1.Clinical and MRI findings in children with transverse myelitis

Objective To explore the clinical, MRI features and relation of them in children with transverse myelitis. Methods We analyse retrospectively the clinical and MRI features of 10 patients with transverse myelitis. Results 6 patients with prodromal period had lower limbs and quadriplegia. Most of them had dysfunction of superficial sense and sphincter, and the former was light and recovered quickly. The T2WI in all 10 patients shows high intensity, with blurry border. Only 4 patients of them had light augmentation on spinal cord. We found no relationship of the traumatic range or augmentation of spinal cord and the prognosis (P0.05). Conclusion In children with transverse myelitis, the T2WI is the most sensitive, credible and differential non-traumatic examinational procedure. It seems that children patients have different features compared to adults. For children patients, they have slight sensory disturbance and recover quickly. Not all patients show augmentation on spinal cord. It may has the South North regional difference.
2.Antimicrobial resistance analysis on clinically isolated Escherichia coli and detection of class I integrons

Objective To study antimicrobial resistance of clinically isolated Escherichia coli (E.coli),the preva-lence of integrons in E.coli,and relation of integron with antimicrobial resistance of E.coli.Methods E.coli isola-ted from three hospitals of Guangdong Province from 2010 to 2012 were collected,and antimicrobial susceptibility testing was performed by Kirby-Bauer method;integrons were detected by polymerase chain reaction (PCR),and gene cassette was analyzed by sequencing.Results A total of 156 E.coli isolates were collected,antimicrobial sus-ceptibility testing showed that resistance rate of E.coli to most penicillins,cephalosporins,fluoroquinolones,amin-oglycosides and sulfonamides were over 50%;the resistance rate to antimicrobials < 10% included cefoperazone/sulbactam(0),imipenem(3.85%),cefotetan(4.35%),ertapenem(7.69%)and piperacillin /tazobactam (8.97%);The positive rate of class I integron was 57.69%(90/156);the positive rate of class I integron in multidrug-resist-ant and non-multidrug-resistant E.coli was 66.00% (66/100)and 64.71 % (22/34)respectively,the difference was not statistically different (P >0.05),but compared with sensitive E.coli (9.09%,2/22),the difference was statisti-cally different (P<0.01 ).There were nine types of integron-drug resistant gene cassettes in the variable regions, most of which contained aadA and dfrA.Conclusion Antimicrobial resistance of E.coli is serious;high incidence of class I integrons are widely found in E.coli,and is closely related with drug resistance of E.coli,class I integrons mainly mediated aminoglycosides,sulfonamides and beta-lactams resistance.
3.Effect and mechanisms of pioglitazone on post operative cognitive function induced by surgery in elderly rats

Objective To investigate the effect and mechanisms of pioglitazone on post operative cognitive function induced by surgery in elderly rats.Methods Elderly SD rats,male and weighing 220-250 g,were randomly divided into control group (group Con),surgery group (group S),pioglitazone group (group P)and pioglitazone/surgery group (group PS).The level of glycemia was measured by glucometer after completing the surgical model.The expression of advanced glycation endproducts (AGEs)in hippocampal tissues was detected by western blot and reactive oxy-gen species (ROS)and IL-6 by ELISA 1 2 h after surgery.Morris water maze was used for evaluation of cognitive function 4 days after surgery.Results Compared with group Con,group S showed a sig-nificant increase in the expression of AGEs and IL-6 and the level of ROS (P<0.05);Compared with group S,the expression of AGEs and IL-6 and the level of ROS decreased significantly in group PS (P<0.05).Compared to group Con,the surgery increased the average time of escape latencies on the 8th days and 10th days after surgery and reduced the platform-crossing times in the Morris water maze test (P<0.05).Compared with group S,group PS showed a significant decrease in the average time of escaping latencies on the 10th day after surgery (P<0.05),and an increased platform-cross-ing times in the Morris water maze test (P<0.05).Conclusion These results suggest that pioglita-zone attenuates postoperative cognitive function and its mechanism may be related to the decrease of the expression of AGEs and IL-6 and the level of ROS.
4.Role of quadrate lobe hepatectomy in the management of complex iatrogenic high biliary tract injury

Objective:To evaluate partial ventral hepatectomy in the treatment of patients with complicated iatrogenic high bile duct injury.Methods:The clinical data of 8 cases of complicated iatrogenic high bile duct injury treated with the assistance of hepatic ventral segmentectomy from Mar 2013 to May 2020 at Hunan Provincial People's Hospital was retrospectively analyzed.Results:Among the 8 patients, 5 patients underwent partial Ⅳb lobectomy, and 3 patients received partial Ⅳb and Ⅴ segmentectomy of the liver. All the operation was successful without death in hospital. One case developed subphrenic infection and seroperitoneum, which was healed by anti-infection treatment and abdominocentesis. The postoperative follow-up time was 5-90 months, and all of patients are doing well. There was no stenosis in intrahepatic bile duct by postoperative cholangiography or MRI.Conclusions:Quadrate lobe hepatectomy provides a wide view for the treatment of complicated iatrogenic high bile duct injury by fully opening the first porta hepatis and exposing the primary and secondary bile duct branch helping establish a wide patent tension free bile duct-jejunostomy.
