1.Online health education by family physician based on the wearable devices

Objective To explore the effect of online health education by family physician using the wearable devices, and provide the basis in theory and practice to improve the level of primary health care in community. Methods A total of 132 community residents (78 for male and 36 for female, all of them are WeChat user) with hypertension in one residential area who had regularly participated in the activity of volunteer blood pressure measurement organized by community health service center in wujiaochang street from November 2015 to February were chosen as the participants. The residents who wore the wearable device given by the community hospital uniformly were set as the study group (60 people), while the remaining 72 residents were set as the control group. Both groups received the daily online health education provided by the family physicians through their smartphone. We compared the diversity of health cognition, controlling behavior and controlling results to blood pressure of both groups before and after the studyrelatively, then we comprehensively analyzed the effect of use of wearable devices in family physicians ' online health education. SPSS 18.0 was used for data analysis. The categorical data of two groups were compared with chi-square test and the comparison of continuous data was performed with t test. Results Totally 114 community residents were qualified in the study, and the initial backgrounds of study group and control group had no significant difference. After receiving the online health education information for three months, both groups achieved improvement on the cognition and related self-control behavior of hypertension except for several items, and the study group did better than control group in most items. The physical examination showed that the systolic blood pressure of the study group after the intervention [(131.46 ± 12.89) mmHg] (1 mmHg=0.133 kPa) was lower than before [(137.24 ± 12.27) mmHg] and the difference was significant (t=9.2, P<0.01); the diastolic blood pressure of the study group after the intervention [(78.29±8.91) mmHg] was lower than that of before intervention [(80.75±10.25) mmHg] and the difference was significant (t=2.3, P<0.05). Meanwhile, after intervention the systolic blood pressure of study group [(131.46 ± 12.89) mmHg] was lower than that of control group [(133.27 ± 12.7) mmHg] and the intervention was significant (t=2.1, P<0.05). Conclusion Long-term use of wearable devices help family physicians improve the effect of health education, consequently the community hospitals are responsible to enhance the input and management of the informatization of family physicians' service, and assist them to broaden the form and content of health education.
2.Diagnosis and treatment of sacroiliac joint pain with a technique combining intra-and peri-articular injection after lumbar fusion surgery

Objective To investigate the mechanisms of sacroiliac joint pain after lumbar fusion surgery and to present the clinical outcomes after a combining intra-and peri-articular injection.Methods Totally 20 male and 15 female patients (48-75 years old) from January 2013 to December 2016 were retrospectively included in the present study.The patients were all with sustained low back and hip pain after prior posterior lumbar interbody fusion surgery.Nine cases were diagnosed with lumbar disc herniation,22 cases with lumbar stenosis,and 4 cases with degenerative lumbar spondylolisthesis.Ten cases were performed with single level fusion,16 cases with two level fusion,9 cases with 3 or more level fusion.Autogenous iliac bone graft was not applied in any of those patients.The pain of the patients was confirmed from the sacroiliac joint through specific symptoms and signs.They were divided into two groups and were treated with either standard intra-articular injection (17 cases) or a combine of intra-and peri-articular sacroiliac injection (18 cases).Peri-articular injection was conducted at 1 cm above the inferior margin of the sacroiliac joint.Recover ratios of visual analogue scale (VAS) and Oswestry disability index (ODI) at 2 weeks post-operatively were recorded and were compared between the two groups.Results No statistical difference was found in gender,fusion location,fusion levels,pre-operative VAS and ODI score between the two groups (P > 0.05).The combination of intra-and peri-articular sacroiliac injection showed significantly better results than the single intra-articular injection in VAS score immediately after injection (t=2.159,P=0.038),VAS score at 2 weeks after injection and ODI score at 2 weeks after the injection (t=2.705,P=0.011;t=2.156,P=0.039,respectively).Conclusion Both intra-and extra-sacroiliac joint diseases may lead to sacroiliac joint pain after lumbar fusion surgery.A single intra-articular sacroiliac injection could not provide optimistic outcomes.Further extra-articular injection is required at approximate 1 cm above the inferior margin of the sacroiliac joint.The technique combining intra-and peri-articular injection could guarantee improved early clinical outcomes.
3.Spatial-temporal-multicomponent analysis of the characteristic of the incidence of hepatitis E in Zhejiang Province

Objective To analyze the spatial-temporal characteristic of the incidence of hepatitis E (HEV) in Zhejiang Province.Methods Data related to HEV cases in Zhejiang Province in 2015 were collected from the Infectious Disease Reporting Information System in the Information System of Disease Prevention and Control of China.The ArcGIS (10.0) was used to analyze the spatial autocorrelation,R software was used to explore the spatialtemporal-multicomponent of the incidence based on the epidemic component,autoregressive component and endemic component.Results There were 1 738 cases identified and the incidence of HEV in Zhejiang Province in 2015 was 3.16/105.There were obvious regional clustering of the cases (P < 0.05),the northwest regional of Zhejiang had higher incidence of aggregation.The impact of the epidemic component was strong in most of the districts of Hangzhou Municipality,parts of Huzhou and Jiaxing Municipality.The impact of autoregressive component was strong in the main urban area of Hangzhou Municipality,Lin'an and Wenlin.The impact of endemic component was relatively lower and autoregressive component was the same in a whole area.Conclusion The spatial-temporal characteristic of the incidence of HEV in different areas are heterogeneous,it is suggested that the risk factors maybe different and targeted strategies should be taken to control the disease.
4.Spatial analysis and prediction of severe fever with thrombocytopenia syndrome in Zhejiang province, 2011-2015

Objective To understand the distribution of the severe fever with thrombocytopenia syndrome (SFTS) in Zhejiang province,and predict the incidence and the probability of SFTS outbreak.Methods Based on the cases of SFTS from 2011-2015,software ArcGIS 10.0 was used to analyze the spatial distribution,Moran's I and Getis-Ord Gi were used to analyze the spatial autocorrelation.The incidence trend was explored by trend surface analysis,and the prediction was made by Kriging interpolation.Results The incidence of SFTS increased and the distribution expanded in Zhejiang from 2011 to 2015,the seasonal and the demographic characteristics of SFTS were similar to the previous research;there were regional clustering of the cases (P<0.001);a downward trend was observed from northeast to southwest in terms of incidence of SFTS;the second-order disjunctive Kriging interpolation based on circular model and the indicator Kriging interpolation based on exponential model had higher prediction accuracy,the probabilities of outbreak in Anji,Daishan and Tiantai were high,the prediction deviation of inland was less than that of edge area.Conclusion The prediction of SFTS by Kriging interpolation had high accuracy,the incidence of SFTS was higher and the distribution of SFTS was larger than the results of surveillance,the risk areas for epidemic were Anji,Daishan,Ninghai,Tiantai,Sanmen and Linhai.
5.Study on the Application of Named Entity Recognition in Electronic Medical Records for Lymphedema Disease

Purpose/Significance The paper discusses the application of artificial intelligence technology to the key entity recognition ofunstructured text data in the electronic medical records of lymphedema patients.Method/Process It expounds the solution of model fine-tuning training under the background of sample scarcity,a total of 594 patients admitted to the department of lymphatic surgery of Beijing Shijitan Hospital,Capital Medical University are selected as the research objects.The prediction layer of the GlobalPointer model is fine-tuned according to 15 key entity categories labeled by clinicians,nested and non-nested key entities are identified with its glob-al pointer.The accuracy of the experimental results and the feasibility of clinical application are analyzed.Result/Conclusion After fine-tuning,the average accuracy rate,recall rate and Macro_F1 ofthe model are 0.795,0.641 and 0.697,respectively,which lay a foundation for accurate mining of lymphedema EMR data.
