1.Endocardial mapping and ablation of multielectrode basket catheter in right atrial tachycardias

Objective The aim of our study was to examine the utility of a mutielectrode basket catheter (MBC) in the mapping and Ablation of atrial tachycardias (ATs). Methods and Results This study of ATs was conducted in 15 patients. The MBC recording and pacing capabilities, mapping performance and ablation with MBC was evaluated. Stable endocardial electrograms were recorded in 88±4% of electrodes. 64±5% electrode pairs could be used to stimulate the heart. Earliest endocardial activation was recorded from MBC and standard catheters were 41±9 and 45.9±8 m before the onset of P-wave,respectively (P=0.21). Ablation successed in 7 patients (87.5%). Conclusion The MBC allows comprehensive and reliable endocardial mapping during ATs, which facilitates successful ablation in the patients with ATs.
2.Isolation and Purification of Polysaccharide A from Isatidis indigotica and Study on Its Structure

OBJECTIVE: To extract and isolate Isatidis indigotica polysaccharide A(with uniform molecular weight) and to study its structure.METHODS: Hydrosoluble crude polysaccharides were extracted from Isatidis indigotica before being separated and purified by column chromatography to obtain polysaccharide A.The purity and relative molecular weight of polysaccharide A were determined by high performance gel chromatography(HPGPC) and the chemical structure each polysaccharide constituent was studied by UV detection,IR spectral analysis,NMR detection,periodate oxidation and Smith degradation methods.RESULTS: The average molecular weight of polysaccharide A was 11 700.Arabinose and galactose were the major monosaccharide;? was the major configuration of glycosidic bond was predominantly,and 1→3 was the main linkage while 1→4 and/or 1→2 linkages were also presented in polysaccharides A.CONCLUSION:Isatidis indigotica polysaccharides A is a new compound,and its purity remains as high as above 96%.
3.Dose-response relationship between pre-pregnancy body mass index and gestational diabetes mellitus

Objective:
To explore the dose-response relationship between pre-pregnancy body mass index (BMI) and gestational diabetes mellitus (GDM), so as to provide insights into the cut-off values of pre-pregnancy BMI and optimizing GDM prevention and control strategies.
Methods:
Pregnant women that admitted to Zhengzhou Central hospital in 2021 were recruited, and demographics, family history, pregnancy and delivery history and blood glucose levels during pregnancy were collected. The dose-response relationship between pre-pregnancy BMI and GDM was analyzed using restricted cubic spline (RCS) analysis. The predictive ability of pre-pregnancy BMI for GDM risk was evaluated using receiver operating characteristic (ROC) curve.
Results:
A total of 2 279 participants were included in the study. The median age was 29.0 (interquartile range, 5.0) years. The median pre-pregnancy BMI was 21.1 (interquartile range, 3.8) kg/m2. There were 312 underweight women (13.69%), 825 women with low-normal weight (36.20%), 730 women with high-normal weight (32.03%), 345 overweight women (15.14%) and 67 obese women (2.94%).The prevalence of GDM was 17.20%. RCS analysis suggested a linear dose-response relationship between age, pre-pregnancy BMI and GDM (P<0.05). When pre-pregnancy BMI was higher than 21.1 kg/m2, the risk of GDM increased with pre-pregnancy BMI (P<0.05). When women aged over 29.0 years, the risk of GDM increased with age, and the dose-response relationship of GDM caused by pre-pregnancy BMI was stronger in the women aged over 29.0 years than in the women aged 29.0 years and below (P<0.05). The area under curve (AUC) was 0.654 (95%CI: 0.624-0.684). If the cut-off value of pre-pregnancy BMI was 23.0 kg/m2, the Youden index, sensitivity and specificity was 0.238, 0.472 and 0.766, respectively. If it was 24.0 kg/m2, the Youden index, sensitivity and specificity was 0.195, 0.342 and 0.853, respectively. If it was 21.1 kg/m2, the Youden index, sensitivity and specificity was 0.213, 0.676 and 0.537, respectively.
Conclusions
There is a linear dose-response relationship between pre-pregnancy BMI and GDM, and higher than 21.1 kg/m2 of the pre-pregnancy BMI could increase the risk of GDM.
4.Perioperative care of pediatric patients with tethered cord syndrome

Objective To discuss perioperative nursing of pediatric patients with tethered spinal cord syndrome (TCS). Methods 35 pediatric patients with TCS underwent surgical treatment. Pre-operative nursing included psychological care for the parents to eliminate their fear and worries, prona-tion position practice and clean of skin for the children patients, post- operative care included monitor-ing of vital signs, maintaining of proper positions,close observation of the wound, skin care and rehabili-tation training. Results After intensive care, all cases went through the perioperstive period without bed sores. During follow- up disappearance of clinical symptoms occurred in 24 cases, 3 cases got allevia-tion,7 cases with no improvement, and 1 case with deteriorated clinical symptom. Conclusions Preopera-tive care for patients with tethered cord syndrome can provide doctors with the diagnosis and treatment reference, and it plays an important role in treatment and rehabilitation of children patients.
5.Study of pituitary glands in normal adults by high-resolution MRI

Objective ·To study changes of morphological characteristics of pituitary glands Objective · To study changes of morphological characteristics of pituitary glandsin normal adults by high-resolution magnetic resonance imaging (MRI). Methods · A total of 245 healthy adults were scanned by 3.0T MRI to measurewidths,lengths,heights and volumes of pituitary glands and observe pituitary gland morphology. Results · The average height of pituitary was (5.47±1.33) mm inmales and (6.06±1.32) mm in females; the average volume of pituitary was (614.07±125.52) mm3 in males and (660.00±117.35) mm3 in females. The difference betweenthe genders was statistically significant (P<0.01). The pituitary height was positively related to the pituitary volume (r=0.829,P<0.01). Except the pituitary lengths, there was statistically significance in the pituitary widths, heights and volumes among different age groups (P<0.05), and all of them were negatively correlated with age (P<0.01). Conclusion · The pituitary heights and volumes peak in the 18-29 age group in adults and tend to decline with increased age. In female subjects, however, there is a tendency of pituitary heights and volumes to increaseagain in the 50-59 age group. The average height and volume of female pituitary glands are significantly greater than those of males.
6.Effect of Dachengqi decoction on pancreas aquaporin 1 in rats with acute necrotizing pancreatitis

Objective To detect the expression of aquaporin 1 in pancreas of rats with acute necrotizing pancreatitis (ANP) and to study the effect of Dachengqi decoction on it.MethodsOne hundred and sixty male SD rats were randomly divided into control group ( C group,n =32 ),ANP group ( n =32),Dexamethasone group (De group,n =32),Acetazolamide group (A group,n =32) and Dachengqi decoction group (DD group,n =32).ANP model was induced by retrograde injection of 5% sodium taurocholate into the biliary and pancreatic duct.Rats in De group received dexamethasone (4 mg/kg) intravenously after ANP induction; while rats in A group received 1 ml acetazolamide via gastric lavage 2 h before ANP induction; rats in DD group received 2 ml Dachengqi decoction via gastric lavage 48,24,2h before ANP induction; rats in C group received laparotomy.Eight rats in each group were sacrificed at 3 h,6 h,12 h and 18h after induction of ANP models.Quantity of ascites and levels of serum amylases were measured.Pathological changes in pancreas tissue were detected by HE and electron microscope.Capillary permeability in pancreas tissue was detected by Evans Blue (EB) extravasations method.AQP1 expression in pancreas tissue was detected by real-time PCR and Western blotting.ResultsLevels of serum amylase in ANP group was significantly higher,and the pancreatic injuries were obvious ; the levels of serum amylase in De group and DD group was lower than that in ANP group,and the pancreatic injuries were attenuated.The levels of serum amylase in A group were higher than that in ANP group,and the pancreatic.injuries were more severe than that in ANP group.Six hours after ANP induction,the levels of EB in pancreas were (13.44 ±2.56),(126.35 ± 14.80),(86.31 ± 14.46),( 108.99 ± 15.07 ),(78.29 ± 16.85 ) mg/L In C group,ANP group,De group,A group and DD group,and the expression of AQP1 mRNA in pancreatic tissue was ( 170.07 ± 22.48 ) %,( 83.93 ± 8.98 ) %,( 117.09 ±10.70 ) %,( 69.00 ± 8.98 ) %,( 112.82 ± 11.79 ) % ; and the expression of AQP1 protein was 0.23 ± 0.06,0.10 ±0.02,0.32 ±0.03,0.13 ±0.02,0.45 ±0.04.The content of EB in ANP group was higher than that in C group,while the expression of AQP1 mRNA and protein in ANP group was significantly lower than that in C group (P < 0.05 ).The content of EB in De group and DD group was significantly lower than that in ANP group,while the expression of AQP1 mRNA and protein was significantly higher than that in ANP group (P < 0.05).ConclusionsAQP1 plays an important role in the pathogenesis of capillary endothelial barrier dysfunction in rats with ANP.Dachengqi Decoction can attenuate pancreatic injuries of rats by regulating the expression of AQP1.
7.Efficacy Comparison among Three Kinds of Surgical Operation for Type 2 Diabetes Mellitus in Rats

This study was aimed to compare the efficacy of gastric banding (GB), Roux en-Y gastric bypass (RYGBP) and biliopancreatic diversion (BPD) in the treatment of rats with type 2 diabetes mellitus (T2DM). Ani-mal models of T2DM were induced by streptozotocin (STZ) injection and high-sugar-fat diets. A total of 70 T2DM rats were randomly allocated into the GB group (G group, n = 20), RYGBP group (R group, n = 20), BPD group ( B group , n = 20 ) , and the sham operation group ( S group , n = 10 ) . The fasting blood glucose ( BG ) , triglyceride ( TG ) , total cholesterol ( TC ) and insulin ( INS ) content were determined before and 1 , 2 , 3 , 4 , 8 , 16 weeks after operation. The insulin sensitivity index (ISI) was calculated. The mortality and complications were ob-served in each group. The results showed that the fasting weight of the GB group, RYGBP group and BPD group were (324.4 ± 22.5) g, (338.9 ± 17.5) g, (333.3 ± 28.4) g, respectively. The BG content was (12.44 ± 1.29) mmol/L, (9.70 ± 0.81) mmol/L, (11.93 ± 2.39) mmol/L, respectively. The TC content was (2.32 ± 0.45) mmol/L, (2.22 ± 0.79) mmol/L, (2.13 ± 0.31) mmol/L, respectively. The TG content was (1.38 ± 0.32) mmol/L, (1.16± 0.41) mmol/L, (1.23 ± 0.35) mmol/L, respectively. The ISI were (-6.38 ± 0.29), (-6.67 ± 0.24), (-6.65 ±0.23), respectively. And the INS content of the RYGBP group were (69.43 ± 18.73) mU/L. There were signifi-cant differences between before and after operation on the 16th week ( P < 0 . 05 , P < 0 . 01 ) . The mortality rate was 5% in the GB group, 20% in the RYGBP group, and 35% in the BPD group. It was concluded that the GB, RYGBP and BPD are effective in reducing blood glucose and blood lipids in the treatment of rat with T2DM. The treatment effect is obvious in the improvement of insulin resistance ( IR ) .

Objective To explore the values of CT virtual gastroscopy (CTVG), three dimensional (3D) and axial imaging by multi slice spiral CT (MSCT) in the preoperative assessment of gastric carcinoma. Methods Fifty two patients with gastric carcinoma proven by means of biopsy underwent CTVG, 3D imaging using the surface shaded display (SSD) and Raysum, and axial imaging. MSCT was first performed after the ingestion of gas, and a second tri phase MSCT scan was performed after the ingestion of water. The results were compared with those of fibrous gastroscopy (FG) and surgical pathological findings. Results The detection rates of gastric lesions with CTVG, 3D and axial imaging by MSCT were 98.1%, 96.2%, and 96.2%, respectively. 10.Surgical treatment of gastric cancer in the elderly

Objective To explore the clinical characteristics and surgical treatment of gastric cancer in elderly patients.Methods The clinical data of 431 patients older than 70 years with gastric cancer from 1984 to 1998 were analyzed retrospectively.Results The main clinical manifestations were upper abdominal pain,distension or upset,loss of appetite and weight.The patients with clinicalⅢ,Ⅳ stage accounted for 64.2 percent.Pathological findings showed no specificity.Radical gastrectomy was performed in 215 cases(49.9%),and palliative resection was done in 122 ones(28.3%).The operative mortality and postoperative complication rate were 5.1%and 25.5%in the patients respectively.The 5-year survival rate was 29.5%in the patients with surgery,53.2%with radical gastrectomy and 10.7%with palliative resection.There were significant differences in 5-year survival rate between the radical and palliative groups.Conclusions The majority of elderly patients are clinical Ⅲ,Ⅳ stages and their operative complication rate is high.The key to increase the survival rate and life quality of elderly patients with gastric cancer is early diagnoses,strong perioperative management and suitable radical surgery.
