BackgroundWith the exacerbation of population aging, the number of elderly patients with mental disorders has increased significantly. The high prevalence of pulmonary infection in the aging population has largely contributed to the increased medical burdens and mortality rate, so preventing pulmonary infection in elderly patients with mental disorders has become a critical challenge in clinical practice. www.crd.york.ac.uk/PROSPERO注册号:CRD42024553735 ObjectiveTo investigate the prevalence and influencing factors of pulmonary infection in elderly patients with mental disorders, so as to provide references for preventing the occurrence of pulmonary infection in this population. MethodsOn July 1, 2024, computer searches of CNKI, China Biomedical Literature Database (CBM), Wanfang, PubMed, Web of Science, Embase, Cochrane Library and Ovid were conducted from the inception of each database to June 2024. Two researchers independently conducted literature screening, data extraction and quality assessment. Meta-analysis was performed using Stata 16.0. ResultsA total of 17 literature (14 in Chinese, 3 in English) involving 75 724 elderly patients with mental disorders were included. Meta analysis revealed that the incidence rate of pulmonary infection in elderly patients with mental disorders was 20.8% (95% CI: 0.154~0.263). Subgroup analysis indicated that the incidence rates of pulmonary infection among patients with dementia, schizophrenia, depression and unspecified mental disorders were 21.9% (95% CI: 0.182~0.256), 20.6% (95% CI: 0.129~0.283), 18.4% (95% CI: 0.136~0.232) and 5.2% (95% CI: 0.430~0.062), respectively. In terms of influencing factors, the following were identified as risk factors for pulmonary infection in elderly patients with mental disorders: comorbid diabetes mellitus (OR=1.29, 95% CI: 1.24~1.34), prolonged bed rest (OR=2.41, 95% CI: 2.10~2.76), dysphagia (OR=1.76, 95% CI: 1.53~2.03), smoking history (OR=6.27, 95% CI: 5.97~6.59), irrational use of antibiotics (OR=2.10, 95% CI: 1.79~2.45), hypoalbuminemia (OR=1.57, 95% CI: 1.35~1.83), duration of illness (OR=2.05, 95% CI: 1.79~2.36), age (OR=9.04, 95% CI: 8.44~9.68), length of hospital stay (OR=2.68, 95% CI: 1.65~4.34), use of proton pump inhibitors (OR=1.10, 95% CI: 1.06~1.14), history of chronic lung disease (OR=1.50, 95% CI: 1.43~1.57), poor oral hygiene (OR=3.66, 95% CI: 1.01~13.23), comorbid tumors (OR=3.12, 95% CI: 2.18~4.48), more than two somatic complications (OR=4.01, 95% CI: 1.08~14.86), invasive procedures (OR=3.31, 95% CI: 1.81~6.04) and disorders of consciousness (OR=5.57, 95% CI: 2.18~14.24). ConclusionElderly patients with mental disorders suffer a relatively high prevalence rate of pulmonary infection, and its prevalence rate varies among different types of mental disorders, with the highest rate being observed in patients with dementia. The factors contributing to the development of pulmonary infection are found to include age, duration of illness, smoking history, comorbid somatic complications, dysphagia, disorders of consciousness, hypoalbuminemia, prolonged bed rest, oral hygiene status, irrational use of antibiotics, use of proton pump inhibitor, length of hospital stay and invasive procedures. OBJECTIVES:
To explore the mechanism of Shenqi Buzhong (SQBZ) Formula for alleviating mitochondrial dysfunction in a rat model of chronic obstructive pulmonary disease (COPD) in light of the AMPK/SIRT1/PGC-1α pathway.
METHODS:
Fifty male SD rat models of COPD, established by intratracheal lipopolysaccharide (LPS) instillation, exposure to cigarette smoke, and gavage of Senna leaf infusion, were randomized into 5 groups (n=10) for treatment with saline (model group), SQBZ Formula at low, moderate and high doses (3.08, 6.16 and 12.32 g/kg, respectively), or aminophylline (0.024 g/kg) by gavage for 4 weeks, with another 10 untreated rats as the control group. Pulmonary function of the rats were tested, and pathologies and ultrastructural changes of the lung tissues were examined using HE staining and transmission electron microscopy. The levels of SOD, ATP, MDA, and mitochondrial membrane potential in the lungs were detected using WST-1, colorimetric assay, TBA, and JC-1 methods. Flow cytometry was used to analyze ROS level in the lung tissues, and the protein expression levels of P-AMPKα, AMPKα, SIRTI, and PGC-1α were detected using Western blotting.
RESULTS:
The rat models of COPD showed significantly decreased lung function, severe histopathological injuries of the lungs, decreased pulmonary levels of SOD activity, ATP and mitochondrial membrane potential, increased levels of MDA and ROS, and decreased pulmonary expressions of P-AMPKα, SIRTI, and PGC-1α proteins. All these changes were significantly alleviated by treatment with SQBZ Formula and aminophylline, and the efficacy was comparable between high-dose SQBZ Formula group and aminophylline group.
CONCLUSIONS
SQBZ Formula ameliorates mitochondrial dysfunction in COPD rats possibly by activating the AMPK/SIRT1/PGC-1α pathway.

Objective To compare the effect of three-dimensional visual (3DV) model, three-dimensional printing (3DP) model and computer-aided design (CAD) modified 3DP model in video-assisted thoracoscopic surgery (VATS) sublobular resection. Methods The clinical data of patients who underwent VATS sublobular resection in the Affiliated Hospital of Hebei University from November 2021 to August 2022 were retrospectively analyzed. The patients were divided into 3 groups including a 3DV group, a 3DP group and a CAD-3DP group according to the tools used. The perioperative indexes and subjective evaluation of operators, patients and their families were compared. Results A total of 22 patients were included. There were 5 males and 17 females aged 32-77 (56.95±12.50) years. There were 9 patients in the 3DV group, 6 patients in the 3DP group, and 7 patients in the CAD-3DP group. There was no statistical difference in the operation time, intraoperative blood loss, drainage volume, hospital stay time or postoperative complications among the groups (P>0.05). Based on the subjective evaluations of 4 surgeons, the CAD-3DP group was better than the 3DV group in the preoperative planning efficiency (P=0.025), intuitiveness (P=0.045) and doctor-patient communication difficulty (P=0.034); the CAD-3DP group was also better than the 3DP group in the overall satisfaction (P=0.023), preoperative planning difficulty (P=0.046) and efficiency (P=0.014). Based on the subjective evaluations of patients and their families, the CAD-3DP group was better than the 3DP group in helping understand the vessel around the tumor (P=0.016), surgical procedure (P=0.020), procedure selection (P=0.029), and overall satisfaction (P=0.048); the CAD-3DP group was better than the 3DV group in helping understand the tumor size (P=0.038). Conclusion CAD-modified 3DP model has certain advantages in pre-planning, intraoperative navigation and doctor-patient communication in the VATS sublobectomy.

Chinese Journal of Otorhinolaryngology Head and Neck Surgery 2024;59(8):785-791
Objective:To analyze the clinical characteristics, curative effect related factors and follow-up situation of bilateral sudden sensorineural hearing loss (BSSHL).Methods:The clinical data of 169 patients(338 ears) with BSSHL were retrospectively summarized, and the demographic characteristics, predisposing factors, concomitant symptoms and diseases, and audiological characteristics were statistically described. Additionally, influencing factors of curative effect and prognosis were statistically analyzed. Results:Among the 169 patients, 50.9% (86/169) of patients had at least one incentive, with cold and fatigue being the most common incentives(both 23/169). There were high rates of accompanying symptoms including tinnitus (150/169, 88.8%) and dizziness (100/169, 59.2%). Hypertension(49/169, 29.0%)and diabetes(23/169, 13.6%)were the most common concomitant diseases observed. Most cases exhibited all frequencies involvement, with flat type and total deafness type accounting for 83.1%(281/338 ears). The most common degree of hearing loss was total deafness(86/338 ears), with approximately 60.1%(203/338 ears) of the cases being severe or worse. The total effective rate of treatment was only 29.0%, but it increased to 39.5% for patients with course of disease≤14 days, however, when course of disease>30 days, the effective rate decreased sharply to 3.6%, showing a significant difference between these two groups( χ2=13.776,<0.01). Different types of hearing curves showed statistically significant difference in efficiency( χ2=14.782, P<0.01). Comparing the hearing improvements of 28 BSSHL patients from admission to discharge and from discharge to follow-up, it was found that the hearing improvement of the two groups showed statistically significant difference at the frequencies of L-250 Hz, L-500 Hz, R-125 Hz, R-250 Hz and R-500 Hz( Z value was -2.495, -3.083, -3.970, -3.388 and -3.264 respectively, all P<0.05). The proportion of patients with elevated serum IgE was much higher than that of the normal population. Conclusion:BSSHL patients suffer from serious hearing loss, and many also experience tinnitus and vertigo symptoms. Due to the poor efficiency of treatment, it is better for patients to be treated within 30 days of onset. For patients of hearing loss in the low frequency range, hearing improvement is more significant during hospitalization. And the occurrence of BSSHL may involve an immune mechanism.

Objective To compare the results of accumulation and continuation methods in indoor radon measurement. Methods The radon concentrations in 30 households in 7 provincial capital cities of China were simultaneously measured using both accumulation and continuation methods. Results The radon concentration measured by accumulation method in 30 households ranged from 21 to 323 Bq/m3, with a median M(P25, P75) of 70.5 (43.8, 111). The radon concentration measured by the continuation method ranged from 16.1 to 258 Bq/m³, with a median M(P25, P75) of 100 (51.3, 141). The average relative percent deviation between the two measurement methods was 41.8%. There were significant differences among the measurement results at 8, 16, and 24 h by the continuation method. Pairwise comparisons showed there were statistical diffferences between 8 h and 16 h, as well as between 8 h and 24h; however, no statistical significance was found (P < 0.05), between 16 h and 24 h. Conclusion The overall continuous measurement results of the 30 households were higher than those of cumulation method. The comparison between accumulation and continuation measurement results did not show a high level of correlation (r=0.49). The continuation method is significantly affected by environmental factors, and the length of the measurement period can affect the measurement results. The selection of indoor radon measurement methods should consider the purpose, sample size, and environmental conditions.
