1.Evaluation of medical efficiency before and after Sanming healthcare reform

Objective To evaluate the medical efficiency of sample hospitals in Sanming city before and after Sanming health care reform.Methods One tertiary hospital,two general county hospitals and two TCM county hospitals in Sanming were sampled for the study.Two dimensions(the intensity index of medical work and work efficiency index)were selected.In addition,six indexes were identified,namely the number of discharged patients,number of outpatients and emergency diagnostics,doctor′s daily workload per capita for inpatients,bed turnover times,bed utilization rate and average days of stay.Descriptive trend analysis and comprehensive index method were applied in turn to evaluate the longitudinal changes in medical efficiency at the sample hospitals before and after the medical reform.Results The five sample hospitals in Sanming were found with less days of stay in average,and three were found with year-by-year rise of medical efficiency index,with one of them rising from 1.57 to 2.42.Conclusions Sanming′s health care reform has effectively improved internal operational efficiency at the sample hospitals,and has well addressed the relationship of medical service efficiency and scale expansion.On the other hand,the relationship between work intensity and medical efficiency needs to be further balanced,resource allocation to be further optimized,and more priority to be put in the development of TCM hospitals.
2.Hospital Infection Prevalence Rate:Investigation and Analysis

OBJECTIVE To understand the status quo for hospital infection hospital infection to provide a scientific basis. METHODS A cross-sectional survey was taken combined with the hospital bedside investigation and records investigation. RESULTS In 1033 cases,the prevalence rate was 3.87%,and the infected sites were the respiratory site,superficial incision,skin and soft tissue. Utilization rate of anti-bacterial drugs was 60.31%,the pathagen detection rate before treatment was low only 7.73%. CONCLUSIONS Prevalence rate survey method is simple and reliable,it may be the basic reflect of hospital infection. Further strengthening the management of invasive operations,regulateing the rational use of antibiotics status quo,improving the detection rate of pathogens and reduceing preventive medication and antibiotic usage are all evitable.
3.Relationship between matrix attachment region-binding proteins and skin squamous cell carcinoma

Objective To estimate the relationship between matrix attachment region-binding proteins (MARBPs) and skin squamous cell carcinoma (SCC), and to screen for highly-associated MARBPs. Methods Skin specimens were resected from the tumor and tumor-adjacent normal tissue of 12 patients with SCC. Normal skin tissue from the abdomen of 2 of the 12 patients served as the control. Reverse-transcription PCR was performed to detect the mRNA expressions of seven MARBPs, including CUX1, Lamin Bl, Nucleolin, hnRNP U,SAFB2, SATB1 and YBX1, in these specimens and in a human keratinocyte cell line HaCaT and two cell lines Colo-16 and A431 derived from human SCC. Results No evident expression was observed for the seven MARBPs in the normal control specimens, while the MARBPs were expressed in the tumor tissue and tumoradjacent tissue specimens, Colo-16 cells and A431 cells at different degrees and frequencies. In HaCaT cells,all the 7 MARBPs except SATB1 were expressed. Statistical analysis showed a significant difference in the expression of CUX1, Nucleolin and YBX1, but not in that of Lamin Bl, hnRNP U, SAFB2 or SATB1 between the tumor and tumor-adjacent normal tissues. Conclusion Some MARBPs such as CUX1, Nucleolin and YBX1 may participate in the development of skin squamous cell carcinoma.
4.Study of correlation dimension on EEG.

The study of non-linear EEG is of great significance in clinical practice and research work. This paper has gone into the feasibility of calculating the correlation dimension and has developed some subjects with the characters of correlation dimension and the difference under four conditions: (1) passive eyes closed(PEC); (2) mental arithmetic with eyes closed(MAEC); (3) passive eyes open(PEO); (4) mental reasoning with eyes open (MRED). The results show it is feasible and meaningful to calculate correlation dimension and the correlation dimension can reflect the regular patterns of mental activity.

6.Study of Expression of Pneumococcal Surface Protein and Immunogenicity

The specific fragment of Pneumococcal surface protein A(PspA)and Pneumococcal Surface Adhesin A(PsaA)gene was amplified by PCR from Streptococcus pneumonia 5 and Streptococcus pneumonia 19.The amplified fragnent of PspA and PsaA gene was ligated into pET-27b(+)vector and transformed into BL 21 E.coli for expression and obtain the expressive production of PspA and PsaA.Induced by IPTG,the expression level was as high as 75 % of the total disolube protein.The result showed that the recombinant plasmid could express a specific 75 kDa and 37 kDa fusion protein in E.coli BL 21,which showed the good immunogenicity and a broadly cross reactivity with the other serotypes.
7.Early postoperative enteral nutrition compared with parenteral nutrition after hepatectomy:A prospective randomized study

Objective To evaluate the impact of early enteral nutrition (EEN) compared with parenteral nutrition (PN) on patients after hepatectomy. Methods Seventy-eight patients undergoing liver resection were randomized prospectively into two groups: EEN group receiving early enteral nutrition (n=35) and PN group receiving parenteral nutrition (n=43). The patients in both groups received isocaloric and isonitrogenous nutritional formulas 24 h after operation and the formulas were stopped on postoperative day 7. The general conditions, liver function tests, clinical complications,and clinical nutritional variables at three time points that included preoperative phase, postoperative day 1 (POD 1) and postoperative day 8 (POD 8) were observed. Results No significant differences were found in length of hospital stay, liver function and clinical nutritional variables between the 2 groups. In the EEN group, the serum prealbumin level almost returned to the preoperative level on POD 8. The nutritional complication rate of the EEN group was increased significantly but it was milder than that of the PN group. The time of gut function recovery in the EEN group was shorter than that of the PN group. The costs of nutritional drugs showed a significant decrease in the EEN group.Conclusion Early enteral nutrition is safe, rational and effective in patients who have undergone hepatectomy. Early enteral nutrition is better than parenteral nutrition in promoting liver function recovery, liver protein synthesis, postoperative recovery of gut function and decreasing costs of nutritional drugs.
8.The expression of TLR4 in fiver cell treated with cyst fluid of Echinocaccus granulosus

Objective To explore the signal transduction pathway of cyst fluid of Echinococcus granalosus in anti-parasite mechanisms through investigating the effect of cyst fluid on the expression of Toll-like receptor(TLR) in cells. Methods Changes of TLR4 and TGF-β1 expression of 7404 liver cells were detected by quantitative PCR. Results After treatment with increasing concentration cyst fluid the expres-sion of TLR4 was reduced. TGF-β1 expression of liver cells increased with the dose. TLR2 expressions in each group were very low. Conclusion Cyst fluid can increase the expression of TLR4, suggesting that the TLR4 signal transduction pathway involve anti-cyst fluid of Echinococcus granulosus. High concentrations of cyst fluid contribute to TGF-β1 expression which plays a role in immune evasion.

Amniotic lfuid embolism refers to the accessible amniotic lfuid component of maternal circulation leading to a series of severe symptoms, such as pulmonary embolism, shock or even death. Up till now, AFE is a diagnosis of exclusion. The sensitivity and specificity of these methods were poor. With the increasing maternal mortality caused by AFE in forensic medicine, it is important to seeking for more precise diagnosis method. This review intends to provide methodological reference for forensic identification through the research progress of the diagnosis of AFE.
