1.Value of Hypoechoic Lesions in the Diagnosis of Prostate Cancer with Transrectai Ultrasonographically Guided Biopsies.

We studied the efficacy of directed and random, transrectal sonographically guided biopsies in the diagnosis of prostatic carcinoma. Between January 1994 and June 1995, 33 men underwent transrectal sonographically guided biopsies of the prostate because they had clinical findings suggestive of prostatic carcinoma. Such findings included abnormal results on digital rectal examination and elevated levels of prostate-specific antigen. Of the 33 patients evaluated sonographically, 18 had hypoechoic lesions and 1S had no hypoechoic lesions. Of the 18 patients with a hypoechoic lesion, biopsy results were positive for carcinoma in 7 patients (35%). Of the 15 patients with no hypoechoic lesion, biopsy results were positive for carcinoma in 3 patients (20%). Of the 18 patients with a hypoechoic lesion, 16 had a hypoechoic lesion in the peripheral zone and 2 had a hypoechoic lesion in the transition zone. Of the 16 patients with a hypoechoic lesion in the peripheral zone, biopsy results were positive for carcinoma in 6 patients. Of the 2 patients with hypoechoic lesion in transition zone, biopsy results were positive for carcinoma in 1 patient. While 47% of patients with abnormal DRE findings and elevated PSA level had prostatic cancer, 13% of patients with biopsy-proved cancer had normal DRE findings and elevated PSA level. Of the 12 patients, 7 patients with biopsy-proved cancer had abnormal DRE findings, elevated PSA level and abnormal TRUS. The author conclude, therefore, that in` suspicious patients, sonographically guided transrectal biopsies of hypoechoic lesions may not be reliable as the sole method of detecting prostatic carcinoma. The yield of carcinoma detection increase significantly when random biopsies are performed in patients in which lesions are not found on transrectal sonograms. The optimal biopsy technique includes a transrectal sonographically guided core through any hypoechoic lesion in the nperipheral zone followed by bilateral segmental random biopsies.

2.Renal Angiomyolipoma, 7 Cases.

Renal angiomyolipomas are uncommon benign neoplasms composed of mature fat tissue, thick-walled blood vessels and smooth muscle in varying proportions, which art often associated with tuberous sclerosis. During the last 5 years, 7 patients of renal angiomyolipoma were managed at Hanyang University Medical College. All cases were unilateral and were not associated with tuberous sclerosis. One patient was managed by radical nephrectomy because we could not distinguish from renal cell carcinoma. In 6 patients preoperative diagnosis were possible and one of them was managed by nephrectomy due to spontaneous rupture with severe bleeding and 5 patients were managed by conservative treatment. One patient was managed by enucleation of angiomyolipoma, and 2 patients were successfully managed with selective renal angioinfarct. Another 2 patients were incidentally found, small sized asymptomatic cases, so they were managed byobservation. All 7 patients were free of recurrence for follow up.

Over Past years people have known rich medical information and knowledge through mass-media. It is needed to develop medical database, which makes people get proper medical information, So we made an effort to analysis health and medical consultation data. People easily access internet hospital of Gachon Medial School Gil Medical Center, coorperated with MIDAS Dongailbo. 4020 cases of "Questions and Answers "were conducted to access from March 25th, 1997 to March 25th, 1998. Using Java Servlet, Sybase, and Apache Web Server, we classified data. Those data present over 4000 databases, such as text (3644 cases), image (212 cases), moving picture (21 cases), and medical dictionary (416 dases). People can search symptome, or disease, what they want to know. Our database as a medical service is enhance people know about medical knowledge and information, so that they can take care of their health and prevent illness in advance.

4.Short-term Effect of Partially Hydrolyzed Formula on the Prevention of Development of Atopic Dermatitis in Infants at High Risk.

This short-term, prospective study was aimed to assess the effects of partially hydrolyzed formula (PHF) on the prevention of the development of atopic dermatitis in infants at high risk. The infants of parents with allergy symptoms and serum total IgE over 200 kU/L were divided into 3 groups by their feeding patterns: PHF group (n=15), standard formula (SF) group (n=32), and breast milk (BM) group (n=22). No allergenic food was given during the study period of 6 months, and breastfeeding mothers avoided egg ingestion. Their atopic symptoms were monitored every 2 months. The cumulative incidence and prevalence of atopic dermatitis at the age of 6 months were significantly less in the PHF group than in the SF group (47% vs.78%, p<0.05; 20% vs. 59%, p<0.05). Those rates of the PHF group were also less than those of the BM group, but they were not statistically significant. There was no difference in the onset age and disease severity. These results suggest that early feeding of PHF to infants at high risk has a short-term preventive effect on the development of atopic dermatitis during the first 6 months of life. Long-term preventive effects should be evaluated.

5.The Use of an Implantable Loop Recorder in Patients With Syncope of Unknown Origin.

BACKGROUND AND OBJECTIVES: Possible mechanisms of syncope often remain unknown despite the performance of extensive cardiological and neurological tests. An implantable loop recorder (ILR) has been introduced to monitor the heart rhythm continuously over a year. We evaluated the diagnostic value of the use of the ILR for unexplained syncope. SUBJECTS AND METHODS: Between 2006 and 2007, an ILR was implanted in 9 patients (7 male, 2 female, mean age 55+/-17 years) where syncope remained unexplained after extensive diagnostic tests. We analyzed the recorded electrocardiogram signal in the memory of the ILR. RESULTS: During a follow-up period of 8.8+/-7.3 months, arrhythmia was detected in five patients. Two patients had a sinus pause and received a permanent pacemaker, and one patient had sustained ventricular tachycardia and fibrillation and received an implantable cardioverter defibrillator. One patient had micturition syncope with sinus pause and is waiting for permanent pacemaker implantation, and one patient had symptomatic paroxysmal atrial fibrillation and was administered anticoagulation therapy. Inappropriate auto-activations such as a pseudopause or a decreasing signal were also noted. CONCLUSION: ILR monitoring seems to be a useful diagnostic tool to identify the arrhythmic cause in patients with unexplained syncope.
