1.Function and application of exosomes in malignant tumors

Exosome is a sort of vesicle structure attached to cell membrane and released by cells, which contains numerous bioactive molecules, such as proteins, lipids, mRNA, microRNA and DNA fragments. These bioactive molecules are involved in the regulation of many biological processes, not only the participation in the occurrence and development of diverse diseases, but also the close relationship with the invasion and metastasis of tumors. The recent study has indicated that lots of exosomes released from cancer cells participate in a variety of pathological processes as a messenger of cell functions. This article will elaborate the function and application of exosomes in some malignant cancers.
2.Expression of intermedin in lung cancer patients and its clinical significance

Objective To investigate the expression of intermedin (IMD) in plasma and tissues of lung cancer patients compared with control group and to explore the relationship of IMD with the stage and pathological type of lung cancer. Methods The content of IMD in plasma of 88 lung cancer patients measured using ELISA, 36 lung cancer tissue using immunohistochemistry, compared with control groups. Results Healthy control group IMD level [(38.68±12.65) pg/ml] was lower than lung cancer group [(81.61 ± 30.78) pg/ml] (t =-5.818, P <0.05); There was no significant difference of IMD between small cell [(68.61 ± 30.01) pg/ml] and non-small cell lung cancer [(75.51 ±32.74) pg/ml] (t =-0.680, P >0.05); IMD in stage Ⅳ is higher than stage Ⅰ - Ⅲ (t =-3.444, -3.093, -3.955, P <0.05); IMD with distant metastasis is significantly higher than that without distant metastasis (t =8.052, P =0.000). IMD expression in lung cancer tissues [23/36 (63.9 %)] is significantly higher than adjacent tumor tissues [5/21 (23.8 %)] (x2= 8.525, P <0.05). IMD in Stage Ⅲ[14/17(82.4 %)] is significantly higher than in stage Ⅰ [1/5 (20.0 %)] (x2 = 6.924, P =0.009). Conclusion The expression of IMD in lung cancer patients is significantly higher than control groups. Expression has correlation with stage and metastasis, which might play a vital role in the pathogenesis of lung cancer.
3. Relationship between nitric oxide release from pig iliac endothelial cells caused by Exendin-4 and change in intracellular calcium

Objective To study the effects of Exendin-4 (Ex-4) on the function of endothelial cells by investigating the relationship of Ex-4 induced nitric oxide (NO) release from pig iliac endothelial cells (PIEC) and changes in intracellular calcium. Methods Glucagon-like peptide-1 receptor (GLP-1R) was confirmed to be present in PIEC by immunofluorescence assay. Intracellular NO production was probed using DAF-FM DA, and then measured with fluorescence microplate reader at the timepoints of 0, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11 and 12h. The intracellular Ca2+ concentration was probed with Fura-2 AM at 0-30min, 4-4.5h and 8-8.5h and measured with fluorescence microplate reader. The relationship between Ex-4 induced NO release in PIEC and intracellular Ca2+ concentration was evaluated. Results GLP-1R was expressed in PIEC. Ex-4 increased the intracellular NO level in PIEC in a time-dependent manner. NO level was significantly higher 4 hours after Ex-4 treatment compared with that of 0-hour group (P<0.05). The intracellular NO level reached the top value 8 hours after Ex-4 treatment (P<0.05). The intracellular Ca2+ concentrations were significantly higher during the periods of 0-30min, 4-4.5h and 8-8.5h after Ex-4 treatment than those of their control groups (2.042±2.115 vs 0.634±0.352, P<0.01; 0.413±0.154 vs 0.113±0.111, P<0.01; 0.309±0.133 vs 0.063±0.120, P<0.01). During the period of 0-30min after Ex-4 treatment, the intracellular Ca2+ concentration increased gradually (fitted curve: y=0.106x+1.326); while during the periods of 4-4.5h (y=0.003x+0.374) and 8-8.5h (y=-0.001x+0.324) after Ex-4 treatment, the intracellular Ca2+ concentrations, and the NO levels were higher than those of their control groups, but without significant variation. Conclusions Ex-4 may increase the NO release in endothelial cells, and intracellular calcium ions may participate in the process.
4.Effects of Jiangya Capsule Combined with Nimodipine on Depressurization and Quality of Life in Elderly Patients with Isolated Systolic Hypertension

Objective To observe the effect on depressurization and quality of life through Jiangya capsule combined with nimodipine treatment.Methods Randomized,blind and control trial was applied.One hundred and twenty elderly isolated systolic hypertension (EISH) patients were randomly assigned to Jiangya capsule combined with nimodipine group (treatment group) 60 cases (6 cases were lost and 54 cases were in analysis) or nimodipine group (control group) 60 cases (13 cases were lost and 47 cases were in analysis).Treatment group was administrated with Jiangya capsule and nimodipine,and control group was administrated with nimodipine and Jiangya capsule placebo.The course of treatment was 4 weeks.The blood pressure and quality of life of the two groups were observed before and after treatment.Results SBP was obviously decreased in treatment group compared with control group (P0.05).Symptom scores of yin deficiency and yang hyperactivity syndrome in treatment group was improved and superior to the other syndromes in treatment group and control group (P
5.CLINICAL APPLICATION OF THE MEDIAL MULTIPLEX FLAP PEDICLED WITH THE POSTERIOR TIBIAL VESSEL

Objective To investigate the clinical results of the medial multiplex flap pedicled with the posterior tibial vessel. Methods Twelve cases with soft tissue defects and bone defects of limbs were treated with the medial multiplex flap pedicled with the posterior tibial vessel from September 1992 to May 1999. Among them, bone and soft tissue defects following opened fracture in 7 cases, chronic ulcer following chronic osteomyelitis in 2 cases, melanoepithelioma in 2 cases, bone and soft tissue defects following osteoma resection in 1 case. The bone defect area was from 2.5 cm×5.0 cm to 4.5 cm ×11.0 cm. Free graft was performed in 5 cases, bridged transposition in 3 cases and reversal transposition in 4 cases, among them, periosteal myocutaneous flap with autogenous or allogeneic bone grafting in 8 cases, myocutaneous flap in 4 cases. The area of the flaps from 6 cm ×8 cm to 12 cm×25 cm. Results All flaps were healed by first intention, but in the distal fragments of bigger flaps were partially necrosed in 2 cases. In 10 cases bone healing were obtained after 16 weeks of operation according to the X-ray photos. All cases were followed up from 6 to 18 months. All cases achieved satisfactory result but 1 case died because of lung metastasis of osteoma. Conclusion The multiplex graft pedicled with the posterior tibial vessel is an ideal graft for repairing the large soft tissue defects and bone defects, because it has such advantages as adequate blood supply, big vascular diameter, long pedicle and big dermatomic area.
