OBJECTIVE:To establish a method for the simultaneous determination of flavonoids components in Astragali Ra-dix,and to explore the relationship among flavonoids components, varieties, origins and planting patterns. METHODS:HPLC was performed on the column of Venusil ASB with mobile phase of acetonitrile-0.3% formic acid (gradient elution) at a flow rate of 1.0 ml/min,detection wavelength was 260 nm,and column temperature was 25 ℃. Medicinal material quality of Astragalus mem-branaceus (Fisch.) Bge. var. mongholicus (Bge.) Hsiao and A. membranaceus (Fisch.) Bge of wild and cultivated from different province was compared. RESULTS:The linear range of the mass concentration was 0.008 9-2.224 mg/ml for calycosin glucoside (r=0.999 5),0.005 2-1.3 mg/ml for ononin(r=0.999 6),0.002 8-0.697 6 mg/ml for calycosin(r=0.999 9)and 0.002-0.5 mg/ml for formononetin (r=0.999 9);RSDs of precision, stability and reproducibility tests were lower than 1%;recoveries were 99.52%-100.74%(RSD=0.41%,n=6)for calycosin glucoside,98.84%-100.60%(RSD=0.60%,n=6)for ononin ,98.47%-101.74%(RSD=1.08%,n=6)for calycosin,100.10%-101.59%(RSD=0.32%,n=6)for formononetin. In terms of varieties, the contents of calycosin glycosides, ononin and flavonoids in A. membranaceus(Fisch.)Bge. var. mongholicus(Bge.)Hsiao were higher than those of A. membranaceus (Fisch.)Bge,but the contents of calycosin and formononetin were less than those of A. membranaceus (Fisch.)Bge;in terms of origins, calycosin glycosides and flavonoids of Inner Mongolia and Shanxi held the highest contents,fol-lowed by those of Northeast China and Gansu, and lowest in Shandong,Anhui and Shaanxi;in terms of planting patterns, the con-tents of calycosin glycosides, ononin and flavonoids of wild Astragali Radix were higher than those of cultivated varieties, and the contents of calycosin and formononetin of cultivated varieties were higher than those of wild ones. CONCLUSIONS:The method is simple, stable and reproducible, and can be used for the simultaneous determination of flavonoids components in Astragali Radix. The flavonoids components show great differences in Astragali Radix from different origins, and they are affected by varieties,ori-gins and planting patterns.

Objective:To analyze the correlations between the prognosis of patients with pancreatic cancer and macrophage infiltration, and to find the differential gene correlated with macrophage infiltration in patients with pancreatic cancer through bioinformatics. Methods:A total of 32 patients with pancreatic cancer admitted to the First Hospital of Lanzhou University from June 2015 to December 2018 were selected as the research objects, including 19 males and 13 females, with the age of (61.8±2.8) years. Cancer tissues, adjacent tissues, and related clinical data were collected. F4/80 (macrophage marker) immunohistochemical staining was performed on the samples. The survival time was followed up and its correlation with the above indexes was analyzed. The pancreatic cancer data from The Cancer Genome Atlas (TCGA) database was used for bioinformatics analysis. Results:The survival time of pancreatic cancer patients was negatively correlated with degree of macrophage infiltration in cancer tissues ( r=-0.522, P=0.002), but not with adjacent tissues ( r=0.168, P=0.358). The degree of macrophage infiltration in cancer tissue combined with preoperative serum carbohydrate antigen 19-9 (CA19-9), tumor TNM stage and vascular invasion can predict survival up to 47.4% of the survival time ( R2=0.474). TCGA database bioinformatic analysis showed that in pancreatic cancer there were 95 differentially expressed genes significantly correlated with M2 macrophage infiltration, among which JPH3 (positive correlation) and IL17REL (negative correlation) were the main genes. Conclusion:The degree of macrophage infiltration in cancer tissue can be used as a prognostic factor for patients with pancreatic cancer, and the combination with preoperative serum CA19-9, tumor TNM stage and vascular invasion is more accurate in predicting the prognosis. The related mechanism of M2 macrophage infiltration can be studied around the differential genes such as JPH3 and IL17REL.

Objective:To evaluate the role of glutathione S-transferase μ1 (GSTM1) expression in mild hypothermia-induced mitigation of cerebral ischemia-reperfusion (I/R) injury and the relationship with microglial polarization. Methods:Eighty clean-grade healthy male Sprague-Dawley rats, aged 8 weeks, weighing 260-280 g, were divided into 4 groups ( n=20 each) using a random number table method: sham operation group (S group), cerebral I/R group (I/R group), mild hypothermia group (H group), and GSTM1 inhibitor + mild hypothermia group (IH group). The rat model of cerebral I/R injury was prepared using the filament occlusion method. The filament was removed to restore blood flow after the left middle cerebral artery was blocked for 2 h, and the rats′ brain and rectal temperature were maintained at 36-37 ℃ during the period. The vessels were only isolated and ligated without occlusion in S group. In H group, the entire body was wiped with 75% ethanol immediately after removing the filament, and the brain and rectal temperatures were maintained at 32-33 ℃ for 3 h, and the other procedures were the same as those previously described in I/R group. In IH group, GSTM1 inhibitor itaconic acid 8.6 mg/kg was intraperitoneally injected at 24 and 1 h before developing the model, and the other procedures were the same as those previously described in H group. Neurological deficits were evaluated using a modified neurological severity score (mNSS) at 24 h of reperfusion, and then the animals were sacrificed and the brains were removed for observation of cerebral infarction (by TTC staining) and for determination of the expression of GSTM1, M1-type microglial marker inducible nitric oxide synthase (iNOS), and M2-type microglial marker arginase-1 (Arg-1) (by Western blot), expression of GSTM1, iNOS and Arg-1 mRNA (quantitative real-time polymerase chain reaction) and contents of interleukin-6 (IL-6), IL-10, tumor necrosis factor-alpha (TNF-α) and transforming growth factor-beta (TGF-β) (by enzyme-linked immunosorbent assay). Results:Compared with S group, the mNSS and percentage of cerebral infarct size were significantly increased, and the expression of iNOS and Arg-1 protein and mRNA was up-regulated, the expression of GSTM1 and mRNA was down-regulated, and the contents of IL-6, TNF-α, IL-10 and TGF-β were increased in the other three groups ( P<0.05). Compared with I/R group and IH group, the mNSS and percentage of cerebral infarct size were significantly decreased, and the expression of iNOS protein and mRNA was down-regulated, the expression of Arg-1 protein and mRNA and GSTM1 was up-regulated, the contents of TNF-α and IL-6 were decreased, and the contents of TGF-β and IL-10 were increased in H group ( P<0.05). Conclusions:Up-regulated expression of GSTM1 is involved in mild hypothermia-induced mitigation of cerebral I/R injury, which is associated with inhibition of microglial polarization toward the M1 phenotype and promotion of polarization toward the M2 phenotype.

Objective:To develop an area under curve (AUC)-based nomogram to predict vancomycin-associated nephrotoxicity in critically ill patients. Methods:This retrospective cohort study included adult patients treated with vancomycin in the intensive care unit at a tertiary teaching hospital from January 2015 to December 2017. Baseline clinical characteristics before vancomycin treatment and pharmacokinetic parameters were collected to establish a prediction model of nephrotoxicity. Univariate analysis was used to screen variables, and multivariate logistic regression analysis was used to establish the prediction model and nomogram. Results:A total of 159 patients met the inclusion criteria, sixty-four were included in the final analysis. Sixteen patients (25%, 16/64) developed vancomycin-associated nephrotoxicity. The following variables were incorporated into the prediction model: vancomycin AUC, estimated glomerular filtration rate (GFR), and combined nephrotoxic drugs. The following equation was established to calculate the probability of nephrotoxicity: logit (P)=-4.83+0.009×AUC-2.87×1 (if GFR>60 ml/min)+2.53×1 (if number of combined nephrotoxic drugs≥2). A nomogram was generated based on the equation. The receiver-operating characteristic curve demonstrated that the AUC of the prediction model was 0.927 (95% CI 0.851-1.000). The cut-off value of the probability of nephrotoxicity was 26.48%. The sensitivity and specificity were 87.5% and 87.5% respectively. Conclusion:The incidence of vancomycin-associated nephrotoxicity is high. The AUC-based nomogram can effectively predict vancomycin-associated nephrotoxicity in critically ill patients.

OBJECTIVE To explore the grouping efficacy of diagnosis related group (DRG) and the influential factors of hospitalization cost in diabetes cases, and to provide theoretical support for improving DRG payment system, reducing medical cost and enhancing the efficiency of medical insurance funds. METHODS The information of 4 368 diabetic patients who were hospitalized in a 3A hospital in Xi’an from January 1, 2021 to June 30, 2023 was retrospectively analyzed, and DRG grouping of them was summarized; the hospitalization costs of patients in different DRG groups were analyzed by using one way ANOVA and Bonferroni multiple comparison. Coefficient of variation (CV) was used for evaluation within the group, and the influential factors of hospitalization costs were analyzed by one-way linear regression analysis and multi-factor linear regression analysis. RESULTS & CONCLUSIONS The CV values of the four DRG groups were all lower than 0.8, indicating good grouping results and good consistency within the group; the difference of hospitalization cost among the four groups was statistically significant (P<0.05), and the hospitalization cost of China Healthcare Security-DRG version 1.1 FW11 group was significantly higher than those of other three groups (P<0.05). Length of stay, drug cost, the number of other diagnoses, test cost and payment method have significant positive effects on the hospitalization cost of diabetic patients. Whether there is pharmacist intervention has a significant negative influence on the hospitalization cost of patients. Under the DRG payment method, medical institutions can consider multidisciplinary linkage and incorporate a variety of management and service tools, including pharmacist’s intervention, to develop refined management measures, to reduce the economic burden of patients’ families and society.

Cancer is one of the major fatal diseases that seriously threaten human health, and its burden needs to be solved urgently. Health technology assessment (HTA) can provide scientific evidence-based basis for cancer diagnosis, treatment, prevention and related policy formulation. Cost-utility analysis is the gold standard for economic evaluation in HTA, and the accurate measurement of its health utility is one of the key elements to determine the accuracy of its results. This article focuses on systematic introduction of direct measures, multi-attribute health utility scales, and mapping methods in the field of cancer measurement and reviews their applications in cancer patients. Among them, direct measures are complex, costly, and require a high level of subject knowledge; multi-attribute health utility measures are currently the preferred method for measuring health utility in cancer patients; with the continuous development and refinement of disease-specific utility measures in multi-attribute health utility instruments, the mapping method may gradually decrease in future applications. This paper can provide a reference for the selection of health utility measurement tools for HTA in the field of cancer, and provide evidence-based basis for optimizing resource allocation and policy formulation in the field of cancer.
