1.Analysis of the Antineoplastics Used in Tumour Ward of Ruijin Hospital

AIM: To investigate the current situation and future trend of use of antineoplastics.METHODS: By using the methods of order of sum of money and order of frequency,the antineoplastics,annually used in Rujin Hospital's tumor ward,were analysed.RESULTS: 30 different kinds of antineoplastics for injection and 12 different kinds of tablets that used to assist the chemotherapy were often used.Some new chemotherapeutants were at the top of the order of sum of money and tablets were often used,too.CONCLUSION: It is important to speed up the development of Chinese domestic chemotherapeutants,cut down the expenses for chemotherapy,steadily improve the rationality and effectiveness of the medicine used.
2.Effects of glucose concentration on differentiation of rat pancreatic duct stem cells

BACKGROUND: Glucose is an important factor on differentiation of pancreatic duct stem cells, it relates to the quantity and secretion function of insulin-producing cells after differentiation.OBJECTIVE: To compare the insulin secretion capacity of the differentiated rat pancreatic stem cells induced by various glucose concentrations.METHODS: Rat stem cells were isolated and purified from pancreatic duct cells using collagenase V and Ficoll-400. These stem cells were randomly divided into 10 groups. Every group was induced to culture, proliferate, differentiate and form insulin- producing cells in vitro. The differentiation of all groups was performed in medium with different concentrations of glucose. The immunofluorescence staining was used to identify the pancreatic duct stem cells. The electrochemical luminescence method was used to detect the insulin release from stem cell differentiated islets.RESULTS AND CONCLUSION: The stimulation index of glucose 20.6, 25.6, 30.6 mmol/L groups was higher than that in other groups (P < 0.05), but there was no difference between each two groups among these three groups (P > 0.05). The insulin releasing of glucose 15.6, 20.6, 25.6 groups was higher than that in other groups (P < 0.05), but there was no difference between each two groups among these three groups (P > 0.05). The best insulin secretion capacity of insulin-producing cells can be gained by controlling concentration of glucose as 20.6-25.6 mmol/L when pancreatic duct stem cells differentiated into insulin-producing cells.
3.Analysis and Optimization of Work Efficiency of Intelligent Dispensing System in Outpatient Pharmacy of Our Hospital

OBJECTIVE:To improve the working efficiency of the drug delivery system. METHODS:The kinds of drugs in delivery system and manual dispensing window were adjusted through comparing the number of prescriptions and drug box dis-pensed by the system and manual window in our hospital before adjustment (Jun. 2014),combining with analysis of EIQ(En-try-item-quantity)-ABC. The improvement effect was evaluated through analyzing related index after adjustment(Aug. 2014). RE-SULTS:The data before adjustment showed the workload of the delivery system was lower than that of manual delivery in term of the number of drug box (P=0.000),and they were similar in term of the number of prescription (P=0.122). The drug delivery system was significantly better than manual delivery in terms of two indexes after resetting the type of drugs in the system(P=0.000). CONCLUSIONS:Optimized delivery system can effectively improve the working efficiency.
4.Nectin-4 expression in pancreatic cancer and its clinical significance

Objective To investigate the expressions and the clinical significance of nectin-4 in pancreatic carcinoma and the relationship with clinicopathological features. Methods Immunohistocbemical techniques were used to detect nectin-4 expression in pancreatic carcinoma tissues (n = 40) and normal pancreatic tissues (n = 12 ), and the relationship between the expressions and clinicopathological parameters was analyzed. Results The IOD and area of nectin-4 were 2. 43 ± 0.75 and 9. 73 ± 1.86 in pancreatic carcinoma tissues, which were significantly higher than those in the normal pancreatic tissues( P < 0.01 ).The expression of nectin-4 was not correlated with patients demographics ( P > 0.05 ), and the protein expression was correlated with histopathologic grade ( P < 0.01 ) and lymph metastasis ( P < 0.05 ).Conclusions The high expression of nectin-4 in pancreatic carcinoma tissues suggests that its high expression may be correlated with the malignant degree of the carcinoma. nectin-4 can be considered as a reference index of differentiation, metastasis and prognosis in pancreatic carcinoma.
5.Prolongation of rat pancreatic islet functions by co-culture with mesenchymal stem cells (MSCs)

ObjectiveTo observe the effect of mesenchymal stem cells (MSCs) on enhancing rat islets viability and function in vitro by a pretransplant co-culture.Methods4-week-old Wistar rats were used as donors, bone mesenchymal stem cells were isolated and subcultured. Islets of Wistar rats were isolated and purified by one-step single-layer Histopaque-1077. Then islets were divided into four groups randomly, 2 groups co-cultured with mesenchymal stem cells (MSCs) (one using low-glucose medium; the other using high-glucose medium ); 2 groups were cultured alone (low-glucose medium; high-glucose medium), each group was further stratified into 3 subgroups(3, 7, 14 d); the survival and functionality of these islets were observed and evaluated. The amount of glucose stimulated secreted insulin were measured wth a rat/mouse insulin enzyme-linked immunosorbent assay (ELISA) kit and stimulation index was also calculated.ResultsCompared with those not co-cultured, islets co-cultured with MSCs demonstrated significantly higher survival rates and viability both in 3th, 7th and 14th day ( P < 0. 01 ); furthermore, cocultured islets revealed higher levels of glucose stimulated insulin secretion and secretion indexes in 7th day (P<0.01).ConclusionRat islet cells co-cultured with MSCs have longer in vitro survival and better functions.
6.Pancreatic stem cells differentiation and allograft for the treatment of diabetes

Objective To investigate the potential of pancreatic stem cells (PSCs) directed differentiation in vitro, and to evaluate the effects of differentiated PSCs allograft on the treatment of diabetes.Methods The PSCs of adult Wistar rats were separated and purified in vitro. The surface of PSCs was determined by immunofluorescence staining, and then it was stimulated by hepatocyte growth factor (HGF) and nicotinamide to induce directed differentiation. Dithizone dyeing was used to determine the islet-like cells after induction, and ELISA staining method was used to detect the insulin levels. Streptozotocin peritoneal injection was used to induce the diabetic rat mode. 40 rats were randomly allocated into pancreatic islet cells allograft group (experiment group) and placebo group. The serum insulin and glucose levels 1 d before transplantation and 1, 2, 3, 4 week after transplantation were measured. Results PSCs of adult Wistar rats were successfully obtained, and the expression of CK19, Pdx-1 and Nestin on cell surface was positive. Dithizone dyeing for directed differentiation cells showed brownish red color. The cells could express and secrete insulin after hyperglycaemia stimulation. The serum insulin and glucose levels 4 week after transplantation were (11.41 ±1.52) mU/L and (8.22 ± 2.7) mmol/L, which were (9.30 ± 1.56) mU/L and (12.23 ± 3.8) mmol/L in the placebo group, and difference was statistically significant (P<0.05). Conclusions PSCs can be induced and directed differentiated in vitro into islet-like clusters with insulin secretion function. And its allograft has the potential for the treatment of diabetes.
7.Risk analysis of intravenous thrombolysis in acute cerebral infarction with cerebral microbleeds by SWI

Objective To investigate the risk factors and the influence of intravenous thrombolysis of acute cerebral infarction with cerebral microbleeds(CMBs)by SWI.Methods 1 64 patients with acute cerebral infarction were enrolled in this study.All pa-tients were scanned with routine MRI and SWI.According to the presence of CMBs on SWI,the patients were classified into two groups:CMBs group(73 cases)and non-CMBs group(91 cases).Past history was recorded and risk factors of CMBs were explored. 76 cases patients(including 35 cases of CMBs group and 41 cases of non-CMBs group)were treated by intravenous thrombolysis and rescanned with routine MRI and SWI to compare the changes in the number of CMBs and hemorrhage transformation 24 hours after thrombolysis.Results The difference age,hypertension,lacunar infarction and leukoaraiosis between the two groups were significant (P <0.05).The difference of CMBs and hemorrhagic transformation between the two groups treated by thrombolysis were not sig-nificant(P >0.05).Conclusion Acute cerebral infarction with CMBs are influenced by age,hypertension,lacunar infarction and leu-koaraiosis.Thrombolysis in acute cerebral infarction with CMBs can not augment the incidence of hemorrhagic transformation.
8.Evaluation of combination therapy with tolterodine and tamsulosin for the treatment of the double J stent-related symptoms

Objective To evaluate the effect of combination therapy of tolterodine and tamsulosin in improving symptoms in patients with indwelling double-J ureteral stents.Methods A total of 96 patients underwent placement of a double-J ureteral stent after retrograde ureteroscopy for urinary stone disease,which were prospectively randomized ( random numbers table) into two groups.The tamsulosin group ( n =48) was administered tamsulosin 0.2 mg once daily,the combination group (n =48) was administcred tamsulosin 0.2 mg once daily and tolterodine 2 mg twice daily.All the patients completed a validated Ureteral Stent Symptom Questionnaire (USSQ),the Overactive Bladder Symptom Score (OABSS),the International Prostate Symptom Score (IPSS) one day before he placement and 4 weeks after stent placement of stent.Results The mean urinary symptom index score (25.4 ± 4.0 vs 15.3 ± 2.9,P < 0.0001 ),the mean pain index score (15.0 ±2.6 vs10.3 ±2.1,P <0.0001),the mean general health index score (15.4 ±2.2 vs 11.0±1.5,P<0.0001),the work performance index score (13.0±2.2vs9.7±1.9,P<0.0001),the sex index score (3.6 ± 1.1 vs 2.2 ± 0.9,P <0.0001 ) improved significantly in combination group than those in tamsulosin group.There was a statistically significant difference between tamsulosin group and combination group in the IPSS score ( 12.8 ± 2.0 vs 9.2 ± 1.7,P < 0.0001 ) and OABSS score ( 6.7 ± 1.4 vs 4.2 ± 1.4,P < 0.0001 ) at the 4 weekfollow-up.Conclusions Indwelling ureteral stents have a significant impact on health related quality of life.It is effective for improving symptoms in patients with indwelling double-J ureteral stents by combination therapy with tolterodine and tamsulosin.
9.Investigation and analysis of chronic prostatitis and sexual dysfunction.

OBJECTIVETo investigate the prevalence of sexual dysfunction in chronic prostatitis patients and its relation with patients' age and the course of disease.
METHODSQuestionnaire surveys were completed by 2 000 subjects who met the criteria for chronic prostatitis after clinical examination and expressed prostatic secretion analysis. Included in the questionnaire were: (1) age, height, weight, occupation, course of disease and treatment received; (2) NIH-CPSI; and (3) IIEF-5.
RESULTSThe prevalence of sexual dysfunction in patients with chronic prostatitis was 49.0% : 26.2% of the subjects with premature ejaculation, 15.0% with erectile dysfunction and 7.7% with both premature ejaculation and erectile dysfunction.
CONCLUSIONThe prevalence of sexual dysfunction in patients with chronic prostatitis is high, which is negatively correlated with both the age of the patients (P < 0.001) and the course of the disease (P < 0.001).

10.Predictive efficacy of magnetic resonance spectroscopy in patients with glioblastoma multiforme during and after postoperative radiotherapy with concomitant/adjuvant temozolomide

Objective To explore the predictive ability of magnetic resonance spectroscopy (MRS) in overall survival (OS) and progression-free survival (PFS) of patients with glioblastoma multiforme (GBM) before,during,and 2 months after radiotherapy with concomitant/adjuvant temozolomide (TMZ).Methods GBM patients,admitted to our hospital from January 2011 to January 2016 and confirmed by pathology,were chosen in our study;all patients underwent postoperative three-dimensional conformal radiotherapy with concomitant/adjuvant TMZ.And 3D-MRS was performed before,during,and 2 months after radiotherapy,the levels of N-acetyl-aspartic acid (NAA),choline (Cho) and creatine (Cr),and ratios of Cho/NAA,Cho/Cr and NAA/Cr in the GBM/edge of surgery side and the normal brain tissues were observed.The survival curve,median overall survival (mOS) and median progression free survival (mPFS) of patients with standardized Cho decreased<30% and patients with standardized Cho decreased>30% 2 months after radiotherapy were compared.Results Twenty-one patients finished the scheduled MRS for 3 times.Until the end of our study,16 patients died and 5 survived.Standardized Cho gradually decreased before,during,and 2 months after radiotherapy (2.08±0.22,1.45 ±0.21 and 1.16±0.18),with significant differences (P<0.05).Standardized Cho after radiotherapy was significantly decreased as compared with that before radiotherapy (P<0.05).Ratios of Cho/NAA and Cho/Cr in the GBM/edge of surgery side were significantly higher than those in the normal brain tissues (P<0.05),and ratio of NAA/Cr in the GBM/edge of surgery side was significantly lower than that in the normal brain tissues (P<0.05).Ratio of Cho/NAA gradually decreased before,during,and 2 months after radiotherapy,with significant differences (P<0.05).As compared with patients with standardized Cho decreased<30% 2 months after radiotherapy,patients with standardized Cho decreased>30% 2 months after radiotherapy had significantly decreased rates of OS and PFS,and statistically shorter mPFS and mOS (4.5 vs.13.5,10.9 vs.25.3,P<0.05).Conclusion The changes of standardized Cho 2 months after radiotherapy have high prognostic significance for PFS and OS.
