Objective To evaluate the efficacy and safety of local infiltration analgesia in the multimodal analgesia protocol. Methods Sixty patients who were scheduled to undergo TKA were randomly divided two groups:local infiltration analgesia (LIA) group (n=30) or the non-local infiltration analgesia (N-LIA) group (n=30).All patients were given Celecoxib 200 mg bid,3 days preoperative, and a single-injection femoral nerve block (SFNB) half an hour before the surgery (ropivacaine 3.3 g/L,30 ml).The LIA group was given local infiltration analgesia with ropivacaine (2.5 g/L,60 ml) and 0.1 mg epinephrine before suture the operative incision. The N-LIA group didn't do the LIA.Both of the two groups didn't use the patient controlled analgesia. The VAS scores, the knee joint range of motion, the muscle strength of quadriceps femoris and the side effects and complications were recorded. Results The VAS scores were lower in LIA group than in the N-LIA group, these scores at 2 h to 48 h after surgery at rest and after 24 h at motion had statistical significance. The range of motion and the muscular strength of quadriceps femoris in the LIA group were better than in the N-LIA group. In the LIA group the use of opioids was less and the side effects were lower. The average length of hospital stay after the operation was shorter in the LIA group than the N-LIA group. Conclusion This multimodal perioperative analgesia protocol that include SFNB and LIA offered improved pain control and minimal side effects to patients undergoing TKA.

This study shows the case of a patient with peroneus brevis tendon rupture in ankle fracture. The patient com-plained of swelling, pain and activity limitation in her right ankle caused by sprain. She was diagnosed with ankle fracture, supina-tion-adduction (Lauge-Hansen typing) by medical history, physical examination and imaging. The peroneus brevis tendon was not-ed complete ruptured when suturing the incision after ORIF, which was then repaired by "8" shaped suture. Based on literature re-view, the injury mechanism of the case may be because of overload inversion force toward to lateral inferior from the fracture of fib-ula on the peroneus brevis tendon. Additional attention should be paid to patients who suffered from an supination-adduction ankle fracture for whether peroneus brevis tendon tears before and during surgery.

Objective:To deeply explore the pain experience of patients with rheumatoid arthritis, so as to provide a basis for the practical interventions in the next step. Methods:Using the phenomenological research method, 18 patients with rheumatoid arthritis who experienced pain in the First Hospital of Soochow University from September 2020 to January 2021 were selected for semi-structured interviews, and the Colaizzi 7-step analysis method was used for data analysis. Results:The pain experience of patients with rheumatoid arthritis were summarized into six themes. Pain was complex and erratic, pain relief strategies were ineffective, pain negatively affected daily life, expected more pain relief, seeking help selectively when pain occurs, and experienced pain brings positive change. Conclusions:Medical staff must pay attention to the real experiences and demands of pain in rheumatoid arthritis patients, use information technology and multidisciplinary collaboration to provide patients with effective pain management strategies and encourage patients to make more positive changes to relieve pain symptoms.

Objective:To explore the feasibility of the tumor-targeted near infrared (NIR) fluorescent dye NIR-T for clearly and accurately delineating incisal edge of breast cancer. Methods:Firstly, the breast cancer xenograft subcutaneous tumor was established in BALB/c nude mice, and dye NIR-T was administrated intratumorally to evaluate the tumor-to-normal tissue (T/N) ratio of the incisal edge of breast tumor. Under the guidance of fluorescence signals, the tumor and peritumoral tissues were resected and collected respectively, and the accuracy of the tumor incisal edge delineated by NIR-T was evaluated by pathological analysis. Studies were further performed on fresh isolated human breast tumor tissues. After intratumoral injection of NIR-T, tumor and peritumoral tissues were collected with the guidance of fluorescence signals, and the accuracy of breast tumor incisal edge delineated by NIR-T was evaluated by pathological analysis. Results:Dye NIR-T was able to clearly delineate the incisal margin of subcutaneous breast tumor in BALB/c nude mice with a T/N ratio of up to 15, and was used to successfully guide the rapid resection of the tumor, and subsequent intraoperative pathological analysis confirmed that the tumor was absolutely removed; NIR-T could be used to clearly delineate the resection margin of breast tumors and its precision was determined to be 100% via pathological analysis. Conclusion:The tumor-targeted NIR fluorescent dye NIR-T was able to clearly and accurately delineate the resection margin of breast tumors, which would promote the precision of tumor resection and shorten the operation time.

BACKGROUND:Zinc-based alloy medical implant materials have excellent mechanical properties, complete degradability and good biocompatibility, and are mainly used in orthopedic implants, cardiovascular stents, bile duct stents, tracheal stents, nerve catheters, etc. OBJECTIVE:To review the research progress of biodegradable zinc-based alloys in bone defect repair and prospect the promising research direction and achievements of zinc-based materials. METHODS:After searching PubMed,Web of Science,WanFang Data, and CNKI databases from the establishment of the database to June 2023,various relevant articles on biodegradable zinc-based alloys for bone implant material research were collected. The basic characteristics of biodegradable zinc based alloys were summarized, and the role of zinc-based alloys in promoting bone tissue repair was sorted and summarized. The current research hotspots and shortcomings were discussed. RESULTS AND CONCLUSION:(1)Zinc-based alloys have good biocompatibility.Using zinc-based alloys as the matrix material, with the help of scaffold structure construction technology and coating optimization process, the bone conductivity of zinc-based alloys will be effectively improved, and their degradation products will have efficient bone induction to regulate the gene expression of osteoblasts and osteoclasts, thereby promoting the repair and reconstruction of bone defects.(2)However,in the research on optimizing zinc-based alloys, the coating process is relatively insufficient, and additive loading technology is still lacking.(3)Zinc-based alloys have excellent mechanical and biological properties.Through special processes, their bone conductivity and osteoinductivity can be increased to effectively improve their ability to promote bone repair and reconstruction, and it is expected to further achieve the development of personalized transplant materials.Further research and development are needed to optimize the integration of coating and additive loading technologies into zinc-based alloys.

A motion unit for sucking robot with a stable motion, convenient operation and process simulation is introduced. The key parameters and process data of the sucking operation were obtained from the clinical work, which provided the basis for the design of the sucking robot motion unit. According to the points of sucking action, robotic thumb, forefinger and metacarpophalangeal joints were used to grip the suction tube, and the servo and arm structure were used to simulate the motion of the wrist and elbow to complete the rotation and push of the sputum suction tube. The feasibility is verified through the advanced sputum suction training model. The movement unit is stable in movement, and can smoothly complete the clamping, feeding, back off protection and rotating tube removal of the sputum suction tube, so as to achieve effective sputum suction.
