[Objective] To search for a new technique for incarcerated annular mixed hemorrhoids (IAMH) . [Methods] One hundred and eighteen cases of IAMH (group A) were treated by double denenate excision-ligation combined with pad preservation and 53 eases (group B) by external-exfoliation and internal-ligation. Cure rate, wound-healing time, anal function and postoperative complications were observed in the two groups. BACKGROUND:Non-operation therapies are commonly used in the treatment of cervical spondylopathy. All of these therapies involve biological mechanics; especially for traction and massage of the cervical vertebrae, they have an obvious property of biological mechanics in rehabilitation of the cervical vertebrae. OBJECTIVE: To probe into the biological mechanics of different rehabilitation methods through comparing the intervention effects between traction and traction combined with massage on cervical spondylopathy.DESIGN: A case-controlled observation. SETTING: Rehabilitation Center of West China Hospital of Sichuan University. PARTICIPANTS: Among the patients who came to the Rehabilitation Center of West China Hospital of Sichuan University between October 2002 and July 2003 for the treatment of spondylopathy, totally 52 patientsmet the criteria for non-operation therapy. They participated in the experiment voluntarily and were randomly divided into 2 groups: the experimental group and the control group, with 26 patients for each. METHODS: ①The patients in the experimental group were treated by traction in combination with massage. Traction was performed once a day,30 minutes each time, 5 times as a course of treatment, and one or two courses of treatment in total. Massage was performed for 8 to 10 minutes each time, once a day, 5 times as a course of treatment, and one or two courses in total. ② The patients in the control group were treated with traction only. Cervical Spondylopathy Therapeutic Effect Rating Scale was adopted to evaluate the effects, three items were selected to set as our scoring criterion, which included clinical symptoms, physical examinations and activities of the daily life (ADL). The improvement index was figured out on the basis of the pre-treatment score and post-treatment score. The improvement index is worked out by the following equation: improvement index = (The post-treatment score-pre-treatment score) / the post-treatment score. The improvement index was evaluated by physicians after the treat ment. Effectual: Clinical symptoms and body symptoms were obviously improved; work, learning, and daily life are not affected. Effective: Clinical symptoms and body symptoms have improved , and work, learning, and the daily life are partially affected. In-effective: There was no improvement in clinical symptoms and body symptoms and showed no effect on the work ,learning, and the daily life. MAIN OUTCOME MEASURES: ① Score of the treatment of cervical spondylopathy of the patients before and after the treatment. ② Intervention effect on the patients after the treatment RESULTS: Totally 52 patients with cervical spondylopathy entered the stage of result analysis with none missing in the midway. ① Comparison of the scores of the treatment of cervical spondylopathy of the patients before and after the treatment: After the treatment, the score in the experimental group was significantly higher than that in the control group [ 16.431 ±3.212,13.147 ±3.036 ( t =4.676, P ＜ 0.01 )], and the improvement index of the patients in the experimental group was significantly higher than that in the control group [0.505±0.163,0.368±0.145 (t=3.860, P ＜ 0.01 )]. ② Comparison of the intervention effects on the patients after the treatment: The effectual rate of the experimental group was higher than that of the control group (80.8%,46.2%).CONCLUSION: The approach of traction combined with massage is superior to simple traction. Different rehabilitation approaches have different properties in biological mechanics. It has showed that the force change is important to therapeutic effectiveness on cervical spondylopathy.

In recent years, the emergence of single-port laparoscopic technology promoted the development of the technology of laparoscopy, but deficiencies like equipment conflict or lack of triangulation severely hampered the clinical application of this technology. The appearance of single-port laparoscopic flexible instruments makes up for those deficiencies to a great extent. The element is to add a wrist joint to the tip of the instrument to form triangulation and reduce equipment conflict. But this technique is still in an early stage of development. The purpose of this article is to summarize the characteristics and problems of existing flexible single-port laparoscopic instruments, and to provide information for the further improvement, finally the future development was expected.

Objective To study the effects of PNESF at different voltages on the life spans of ascites tumor bearing mice.Methods Forty ascites tumor bearing mice were divided into 5 groups and exposed to PNESF at 0,10,17,23 and 30 kV,respectively,for 30 minutes daily on 4 successive days.All of the animals then received routine care until death.Their eating habits,body weights and life spans were observed.A straight line fitting method was used for statistical analysis. Results The body weights of the mice exposed to 17 kV and 23 kV PNESF increased quickly during the four days of PNESF exposure.The weight gain in all groups declined after PNESF exposure was stopped.Thereafter the body weights of the mice exposed to the 17 kV PNESF increased more slowly than those in the control group.The mice exposed to the 23 kV PNESF gained weight more quickly than other groups.The mice exposed to the 17 kV PNESF lived the longest,demonstrating a lengthening of life of 25.5%compared with the control group.Conclusion Exposure to a PNESF of appropriate intensity might significantly increase the life span of ascites tumor bearing mice.

This study was aimed to explore the medication rules of Chinese herbal medicine compound in the treatment of primary dysmenorrheal based on data mining.Relevant literatures on Chinese herbal medicine compound in the treatment of primary dysmenorrhea were retrieved from June 2006 to June 2016 in the Chinese Journal Full-text Database (CNKI) to establish a literature database.The Excel 2010,SPSS 22.0 and SPSS Modeler 14.2 software were used in the frequency analysis,cluster analysis,association analysis,and etc.The results showed that literatures related to Chinese herbal medicine compound in the treatment of primary dysmenorrhea,there were 74 compound formula and 76 kinds of Chinese herbal medicine involved.Angelica was the core single Chinese herbal medicine.Chinese herbal medicine with high-frequency usage was the type of removing blood stasis,invigorating qi and blood.Cluster analysis can form 5 clustering combination.Primary dysmenorrhea was mainly clustered to two syndromes,which were qi and blood deficiency with blood stasis,liver and kidney deficiency with qi stagnation and blood stasis.The association rules analysis showed that there were a total of 25 rules,with the most common combination of two or multiple combination from four kinds of Chinese herbal medicine,including Angelica,Radix Paeoniae Alba,Rhizoma Chuanxiong,and Rhizoma Corydalis.It was concluded that blood stasis was the direct cause of primary dysmenorrhea,many of which belonged to the deficiency of root (i.e.,deficiency of qi,blood and cold deficiency) and excess of branch.Chinese herbal medicine treatment should be focused on the regulation of both qi and blood,invigoration of both liver and kidney,as well as harmonization of the blood.

This study was aimed to summarize and analyze acupoint selection rule of acupuncture in the treatment of primary dysmenorrhea in recent years,and to guide acupoint selection and compatibility in clinical practice.Relevant literatures in recent years were retrieved from the China National Knowledge Infrastructure (CNKI) full-text database.And then,literatures involving acupuncture prescriptions were summarized.Data were integrated and input.Finally,the SPSS software was combined to analyze the data of acupoint selection in the acupuncture prescription to achieve the conclusion.The results showed that in the acupuncture treatment of primary dysmenorrhea,the main meridian selections were the Ren Meridian and the Spleen Meridian of Foot Taiyin.The generally used acupoints were SP6-Sanyinjiao,RN4-Guanyuan,SPS-Diji,and etc.The multi-point compatibility therapy was used.It was concluded that although the advantages of acupuncture in the treatment of primary dysmenorrhea are prominent,the pathogenesis is mainly related to the liver,spleen and kidney,which are recognized by most physicians.However,there are still large differences in acupoint compatibility and acupoint selection.Hence,there are still no unified,scientific and standardized diagnosis and treatment standards.Further exploration should be made to guide clinical practice.

Liver transplantation is the only way to treat end-stage liver disease. In order to overcome the shortage of donor, marginal donors have been used widely, which bring about a series of problems. Machine perfusion can stimulate the circulation in vivo and is beneficial for the protection of liver. It could also improve the graft function and reduce postoperative complications, which makes it a hot spot in recent years. The aim of this study is to summarize the current status and prospects of application of machine perfusion on clinical liver transplantation.

ObjectiveTo investigate the clinical value of endoscopic esophageal submucosal tunnel resection of gastric fundus-cardiac tumors originating from muscularis propria.Methods Clinical date of 18 patients with gastric fundus-cardiac submucosal tumors originating from muscularis propria who underwent endoscopic esophageal submucosal tunnel resection from January 2011 to December 2011 were retrospectively collected and analyzed.ResultsAll lesions were successfully and completely resected in 18 patients,with sizes ranging from 0.7 cm to 7.2 cm,mean (2.43 ± 1.91 ) cm.Pneumoretroperitoneum,pneumomediastinum and pneumohypoderma occured during the procedure in 2 cases,but spontaneously resolved in 3 days.Fever with increased WBC within 24 h after the procedure occurred in one patient,and was cured in two days with antibiotics.There were no severe complications including bleeding,perforation or death.All patients could have liquid diet 3 days later after the operation.Follow-up endoscopy at 1 week after the operation showed a healing of esophageal incision.ConclusionEndoscopic esophageal submucosal tunnel resection is a safe and effective method for gastric fundus-cardiac submucosal tumors originating from the muscularis propria,lessening the difficulty of traditional endoscopic resection.

Objective To investigate the role of preoperative peripheral blood neutrophil to lymphocyte ratio (NLR),platelet to lymphocyte ratio (PLR),prognostic nutritional index (PNI) in the prognosis of patients with hepatitis B virus (HBV)-related hepatocellular carcinoma (HCC) after radical operation.Methods This is a retrospective study,involving 426 surgically resected hepatitis B related hepatocellular carcinoma cases in a single center from 2003 to 2012.Results Kaplan-Meier analysis showed patients in NLR ≤ 1.62 group achieve higher rate of recurrence-free and overall survival than that in the NLR ＞ 1.62 group,the difference was statistically significant (P ＜ 0.005);Also PNI ＞ 49.42 group showed higher rate of overall survival significantly than PNI≤49.42 group (P ＜ 0.005).The results of Cox regression multivariate analysis further suggested that both NLR ＞ 1.62 (HR 1.74,P =0.007) and PNI ≤49.42 (HR 0.70,P =0.021) were independent risk factors for overall survival,NLR ＞ 1.62 (HR 1.45,P =0.03) was also an independent risk factor for recurrence-free survival.Conclusion The preoperative NLR and PNI may be independent risk factors for prognosis of patients with HBV-related HCC after radical operation.
