in 103 subjects with type 2 diabetes mellitus and 105 nondiabetic subjects.All subjects were biologically unrelated Chinese without a family history of DM.Results There were 7 alleles ranging from 151bp to 163bp in all subjects.The frequencies were 0.01 to 0.37,2 alleles were more common in diabetic patients than in nondiabetic subjects (RR=16.04, P
6.Changes of Alpha Competitive Structure after Administration of Single Dose Methylphenidate in Children with Attention Deficit Hyperactivity Disorder

Objective:To investigate the effects of stimulant medications on the EEG of children with attention deficit hyperactivity disorder(ADHD) by measuring the changes of the alpha competitive structure after administration of single dose of methylphenidate,and to explore the neuropathological mechanism of ADHD.Methods:The study involved 110 ADHD children who met the DSM-Ⅳ diagnostic criteria of ADHD and 110 normal children matched by age and sex.Their continuous EEG data were collected before and 2 hours after taking methylphenidate(10 mg) and were analyzed with EEG-encephaloflutuographic technology(EEG-ET).Results:(1)After taking methylphenidate,the main frequency of alpha band obviously increased [(8.97?0.94) Hz vs.(9.40?0.99) Hz,P0.05].Conclusion:Single dose methylphenidate can ameliorate the brain self-organization in ADHD children and improve the lower efficient state,and produce changes in the EEG towards normalization.
8.Study of candidate genes in type 2 diabetes mellitus in Chinese Han populations in Shandong

Objective To investigate the relationship between polymorphisms of candidate genes 〔glucokinase (GCK) gene, NIDDM1(D2S140) gene, fatty acid bindingprotein2(FABP2)gene〕and type 2 diabetes mellitus (DM) in Shandong Chinese Han population. Methods Short tandem repeat (STR) region polymorphisms of GCK gene, NIDDM1 gene and FABP2 gene were analyzed with the polymerase chain reaction and polyacrylamide sequencing gel in 107 patients with type 2 diabetes mellitus and 105 normal subjects. The subjects studied were unrelated and they are Chinese Hans living in Shandong province and no diabetes family history was revealed. Results A5 allele of GCK gene and B2 allele of NIDDM1 gene were positively related with type 2 diabetes (RR=15.70, P0.05). Logistic regression analysis showed that A5 allele and B2 allele were among the risk factors of type 2 diabetes. Conclusion Polymorphic microastellite repeat markers at the glucokinase gene and the NIDDM1 (D2S140) gene locus are associated with type 2 diabetes in Chinese. The polymorphism at the FABP2 locus is not a genetic marker for type 2 DM in Chinese Hans.
9.MRI Manifestation of Temporomandibular Joint after Trauma

Objective To investigate MRI manifestations of temporomandibular joint trauma,emphasized on the displacement and deformation of the meniscus.Methods Thirteen patients with TMJ trauma were examined by MRI with 3-inch dual surface coils.T1WI and T2WI images in oblique sagittal and coronal plane were acquired in both closed-and open-mouth positions.Results Twenty(20/26,76.9%) joints had condylar fractures.Nineteen(19/20,95%) discs in 12 patients displaced anteromedially with the condylar fragments,six(6/20,30%) discs showed distortion and deformation.The other one patient had unilateral condylar fracture companied with avulsion and lateral dislocation of ipsilateral disc(1/20,5%),and the opposite disc also showed slight anterior displacement.Effusion was observed in 12(12/20,60%) joints with condylar fractures.The location of the discs didn't show obvious change between closed-and open-mouth positions.Conclusion MRI is an effective method for demonstrating TMJ injuries,and is useful for detecting the changes of morphology and position of the discs.

Objective: To study the e ects of Heying Anxin Fang on cardiac function and intracellular Ca2+ concentration in cadiocyte of chronic congestive heart failure rats.
