Objective:To analyze the analgesic effectiveness of dezocine after laparoscopic cholecystectomy. Methods: Totally 80 patients with laparoscopic cholecystectomy were randomly divided into the observation group and the control group. The control group was given tramadol 0. 2 mg·kg-1 for anesthetize, the observation group was given dezocine 0. 1 mg·kg-1 analgesia. Conducted according to a simgle drug additional pain status of the patient. After the surgery, the time of surgery, VAS and BCS pain condition score of the pa-tients were analyzed and compared, and the incidence of adverse reactions of the two groups were recorded. Results:After the surgery, the operation time, opening eyes, awaking time and extubation time showed no significant difference in the two groups (P>0. 05). VAS scores of the observation group in 30min, 1h, 2h and 6h after the surgery were significantly lower than those of the control group, and BCS score was significantly higher than that of the control group (P<0. 05). AIM:To observe the preliminary efficacy of conbercept injected intravitreally for the treatment of wet age-related macular degeneration(wAMD).METHODS:Seventeen wAMD patients (18 eyes) were selected to receive conbercept injection.All patients were given a single conbercept injection every month,3 times.Before and after 1,2,3mo of the injection,the best corrected visual acuity (BCVA),intraocular pressure (IOP,measured by Non-contact tonometer),fundus photography,fundus fluorescein angiography(FFA),indocyanine green angiography(ICG),optical coherence tomography(OCT) examination and the complications incidence were compared.RESULTS:Three months after conbercept injection,the BCVA improved in 15 eyes (83%),stable in 3 eyes (17%).Before treatment,the average central macular thickness was 421.72±54.43μm,at 1 and 2 and 3mo after treatment,the average central macular thickness was 337.89±25.88μm,293.56±26.87μm,266.89±19.10μm respectively.There were significant differences compared with before and after injection(P<0.05).In the final follow up,FFA and ICG showed that the leakage in macular area disappeared in 15 eyes (83%),still existed in 3 eyes (17%),in those 3 eyes the injection was given for one or two times till the leakage disappeared.Elevated intraocular pressure occurred in 2 cases (26mmHg,23mmHg),after 1d down to normal.Another patient showed postoperative envy,given left ofloxacin eye drops after 2d,then back to normal.There was no serious ocular adverse reactions.CONCLUSION:Intravitreal injection conbercept for wAMD can significantly improve the visual function,reduce the macular edema and the leakage with higher safety and less complications.However the prolonged efficacy needs further observation.

Objective To assess the feasibility of using ultra sonic endoscopy in retroperitoneal laparoscopic nephron-sparing surgery for treatment of endogenous renal tumor, and to summarize our clinical experience. Methods A female patient, aged 28 years old, was found to have a mass (diameter1.4cm×1.0cm, clinical stage T1aN0M0)in the right upper part of the kidney. Retroperitoneal laparoscopic nephron? Sparing surgery was performed. The tumor could not be accurately located during the operation due to the smooth renal surface; then ultrasonic endoscopy was used to locate the tumor; and color Doppler mode was used to observe the blood supply of the tumor and its relation with surrounding tissues. The nephron-sparing surgery was performed following the guidance of ultrasonic endoscopy positioning; the integrity of tumor resection and surgical margin were also observed. Results The involvement of the tumor an dits blood supply were clearly displayed by ultrasonic endoscopy. Nonoticeable signals of blood flow were seen around the tumor after blocking the renal artery, and the tumor was totally and thoughly removed with a negative margin of 0.5-1.0cm.There was no transfer to opening surgery. The procedure of ultrasonic endoscopy last ed for 5 min. Post operative pathological results in dicated angiomyolipoma with negative margins. Conclusion Our initial clinical practice suggests that ultrasonic endoscopy is safe and benificial for retroperitoneal laparoscopic nephron-sparing surgery in treatment of endogenous renal tumors, especially for observing the tumor location, tumor blood supply, and the integrity of resection.

The daily intake levels and trend of polychlorinated dibenzo-p-dioxins(PCDDs), polychlorinated dibenzofurans(PCDFs), polychlorinated biphenyls(PCBs) of general population of several developed countries are reviewed. Based on the data of China, PCDD-Fs levels in Chinese human milk and blood were less than that of other countries, in addition, a daily intake of PCDD-Fs for Chinese general population was estimated in the present paper, the tolerable daily intake of Chinese general population was less than that the World Health Organization proposed.

Objective To study the clinical practical use of the model for end-stage liver disease(MELD) and assess its validity in predicting the prognosis of the.chronic severe hepatitis patients.Methods The clinical data of 391 patients with chronic severe hepatitis among 2003～2005 in our hospital were analyzed,and they were graded with MELD formula.These patients were divided into plasma exchange group and none plasma exchange group according to plasma exchange treatment or not.The death rate was observed within three months.The validity of the model in predicting the prognosis of the patients with severe hepatitis was determined by the concordance(c)-statistic.Results 236 patients died within three-months and the mortality was 60.6%.The mortality of patients in PE group whose MELD score were less than 30 and 30～40 was 25% and 75% respectively,but in non-PE group they were 42.0%,90.7% respectively,there were significant difference between PE group and non-PE group(P0.05).The optimal cut-off values of MELD to predict the prognosis of patients were 30 in PE group whose sensitivity and specificity were 66.7% and 86.4%,and its c-statistic was 0.823(95% CI 0.734～0.912);and in non-PE group they were 27,76.4%,79.1% and 0.832(95% CI 0.787～0.877) respectively.Conclusions The mortality of the patient with chronic severe hepatitis increases with the increase in the MELD score.The MELD can accurately predict the short-term prognosis of patients with chronic severe hepatitis.
