1.Clinical value of right lateral position with real-time monitor for capsule endoscopy

ObjectiveTo investigate the clinical value of right lateral position with real-time monitor for capsule endoscopy.MethodsA total of 80 patients were randomly divided into two groups.The observation group assumed the right lateral position,while the control group was in upright,standing or sitting position.The capsule endoscope was monitored real time.The patients were allowed to move after the capsule endoscope passed pylorus.Gastric transit time,small bowel transit time,small bowel examination completion rate and positive detection rate were compared between the two groups.ResultsThe mean gastric transit time of the observation group was (31.7 ± 29.8)min,which was significantly shorter than that in the control group (62.6 ± 55.9) min ( U =559.000,P =0.020).The mean transit time of small bowel was (221.3 ±78.8) min in the observation group,which was not significantly different from that of the control group ( t =0.511,P =0.611 ).The examination completion rate of whole small bowel was 95.0% (38/40)in the observation group,which was significantly higher than that of the control group (x2 =5.165,P =0.023).The positive detection rates were 75.0% (30/40) and 65.0% (26/40),respectively,which were not significantly different (x2 =0.952,P =0.329).ConclusionThe right lateral position with real-time monitor during capsule endoscopy is of better clinical value.
2.Hyaluronic acid protects tissue engineering cartilage from the inhibitory effect of nitroprusside sodium

Objective To investigate the protective mechanism of hyaluronic acid (HA) antagonistic to nitmprusside sodium (SNP) on the tissue engineering cartilage. Methods Alginate culture for two weeks was used to recover phenotype of dedifferentiated chondrocytes. Differentiation state of chondrecytes was analyzed by immunostaining. The growth of alginate-recovered chondrocytes on the chitosan-based scaffold was observed by scanning electron microscope. After cultured for 3 weeks, this tissue engineering cartilage was treated with SNP in the absence or presence of HA combined with specific β1 integrin blocking antibody collagen type Ⅱ and aggreean were detected by RT-PCR and Western blot. Results Collagen type Ⅱ expression in dedifferentiated chondrocytes was significantly enhanced by alginate bead culture. The chitosan-based scaffold supported cell adhesion, proliferation and migration. A dose-dependent inhibitory effect on the expression of collagen type Ⅱ and aggrecan was observed when tissue engineering cartilage was treated with SNP alone. HA significantly promoted collagen type Ⅱ, and aggrecan expression antagonistic to low concentrations of SNP (p<0.05). However, the specific β1,integrin blocking antibody abrogated the effects of HA. Conclusion Alginate culture recovers the phenotype of dedifferentiated chondrocytes. HA abrogats the inhibitory effect of SNP via β1 integrin signal pathway to protect tissue engineering cartilage.

AIM:To establish HPLC fingerprints of Platycodon grandiflorum in different picking times in Henan province.METHODS:HPLC chromatography condition:Hypersil C_(18) column(250 mm×4.6 mm,5 μm);the mobile phase was acetonotrile with 0.05% phosphoric acid and the gradient elution mode was applied in chromatographic separation;The flow rate was 0.5 mL/min;The temperature of column was 30℃;the detective wavelength was set at 210 nm;RESULTS:The overall similarity in samples collected in autumn was higher than that in spring,and autumn was the appropriate for harvest.CONCLUSION:This method is simple and accurate with a good reproducibility.It provids a reliable scientific basis for the quality control of Platycodon grandiflorum.

AIM: To establish HPLC-fingerprints and quantitatively determine platycodin-D from Platycodon grandiflorum.METHODS: HPLC analysis was carried out on Hypersil C18 column(250 mm ? 4.6 mm,5 ?m),with a mobile phase of acetonitrile-0.05 mol/L phosphoric acid system,gradient elution,with a flow at 0.5 mL/ min,an ultraviolet detection wavelength was at 210 nm for fingerprint and at 206 nm for platycodin-D,column tem-perature at 30 ℃.RESULTS: Twelve common peaks were identified in chromatograms with reference to platycod-in-D peak from the 18 batches of the samples.CONCLUSION: The method of the HPLC-fingerprint and quantita-tive analysis is rapid,simple and accurate with a good reproducibility and can be used for the quality control of Platycodon grandiflorum.
6.Effect of triptolide on apoptosis of nasopharynx cancer

Aim Toexploretheinhibitioneffectof triptolide on nasopharynx cancer, and the mechanism. Methods Theinhibitionofcellproliferationwasde-tected by MTT assay;the cell apoptosis was analyzed by flow cytometry with propidium iodide staining. The ex-pressions of glucose regulated protein 78 ( GRP-78 ) , Akt and pAkt in cells were examined by Western blot;the effect of triptolide on reactive oxygen species ( ROS) accumulation was detected by ROS Fluorescent Probe-DHE.Results MTTassayshowedthatthe growth of nasopharynx cancer was inhibited by triptol-ide , and the inhibition occurred in a dose and time-de-pendent manner following triptolide treatment in CNE-2Z nasopharynx cancer cells. Propidium iodide staining revealed that the apoptosis of CNE-2 Z cells was in-duced remarkably by triptolide. After CNE-2Z cells treated with 25, 50,100 nmol·L-1 of triptolide for 24 h, the apoptosis rate was 14%,26. 9% and 34. 4% re-spectively. Western blot experiment showed that the expression of GRP-78 had no significant change follow-ing triptolide treatment in CNE-2 Z nasopharynx cancer cells for 24 h, but the expression and the phosphoryla-tion level of Akt were strikingly decreased. The experi-ment of ROS uncovered that CNE-2 Z nasopharynx cancer cells increased generation of ROS after treat-ment with triptolide for 4 hours, and acted cells in a dosedependentmanner.Conclusions Triptolidecan inhibit the growth of CNE-2 Z nasopharynx cancer cells in a dose and time-dependent maner. The mechanism may be related with the point that triptolide can induce oxidative stress, incease ROS, inhibit the expression and the phosphorylation level of Akt,then promote the apoptosis of CNE-2Z cells.
7.Identification and antimicrobial susceptibility of 52 pathogenic fungi causing candidemia in Nanchang City of Jiangxi Province

Objective To explore the constitute and antimicrobial susceptibility of pathogenic fungi causing candidemia in Nanchang City of Jiangxi Province.Methods Candida spp.isolated from blood specimens of patients at a hospital in Nanchang in March-October 2015 were collected, fungal strains were identified by amplifying the internal transcribed spacer (ITS) and large ribosomal subunit (D1/D2 region of 26rRNA), antifungal susceptibility of fungi was detected.Results A total of 1 332 positive blood culture specimens were collected, including 74 fungal positive specimens, accounting for 5.56%, 52 strains of Candida spp.were obtained, most were Candida tropicalis (n=17,32.69%),followed by Candida albicans(n=16, 30.77%) and Candida parapsilosis complex (n=16, 30.77%).Identification results of ITS and D1/D2 region were identical.52 strains of Candida spp.were sensitive to both micafungin and caspofungin, epidemiological cutoff value(ECV) of amphotericin B showed that 52 strains were all wild type.Resistance rates of Candida tropicalis to fluconazole and voriconazole were 29.41% and 17.64% respectively, ECV of itraconazole and posaconazole showed that wild type accounted for 82.35% and 94.12% respectively;resistance rates of Candida albicans to fluconazol and voriconazole were 93.75% and 81.25% respectively, ECV of itraconazole and posaconazole showed that wild type accounted for 75.00% and 81.25% respectively;Candida parapsilosis complex strains were sensitive to both fluconazole and voriconazole, ECV of itraconazole and posaconazole showed that all were wild type;all Candida glabrata strains had intermediate resistance rates to fluconazole, ECV of voriconazole, itraconazole, and posaconazole showed that wild type accounted for 66.67%, 100.00%, and 100.00% respectively.Conclusion Candida tropicalis is the most common pathogenic fungus causing candidemia in Nanchang of Jiangxi, followed by Candida albicans and Candida parapsilosis complex.Azole, echinocandin, and amphotericin B are still first-line antifungal agents.
8.Pathogenic detection of infectious diarrhea in patients in a district of Beijing from 2011 to 2013

Objective To analyze the distribution and pulsed-field gel electrophoresis (PFGE)of pathogenic bacteria causing infectious diarrhea in a district of Beijing from 2011 to 2013,and provide basis for tracing infection sources.Methods A total of 1 179 stool specimens of infectious diarrhea from patients in a diarrhea outpatient department from January 2011 to December 2013 were collected,all isolated pathogens were identified by serotyping and PFGE analysis.Results 330 enteric pathogens were isolated from 1 179 specimens,the top 4 bacteria were Shi-gella spp .(28.18%,n=93),Salmonella spp .(20.91 %,n=69),Vibrio parahaemolyticus (13.33%,n =44),and diarrheagenic Escherichia coli (3.33%,n = 11 ).18 Shigella sonnei isolates were identified as 8 PFGE patterns, clustering similarity was close to 88%;69 Salmonella spp .strains belonged to 18 serotypes and 41 PFGE patterns, Salmonella senftenberg and Salmonella enteritidis had dominant patterns;no dominant PFGE patterns were obviously identified among 23 strains ofVibrio parahaemolyticus .Conclusion The serotypes and PFGE patterns of pathogenic bacteria in infectious diarrhea in past three years showed a wide distribution characteristics,the dominant PFGE patterns of Salmonella spp .and Shigella spp .need to be paid more attention,and outbreak of infectious diarrhea caused by Salmonella spp .and Shigella spp .should be alerted.
9.Modulation of Wnt/β-catenin signaling pathway by irbesartan in highglucose-induced tubular epithelial-mesenchymal transition

Aim To investigate the effects of irbesartan on Wnt/β-catenin signaling pathway in tubular epithelial-mesenchymal transition(EMT)in HKCs induced by high-glucose.Methods Human kidney proximal tubular epithelial cell line(HKCs)cultured in vitro was divided into four groups:normal-glucose group,mannitol control group,high-glucose group and high-glucose plus irbesartan group.Immunocytochemistry staining was used to observe the expression of β-catenin;the protein expression of Wnt4,β-catenin,E-cadherin and α-SMA was assessed by Western blot;Wnt4 and β-catenin mRNA were detected by reverse transcription-polymerase chain reaction(RT-PCR).Results Compared with normal-glucose and mannitol control group,both the protein and the mRNA of Wnt4 were up-regulated in HKCs stimulated by high-glucose.α-SMA expression significantly increased but E-cadherin decreased in HG group.The cytoplastic and nuclear fraction of β-catenin enhanced with highglucose stimulation.But no difference of the total protein and mRNA of β-catenin was observed between highglucose-treatment and control groups.Highglucose induced Wnt4 and β-catenin expression in a time-dependent manner,both peaking at 24 h.Irbesartan reduced the promotional effect of HG on Wnt4 and α-SMA expression,and nuclear translocation of β-catenin.HG-mediated inhibition of E-cadherin was also restored by irbesartan.Conclusion These data supported a functional role for Wnt/β-catenin signaling pathway during epithelial-mesenchymal transition of HKCs induced by high glucose and suggested that irbesartan might reverse tubular EMT by regulating activity of Wnt/β-catenin pathway.
10.Custom-made artificial hip replacement for 26 patients with hip tumors

Clinical data of twenty-six patients with primary tumors involving hip ioint treated surgically in Renmin Hospital of Wuhan University between March 1999 and May 2005 were retrospectively analyzed.The diagnosis of all patients was confirmed by pathohistology.There were 3 cases of chondrosarcomas.6 osteosarcomas,1 synovial sarcoma,14 giant cell tumors and 2 aneurysmal bone cysts.Seventeen cases were treated with custom-made total hip replacement prosthesis,7 with custom-made dipolar femoral head prostheses and 2 with saddle prostheses.They were followed up for 18 months to 6 years with an average of 4 years and 3 monts.Local relapse rate and final limb salvage rate was 40%and 60% in 10 patients with malignant bone tumors:125%and 875%in 16 patients with low-grade malignant bone tumors.According to Enneking(MSTS)evaluation criteria.the average score was 19 with an excellent and good rate of 76.9%.Custom-made artificial hip prosthesis is a satisfactory method in limb salvage operations for patients with malignant or low-grade malignant bone tumors jn hip joint.
