A comparison is made on the healthcare systems, economy, informationization environment, developers, investment and compensation mechanisms in nine countries featuring early startup, rich experiences, and relatively higher ratio of success in their development projects. This lead to a discussion focusing on the construction steps and development strategies for regional health informationization, and an analysis of the present regional health informationization overseas, common measures taken, and key factors for successful regional health informationization.Based on these, recommendations were made on development of regional health informationization compliant to China's circumstances.

By means of literature review, the paper made a comparative study of key development projects of healthcare information in China over the last decade,in terms of their project characteristics, economic environment, development entity, investment and compensation practice, development strategy, coverage, experience and promotional values.Setbacks identified include the following:discrepancy in regional development stages of healthcare information; successful pilots are limited to scientific research samples; lack of regional models; systems separation; poor continuity and coordination; waste and insufficiency of resources in the investment and construction among others. The authors recommend a better exchange of experiences in building the regional healthcare information service platforms, greater investment, better compensation mechanism, better top-tier design, and maximized coverage within reach of supervision and technology.

Objective Radiological grade of splenic injury was seldomly used in China trauma center now, though it had been established in 1994 by American Association for Surgery of Trauma (AAST) and widely used. The present study is aimed to analyze the imaging grade and clinical characteristics of traumatic splenic rupture in children, discuss the feasibility of conservative treatment, and the role of radiographic grading during clinical treatment. Methods Information (including age, gender, severity based on radiological findings, treatment strategies, and clinical outcome) regarding 59 hospitalized splenic injury patients whose injuries occurred between 2008 and 2014 was retrospectively analyzed. Results Between 2008 and 2014,59 pediatric patients with splenic injury were treated in our institution.Median age was 9.5 years (range,3 months to 16 years).Of all patients,41 (69.5%) were male. The injuries were primarily caused by traffic crash (45.7%),stumbling/falling from a height (38.9%).According to AAST,5 cases were grade Ⅰ,26 patients grade Ⅱ (44.1%),and 21 cases grade Ⅲ (35.6%),6 over grade Ⅳ,and only one was unclear.Of all patients,25 cases were with the other organs complications. All patients underwent fasting, bed rest, and antibiotics.Only 1 case was transferred to operation during the conservative treatment.Forty-nine patients underwent with CT scan over 2 times. Conclusion Imaging classification helps guide clinical treatment.Conservative treatment is feasible for traumatic splenic injury in children.Early imaging classification of splenic injury may be helpful in clinical judgment, and reduce children radiation exposure.

Objective To detect the expression of advanced glycation end-products (AGEs) in the sun exposure and non-exposure skin,to observe the elastic protein in the same location, and to explore the relationship between non-enzymatic glycation reactions and,photo-aging skin morphological changes. Methods In the exposure and non-exposure specimens from 30 patients, elastic fibers were stained with Gomori staining, and immunohistochemistry for AGEs was performed. Results AGEs expressed clearly positive in all elastic fibers of degeneration markedly (+ + above) of exposure skin, and appeared line-like in markedly(+ + above)elastic fibers. While in all non-exposure skin,AGEs expressed negative. In the exposure skin with middle or old age, compared with non-exposure skin,AGEs expressed more and elastic fibers were hyperplasia with thickening, curling and irregular distribution. The skin parts of AGEs expressed accorded with where elastic fibers degenerated. Conclusions Ultraviolet radiation can induce denaturation of elastic fibers and non-enzymatic glycation may contribute to the mechanism of skin photo-aging.

Objective To observe the difference in treament of thoracolumbar vertebral bodies fractures be- tween AF nail and Dick nail. Methods From March 1998 to March 2007,85 cases of thoracolumbar vertebral bod- ies fractures were followed up.20 cases were fixed with Dick nail, and 65 cases with AF nail. Results The mean,fol- low-up period was 12 months.By comparison of the operating rime, bleeding amount, the recovery rate of vertebral height, the reduction of Cobb angle and capacity of vertebral canal,AF nail was much better than Dick nail. But there was no marked difference in the recover of nerve function. Conclusion AF nail has more power to reduce vertebral height and is easier to set than Dick nail.It will be worthy of more and wider application in basic level hospitals.

A multiplex real-time PCR was developed to detect ctxA of Vibrio cholerae, gyrB and tdh of Vibrio parahaemolyticus simultaneously. The multiplex real-time PCR were evalidated by detection for the three genes in 47 toxigenic V. cholerae O1 and O139 strains (ctxA+; O1=3, O139=44), 25 non-toxigenic V. cholerae strains (ctxA-; O1=12, O139=6, non-O1 and non-O139=7), 116 V. parahaemolyticus strains with or without tdh (73 or 43) and 9 other bacteria strains. The specificity and sensitivity of the multiplex real-time PCR in detection for the ctxA and the tdh genes in the strains tested were both 100.0%, compared to the results by routine PCRs. In the detection for V. parahaemolyticus specific gyrB using the multiplex real-time PCR, all of 116 V. parahaemolyticus strains were positive, and 9 other strains and 72 V. cholerae strains were all negative. The multiplex real-time PCR is a sensitive, specific and quick assay not only for detecting virulence genes of V. cholerae and V. parahaemolyticus but also for identifying V. parahaemolyticus at species level. In addition, two real-time PCRs for detection of V. parahaemolyticus virulence genes trh1 and trh2 were also developed.

Microbiology 2008;0(11):-
A strain Actinobacillus succinogenes CGMCC 1593 was selected as the parent strain. After UV-EMS and UV-DES treatments respectively, seven mutated strains with subtle improvements in acid tol-erance were obtained, and were subjected for recursive protoplast fusion.Through three rounds of genome shuffling, four shuffled strains with both higher yield and acid tolerance were obtained. The shuffled strain namely F3-21 could even survive at pH 5.2.The comparison of the shuffled strains and the parent strain for succinic acid production was also studied here. After 48 h of shake-flask fermentation, the succinic acid concentration of F3-21 was 48% higher than that of the parent strain. When F3-21 was carried out in a 5 liter stirred bioreactor with pH controlled 5.6~6.0,the accumulation of succinic acid in 48 h fermentation attained 38.1 g/L,which was increased by 45% compared with that of the parent strain(26.2 g/L).While pH was controlled at 6.5~7.0,the production of succinic acid in 32 h fermentation attained 40.7 g/L.When F3-21 was carried out in fed-batch fermentation, succinic acid concentration of 67.4 g/L was reached in 72 h fer-mentation.These results indicated that the genome shuffling could improve the acid tolerance and the suc-cinic acid production of A.succinogenes CGMCC 1593.
