1.Validation and verification of L3-L5 lumbar three-dimensional finite element model

BACKGROUND:Creation of a precise finite element model is an important basis for the finite element mechanical analysis of the spine. The reports on the precise finite element model are less.
OBJECTIVE:To create L 3-L 5 lumbar three-dimensional finite element model and validate this model with normal CT data.
METHODS:A 39-year-old male healthy volunteer with the height of 175 cm and weighted 65 kg was selected, then the L 3-L 5 lumbar spines were scanned with 16 row spiral CT to obtain 101 CT images with the thickness of 1.25 mm. Solid geometric model was established with Geomagic9.0 software, then determined the unit type,
divided the finite element mesh, and established the finite element model for loading and calculating.
RESULTS AND CONCLUSION:A L 3-L 5 lumbar three-dimensional finite element model was established. It
included 213 736 nodes and 799 779 elements. The ranges of motion of L 3-L 4 and L 4-L 5 segments of the model were consistent with cadaveric biomechanical testing results, verified the effectiveness of the model, so the
model could be used for experimental research.
2.Comparing of dose distribution between intensity-modulated radiotherapy simultaneous integrated boost and conventional radiotherapy for cervical cancer patients

Objective To assess the feasibility of applying intensity-modulated radiotherapy(IMRT)simultaneous integrated boost(SIB)to replace conventional radiotherapy(CR)plus brachytherapy of whole pelvis in locally advanced cervical eaneer(LACC).Methods Five LACC patients based difference position of uterus were chosen and worked out CR and IMRT SIB plans respectively.Dose distributions were compared between IMRT SIB and CR.Results When uterus was in ante-,neutral-,retro-pnsition and deviation respectively,IMRT SIB could provide enough and homogeneous dose distribution for target volume and reduce irradiated volumes and doses for organs at risk(recta,bladder and small intestine)than CR.The doses of the A,B,and fundus of uterus were higher in IMRT SIB than CR.However,in ease of small intestine was close to or encircled the uterus,the targets volume dose would be inadequacy.Conclusions LACC IMRT SIB's dose distribution is better than CR(except excess ante-position)and may help to treat those patients who couldn't be suitable with brachytherapy.
3.Influence on the proliferation of breast cancer with silencing YKL-40 by siRNA interference

Objective:To explore the relationship between YKL-40 and the proliferation of breast cancer and its mechanism.Methods:The expression of YKL-40 in breast cancer MCF-7 cells was detected by immunofluorescence assay.Fluorescence microscope was used to observe the conversion efficiency,and Real-time PCR was used to screen the most effective YKL-40 siRNA.Expression levels of PI3K,P-PI3K,AKT,P-AKT of the PI3K/AKT pathway associated proteins was test by Western blot.At the same time,MTT and flow cytometry were validated by YKL-40 siRNA treatment of human breast cancer MCF-7 ceils,the differences of 24h,48h and 72h groups of cell proliferation ability and cell cycle.Result:MCF-7 cell express YKL-40 protein,mainly located in the cytoplasm.Real-time PCR show that siRNA01,siRNA02,siRNA03 compared with NC group YKL-40 gene silencing effect is remarkable.Among of them the strongest silencing effect is siRNA02 (P<0.01),Western blot show the experimental group than the control group,Total PI3K and AKT remain unchanged while P-PI3K,P-AKT expression decreased (P<0.05).In the experimental group,the number of G1 cells in the control group was increased (P<0.01),while the S phase cells decreased (P<0.01).MTT results showed that the experimental group compared with the control group,the proliferation ability is decreased(P<0.01).Conclusions This study suggests that YKL-40 can be used as the upstream regulatory factor of PI3K/AKT signaling pathway and affect the process of cell cycle in breast cancer,and then regulate the proliferation of breast cancer,YKL-40 may be a crucial target for the treatment of breast cancer.
4.Comparison of the impact of laparoscopic and open surgery on immune function in colon cancer patients

Objective To study the impact of laparoscopic radical resection and open radical resection on immune function in patients with colon cancer. Methods 110 cases received complete mesocolic excision surgery of the right colon cancer patients, according to the different surgical methods divided into observation group and control group, the observation group adopted laparoscopic surgery while the control group treated with open surgery, then compared the inflammatory stress response and related immunology index changes before and after operation. Results After op-eration, inflammatory stress indicators such as MCP-1, HMGB-1, Glucagon, BG in observation group were signifi-cantly better than that in control group. After operation, WBC, CRP, IL-6 index in the two groups were significantly improved. CRP, IL-6, CD4+, CD8+, NK cell index in observation group was significantly better than that in control group. There were significant differences between the two groups, < 0.05. Conclusion Laparoscopic CME surgery can help to relieve inflammatory stress response, and the immune function is less affected, it is worthy of promoting clinical application.
5.The design and implementation of a medical network examination training system

Network examination and learning has become the main form of test and training for nurses, doctors and pharmacists in the field of medicine and health. The system design of Jsp+Servlet+Extjs 4.1+Tomcat server+Mysql database implements the simulation training and learning function of the medical qualification examination by system positioning , functional development and hierarchical construction . Question bank should be developed and optimized according to the examination syllabus and achievement analysis. Test sites, examples, difficulties and so on have been developed into micro courses. With the sup-port of the system, the micro courses and examination questions have been linked together to achieve the organic integration of examination and guidance. This kind of examination system based on network and on-line learning has good compatibility and strong pertinence. It can be perfected according to the application of learning end.
6.Comparative study of tension free vaginal tape and fascia lata for stress urinary incontinence

7.Analysis of dosimetry and clinical outcome using intensity modulated radiation therapy for early breast cancer patients after breast conservative surgery

Objective To analyze the dosimetfic benefits, clinical effect and side-respond of whole breast using intensity modulated radiotherapy for early breast cancer after conservative surgery. Methods From Oct.2004 to Aug. 2005,103 patients received the whole breast intensity modulated radiation therapy (IMRT). A dosimetric comparison of IMRT with conventional radiotherapy (CR) was performed on each patient. The cosmetic results, clinical effect and side-respond were observed. Results The average volume proportion of 95% and 107% prescribed dose was 95.8% ± 4.90% and 84.0% ± 20.7% (t = 9.60, P < 0.01) with IMRT and CR in clinical target volume, respectively. The V20 (lung volume of aceepted> 20 Gy/all lung volume × 100%) of the ipsilateral lung were 15.70% ± 4.64% and 23. 11% ± 7.88% (t = - 13.3, P < 0.01). The V30of the heart were 4.44% ±3.93% and 15.55% ± 10.89%(t = - 11.3, P< 0.01) with IMRT and CR respectively for sixty-three left side breast cancer patients. The 1- and 2-year excellent rate of good cosmetic outcome was both 100%. The 1-, 2- and 3-year local control rate was 99% ,99% and 98% ,respectively. The 1-, 2- and 3-year disease-free survival rate was 99% ,99% and 96% ,respectively. The Grade 1 and 2 acute radiation skin reaction rate was 95.1% and 4.9%, respectively. Conclusion Compared with conventional radiotherapy, IMRT improves dose distribution of CTV and reduce the dose of normal tissue around CTV;but with better clinical effects and lower side-respond for early breast cancer patients after breast conservative surgery.
8.Correlation between metabolic syndrome and uric acid stone composition

Objective To study the relationship between metabolic syndromes and uric acid calculi.Methods We retrospectively analyzed medical records of 640 adult patients,who were diagnosed as renal calculi and finished the stone composition analysis.Among them,394 cases were male and 246 were female.The averagc age was (50.9 ± 12.8) years old.According to the diagnostic criteria of the MS,the patients were divided as metabolic syndrome group (metabolic syndrome group /MS group and non-metabolic syndrome group (non MS group);the MS group had a total of 238 cases including 151 males and 87 females.The average age was (54.1 ± 11.0) years old.There were 402 cases in non MS group,including 243 males and 159 females,with an average age of (49.1 ± 13.4) years old.We obtained corresponding metabolic indices.By comparing the differences between each group,the statistic were analyzed with SPSS.Results In the MS group,42 cases was diagnosed with uric acid stones,and the nonMS group with 8 cases diagnose with uric acid stones.The MS included obesity,elevated blood pressure,elevated fasting plasma glucose,high serum triglycerides.In the patients obtained,123 had the 4 kinds of metabolic diseases at a time,and 23 (18.7%) cases were diagnosed as uric acid stones.115 patients had the 3 kinds of metabolic diseases in which 19 (16.5%) cases were uric acid stones.190 patients had 2 kinds of metabolic diseases and 5(2.6%) cases were uric acid stones.137 patients had only one kinds of metabolic disease in which 3 were uric acid stones.The multivariate Logistic regression analysis showed that,after the correction of gender and age,obesity patients own the OR as 1.10(0.98-1.24)(P =0.018),high blood sugar had the OR as 1.72(1.19-1.92) (P =0.006),high glycerin three fat had the OR as 0.71(0.48-1.09) (P =0.032) and high HDL-C was 0.15(0.03-0.65) (P =0.01).Conclusions The metabolic syndromes including obesity,hyperglycemia and dyslipidemia obviously might influenced the formation of uric acid calculi.
9.Cardiopulmonary Related Index Change of Diffusing the Lung and Promoting Urination on Cor Pulmonale Rats

This study was aimed to diffuse the lung and promote urination for the observation of cardiopulmonary re-lated index change of cor pulmonale rats to further explore the treatment effect on the pulmonary function, pulmonary arterial hypertension (PAH) and the influence of right heart hypertrophy of rats, in order to further illustrate the ef-fect of diffusing the lung and promoting urination for cor pulmonale. Sixty Wistar rats were randomly divided into the control group, model group and Xiao-Qing-Long decoction (XQLD) group with 20 rats in each group. The AniRes2003 animal lung function analysis system was applied to measure the pulmonary function of rats. And the multi-guide physiological recorder was used in the recording of the pulmonary artery pressure of rats. The conven-tional weighing method was applied to calculate and obtain the change of right heart hypertrophy. The results showed that compared to the control group, symptoms in the model group became severe obviously, which include reduced activity, slow movement and occasional airway sputum sound, and the right heart hypertrophy index of the model group increased obviously (P < 0.01). Compared to the model group, the pulmonary function and pulmonary artery pressure of the XQLD group have obvious difference (P< 0.05). It was concluded that to diffuse the lung and pro-mote urination can effectively improve the pulmonary function, PAH and the right heart hypertrophy index of rats with cor pulmonale (fluid retention). The effect of this method is definite in the treatment of cor pulmonale.
10.Protective influence of edaravone on cellular membrane and mitochondria of replanted rat extremities following ischemia/reperfusion injury due to cryopreservation and rewarming

BACKGROUND: A lot of important organs are worthless for clinical application because they are hard to store for a long time. In addition, tissues or organs which are dealt with cryopreservation also attack ischemia/reperfusion injury with the recovery of blood flow; especially, skeletal muscle is the most involved tissue.OBJECTIVE: To observe the protective influence of edaravone on cellular membrane and mitochondria of replanted rat extremities following ischemia/reperfusion injury due to cryopreservation and rewarming.DESIGN: Randomized contrast animal study.SETTING: Basic Medical College of Southern Medical University; Department of Hand and Foot Surgery, Shandong Provincial Hospital.MATERIALS: The experiment was carried out in the Cryopreservation Laboratory, Shandong Provincial Hospital from April to November 2006. A total of 36 healthy adult male Wistar rats were provided by Experimental Animal Center of Medical College of Shandong University. All rats were randomly divided into control group, cryopreservation group and edaravone group with 12 in each group.METHODS: Femoral artery and vein of rats in control group were exposured, but extremities were not blocked. Rats in other two groups were used to establish ischemia/reperfusion injury models of replanted extremities. Before cryopreservation, their right hindlimbs were cut off and maintained in liquid nitrogen container for 1 month. After the operation mentioned above, the broken limbs were rewarmed, perfused with routine eluant and replanted. Four hours later, blood supply of extremities was recirculated and the samples were selected. Eluant in edaravone group contained 0.5 mg/kg edaravone. Samples of skeletal muscle were selected at the same time point to establish cellular membrane and extract mitochondria. Furthermore, fluorescence polarization of cellular membrane (reflecting liquidity in cellular membrane lipid area), malondialdehyde (MDA) content of mitochondria, superoxide dismutase (SOD) activity and respiratory controlling rate were measured; meanwhile, mitochondrial ultrastructure of skeletal muscle was observed under transmission electron microscope.MAIN OUTCOME MEASURES:①Fluorescence polarization of cellular membrane, MDA content of mitochondria, SOD activity and respiratory controlling rate of skeletal muscle; ②mitochondrial ultrastructure of skeletal muscle.RESULTS: All 36 rats were involved in the final analysis without any loss. ①SOD activity and respiratory controlling rate of mitochondria in skeletal muscle: The values of these two items were higher in edaravone group that those in cryopreservation group (P<0.05).②Fluorescence polarization of cellular membrane and MDA content of mitochondria in skeletal muscle: The values of these two items were lower in edaravone group than those in cryopreservation group (P<0.05). ③Mitochondrial ultrastructure of skeletal muscle: Injured degree of skeletal muscle was milder in edaravone group than that in cryopreservation group.CONCLUSION: Edaravone can relieve ischemia/reperfusion injury of skeletal muscle and protect cellular membrane and mitochondria of skeletal muscle. Its mechanism may be related to directly inhibiting hydroxy free radicals, increasing SOD activity of skeletal muscle, reducing generation of MDA and promoting normal oxidative phosphorylation.
