1.Effects of E 4031 on isolated cardiac function and resting Ca2+ level of myocardial cells from rats with chronic heart failure.

OBJECTIVETo investigate the effects of E 4031, a sodium calcium exchanger (NCX) agonist, on the isolated cardiac function and resting Ca2+ level in myocardial cells from rats with chronic heart failure.
METHODSRats chronic heart failure model was established by abdominal aorta coarctation with; Isolated heart perfusion by Langendorff apparatus was used to detect heart function and the effects of E 4031 on haemodynamic indexes; Myocardial cells of rats in the model group were extracted quickly and co-incubated with calcium fluorescent indicator fluo3/AM and the impact of E 4031 on the fluorescence intensity in myocardial cells were evaluated by confocal microscopy.
RESULTSHeart function of rats in the model group detected by Langendorff perfusion was significantly reduced after 12 weeks, E 4031 at 10 micromol/L could improve their left ventricular developed pressure(LVDP) and systolic / diastolic maximum rate (+/- dp/dtmax). Compared with the control and sham operation groups, the resting Ca2+ fluorescence intensity of the myocardial cells of rats in model group was at a higher level and went through a process of transient rise and drop, then stably remaining at a low level after co-incubated with 10 micromol/L E 4031.
CONCLUSIONE 4031 can improve the isolated heart function of rats with chronic heart failure, which may be associated with its enhancing the activity of NCX in the myocardial cell membrane and stabilizing intracellular Ca2+ level.

2.Orthotopic heart transplantations for end-stage heart diseases

Objective To summarize the outcomes and clinical experience of orthotopic heart transplantations in Shanghai Zhongshan Hospital.Methods From May 2000 through October 2005,141 patients,101 males and 40 females,diagnosed as having dilated cardiomyopathy in 118,hypertrophic cardiomyopathy in 2,restrictive cardiomyopathy in 2,end-stage ischemic heart disease in 9, primary malignant cardiac tumor in 4,valvular heart disease in 3 and others in 3,underwent orthotopic heart(transplantations) at our center.The operative procedures included 120 bicaval anastomotic cardiac transplantations,19 conventional Stanford orthotopic cardiac transplantations and 2 total heart transplantations.The immunosuppressive therapy strategies included Cyclosporine A or tacrolimus,corticosteroids and mycophenolate mofetil.For the latest 28 patients,induction therapy with Daclizumab was applied.Results There were 3 operative deaths with an operative survival of 97.9 %.During the follow-up from 1 month to 65 months,the actuarial survival rate was 90.8 % after 1 year,84.6 % after 3 year and 81.4 % after 5 year.Acute rejection,infections,graft failure and metastasis of primary cardiac tumor were the main causes of death in the 1st postoperative year,while acute rejection and graft coronary vasculopathy were the leading causes of death thereafter.Postoperative complications included acute rejection,infections,renal dysfunction and graft failure.Conclusions Orthotopic heart transplantation was proved to be a reliable choice for endstage heart disease with excellent outcomes.More attention should be paid to surveillance and management of acute rejection, infections and graft coronary vasculopathy in the long-term follow-up.
3.Role of Apoptosis in Acute Rejection of Pancreaticoduodenal Transplantation in Rats

Objective To investigate the roles of cell apoptosis and the gene expressions of Fas, FasL, bcl-2 and bax in acute rejection of pancreaticoduodenal transplantation and to evaluate the function of duodenum biopsy for early detection of rejection in rats. Methods Wistar and SD rats were divided into two groups: ①Wistar rats that underwent allogenic pancreaticoduodenal transplantation from the organs of SD rats; ②Wistar rats that received homogenic transplantation. The grafts were then harvested on day 3, 5 and 7 after the transplantation, and all graft samples were observed with HE staining and TUNEL was also used to detect apoptotic cells. The expressions of Fas, FasL, bcl-2 and bax were measured by immunochemical method. According to Nakhleh’s score, pathologic features of transplanted pancreas and duodenum were ranged from one to three scores in order. Results The percentage of same or different scores between the pathological scores of pancreas and duodenum in allogenic pancreaticoduodenal transplantation group were 61.1% (11/18) and 38.9% (7/18) respectively, and there were 6 specimens of pancreatic tissue got higher scores with only one higher score for duodenum. 4.Water Quality of Product Water from Centralized Water Supply System in Hunan, 2006

Objective To explore the water quality of the product water from the centralized water supply system in Hunan, 2006. Methods The sanitary quality of the product water samples collected from 28 centralized water supply system in cities and 39 centralized water supply system in counties in Hunan, 2006, which distributed among the area of Xiangjiang River, Zishui River, Yuanjiang River and Lishui River, were determined and evaluated according to the Standards for Drinking Water Quality (GB5749—2006). Results The eligible rate of the water sample was 70.1%. Among 74 water quality indexes, 12 indexes (such as antimony,aluminum,pH value,manganese and so on) exceeded the standard limits in degrees in some centralized water supply systems. Most of the routine indices of water quality were unqualified and the reference indices were qualified. 5.Erythromycin and alendronate inhibit aseptic loosening in murine osteolysis model

[Objective]To observe whether erythromycin and alendronate are effective in treating aseptic loosening in a murine osteolysis model. [Method]Air pouches were established in the back of BALB/c mice by injecting sterile air subcutaneously. After the air pouches were mature, grafts of calvaria from syngeneic mouse donors were implanted in the air pouches and polyethylene debris was injected into the air pouches. Mice were divided into 4 groups. In the first group, saline was injected into the air pouches.In the other three groups, polyethylene debris was used. Saline,erythromycin and alendronate were injected intraperitoneally separately.Pouch tissues were collected at 14 days after polyethylene debris administration for molecular and histologic analysis.[Result] Both erythromycin and alendronate inhibited osteoclastogenesis and bone resorption, but alendronate was not as effective as erythromycin in anti-inflammation.[Conclusion]Both erythromycin and alendronate can be promising drugs for the treatment of aseptic loosening.
6.Prognostic evaluation of posterior wall fractures of the acetabulum by two scoring systems

[Objective]To evaluate the outcome inforation provided by two scoring systems after operative treatment of elementary posterior wall fractures of the acetabulum.[Method]Thirty-two patients with elementary posterior wall fracture of the acetabulum were included in the study.Functional outcomes were assessed with use of the eight health dimensions of the short form(SF)-36,and clinical outcomes were evaluated with use of the modified Merle d' Aubign? score.The SF-36 scores were compared with previously reported normal values and the correlation was analyzed between the modified Merle d' Aubign? score and score of each SF-36 health dimension.[Result]The mean modified Merle d' Aubign? score was 16.28 (range 13-18,SD 1.3),indicating overall good-to-excellent clinical results.However,all SF-36 health dimensions were significatly lower than the expected forms,with a significant statistical difference(P

AIM:To investigate the relationship between serum ghrelin level and body composition including bone mineral content (BMC),fat mass(FM) & lean mass(LM) in premenopausal women with different thyroid functional status.METHODS:We measured the serum ghrelin levels by radioimmunoassay (RIA),the serum levels of free triiodothyronine (FT3),free thyroxine (FT4) and sensitive thyroid-stimulating hormone (sTSH) by chemiluminescene immune assay.The total body composition including total BMC,FM and LM was measured by dual-energy X-ray absorptiometry (DXA) in 71 premenopausal women with different thyroid functional status (33 hyperthyroidism,18 hypothyroidism and 20 normal subjects).RESULTS:(1) The level of serum ghrelin in patients with hyperthyroidism was significantly lower than that in patients with hypothyroidism (P0.05).The serum ghrelin level was correlated negatively with FT3(r=-0.318,P0.05).CONCLUSION:The serum ghrelin level in the premenopausal women with different thyroid functional status may correlate with the total BMC,LM and body weight.

Objective:Performance appraisal of large-scale medical equipment has a significant impact on hospital management. Process equipment performance evaluation of digital is an important part of digital medical&hospital. How to fully utilize the large number of advanced medical equipment resources, how to monitor and manage equipment are focuses that hospital administrators pay much attention to. Methods: The effective performance appraisal of large-scale medical equipment can promote the resource allocation optimization, accurate and scientific management, reasonable and benign application, and then allows large-scale medical equipment fully and effectively applied to clinical practice. Results: Collecting basic data is the basis for performance appraisal of large-scale medical equipment; its instant validity, accuracy, and scientific can enhance the operability and the scientific accuracy of the performance appraisal for large-scale medical equipment. Conclusion:The acquisition of basic data has few specialized research in previous studies, the rules set up for basic data acquisition of large-scale medical equipment in this paper can improve the timeless, accuracy and normalization of the basic data using in the performance appraisal of large-scale medical equipment.
