Spine surgery is a branch of science bridging neurosurgery and orthopedics.In our country,spinal neurosurgery started relatively late and its development is unbalanced regionally despite rapid development in recent years.In this paper, we make an objective evaluation of the development and present situation of spinal neurosurgery in China and its existing problems.We also introduce some theoretical knowledge about stability of the spine as well as some methods commonly used in clinic to protect and reconstruct spinal stability. We urge neurosurgeons to pay more attention to the learning of and research on spinal stability and biomechanics,and perform spinal surgery vigorously and assuredly.

5.Evaluation for disease-specific quality of life in children with sleep-disordered breathing

Objective To evaluate the impact of sleep-disordered breathing(SDB)on the quality of life(QOL)in children with SDB and investigate the value of tonsillectomy and adenoidectomy in improving their QOL.Methods Questionnaire of disease-specific quality of life for children with obstructive sleep apnea with 18 items(OSA-18)was used to evaluate QOL in 168 children with SDB within four weeks before and six to 12 months after surgical operation,and correlation between OSA-18 scores to apnea hypopnea index(AHI)and minimum oxygen saturation(MiO2)in 62 of them was evaluated with polysomnography(PSG),with 50 healthy children without snoring as controls.Results Overall scores of OSA-18 in children of SDB group before surgical operation were significantly higher than those in children of control group(t=15.12,P<0.01).and QOL was seriously affected in 30.4 percent of the children with SDB.Scores of OSA-18 correlated to AHI and MiO2 before surgical operation.QOL was improved significantly in 73.8 percent of children with SDB after tonsillectomy and adenoidectomy.Conclusions SDB has obvious impact on pediatric QOL,which can be improved by tonsillectomy and adenoidectomy.There exists significant correlation between OSA-18 scores and objective indicators of PSG,and the former can be used as clinical diagnosis for SDB in children and as quantitative evaluation for the effectiveness of intervention.
6.Prediction meaning of daytime sleepiness evaluation for patiens with sleep apnea sydrome combined with cerebral vascular disease occurrence

Objective To investigate whether scores of epworth sleepiness scale (ESS) assessing daytime sleepiness were associated with sleep apnea syndrome(SAS)combined with cerebral vascular disease and ESS could be used to predict cerebral vascular disease occurrence.Methods A group of 62 SAS combined with transient ischemic attack(TIA), a group of 53 SAS with combined hypertension and the other group of 61 SAS combined without other diseases were monitored by polysomnography(PSG), daytime sleepiness of these SAS patients were assessed by ESS at the same time. 7.Effects of comprehensive pelvic floor rehabilitation program on pelvic floor function in patients with perimenopausal female sexual dysfunction

Objective To investigate the effects of comprehensive pelvic floor rehabilitation program on pelvic floor muscle strength, female sexual function index (FSFI) score and sexual satisfaction scale for women (SSS-W) score in patients with perimenopausal female sexual dysfunction. Methods The clinical data of 120 patients with perimenopausal female sexual dysfunction were retrospectively analyzed, and all the patients were treated with estrogen and progesterone replacement therapy. Among the patients, 80 patients were treated with Kegel training program (control group), and 80 patients were treated with Kegel training program combined with comprehensive pelvic floor rehabilitation program (treatment group). The pelvic floor muscle strength, FSFI score, SSS-W score, incidence of algopareunia and sexual life frequency were compared between 2 groups. Results The pelvic floor muscle strength after treatment in treatment group was significantly better than that in control group. The Ⅲ,ⅣandⅤgrade of pelvic floor muscle strength in treatment group was in 5, 42 and 33 cases, and that in control group was in 21, 47 and 12 cases, and there was statistical difference (P<0.05). The FSFI scores and SSS-W scores after treatment in treatment group were significantly higher than those in control group:(32.24 ± 3.70) scores vs. (28.51 ± 2.95) scores and (139.80 ± 16.17) scores vs. (122.33 ± 13.75) scores, and there were statistical differences (P<0.05). The incidence of algopareunia after treatment in treatment group was significantly lower than that in control group:10.00%(8/80) vs. 55.00%(44/80), and there was statistical difference (P<0.05). The sexual life frequency after treatment in treatment group was significantly better than that in control group, the 0, 1, 2 and≥3 times/month of sexual life frequency in treatment group was in 0, 8, 20 and 52 cases, and that in control group was in 5, 34, 24 and 17 cases, and there was statistical difference (P<0.05). Conclusions Comprehensive pelvic floor rehabilitation program in patients with perimenopausal female sexual dysfunction can efficiently promote the recovery process of pelvic floor muscle strength, improve FSFI score and SSS-W score, and increase the degree of sexual satisfaction.
8.Comparison of the efficacy of intraspinal tumor resection under quadrant channel and hemilaminectomy surgery

Objective To compare the clinical efficacy of intraspinal tumor resection under quadrant channel and hemilaminectomy surgery.Methods We retrospectively studied 33 patients with intraspinal tumor with the lesion length of less than 5 cm hospitalized in our hospital from July 2013 to July 201 5.Of the 33 patients,1 6 patients received intraspinal tumor resection under quadrant channel and 1 7 patients received intraspinal tumor resection under hemilaminectomy surgery.We compared various indicators in the two groups,including the length of surgical incision,surgical operation time,bleeding volume during operation,time for the first ground activity after surgery,length of hospital stay (LOS)after surgery,visual pain simulation (VAS)score 1 week after surgery, and JOA score excellent and good rates.Results The patients who received quadrant channel intraspinal tumor resection outperformed those who received hemilaminectomy surgery regarding the length of surgical incision, bleeding volume during operation,time for the first ground activity after surgery,LOS after surgery,and VAS score 1 week after surgery (P <0.05 ).Conclusion Intraspinal tumor resection under quadrant channel is a safe and effective therapy because of small trauma,less bleeding,quick recovery and curative effect.
9.Modified surgical treatment for Arnold-Chiari Ⅰ malformation complicated with syringomyelia

Objective To evaluate the methods and effects of modified surgical treatment for Amold-Chiari Ⅰ malformation complicated with syringomyelia.Methods Twelve patients with ArnoldChiari Ⅰ malformation complicated with syringomyelia,underwent a modified surgical treatment,effects were observed.Results During the first follow-up,sensation and motion function were significantly improved in 10 cases,unchanged in 2 cases.During the second follow-up,2 cases previously unchanged were improved,while the other were with no change.During the first follow-up,lesion showed on MRI disappeared in 5 cases,reduced in 4 cases,no significant change in 3 cases,no case extend,3 cases with hydrocephalus ventricle was significantly reduced.The second follow-up,MRI showed that there was no change compared with the first follow-up.Conclusion The remove of the pia mater of tonsillar hernia + reconstruction the cisterna magna have good effect on surgical therapies for Arnold-Chiari I malformation complicated with syringomyelia.
10.A new first order peak location shimming method

Objective To develop a fast shimming method for permanent magnet MRI system. Methods Peak location method (PLM) was improved by calculating the eddy current effect, and the time deviation was replaced by their phase to increase the precision. Results Experiment showed that the new method took less than 45 s, which was 3 times faster than conventional method while providing the same homogeneity. The result of PLM satisfied the requirement of homogeneity. Conclusion This new method can significantly improve the homogeneity in a short time and satisfy further fast pulse sequence development on 0.5T permanent magnet MRI system, which can be generalized for any permanent magnet first order shimming.
