1.Comparison of clinical efficacy and complications of two operation methods in the treatment of breast cancer

Objective To investigate the clinical effects and complications of modified radical mastectomy and breast conserving operation for breast cancer.Methods 110 cases of breast cancer were randomly divided into the two groups,55 patients in each group,the control group took the modified radical mastectomy,while breast conser-ving operation were carried out in observation group,the clinical efficacy and incidence of complications of two groups were observed and compared.Results The operation time,intraoperative blood loss,length of incision and hospitali-zation time of the observation group were (98.47 ±8.65)min,(82.59 ±4.86)mL,(4.80 ±0.87)cm and (8.66 ± 1.77)d,significantly less than (156.52 ±10.23) min,(125.23 ±6.85) mL,(12.25 ±2.03) cm and (15.23 ± 1.85)d in the control group (t=8.12,7.58,9.88,8.72,all P<0.05);3 years survival rate,the rate of distant me-tastasis and local recurrence rate of the two groups had no significant difference (χ2 =1.35,2.03,1.82,all P>0.05);The incidence of complications of the observation group was 3.63%,significantly lower than 18.18% of the control group (χ2 =8.25,P<0.05).Conclusion Breast conserving operation has a significant efficacy for breast cancer,with less trauma,quicker recovery,fewer complications,which is worthy of extensive promotion and application in clinical practice.
2.Application effect of EWS scoring system in resuscitation of emergency department

Objective To use early warning score ( EWS ) to evaluate the prognosis of patients in emergency and resuscitation room and take corresponding measures, and to discuss its application effect in the emergency resuscitation room. Methods Totally 463 emergency critically ill patients admitted in emergency department of Ruijin Hospital from November 2014 to January 2015 were evaluated by EWS, and corresponding measures were taken according to their scores. The relation between EWS score and the time of patients, mortality and prognosis were compared and analyzed. Results Among 463 cases, 26 cases were instant death and accounted for 5.62%. With the increase of EWS score, mortality was elevated, and there were statistically significant differences in mortality of different EWS score period ( P<0. 05 ) , among which mortality was significantly different in cases with different univariate SpO2(P<0.05). Through effective treatment measures, EWS score changes with the change of rescue time, especially 4 hours later, it was the most significant. Improvement rate of patients with EWS score≥6 points was significantly higher than patients with other scores, and the improvement rate was 82.75%; The cure rate of patients with 0 point in EWS score was significantly higher than that of other patients, and the cure rate was 82.61%.Conclusions Patients in emergency resuscitation room were scored by EWS, and compared with APACHE Ⅱscore and GCS score, nurses can collection data easily, and take corresponding measures, which is easy for nurses make early warning and assess patients′disease condition, so as to improve the nursing quality of patients in resuscitation room.
