2.Preparation of Zhitong sustained-release tablet and its release property in vitro

Objective To prepare sustained-release tablet contained paeoniflorin and evaluate its drug release mechanism.Methods Orthogonal design was used to obtain the best formula.Hydroxypropykmethyl cellulose(HPMC K4M)and ethylcellulose(EC 20CPS)were used as hydrophilic matrix material,and PVP which dissolved in ethanol was used as adhesive material,then the sustained-release tablet was prepared by the wet granule compression technique.The best formula was determined by powder technology,including compression ratio,angle of repose,flow rate,and angle of contact as indexes.The optimum formula was obtained by comprehensive scoring method.The drug release in vitro was determined,and the release formula was made to investigate the release mechanism.Results The optimized prescription was with HPMC 20%,EC 20%,PVP 0%,and ethanol concentration 80%.The dissolution curves in vitro showed that the drug release could be best described by the Higuchi equation(Q=0.367 5 t1/2-0.165 8,r=0.993 2)and Peppas equation(lnQ=0.72 lnt-1.608 6,r=0.991 8).It showed that the release mechanism was Fick diffusion and backbone corrosion.The release rate could meet the requirement of quality control on sustained-release tablet in Chinese Pharmacopoeia(2005).Conclusion The method of content determination is simple and sustained-releasing rate is significant.The release mechanism follows the kinetic model of sustained-release tablet.It is worth doing further research for the clinical use.
3.Anxiolytics and Antidepressants Contribute to Symptomatic Improvement in Patients with Primary Hemifacial Spasm Coupled with Anxiety and Depression

Objective To evaluate the symptom improvement of anxiolytics and antidepressant to hemifacial spasm(HFS)patients with anxiety and depression. Methods A total of 60 HFS patients with anxiety and depression were enrolled for the study and divided into experimental group and control group. Experimental group were treated with escitalopram and BTX-A ,and control group only received BTX-A. SAS and SDS were used to test all the patients before and after treatment. Results Both experimental group and control group were significantly improved after BTX-A therapy. There was no different between two groups at 1 week and 1 month,respectively. At 2 weeks and 3 months,more obvious improvement was seen in experimental group. The SAS and the SDS standard average scores of experimental group were significantly decreased compared with the control group. There was statistical significance in control group after 1 month and 3 months treatment(P<0.05),and in experimental group,the differences appeared after 2-week treatment(P<0.05). Additionally,after 1 month treatment,there were statistical differences between the two groups(P<0.05),and experimental group shown a longer effective period compared with control group. Conclusion Anxiety and depression state serves as a complication of HFS ,which is positively correlated with HFS. HFS patients with anxiety and depression benefit better from escital-opram and BTX-A combination therapy than BTX-A alone.
4.The experimental studies on the effect of methionine deprivation with chemotherapy to human primary gastric cancer cells

Objectives:To study the methionine dependence(Met dependence) of human primary gastric cancer cells in vitro when Met in the culture medium was replaced by its precursor homocysteine(Hcy),and the effect of methionine starvation in combination with chemotherapeutical agents on gastric cancer cells. Methods:Fresh and sterile gastric cancer samples were managed to single cell suspensions and then were cultured in Met -Hcy + and Met +Hcy - medium separately,the proliferation of tumor cells in different culture media was examined by microcytotoxicity(MTT) assay.Meanwhile,the inhibition rate of tumor cells by ADM、DDP、5 FU、MMC and MTX in Met -Hcy +medium was separately tested. Results:①In Met -Hcy + medium,the human primary gastric cancer cell decreased;②Methionine deprivation in combination with chemotherapy enhanced obviously the killing capacity of each chemotherapeutical agent. Conclusions:①Human primary gastric cancer cells in vitro appears Met dependent.②The combined application of Met -Hcy + medium and different chemotherapeutical agents could enhance the antitumor effect of chemotherapy on primary gastric cancer cells.③MTT assay was an efficient way to examine the sensititivity of methionine starvation therapy combined with individualized chemotherapeutical agents.

Methods:Fresh and sterile gaotric cancer samples and normal gastric mucosa were managed into single-cell suspension and were cultured in the Met -Hcy + and Met\++ Hcy\+- medium separately.proliferation of these cells in the two different culture media was examined by cell counter and flow cytometry (FCM). Results:In the Met\++Hcy\+- medium,the growth of primary gastric cancer cells was inhibited,manifesting that the percentage of G 0G 1 tumor cells decreased and that of the S phase increased signiflcantly.However,normal gastric mucosal cells grew well and their cell cycles had no change. Conclusions:Human primary gastric cancer cells are Met dependent in vitro whereas normal gastric mucosal cells have no similar results.

Objective To investigate the role of loss of heterozygosity (LOH) of SMAD2 and its relationship with clinicopathological parameters of primary gastric carcinoma. Methods Fifty cases of primary gastric carcinoma were monitored by polymerase chain reaction -single strand conformation polymorphism (PCR- SSCP) and silver staining to detect SMAD2 LOH. Results The incidence of LOH was 40.0%(22/45) at D18S450 and 33.3%(16/48)at D18S460 . 7.Surgical treatment of gastric cancer in the elderly

Objective To explore the clinical characteristics and surgical treatment of gastric cancer in elderly patients.Methods The clinical data of 431 patients older than 70 years with gastric cancer from 1984 to 1998 were analyzed retrospectively.Results The main clinical manifestations were upper abdominal pain,distension or upset,loss of appetite and weight.The patients with clinicalⅢ,Ⅳ stage accounted for 64.2 percent.Pathological findings showed no specificity.Radical gastrectomy was performed in 215 cases(49.9%),and palliative resection was done in 122 ones(28.3%).The operative mortality and postoperative complication rate were 5.1%and 25.5%in the patients respectively.The 5-year survival rate was 29.5%in the patients with surgery,53.2%with radical gastrectomy and 10.7%with palliative resection.There were significant differences in 5-year survival rate between the radical and palliative groups.Conclusions The majority of elderly patients are clinical Ⅲ,Ⅳ stages and their operative complication rate is high.The key to increase the survival rate and life quality of elderly patients with gastric cancer is early diagnoses,strong perioperative management and suitable radical surgery.
8.Study of the Mechanism of Du Meridian-unblocking and Mind-regulating Acupuncture Pretreatment for Modu- lating miRNA664 and MMP9 in Cerebral Ischemia/reperfusion Rats

Objective To investigate the effect of meridian-unblocking and mind-regulating acupuncture pretreatment on neurobehavioral functions and cerebral water content and explore its mechanism for modulating miRNA664 and MMP9 expressions in cerebral ischemia/reperfusion rats.Methods One hundred and thirty Wistar rats were randomized into group A (electroacupuncture pretreatment) of 30 rats, group B (moxibustion pretreatment) of 30 rats, group C (aspirin pretreatment) of 30 rats, group D (model) of 30 rats and group E (blank control) of 10 rats. Group A received electroacupuncture; group B, suspended moxibustion with moxa sticks; group C, an oral gavage of aspirin 10 mg/kg. A model of cerebral ischemia/reperfusion was made in every group after seven days. Rat neurobehavioral functions were observed and cerebral water content and relative miRNA664 and MMP9 expressions in the cortical region were determined at 24 hours after reperfusion.Results There was a statistically significant difference in the neurobehavioral score between group A, B, C or D and group E (P<0.01), between group A or B and group C (P<0.01) and between groups A and B (P<0.01). There were statistically significant differences in cerebral water content and relative miRNA664 and MMP9 expressions between group A, B, C or D and group E (P<0.01,P<0.05), between group A, B or C and group D (P<0.01), between group A or B and group C (P<0.01,P<0.05) and between groups A and B (P<0.01,P<0.05).Conclusions Meridian-unblocking and mind-regulating acupuncture pretreatment can effectively decrease the neurobehavioral score and cerebral water content and reduce relative MMP9 expression through modulating miRNA664 expression in cerebral ischemia/reperfusion rats. One of the mechanisms of Du meridian-unblocking and mind-regulating acupuncture pretreatment for protecting the brain may be reducing relative MMP9 expression, inducing tolerance to cerebral ischemia and relieving cerebral edema by the modulation of miRNA664 expression.
9.Comparison of the hemostatic effect of total knee arthroplasty in intravenous and intra-articular injection of tranexamic acid

Objective To find a more suitable approach for the application of tranexamic acid(TXA)on total knee arthroplas‐ty (TKA) .Methods Totally 60 patients who met the inclusion criteria from January 2014 to August 2015 in the First Affiliated Hospital of Shihezi University were selected and divided into two groups according to the different route of administration .Group A (n=30) was intravenously injected with 100 mL TXA ,and group B(n=30) was locally injected with 100 mL TXA .Three hours drainage tubes occlusion were carried out after operation in the two groups .The intraoperative and postoperative dominant blood loss ,hidden blood loss indexes and the amount of total blood loss were recorded ,and coagulation indexes and D‐2 polymer were reg‐ularly monitored ,the incidence of thrombosis and postoperative adverse events were also observed .Results The amount of total blood loss in group B[(895 .41 ± 239 .02)mL] was lower than that in group A[(1 020 .89 ± 210 .83)mL] ,and the difference was statistically significant (P<0 .05);the total drainage volume in group B[(294 .33 ± 54 .25) mL] was lower than that in group A [(373 .33 ± 48 .02)mL] ,and the difference was statistically significant (P<0 .05) .After operation ,there was no significant differ‐ence between the two groups in coagulation indexes ,D‐2 polymer and the amount of hidden blood loss (P>0 .05) .No blood trans‐fusion ,symptomatic deep venous thrombosis and fatal pulmonary embolism occurred in the two groups .Conclusion The hemostatic effect of local application of TXA is better than that of intravenous injection in patients′initial TKA .
10.Multi-slice spiral CT in the preoperative stage assessment of gastric carcinoma

Objective To explore the values of CT virtual gastroscopy (CTVG), three dimensional (3D) and axial imaging by multi slice spiral CT (MSCT) in the preoperative assessment of gastric carcinoma. Methods Fifty two patients with gastric carcinoma proven by means of biopsy underwent CTVG, 3D imaging using the surface shaded display (SSD) and Raysum, and axial imaging. MSCT was first performed after the ingestion of gas, and a second tri phase MSCT scan was performed after the ingestion of water. The results were compared with those of fibrous gastroscopy (FG) and surgical pathological findings. Results The detection rates of gastric lesions with CTVG, 3D and axial imaging by MSCT were 98.1%, 96.2%, and 96.2%, respectively. The accuracy of judging gross type of advanced gastric carcinoma using CTVG combined with 3D imaging, FG,and axial imaging was 92.5% , 82.5%, and 75.0%, respectively.Statistical difference existed in CTVG combined with 3D imaging and axial imaging (P
