1.Distribution and Drug Resistance Analysis of 5508 Pathogens

OBJECTIVE To study the distribution and drug resistance conditions of inpatients in our hospital and make us antibiotics use reasonable.METHODS Retrospective analysis was conducted from all the pathogens of the clinical specimens from 2007-2008 and the data were analysed.RESULTS A total of 5508 pathogens were isolated from 2007-2008.Among 2441 pathogens(44.32%),there were Gram-negative bacilli(Enterobacteriaceae 1715 pathogens,Nonfermenters 707 pathogens,Vibrionaceae 19 pathogens),1154 pathogens(20.95%) were Gram-positive bacilli,1906 pathogens(34.60%) were Fungi.There were 331 pathogens which extended spectrum ?-lactamases(ESBLs) of Enterobacteriaceae(19.30%).The resistance of Escherichia coli to amikacin,ceftazidime,gentamicin,ciprofloxacin,piperacillin resistance rates were 10.94%,60.53%,62.08%,82.01%,86.46%,resistance of Enterobacter cloacae to amikacin,ciprofloxacin,gentamicin,piperacillin,ceftazidime resistance rates were 30.61%,46.29%,52.96%,55.20%,53.05%,resistance of Klebsiella pneumoniae to ciprofloxacin,amikacin,gentamicin,ceftazidime,piperacillin resistance rates were 13.04%,15.90%,16.90%,20.17%,30.07%,The resistance of E.coli,E.coli to ceftazidime,gentamicin,ciprofloxacin,piperacillin were not found were than 60.00%,E.cloacae to gentamicin,piperacillin,ceftazidime not more than 50.00%,K.pneumoniae to ciprofloxacin,amikacin,gentamicin,ceftazidime resistance rates more than 30.00%,not found E.coli,E.cloacae,K.pneumoniae PEI-pei South-resistant strains.The resistance of Pseudomonas aeruginosa to imipenem,amikacin,ciprofloxacin were less than 20.00%,the resistance rate of Acinetobacter baumannii more than 40.00%;of 199 methicillin resistant staphylococcus were not found out vancomycin-resistant S.aureus strains.CONCLUSIONS More of the pathogens show multiple drug resistance.It is urgent for surveillance of bacterial resistance and rational use of anti-microbial agents during the clinical therapy.
2.Investigation and Analysis on the Disposal of Medical Waste in Village

OBJECTIVE To know the status of management knowledge and disposal process of medical waste in village.METHODS Questionnaire was conducted among crowds randomly selected from Huishi,taipin,Gangou and Hepan town in Huining country of GanSu province.RESULTS The understanding rate of management regulation of medical waste among crowd in these four town was 12.31%;The touching opportunity to medical waste was very high;When medical waste was manipulated,people of chooseing to bury accounted for 12.15%,choosing to throw 43.35%,choosing localized manipulate accounted for 4.43%;The phenomenon of throwing medical waste was very serious(85.77%).CONCLUSIONS The understanding rate of management regulation of medical waste are not high,wehereas the touching opportunity was very high in the village.At the same time,the phenomenon of throwing medical waste was very serious and correct manipulating rate are not high.So we should strengthen the education of correlated policy,formulate eligible manipulating method,improve and consummate scientific management system of the medical waste in village.
3.Use of dentritic cells pulsed with HLA-A2-restricted MAGE-A1 peptide to generate cytotoxic T lymphocytes against malignant glioma.

This study developed a novel approach of targeting malignant glioma with pMAGE-A1(278-286)-specific cytotoxic T lymphocytes (CTLs) induced from the peripheral blood mononuclear cells of healthy donors by multiple stimulations with human leukocyte antigen (HLA)-A2-restricted pMAGE-A1(278-286) peptide-pulsed dentritic cells. Cytotoxic assays were performed by the colorimetric CytoTox 96 assay to analyze cytotoxic activity of the induced CTLs against various target cells. The induced CTLs showed approximately 45% specific lysis against T2pMAGE-A1(278-286) (pMAGE-A1(278-286) peptide pulsed T2 cells) and U251 (HLA-A2(+), MAGE-A1(+)) at an effector:target ratio of 40:1, and approximately 5% cytolysis against T2pHIV, A172 (HLA-A2(-), MAGE-A1(+)), K562 and T2 cells without being pulsed with peptide at any effector:target ratio. The specific killing activity of the induced CTLs against T2pMAGE-A1(278-286) and U251 was much more obvious than in any other control group (P<0.05). The cytotoxic activity against the T2pMAGE-A1(278-286) and U251 was significantly eliminated by anti-HLA class I mAb W6/32. These results suggest that pMAGE-A1(278-286) epitope may serve as a surrogate tumor antigen target of specific immunotherapy for treating HLA-A2 patients with malignant glioma.
4.An outbreak of suspected Acinetobacter baumannii infection in a general intensive care unit

Objective To analyze the epidemiological characteristics of a healthcare-associated infection(HAI) outbreak due to multidrug-resistant Acinetobacter baumannii (AB) in an intensive care unit(ICU), and provide basis for the prevention and control of HAI.Methods Epidemiological investigation and environmental hygiene monitoring methods were adopted to investigate patients who had pulmonary infection after they were admitted to the ICU of a hospital in April 1-26, 2015, risk factors were analyzed and corresponding preventive control measures were taken.Results A total of 15 cases of extensively drug-resistant AB(XDR-AB) healthcare-associated lower respiratory tract infection occurred.15 strains of AB were isolated from 15 patients.No.1, 3, 4, 5, No.6, 7, 8, 9, and No.10, 11 patients were detected AB with the same antimicrobial susceptibility testing results respectively;occurrence time of infection was relatively centralized.A total of 43 specimens from environmental surface and hands of health care workers(HCWs) were collected, detection rates of AB from bedside cabinet, monitor button, and hands of HCWs were 100.00%(1/1), 75.00%(6/8), and 66.67%(4/6)respectively.AB were not detected from specimens of treatment table, infusion pump button, and doctors' nasal swabs.By late May 2015, there were no new such cases in ICU.Conclusion This AB HAI may be spread through environmental surface and hands of HCWs in ICU, AB isolated from patients is extensively antimicrobial resistant, active treatment of patients and strict disinfection and isolation measures can effectively control further prevalence.
5.Cardioprotective effects of gradual ischemic postconditioning in percutanous coronary intervention

Objective To observe the protective effect of gradual ischemic postconditioning (IP) capable of improving reperfusion on reperfusion injury in patients with ST-segment elevation acute myocardial infarction (STEAMl) undergoing primary percutaneous coronary intervention (PPCI).Methods 102 in-patients with STEAMI undergoing PPCI in the Department of Cardiology in the First People's Hospital of Kunshan City Affiliated to Jiangsu University from February 2011 to August 2014 were enrolled in this study. They were divided into three groups by a random number table: IP group (32 cases), gradual IP group (30 cases) and routine reperfusion group (40 cases). In IP group, after the opening of the infarction related blood vessel, ischemic postconditioning within the first minute of arterial reperfusion was made through three episodes of 1 minute inflation and 1 minute pressure withdrawn of an angioplasty balloon, and then persistent reperfusion was carried out. In the gradual IP group, the patients received three times of gradual angioplasty balloon inflation and denation, 1 minute/1minute, 30 seconds/30 seconds and 15 seconds/15 seconds respectively, presenting the gradual change of IP time. In the routine reperfusion group, after the opening of blocked blood vessel, the patients underwent routine PCI to persistently recover the coronary artery blood supply. The changes of related lead ST segment regression (Sum-STR), incidence of reperfusion arrhythmia, corrected thrombolysis in myocardial infarction (TIMI) franle count (CTFC), peaks of MB isoenzyme of creatine kinase (CK-MB), left ventricular ejection fraction (LVEF) and frequency of adverse events in follow-up period were compared among the three groups.Results The baseline characteristics were comparable in three groups. The incidence of ventricular premature beats was significantly lower in gradual IP group than that in routine reperfusion group [30.0% (9/30) vs. 55.0% (22/40),P < 0.05], and although theincidence of ventricular premature beats was lower in IP group than that in routine reperfusion group [34.4% (11/32) vs. 55.0% (22/40)], no statistically significant difference was found (P > 0.05). The incidence of ventricular tachycardia was significantly lower in IP and gradual IP groups than that in routine reperfusion group [15.6% (5/32), 13.3% (4/30) vs. 40.0% (16/40), bothP < 0.05]. The incidences of ventricular fibrillation, bradyarrhythmia and sinus arrest were lower in IP group and gradual IP group than those in routine reperfusion group, but no statistically significant differences were found (allP > 0.05). In IP group and gradual IP group, the Sum-STR incidence, CTFC, CK-MB peaks were lower than those of routine reperfusion group [Sum-STR: (56.7±18.3)%, (57.3±21.5)% vs. (44.6±21.6)%; CTFC: 25.47±5.37, 24.46±6.41 vs. 31.62±7.56; CK-MB peaks (U/L): 126.3±78.5, 121.6±82.5 vs. 147.4±72.5; allP < 0.05], the effect of gradual IP group being the best among the three groups. The levels of LVEF were slightly higher in IP and gradual IP groups than the level in routine reperfusion group (0.507±0.042, 0.511±0.062 vs. 0.497±0.062), but no statistically significant difference was found (bothP > 0.05). In routine reperfusion group, one patient died because the ventricular fibrillation could not be corrected and another one died of no-reflow during operation. Each group had 1 patient died during the 4 weeks of follow-up after operation, in the routine reperfusion group, one died of refractory heart failure, and the cause of death of other two patients, one in IP group and another in gradual IP group, was considered due to subacute thrombosis in stent. Major bleeding events were not found in each group.Conclusion Gradual IP can ameliorate myocardial reperfusion injury more significantly in patients with STEAMI undergoing PPCI.
6.Investigation on perioperative antimicrobial prophylaxis in 1 304 cases of thyroid operation in Gansu Province

Objective To investigate perioperative antimicrobial prophylaxis in thyroid operation at all levels of hospitals in a province.Methods From January 1 ,2011 to December 31 ,2013,all kinds of thyroid operation in a province were conducted targeted monitor,investigated results were analyzed.Results A total of 1 304 cases of thy-roid operation were investigated,1 294 (99.23%)were elective operation;duration of operation of 1 051 cases (80.60%)were ≤2 hours;antimicrobial use rate was 96.70%;196 patients (15.45%)were administered 30 mi-nutes before operation,887 patients (70.34%)used antimicrobial agents for >48 hours;19.53% of patients used the first generation cephalosporins and clindamycin.Conclusion There are some problems in antimicrobial use in thyroid operation in this province,such as inappropriate choice of antimicrobial agents,improper administration time,long time use of antimicrobial agents,and use antimicrobial agents without indications.Management of anti-microbial agents should be strengthened,so as to promote the rational use of antimicrobial agents.
7.Possibility of blood transfusion in obstetric surgery and its effect on preoperative blood preparation

Objective Preoperative preparation of blood is common.The preoperative blood ordering and transfusion practices for elective obstetric surgery were evaluated in our Renji hospital in order to develop a reasonable preoperative blood preparation schedule for those procedures where a cross-match appears necessary.Methods We evaluated blood ordering practices in total 16 232 elective obstetric surgeries in our hospital from January 1,2013 to June 30,2017 retrospectively.The age of patients,preoperative diagnosis,surgical procedures type,preparation volume of blood,intra-operative transfusion volume of blood,and excessive usage ratio of blood were calculated and analyzed.Results 455 patients transfused intraoperatively total 1 610 U red blood cells.The usage ratio of blood was 2.80% for patients and 4.87% for blood preparation volumes respectively.Placenta previa,ectopic pregnancy,twin pregnancy accounted for the first three of the possibility of intraoperative blood transfusion in obstetric surgeries.Conclusion The relationship between preoperative diagnosis and the risk of blood transfusion in obstetric surgery should be take attention to.Preoperative blood typing,antibody screening and appropriate cross-matching could ensure the safety of obstetric surgery.

9.Clinical application of 18F-FDG PET/CT imaging in diagnosis of focal organizing pneumonia

Objective:To explore the imaging features of 18F-fluorodeoxyglucose (FDG) PET/CT (high-resolution CT, HRCT) imaging in focal organizing pneumonia (FOP). Methods:Patients with solid nodular FOP ( n=45; 33 males, 12 females, age (58±9) years) and early peripheral non-mucinous solid lung adenocarcinoma ( n=47; 21 males, 26 females, age (63±10) years) confirmed by postsurgical pathology between May 2012 and December 2018 in Zhongshan Hospital, Fudan University were retrospectively analyzed. All patients underwent 18F-FDG PET/CT (HRCT) imaging followed by surgery within 3 weeks. The imaging findings and characteristics of the lesions were recorded. Differences of the maximum standardized uptake value (SUV max) and maximum diameter between FOP and adenocarcinoma were analyzed by Mann-Whitney U test or t′ test. Spearman correlation or Pearson correlation analysis was performed to analyze the relation between SUV max and maximum diameter. Binary logistic regression analysis was performed to identify the predictive factors for FOP. Finally, the receiver operating characteristic (ROC) curve analysis was used for evaluation of diagnostic efficiency. Results:The SUV max of FOP was lower than that of lung adenocarcinoma (3.1(1.7, 4.9) vs 6.5(3.8, 9.3); z=-4.598, P<0.01), and the maximum diameter of FOP was smaller than that of lung adenocarcinoma ((18.0±6.3) mm vs (21.8±4.3) mm; t′=-3.424, P<0.01). The SUV max was positively correlated with the maximum diameter in FOP group ( r s=0.509, P<0.01), while the SUV max of lung adenocarcinoma was not correlated with the lesion size ( r=0.076, P>0.05). HRCT of the PET/CT system showed the fusiform shape were more common in FOP ( χ2=9.549, P<0.05). Multivariate regression analysis identified that SUV max≤7.1, diameter≤18.3 mm, and fusiform shape were independent factors to predict FOP, with odds ratio ( OR) of 10.585, 4.674, 9.073, respectively (all P<0.05). ROC curve analysis showed that the area under the curve (AUC) of SUV max≤7.1 combined with diameter≤18.3 mm and fusiform nodule was 0.860, and the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 97.8%(44/45), 70.2%(33/47), 75.9%(44/58), 97.1%(33/34), and 83.7%(77/92), respectively. Conclusion:SUV max≤ 7.1 combined with maximum diameter≤ 18.3 mm and fusiform shape may predict solid nodular FOP.
10.CK19 mRNA in peripheral blood combined with contrast-enhanced ultrasound: the predictive value of axillary lymph node involvement in breast cancer

Objective To evaluate a model for axillary lymph node involvement combining CK19 mRNA with contrast enhanced ultrasound sonography (CEUS) score in operable breast cancer.Methods Operable breast cancer patients planned for sentinel lymph node (SLN) biopsy were enrolled.Preoperative CK19mRNA expressions in peripheral blood and CEUS score of axillary lymph nodes were tested before surgery.In the training set,postoperative sentinel lymph node (SLN) and non-sentinel lymph node (nSLN) pathological results were taken as the gold standard,effective modeling variables were screened,logistic regression was used to establish the prediction model.Parallel control studies were conducted between the validation set and the MSKCC model to evaluate the prediction accuracy and prediction efficiency.Results From Oct 2015 to Nov 2016,359 cases (training set) were enrolled and mathematical formulas for predicting SLN and nSLN were established,respectively.The sensitivity,specificity and AUC of predicting SLN involvement were 91.36%,94.92% and 0.979 respectively.The sensitivity,specificity and AUC of predicting nSLN metastasis were 91.04%,90.53% and 0.932 respectively.From Dec 2016 to Jul 2017,219 cases (verification set) were included.The sensitivity of SLN metastasis predicted by the model was 91.84%,the specificity was 96.69%,and the AUC was 0.979,significantly superior to the MSKCC model (0.739).The sensitivity,specificity and AUC of predicting nSLN metastasis were 95.35%,92.73% and 0.945 respectively,significantly superior to the MSKCC model (0.873).Concolusions Combined with peripheral blood CK19 mRNA and CEUS score,the prediction model for axillary lymph node involvement for operable breast cancer,SLN/nSLN involvement probability can be calculated and qualitative judgment can be made.The overall accuracy and AUC of this model are better than the prediction model of MSKCC.
