Objective To evaluate and compare the immunopathological changes of lumbar disc herniation and discogenic pain. Methods Seventy-one lumbar disc nucleuses were collected intra-operation,and they were divided into four groups. Group A: 30 cases of disc herniation, Group B: 22 cases of sequestered disc herniation, and the patients in Group A and B received discectomy; Group C: 10 cases of discogenic pain were confirmed by discography, and the painful disc was excised through anterior retroperitoneal approach; Group D: 9 cases of lumbar bust fracture who received anterior subtotal corpectomy and discectomy, and the nearly normal caudal disc was collected as control. The disc nucleuses were processed in the following methods: 1) HE stain and pathological observation; 2) Immunocytochemical test using monoclonal antibodies to CD68 and CD45RO molecule for macrophage and T lymphocytes, respectively. The positive cells were counted and analyzed with statistic method. Results 1) Pathological observation with HE stain: compare to control group, the degeneration of nucleus cell was evident in the other groups. There were obvious infiltration of inflammatory cells and focal neovascularization in group A and especially in Group B.In Group C, only diffuse inflammatory cells was found without neovascularization, but the degeneration of matrix was more severe. 2) Positive rate of CD68 cells: Group B (63.6%)＞Group C (40.0%)＞Group A (26.7%)＞Group D (0%). There were significant differences among groups (P＜0.05). 3) Positive rate of CD45RO cells:Group B (59.1%)＞Group A (13.3%), Group C and D were negative, and the positive cell were found in slice of the same site of positive CD68 cells. The differences between each group were significant (P＜0.05). Conclusion The nucleus of herniated disc has evident inflammatory and autoimmunity reaction. The nucleus of discogenic pain is infiltrated with diffuse inflammatory cells and some macrophages, without T lymphocyte and neovasularization, so the autoimmunity course is not evident.

Objective To investigate the changes of platelet aggregation rate and platelet surface CD 41+CD62P+ expression af-ter clopidogrel discontinuation in the patients with percutaneous coronary interventions (PCI)) .Methods The platelet aggregation rates and platelet surface CD41+CD62P+ in 52 PCI patients with oral clopidogrel for near 12 months and discontinuation soon were measured before clopidogrel therapy(T0 ) ,in 1 week after clopidogrel therapy(T1 ) ,1 week before clopidogrel discontinuation(T2 ) , 1 week(T3 ) and 1 month after clopidogrel discontinuation (T4 ) .Results Compared with T0 ,the platelet aggregation rate and the expression of platelet CD41+CD62P+ at T1 were significantly decreased ,the difference showing statistical significance (P<0 .05) , which at T2 maintained the lower levels ;which at T3 were increased ,which at T4 were recovered to those at T0 .The platelet aggre-gation rates at various time points were (44 .20 ± 18 .36)% ,(25 .38 ± 12 .10)% ,(23 .74 ± 8 .15)% ,(51 .79 ± 10 .55)% and(45 .97 ± 16 .42)% respectively ,and the positive rates of CD41+ CD62P+ were(12 .96 ± 11 .48)% ,(3 .93 ± 3 .33)% ,(4 .72 ± 3 .14)% , (13 .90 ± 10 .38)% and(10 .84 ± 8 .13)% ,respectively .Conclusion In the patients treated with 12-month clopidogrel after PCI ,the platelet aggregation rate and the CD41+CD62P+ positive rate are mildly increased at 1 week after clopidogrel discontinuation and gradually returned to the level before discontinuation at 1 month after clopidogrel discontinuation .

Objective To explore the clinical value of combined detection of SCC and CEA in the diagnosis of lung cancer.Methods 382 cases from January 2015 to April 2016 in the respiratory department of our hospital were selected in this survey.Among which 172 cases with lung cancer,110cases with benign lung diseases and 100 healthy person.The CEA and SCC content in serum of all these cases were determined by chemical chemiluminescence,and the diagnostic efficacy of lung cancer by CEA or SCC alone and CEA and SCC combined examination was compared.Results In lung cancer group,the serum levels of CEA and SCC content were significantly higher than those of the normal control group and lung benign disease group(P＜0.05),and benign lung disease group were significantly higher than that of the normal control group(P＜0.05).and compared with squamous cell carcinoma and small cell lung cancer,CEA has the highest content in adenocarcinoma(P＜0.05).Meanwhile,the content of SCC in squamous cell carcinoma was obviously higher than that of small cell lung cancer and adenocarcinoma(P＜0.05),The sensitivity of CEA was significantly higher than that of SCC(P＜0.05),and the sensitivity and negative predictive value in combined detection group was significantly higher than that of CEA or SCC(P＜0.05).Conclusion SCC and CEA could be used to identify different types of lung cancer,and the combination of the two methods could improve the positive diagnosis rate,facilitate the early detection of lung cancer,early treatment,has the promotion value.

Objective To evaluate the diagnostic value and limitation of MRI for chromophobe renal cell carcinoma.Methods MRI features of 5 cases with pathology proved chromophobe renal cell carcinoma were analyzed retrospectively.Results All tumors showed homogenous isoin-tensity or slightly hypointensity on T1 weighted images and isointensity or slightly hyperintensity on T2 weighted images.Slightly hyperintensity were showed on DWI images,the mean ADC value of tumors was 1.42×10-3 mm2/s.On the contrast enhanced images,all the mass showed slight to moderate enhancement ,1 case had spoke-wheel-like enhancement,1 case showed flowed out blood vessels in the tumor.Conclusion The preoperative diagnosis is difficult for chromophobe renal cell carcinoma,MRI is a valuable method which could provide useful information for qualitative diagnosis.

Objective:To explore the expression of IL-21 and the relationship with adenine nucleotide translocator antibody, ( ANT antibody ) in different courses of viral myocarditis ( VMC ) mice, and to explore the role and significance of IL-21 in VMC.Methods:To induced VMC mice model, BALB/c male mice were peritoneally injected ( IP ) with Coxsackievirus B3, mice peritoneally injected with PBS were taken as the controls.On 0,1,2,3,4 and 6 w after IP,the level of IL-21 mRNA in mice myocardic tissue was determined by real-time RT-PCR.The expression of IL-21 protein and ANT antibody in serum was evaluated by EILSA.Results:Comparing with the controls,the expression of IL-21 mRNA and protein increased from 1st week after IP,reaching the peak on the 2nd week,and maintaining a higher trend until 6th week.All the results at each time except that of 0 week were higher than those of controls(P<0.05).The levels of ANT antibody were also increased from 1st to 6th week,reaching the peak on the 2nd week all the results at each time except that of 0 week were higher than those of controls(P<0.05).Pearson analysis showed that there was rela-tionship between IL-21 and ANT antibody( r=0.88,P<0.01).Conclusion:The datas show that local significantly increased levels of IL-21 in myocardium and serum, and have positive relationship with the level of adenine nucleotide translocator antibody, which indicates that IL-21 may play a role in the pathogenesis of mice virus myocarditis.

Objective To explore the suitable chemotherapy measures in different Clonorchis sinensis infectiosity endemic ar-eas. Methods The interventional groups and control groups were set up in the heavy,moderate and light C. sinensis infectiosity areas in Guangdong Province,respectively. In the intervention groups,the chemotherapy was administered among all the residents aged above 3 years in the heavy endemic areas,the chemotherapy was given among the focus populations in the moderate endemic areas,and the chemotherapy was administered among the residents infected with C. sinensis confirmed by stool examinations in the light endemic areas. No measures were carried out in the control groups. Results One year after the interventions,the C. sinensis infection rates of the interventional groups decreased by 47.90%,86.52%and 100%,the abnormal rates of liver B ultrasonic ex-aminations decreased by 26.50%,31.00%and 100%,the ALT abnormal rates decreased by 48.70%,62.30%and 100%,and the AST abnormal rates decreased by 55.50%,39.90% and 100% in the heavy,moderate and light endemic areas,respectively. There was no decreasing trend of above-mentioned indexes in the control groups. Conclusion In the different C. sinensis infectios-ity endemic areas,the different chemotherapy measures should be carried out.

Objective To express the N protein of middle east respiratory syndrome coronavirus ( MERS-CoV) in prokaryotic expression system and evaluate the immunogenicity of the purified recombinant N protein.Methods The gene encoding N protein of MERS-CoV was synthesized and cloned into the vector pET30a to construct the recombinant expression plasmid pET30a-MERS-CoV-N.The transformed E.coli BL21 ( DE3) strains carrying expression plasmid were induced by IPTG to express N protein.The expressed protein was purified by using affinity chromatography.SDS-PAGE and Western blot assays were used to iden-tify the expressed N protein and evaluate its immunogenicity.Results The recombinant N protein was suc-cessfully expressed in soluble form with the size of 46×103 .The results of Western blot assay showed that the recombinant N protein could specifically react with rabbit serum samples positive for antibodies against N protein B-cell epitope peptide and mouse anti-His tag antibodies.No positive band against N protein was found when primary antibody was used with thirty adult serum samples from Beijing in 2008.Conclusion N protein of MERS-CoV was successfully expressed in prokaryotic expression system.The successful expres-sion of N protein laid the foundation for immunological diagnosis of N protein of MERS-CoV and researches on its immunogenicity.

OBJECTIVETo investigate the clinical effects and operative options for the treatment of Forestier disease.

METHODSFrom June 2005 to May 2012, 8 patients with progressive dysphagia due to Forestier disease were treated through anterior approach, their clinical data were retrospective analyzed. There were 6 males and 2 females, aged from 65 to 83 years old with an average of 73 years. Among the patients, osteophytes removal was performed in 3 cases, osteophytes removal with discectomy and fusion was performed in 2 cases, osteophytes removal with corpectomy and fusion was performed in 3 cases. According to Bazaz dysphagia score to assess the improvement of the patients' symptoms before and after operation.

RESULTSAll patients were followed up from 12 to 40 months with the mean of 18.5 months. Seven cases were asymptomatic and 1 case had mild symptom in the last follow-up. Radiographs showed the space enlargement between vertebral body and trachea.

CONCLUSIONIt is effective to treat patients with progressive dysphagia due to Forestier disease through surgical method. And the operative options depend on the stability of cervical spine and the neurological symptoms of the patients.

Objective To analyze and explore the clinical features,pathological characters,treatment and prognosis of rectum carcinoid.Methods From January 1997 to January 2010,69 pathologically diagnosed rectum carcinoid cases were collected from Union Hospital of Tongji Medical College,Huazhong University of Science and Technology and other 7 hospitals.The clinical features,pathological characters,treatment and prognosis were analyzed. Results Of 69 rectum carconid cases,there were 36 males and 33 females.The average age was (49.3±12.9) years.The common symptom were hemotochezia,abdominal pain,constipation,diarrhoea and abdominal distension.There were 55 cases with lesions from the anus less than 8 cm and in other patients,lesions from the anus were all more than 8 cm.66 cases were typical carcinoid,3 cases were atypical carcinoid.40 cases underwent the immunohistochemical staining.The common markers for immunohistochemical staining were Syn,CgA and NSE,the positive percentage were 90％ (36/40),75％ (30/40) and 82.5％(33/40) respectively.A total of 24 patients received endoscopic therapy,44 patients had surgery,1 patient who refused surgery received only life support and symptomatic treatment.Conclusion No specific clinical symptoms of rectum carcinoid,and most were typical carcinoid.Lesions confined to mucosa and submucosa could be considered the endoscopic therapy.
