Objective: To evaluate 3D imaging technology in the preoperative evaluation of breast conserving surgery. Methods: A ret-rospective analysis was conducted using clinical data from 38 patients who underwent breast conserving surgery that was assisted by 3D imaging technology in Xiaogan Hospital Affiliated to Wuhan University of Science and Technology from April 2017 to January 2019. All 38 patients underwent 3.0-T breast magnetic resonance imaging (MRI) examination before surgery, and 3D reconstruction of virtu-al images was constructed through 3D modeling of medical digital imaging and communication (DICOM) data. The predicted resected tissue volume was compared with the volume of the actual resected specimen, and the surgical margin and postoperative aesthetics of the breast conserving surgery were evaluated. Results: The reconstructed 3D model clearly displayed the anatomical structures of the breast, tumor, gland, and blood vessels, and their relationship in 3D spaces. The goodness of fit of the 3D model to the practical sit-uation was 97.4% (37/38). In terms of the resection tissue volume, there was no significant difference between the predicted results (PRTV) and actual results (ARTV) [(61.7 ± 20.1) mL vs. (65.1 ± 20.7) mL, P>0.05]. There was a strong positive correlation between ARTV and PRTV (P<0.01). One patient underwent supplementary secondary surgery, resulting in an incidence of 2.6% (1/38). The postopera-tive satisfaction for breast conserving surgery was 100% (38/38). Conclusions: 3D imaging technology clearly displays the anatomical relationship between breast tumor and surrounding tissues, and correctly assesses breast volume, guiding surgical resection.

BACKGROUND:Abnormal extracellular matrix accumulation and excessive proliferation of fibroblasts are the main manifestations of pathological scars.Excessive proliferation of fibroblasts leads to the production of large amounts of collagen-based extracellular matrix. Therefore,to investigate the role of fibroblast fibrosis in the formation of pathological scar will provide a new idea for revealing the mechanism of pathological scar and biological therapy. OBJECTIVE:To investigate the effect of RAS-selective lethal small molecule 3(RSL3)on the fibrosis of human pathological scar fibroblasts. METHODS:Then cases of pathological scar tissue and normal skin tissue samples from the same individuals, provided by the Department of Burn Plastic Surgery,General Hospital of Ningxia Medical University, were collected.Fibroblasts of human pathological scar and human normal skin were extracted and used in the following experiments. The general condition of the pathological scar tissue and the normal skin tissue was detected by hematoxylin-eosin staining. The appearance of fibroblasts from pathological scar and normal skin were observed by inverted microscope. The fibroblasts were verified by immunofluorescence assay. The cells were treated with different concentrations of RSL3(1,3,5,7,9,11,13 μmol/L).The inhibitory concentration of RSL3 on fibroblasts was detected by cell counting kit-8.Control group(without treatment)and RSL3 intervention group(treated with 7 μmol/L RSL3 for 24 hours)were set up.The mRNA and protein expressions of glutathione peroxidase 4,type Ⅰ collagen, type Ⅲ collagen and α-smooth muscle actin were detected by Qrt-PCR and western blot, respectively.Level of malondialdehyde in cells was detected. The residual scratch area was measured by cell scratch test after 24 hours to calculate the percentage of residual scratch area. RESULTS AND CONCLUSION:The expression of glutathione peroxidase 4 in the pathological scar group was higher than that in the normal skin group(Mrna:t=3.252,P<0.01;protein:t=5.075,P<0.01).The expression of glutathione peroxidase 4 in the pathological scar fibroblast group was higher than that in the normal skin fibroblast group(Mrna:t=10.32,P<0.01;protein:t=26.22,P<0.01).Compared with the control group, the expression of glutathione peroxidase 4 was decreased(Mrna:t=2.798,P<0.05;protein:t=4.643,P<0.01),the content of malondialdehyde was increased(t=2.917,P<0.05),the expression of type Ⅰ collagen(Mrna:t=15.84,P<0.01;protein:t=4.610,P<0.01),type Ⅲ collagen(Mrna:t=28.86,P<0.01;protein:t=7.713,P<0.01)and α-smooth muscle actin(Mrna:t=2.671,P<0.05;protein:t=7.417,P<0.01)were decreased in the RSL3 intervention group.Compared with the control group, the migration ability was weakened in the RSL3 intervention group(t=14.06,P<0.01).To conclude,RSL3 can inhibit the expression of glutathione peroxidase 4 and then inhibit the ability of fibrosis and migration of pathological scar fibroblasts.

Zuoguiwan, one of the classic formulas of traditional Chinese medicine (TCM), has the effects of nourishing genuine Yin, supplementing essence, and replenishing marrow, and it is widely used in the clinical treatment of diseases caused by congenital essence deficiency. This article systematically reviewed the ancient books involving congenital deficiency and the modern research reports on the treatment of congenital deficiency with Zuoguiwan. From the formulation principle of Zuoguiwan and the TCM concept of congenital deficiency, this article discussed the theoretical basis of treating congenital deficiency with Zuoguiwan based on the modern research results. Ancient physicians have discovered that the deficiency of parents can lead to abnormal physical development and weakness of the offspring. ZHANG Jingyue divided congenital essence Qi into genuine Yin and primordial Yang and formulated Zuoguiwan based on the principle of Yin-Yang mutual assistance for supplementing congenital genuine Yin. Experimental studies have shown that Zuoguiwan can up-regulate the expression of connexin 43 (Cx43) protein and improve gap junction intercellular communication (GJIC) function to promote osteogenic differentiation and maturation of bone marrow mesenchymal stem cells and ameliorate abnormal bone development caused by congenital essence deficiency. Zuoguiwan can enhance the expression and activity of Reelin to ameliorate abnormal brain development. It can upregulate the expression of Cx43 protein to intervene in the phosphatidylinositol 3 kinase/protein kinase B (PI3K/Akt) pathway, thus repairing the reproductive functions. Zuiguiwan can promote the development and maturation of T cells and activate the Wnt/β-catenin signaling pathway in the thymus to improve immune functions. In addition, it can promote the expression of β-catenin and inhibit the expression of microtubule-associated proteins 1 light chain 3 (LC3) to attenuate skin dysfunctions. Moreover, Zuoguiwan can guide the differentiation of stem cells via the correlation between essence and vitality in TCM. Zuoguiwan has demonstrated significant therapeutic effects on some diseases in the pediatric, andrological, gynecological, geriatric, internal medicine, orthopedic and skin disease departments. On the basis of the results of experiments and clinical applications, this paper analyzes the specific connotation of congenital deficiency proposes that congenital deficiency should be subdivided into the four aspects of essence, Qi, Yin, and Yang. Congenital essence deficiency as the deficiency of genuine Yin can lead to deficiency of kidney essence in the offspring and dysfunction of kidney storing essence. Pharmacological studies have discovered that Cuscutae Semen, Lycii Fructus, and Achyranthis Bidentatae Radix in Zuoguiwan contain active components such as quercetin and kaempferol, which act on the targets such as recombinant prostaglandin endoperoxide synthase 2 (PTGS2), interleukin-6 (IL-6), mitogen-activated protein kinases (MAPK14), tumor necrosis factor (TNF), TP53, Vascular permeability factor A (VEGFA), and albumin (ALB) to play a role in reproductive system development and hormone responses. Zuoguiwan has unique advantages over Liuwei Dihuangwan in nourishing congenital genuine Yin in the kidney and can achieve better therapeutic effects on the syndromes and diseases caused by congenital essence deficiency. This review is expected to enrich the knowledge about the efficacy and clinical application of Zuoguiwan, provide new perspectives and methods for the prevention and treatment of congenital deficiency and congenital essence deficiency, and give insights into the application of Zuoguiwan in modern healthcare, especially in the nurturing of offspring.

Cancer Research on Prevention and Treatment 2024;51(9):750-755
Objective To analyze the factors influencing axillary lymph-node metastasis (ALNM) in microinvasive breast cancer (MIBC) patients, as well as to establish the risk-prediction model of ipsilateral ALNM in MIBC patients. Methods A total of 4475 primary female MIBC diagnosed by pathology from 2010 to 2015 were searched and screened from the SEER database. The obtained data were used to establish a prediction model for ALNM of MIBC. A total of 2266 primary female MIBC patients diagnosed by pathology from 2018 to 2020 in the SEER database were screened as the external validation cohort. The clinicopathological data of the enrolled MIBC patients were collected. Univariate analysis was used to screen out the factors affecting ALNM in MIBC patients. Statistically significant variables in univariate analysis were included in multivariate logistic regression analysis. The independent factors influencing ALNM in MIBC patients were screened out, and a nomogram was established. The area under the curve (AUC) was calculated by plotting the ROC curve. After plotting the calibration curve, the model was evaluated by Hosmer-Lemeshow goodness-of-fit test. Results A total of 309 patients were diagnosed with ipsilateral ALNM among the 6741 MIBC patients, accounting for about 4.58%. Univariate analysis of the modeling group showed that age, ethnicity, histological grade, pathological type, molecular subtype, and lateral side were associated with ALNM in MIBC patients (P<0.05). Results of multivariate analysis showed that the risk of ALNM was higher in MIBC patients ≤40 years old, black people, histological grade Ⅱ and Ⅲ, and primary right side (P<0.05). Subtype is an independent factor influencing ALNM in MIBC patients. However, the difference in ALNM risk was not statistically significant between the subtype of HR+HER2+, HR−HER2+, HR−HER2− and HR+HER2− subtypes. The AUC of the modeling group was 0.716 (95%CI: 0.682-0.750), the best cut-off value was 0.045, the sensitivity was 0.733, and the specificity was 0.608. The newly established nomogram model was used for the validation cohort, and its AUC was 0.722 (95%CI: 0.667-0.777). The P values of the Hosmer-Lemeshow goodness-of-fit test of the calibration curves in the modeling and validation groups exceeded 0.05. Conclusion The risk-prediction model of ALNM in MIBC patients established by the SEER database has good predictive ability and can thus be expected to serve as a reference for clinical practice.
