1.Effect of recombinant human interleukin-1 receptor antagonist on metabotropic glutamate receptor 5 of the hippocampus following global cerebral ischemia reperfusion

BACKGROUND: Interleukin-1 receptor antagonist can relieve damage of neuron and protect nerve. Aminoglutaric acid can induce exitotoxicity through activating some kinds of aminoglutaric acid receptor, at the same time, can protect nerve through some receptors. But the relationship between them was unclear during the process of cerebral ischemia reperfusion.OBJECTIVE: To investigate the effect of neuroprotective actions of interleukin-1 receptor antagonist on hippocampal neurons, and the relationship between interleukin-1 receptor antagonist and metabotropic glutamate receptor 5 within the process of cerebral ischemic reperfusion injury.DESIGN: Completed randomized controlled study.SETTING: State Key Laboratory of Magnetic Resonance and Atomic and Molecular Physics, Wuhan Institute of Physics and Mathematics, Chinese Academy of Sciences; School of Medicine & Life, Jianghan University.MATERIALS: The experiment was performed at State Key Laboratory of Magnetic Resonance and Atomic and Molecular Physics, Wuhan Institute of Physics and Mathematics, Chinese Academy of Science from May 2004 to January 2005. Totally 32 well-being pure breed male Wistar rats were selected.METHODS: Totally 32 rats were divided into four groups by method of the simple random sampling: normal group (n=8) received no surgical treatment, sham operated group (n=8) subjected to only dorsal and ventral neck midline incisions and gently dissection of the bilateral common carotid arteries free of surrounding nerve fibers without occlusion of the both vertebral arteries and common carotid arteries, saline group (n=8) suffered from the permanently occlusion of the both vertebral arteries by electrocauterization and transient (20 minutes) occlusion of bilateral common carotid arteries respectively, and received the treatment of a 2 μL normal saline injection into right lateral ventricle at the rate of 0.4 μL/minute following a needle withdrawal within 5 minutes, and experiment group (n=8) offered the same procedure of the saline group but for that the equivalent amount of rhIL-1ra took the place of normal saline into the lateral ventricle. Hematein-eosin staining was applied to observe pathological changes and immunohistochemistry (ABC) was used to survey the IR of mGluR5 varieties of the hippocampal neurons.MAIN OUTCOME MEASURES: ① Basic pathological changes of hip pocampus and cerebral cortex; ② mGluR5 immunoreactivity (IR) of hippocampus, the sensitive area to cerebral ischemia.RESULTS: Two rats were excluded from the experiment on account of their convulsion during reperfusion (one of the experiment group, another of the saline group), data of 30 rats was entered final analysis. ①Basic pathological changes of hippocampus and cerebral cortex: The Hemateineosin (H.E) staining showed that there is little pathological discrepancy between the normal group and the sham operated group, apparent neuronal degeneration such as peripheral edema around neurons, pyknosis, karyorrhexis and so onin the hippocampus and cortex of the rats of the saline group compared with those of the previous groups, and also the pronounced lower degree of neuronal degenerationin the rats of the experiment group.② mGluR5 immunoreactivity (IR) of hippocampus, the sensitive area to cerebral ischemia (absorbency): The immunohistochemistry presented that the mGluR5 immunoreactivity (IR) of CA1, CA3 areas of the normal group and sham group rats was strong positive (CA1: 0.54±0.12, 0.54±0.05; CA3:0.57±0.02, 0.58±0.08;P > 0.05) Compared with of the normal group and sham group, the mGluR5 IR of the saline and the experiment groups reduced apparently (CA1: 0.30±0.03, 0.40±0.04; CA3: 0.30±0.04, 0.42±0.06;P < 0.01), but the mGluR5 IR of the experiment group was quite stronger than that of the saline one (P < 0.05).CONCLUSION: IL-1ra, one member of the Interleukin-1 family (system)of the cytokines, and mGluR5, one subtype of the metabotropic glutamate receptors were both involved in the pathophysiological process of the global cerebral ischemia reperfusion injury of the rat. Also, as the antagonist of the proimflammatory medium intetleukin-1 receptor, IL-1ra may show neuroprotection by affecting the mGluR5 expression of the CA1 and CA3 areas of the rat hippocampus within the process of cerebral ischemic reperfusion injury. Furthermore, here it demonstrated that besides IL-1ra, other factors might regulate the expression of mGluR5.

Objective:To fast detect hypotensive drugs illegally added to the flower buds of Panax notoginseng. Methods:A laser Raman spectrometer was used to detect several chemical drugs added to the flower buds of Panax notoginseng, and the spectrum was qualitatively analyzed. Results:Raman spectroscopy could detect chemical antihypertensive drugs added to the flower buds of Panax notoginseng, and the test results were in accordance with those of the traditional method. Conclusion: Raman spectroscopy can be used for the determination of illegal additives in the flower buds of Panax notoginseng.

4.Differentiation of bone marrow mesenchymal stem cells into chondrocyte-like cells under the conditions of transforming growth factor beta and cyclical tensile strain

BACKGROUND:Transforming growth factor-βhas been shown to exert an obvious induction effect on the differentiation of bone marrow mesenchymal stem cells into chondrocytes. Cyclical tensile strain simulates mechanical environment of chondrocytes in the body, and plays an important regulatory role in cellproliferation and differentiation. OBJECTIVE:To discuss the synergy of transforming growth factor-βand cyclical tensile strain in inducing the differentiation of bone marrow mesenchymal stem cells into chondrocyte-like cells. METHODS:A total of 10 2-month-old New Zealand rabbits were selected. Bone needle was used to penetrate the medul ary cavity of bone. 3.0-4.0 mL of bone marrow was extracted for isolation and culture of bone marrow mesenchymal stem cells. Passage 3 cells were randomly assigned to four groups:blank, transforming growth factor-β, cyclical tensile strain and cyclical tensile strain+transforming growth factor-βgroups. After 1, 3 and 6 days, cells were obtained. General morphology was observed using safranin O staining. Glycosaminoglycan levels were detected by alcian blue staining. Matrix metal oproteinase-13 and tissue inhibitor of metal oproteinase-1 levels in supernatant were measured using ELISA. Type II col agen, matrix metal oproteinase-13 and tissue inhibitor of metal oproteinase-1 mRNA relative expression was detected using RT-PCR. RESULTS AND CONCLUSION:Safranin O staining showed fusiform or irregular triangular cells. cellnumber
and matrix secretion increased in each experimental group than in blank group. Glycosaminoglycan levels in the supernatant were greater in the transforming growth factor-βand cyclical tensile strain+transforming growth factor-βgroups than in the blank group (P<0.05). Type II col agen mRNA relative expression was higher in the cyclical tensile strain+transforming growth factor-βgroup than in the blank group (P<0.05). Results indicated that transforming growth factor-βand cyclical tensile strain could induce the differentiation of bone marrow mesenchymal stem cells into chondrocytes, showing an apparent cooperative action.

6.Treatment of displaced intra-articular fractures of the calcaneus using a small lateral incision approach

Objective To investigate the operative treatment of displaced intra-articular fractures of the calcaneus using a small lateral incision approach.Methods Thirty-one patients with 32 intra-articular calcaneal fractures,were treated with open reduction and internal fixation through a small lateral incision approach from October 2004 to April 2006.The skin incision was made from the distal tip of the fibula to the base of the fourth metatarsal.According to Sanders classification,there were 21 typeⅡfractures and 11 typeⅢfractures.The first change of dressing was done on the second day after operation.The residual blood clots in the wounds were cleaned as much as possible when the drainage sheet was removed.The operated foot was bandaged with pressure dressing of cotton pad.Results After an average follow-up of 10.5 months(range,3 to 18 months),no soft tissue com- plications were found,and all had acceptable reduction.The preoperative X-ray film showed that B(?)hler angle was 6.84??9.36?,Gissane angle was 106.04??12.03?.The postoperative X-ray film demonstrated that B(?)hler angle was 32.06?+6.87?,Gissane angle was 115.81??9.48?,and the difference was statistically significant(P<0.01).According to the AOFAS evaluation system for Ankle-Hindfoot,two feet scored 60-70 points,nine feet 70-80 points,16 feet 80-90 points and five feet 90-100 points.Condusion The small lateral incision approach is a good option for management of calcaneus fractures,because it causes minimal soft tissue damage,provides excellent exposure,and leads to convenience for later removal of internal fixators and subtalar arthrodesis.
7.Study on serum autoantibody in patients with polycystic ovary syndrome in child-bearing period

Objective To investigate the association between serum non-organ-specific autoantibodies and polycystic ovary syn-drome(PCOS).Methods A total of 69 patients with PCOS in child-bearing period treated in our hospital were selected as the study group and contemporaneous 69 healthy child-bearing year women were selected as the control group.Serum level of antinuclear anti-bodies (ANA)was measured by ELISA,serum level of anti-dsDNA was measured by colloidal gold spot infiltration assay and the extractable nuclear antigen (ENA)auto-antibodies profiles were measured by Western blot.Results Serum levels of ANA and an-ti-dsDNA antibodies in the study group were significantly elevated compared with the control group with statistical difference.Con-clusion Serum autoantibody positive exists in the patients with PCOS.

Objective To investigate the effects of short-term continuous subcutaneous insulin infusion (CSII) in newly diag-nosed type 2 diabetes mellitus(T2DM ) and to evaluate the treatment regimen conversion after CSII therapy .Methods 72 patients with newly diagnosed T2DM were treated with CSII for 2 weeks .Then they were randomly divided into two groups :the basal insu-lin group(glargine) and the oral anti-diabetic drug(OAD) group .Both groups were followed up for 2 years .Blood glucose ,insulin and HbA1c were measured before and after CSII and during the 2-year follow-up .Results CSII significantly controlled the glucose levels ,reduced the TG ,TC ,LDL levels and the homeostasis model assessment of insulin resistance ( HOMA-IR) and increased the homeostasis model assessment of insulin secretion (HOMA-IS)(all P< 0 .05) .During 1-year follow-up ,HbA1c in the glargine group and the OAD group was (6 .13 ± 0 .47)% and(6 .21 ± 0 .38)% respectively .During 2-year follow-up ,the HbA1c values in the two groups were (6 .91 ± 0 .57)% and(6 .43 ± 0 .62)% respectively .T HOMA-IR and HOAM-IS obtained the long tern improve-ment without significant body weight increase .Conclusion Short-term intensive insulin therapy can effectively control the blood glucose ,improve the sensitivity of insulin and the β-cell function ;after CSII ,adopting basic insulin and oral antidiabetic drugs can a-chieve the ideal glycemic control .
9.Efficacy of the Rehabilitation Strategy for Inpatients with Schizophrenia

Objective:To explore efficacy of the rehabilitative strategy in hospitalized schizophrenics.Method:Sixty-four schizophrenic patients were randomly divided into the intervention group and the control group.Both groups received the anti-psychotics therapy, but the teamwork education,medication management skills training module, symptom management skills training module were only given to the intervention group for ten weeks. And a one-year follow-up was carried out after their discharge from hospital. All subjects were evaluated with the Positive and Negative Syndrome Scale (PANSS), Nurses` Observation Scale for Inpatient Evaluation-30 (NOSIE-30), Insight and the Attitude to Treatment Scale, Social Disability Screening Schedule (SDSS), Morning Side Scale. We also recorded the relapse rate, reemployment rate, and re-hospitalization rate. Result:The clinical outcome of the intervention group was significantly superior to the control group on overall improvement according to PANSS (31.17?3.13 vs 52.34?13.02,7.34?1.08 vs 12.36?4.28,7.76?2.13 vs 13.26?4.50,16.13?1.82 vs 32.17?9.82 ),increase of active factor score of NOSIE-30 (-18.3?3.2 vs 14.4?6.7),and the reduction of inactive factor score of NOSIE-30(9.4?6.2 vs -22.3?7.4).The relapse rate(9.4% vs 54.8%),re-hospitalization rate(3.2% vs 42%),reemployment rate(37.5% vs 9.6%)were also better in the intervention group.Conclusion:The effect of rehabilitative strategy for schizophrenic patients not only improves the symptom, but also their social function.
