1.Therapeutic effect and cost of two-year therapy with interferon compared to adefovir in patients with chronic hepatitis B: a comparative study

Objective To compare the efficacy, tolerance and cost of interferon (IFN) α-2b and adefovir (ADV) in patients with chronic hepatitis B (CHB) for two years. Methods The treatmentnaive outpatients with CHB were treated with IFN α-2b or ADV according to intention to treat.Among 77 patients, 34 were treated with recombinant IFN α-2b 5 MU once every other day subcutaneously (IFN group), 43 were treated with ADV 10 mg/day orally (ADV group). The medications were stopped or the regimens were changed due to intolerant adverse reactions or without effects according to intention to treat. The patients were followed up for 24 months. The therapeutic effects, adverse reactions, compliance and cost of two initial treatments were compared. The data were analyzed by Fisher exact probability test. Results The complete virological response (HBV DNA<500 copy/mL) rates after 12 months of therapy in IFN group and ADV group were 41.2% (14/34)and 67. 4 % (29/43), respectively, while the alanine aminotransferase (ALT) normalization rates were41.2% (14/34) and 93. 0% (40/43), respectively. The rates in ADV group were both significantly greater than those in IFN group (both P<0.01). There were no statistically significant differences of HBeAg negative rate and HBeAg seroconversion rate between the two groups. In IFN group, the expulsion rate was 23. 5% (8/34), the therapy was discontinued in 8. 8% (3/34) of patients due to adverse reactions and the medication was changed in 47.1% (16/34) of patients. In ADV group, there were no adverse reactions associated with medication during 2-year therapy and patients were well tolerant, the expulsion rate was 7.0% (3/43) and the regimen in 9.3% (4/43) of patients was changed (P<0.01). The comparison of therapeutic cost between the two groups showed that the cost of anti-viral therapy, management with adverse reactions and laboratory examinations in IFN group were all higher than those in ADV group. The average cost per person of two years was increased with RMB 4855 yuan in IFN group. Conclusion In HBeAg-positive CHB patients, ADV is cost-effective and suitable choice for initial antiviral treatment.
2.Investigation on patients with familial aggregation of hepatitis B in southeast district of Chongqing *

Objective To investigate the incidence of familial hepatitis B viral (HBV ) infection in the southeast part of Chongqing ,and provide information for its prevention and treatment .Methods 295 patients with HbsAg positive came from 110 families were selected and divided to different groups by gender and marital status .We investigated the infections of HBV in three generations of the immediate families .This research focused on the difference between mother-to-child transmission and father-to-child transmission ,the status of receiving HBV vaccine ,the morbidity and mortality of cirrhosis and hepatic carcinoma ,the cause of death ,and the impact of gender in three generations .Results Of the 110 cases ,the HBsAg positive rate for children with HBsAg positive mothers (more than 66 .7% ) was significantly higher than that with positive father (less than 1 .1% ) ,while there was no significant difference between male spouses and female spouses (22 .2% versus 20 .5% ,P>0 .05) .Only 23 .2% received HBV vac-cine and 2 .7% got immune globulin of HBV .8 .5% of the 295 persons had cirrhosis and 4 .4% of them had primary hepatocellular carcinoma (HCC);19 patients from 16 families died of liver diseases .39 cases of 110 patients got antiviral treatment regularly .Con-clusion As well as father-to-child transmission and close contact ,mother-to-child transmission contributed to familial HBV infec-tion ,and even more common .The high rate of familial HBV infection in this area was associated with low rate of vaccination and immune globulin of HBV .The leading cause of the high incidence of hepatocellular carcinoma and hepatic cirrhosis was substandard treatment .
3.The diagnostic value of RTE and APRI for evaluating the degree of liver fibrosis who with chronic liver disease at early stage

Objective To investigate the diagnostic value of the real-time tissue elastography (RTE)and AST/PLT ratio in-dex (APRI)for the assessment of the early stage liver fibrosis in chronic liver disease patients.Methods Totally 90 patients with chronic liver disease were enrolled in the present study.The RTE score and the area percentage of the blue color (AREA,one of the elastic characteristics),the AST and PLT were recorded.Compared the results among different stages of liver fibrosis reported by biopsy considered as the gold standard for the assessment of liver fibrosis.The accuracy of the diagnosis of liver fibrosis by RTE scores,AREA and APRI were compared.Results RTE scores,AREA and APRI increased with the stage of liver fibrosis. Taking ≥S1 as the diagnosis standard of the early stage liver fibrosis,the area under the curve of ROC for RTE scores,AREA and APRI were 0.88,0.92,0.76 (P <0.05).Conclusion RTE is a new and promising sonography-based noninvasive method for the assessment of hepatic fibrosis in patients with chronic liver disease.Combined RTE with APRI have great clinical significance in di-agnosing the early stage liver fibrosis in chronic liver disease patients.
4.Posterior pedicle screw fixation for treatment of fracture and dislocation of atlantoaxial spine

Objective To investigate the clinical effect of pedicle screw fixation in the treatment of fracture and dislocation of atlantoaxial spine via posterior approach .Methods 19 patients with fracture and dislocation of atlantoaxial spine in this hospital from June 2011 to December 2013 were selected and treated with open reduction and pedicle screw fixation via posterior approach . The X‐radiographs were postoperatively re‐examined at regular time for understanding the correction of fracture and dislocation and implant fusion results ,the neurological functions were evaluated according to the Japanese Orthopaedic Association(JOA) scores . Results All cases got bony fusion without the occurrence of internal fixation loosening ,broken screw or broken rod .The JOA score was improved from preoperative (7 .35 ± 2 .39) points to postoperative (13 .21 ± 2 .53) points (P<0 .05) .Conclusion The posteri‐or atlantoaxial pedicle screw fixation and fusion for treating upper cervical spine injury has satisfactory effect .
5.Antibiotic Resistance and Molecular Epidemiology Profile of Staphylococcus aureus in Hospital-acquired Infection

OBJECTIVE To identify the pop strain of Staphylococcus aureus hospital acquired infection by random amplification of polymorphic DNA(RAPD),and to study the molecular mechanism of antibiotic(resistance),so as to reduce the occurrence of drug resistance and infection acquired in hospital.METHODS 1.DNA from 21 strains of S.aureus were extracted by the phenol-chloroform method and analyzed by using arbitrary(primer) polymerase chain reaction(AP-PCR).2.Amplifying mecA,GyrA and GrlA by PCR,and testing the(variation) of these genes by using Hinf Ⅰ-digested analysis.RESULTS Twenty one S.aureus strains were divided into 3(genetic) types.Type Ⅰ is the pop strain in our hospital which including 12 strains.Fourteen from 17 clinical stains were resistant to meticillin and quinolones,of which 13 strains had mecA except isolate 13064.And they all had(variation) in(GyrA) and/or GrlA.CONCLUSIONS RAPD provides markers for the typing of clinical strains and is suitable for(molecular) epidemiologic studies with high type ability,powerful discrimination,simplicity and(rapidness). Type Ⅰ is the pop S.aureus strain in hospital-acquired infection of our hospital.The majority of these strains are multi-(resistant) to meticillin,quinolones and other antibiotics.
6.Molecular Mechanism of Antibiotic Resistance in Hospital Acquired Staphylococcus epidermidis Infection

OBJECTIVE To study the molecular mechanism of antibiotic resistance in hospital acquired(Staphylococcus) epidermidis infection,so as to reduce the occurrence of drug resistance and infection(acquired) in hospital.METHODS DNA from 18 strains of S.epidermidis were extracted by the phenol-chloroform method,and mecA,gyrA and grlA were amplified by PCR,then the variation of gyrA and grlA was tested by Hinf Ⅰ-(digested)(analysis).RESULTS Fifteen from 18 S.epidermidis strains were resistant to meticillin,and all of them had mecA gene. Eleven from 18 S.epidermidis strains were resistant to meticillin,quinolones and other(antibiotics).And they all had a mutant in gyrA and/or grlA.The mutated spots were gyrA Ser84(TCA→TTA) and GrlA Ser80(TCC→TTC).CONCLUSIONS The majority of hospital acquired S.epidermidis strains are multi-resistant to meticillin,quinolones and other antibiotics,which are caused by acquirement of drug-resistance gene or(mutation) of drug-targeting genes.Medical institutions must strictly standardize the application of antibiotics to(reduce)(development) of drug resistance.
7.Effects of MMP14 on CD8+T cell dysfunction in lung cancer patients by regulating mCD100 shedding and sCD100 generation

The study aimed to elucidate the modulatory role of MMP14 on mCD100 shedding and sCD100 production,and its subsequent effects on CD8+T cell dysfunction in lung cancer patients.Total of 56 non-small cell lung cancer(NSCLC)patients were from January 2020 to January 2023 and compared them with 88 healthy controls.Bronchoalveolar lavage fluid(BALF)was obtained from both tumor and non-tumor sites of the patient group.Peripheral blood mononuclear cells(PBMC)were isolated from both groups,and the expression of CD72 and mCD100 in PBMC were assessed via flow cytometry.CD8+T cells from tumor sites were stimulated with recombinant human MMP14 and CD100.Post-cultivation,supernatant levels of TNF-α and IFN-γ were determined by ELISA,while granulysin B and perforin levels were analyzed through an ELISPOT assay.The rate of target cell death was also observed.Data showed no significant difference in the proportion of CD3+mCD100+,CD3+CD72+ cells,and the average fluorescence intensity of CD72 in CD100 and CD3+ monocytes in CD3+CD8+T cells between the patient and control groups.However,as compared with non-tumor sites,these indexes of tumor sites were significantly elevated.Stimulation with CD100 led to increase in IFN-γ,TNF,perforin,and granulozyme B secretion levels in CD8+T cells.After MMP14 stimulation,the proportions of CD3+mCD10 0+ and target cell death,along with sCD100,TNF-α,IFN-γ,and granulozyme B levels in CD8+T cells from NSCLC tumor sites,were notably increased.Interestingly,the addition of anti-CD100 to MMP14-stimulated CD8+T cells resulted in a significant drop in the levels of sCD100,TNF-α,IL-1β,and granulozyme B,as well as in the proportion of target cell death.Taken together,in NSCLC patients,the inhibition of CD100 shedding in CD8+T cells at tumor sites and the blockade of sCD100 production result in impaired CD8+T cell killing function.MMP14 appears to enhance mCD100 shedding and sCD100 production,thereby potentially restoring the cytotoxic function of CD8+T cells against primary NSCLC cells.
8.Surgical treatment of giant cell tumor of bone around the knee: a multicenter retrospective study

Objective To retrospectively analyze treatment status quo of giant cell tumor of bone around the knee in several institutes,and to investigate risk factors affecting selection of surgical manners,tumor recurrence,and functional outcomes.Methods A total of 222 patients with giant cell tumor of bone around the knee confirmed by pathology,who had undergone surgical treatment in 5 institutes from March 2000 to May 2012,were enrolled in this study.There were 120 males and 102 females,with an average age of 35.5 years.The epidemiology,clinical and radiographic features and risk factors affecting selection of surgical manners,tumor recurrence,and functional outcomes were analyzed.Results Intralesional curettage was performed in 128 patients(57.6%),intralesional curettage combined with partial resection in 13 patients (5.8%),and marginal excision in 79 patients (35.5%).A total of 159 patients were followed up.The local recurrence rate was 19.0% for patients treated with intralesional curettage,8.9% for marginal excision and 0% for intralesional curettage combined with partial resection,and the total local recurrence rate was 14.5%(23/159).The mean duration from primary operation to recurrence was 23.9 months.Univariate analysis indicated that surgical manner was the only factor affecting local recurrence.Pathological fracture,Campanacci grades,Enneking system as well as soft tissue mass had a significant influence on the selection of surgical manners.The mean MSTS score after intralesional curettage was obviously higher than that after marginal excision.Conclusion The surgical manner is the only factor affecting local recurrence and limb function.
9.Misdiagnosis of iatrogenic botulinum toxin poisoning:one case report and review of the literature

Objective To report 1 case of iatrogenic botulinum poisoning misdiagnosed as myasthenia gravis,to discuss the clinical manifestation,distinguishing feature and prognosis of iatrogenic botulinum poisoning combined with literature.Methods We briefly introduced one patient with botulinum toxin poisoning misdiagnosed as myasthenia gravis.A 26-year old female was admitted in June 2017 in Gaochun People's Hospital of Nanjing,who received two injections of botulinum toxin 500 U for a short period of time due to cosmetic needs,then she suffered from fatigue,chest distress,eyelid ptosis and other symptoms.The patient was once misdiagnosed as myasthenia gravis,but the results of neostigmine test and repeated nerve stimulation examination were negative.After careful examination of the body (mandibular swelling) and repeated medical history requiring,the final diagnosis was clarified.After the treatment such as fluid infusion for 10 days,her discomforts were relieved.The literature was retrieved from "US National Library of Medicine National Institutes of Health (Pubmed) "," China National Knowledge Infrastructure (CNKI)" and "Wanfang Medicine Data China Information",a total of 17 articles were included in the analysis,including 30 effective cases.We analyzed the general characteristics,clinical manifestations and prognosis of botulinum toxin injection.Results Of the 30 cases,28 cases (96.7%) were female,2 cases (3.0%) were male,aged from 3 to 80 years old.Most of them were botulinum toxin type A,except 2 cases unknown.The clinical onset began from the injection day to 3 weeks after injection.The duration of the disease lasted from 10 days to 6 months.Most of the prognosis was good after treatment.Conclusion Careful attention should be paid to the serious reaction of botulinum toxin to ensure safe medication use.
10.Investigation of Ultrasound-assisted Extraction of Pogostemon cablin (Blanco) Benth and Its GC-FID Determination

Objective: To investigate the ultrasound-assisted extraction process of Pogostemon cablin. Methods: Taking the extraction time (min), ethanol concentration (%), and liquid-solid ratio (mL · g-1) as influence factors, patchouli alcohol (PA) content as index, single-factor test was applied to the investigation of extraction-process of Pogostemon cablin. By ultrasonic-assisted extraction, the GC-FID was used to determine the PA content in extract. Results: The optimal extraction conditions were as follows: the ultrasonic time was 30 min, the ethanol concentration was 95%, and liquid-solid ratio was 20∶1 mL·g-1. Under this condition, the extraction rate and the content of PA were 32.75%, and0.15% respectively. Conclusion: This extraction method is simple, but has higher extraction rate. It is suitable for the industrial production.
