Objective To evaluate the clinical value of quantitative myocardial acoustic densitometry in myocardial fibrosis in relative to the alteration in serum procollagen type Ⅲ amino-terminal propeptide(PⅢNP) concentration in hypertensive patients. Methods Seventy-five consecutive hypertensive patients were recruited with 75 normal persons served as healthy controls. The indexes of acoustic densitometry were evaluated by echocardiography and the concentrations of the serum PⅢNP peptide by radioimmunoassay. 2.The therapeutic effect of astragalus injection on islet β cells apoptosis

Objective To investigate the therapeutic effect of astragalus injection onβcells apoptosis of type 1 diabetes mice. Methods 32 mice were randomly divided into a control group, a diabetes group, a small dose group and a large dose group of astraglus. Except the control group, mice in the other 3 groups received intraperitoneal injection of STZ in order to induce diabetic mellitus. Then one week after injection of STZ, mice in the small dose group and the large dose group of astraglus were given 30, 60 g/(kg?d) doses of astragalus injection, while the other 2 groups were injected into an equal volume saline for 4 weeks. Serum NO, inducible nitric oxide synthase (iNOS) and insulin levels were detected by enzyme linked immunosorbent assay method, and apoptosis was measured by using a TUNEL assay. Results Compared with the diabetes group, the serum NO (25.81 ± 2.09μmol/L, 18.84 ± 3.95μmol/L vs. 30.34 ± 2.53μmol/L) and iNOS (21.38 ± 4.48μmol/L, 17.00 ± 3.05μmol/L vs. 26.62 ± 2.48μmol/L) levels were significantly reduced in the small dose group and the large dose group of astragalus (P<0.05);But the insulin levels (312.19 ± 66.91 pmol/L, 330.14 ± 56.06 pmol/L vs. 273.56 ± 43.54 pmol/L) and apoptosis ofβcells (23.00%± 16.58%, 27.38%± 18.79%vs. 31.25%± 18.19%) were not significant different among the experimental groups (P>0.05). Conclusion Astragalus injection can reduce the iNOS activity and NO production, but can not effectively decreasingβcells apoptosis.
3.Practice and Thinking of Scoring Diseases Payment under the Control of Total Budget

OBJECTIVE:To study the implementation of scoring diseases payment under the control of total budget in some ar-eas of China,and provide reference for the further improvement of the payment. METHODS:Based on the operation process of scoring disease payment in the control of toal budget,the specific practices of Huaian,Nanchang,Zhongshan and Taiwan area, were compared. RESULTS:It showed differences in the control and allocation of total budget,the establishment of scoring diseas-es,the setup of adjustment coefficient,the development of balance methods,the complementary mechanism and the monitor exami-nation setting. CONCLUSIONS:Whether to classify the diseases according to diagnosis and treatment methods,how to setup ad-justment coefficient appropriately,how to reimburse the patients in stages and how to strengthen supervision are worthy of further thinking and exploring.
4.Study on the medication scope of 581 optimized ancient prescriptions

Objective The medication scope of 581 prescriptions contained in the book of Two hundred one party governance inspection record is studied through data mining.Methods After regulating drug names individually according to the authoritative literature,the statistical range of medication is given using the software Excel 2003.Results A total of 360 kinds of medicines are obtained.Conclusion The spectrum of 581 optimized ancient prescriptions is formed,which is the focus of our further research.
5.The relationship between changes of Tpeak-Tend interval and ventricular arrhythmias in patients with type 2 diabetes mellitus

Objective To analyze the changes of Tpeak-Tend interval (Tp-Te) in patients with type 2 diabetes mellitus (T2DM),and to explore the relationship between the changes of Tp-Te and ventricular arrhythmias. Methods A total of 112 patients with T2DM were selected from the Department of endocrinology of our hospital from June 2014 to May 2016 (T2DM group),and 65 subjects underwent healthy physical examination in our hospital were selected as the normal control group (NC group) during the same period.The general data,fasting plasma glucose (FPG),glycosylated hemoglobin (HbA1c),Tp-Te,Tp-Te dispersion (Tp-Ted) were compared between the two groups.The changes of Tp-Te in patients with or without ventricular arrhythmia in T2DM group were also analyzed. Results The FPG,HbA1c,LDL-C,TC and heart rate were significantly higher in T2DM group than in the NC group (P<0.05).Compared with the control group,V5 lead Tp-Te and Tp-Ted in T2DM group was significantly prolonged (P<0.05).IN T2DM group,V5 lead Tp-Te and Tp-Ted were significantly longer in patients with ventricular arrhythmias than in patients without ventricular arrhythmia (P<0.05).Multiple logistic regression analysis showed that the increase of HbA1c and FPG were independent risk factors for prolonged Tp-Ted,and Tp-Ted was an independent risk factor for ventricular arrhythmia. Conclusion Patients with T2DM are more likely to have prolonged Tp-Ted.The prolonged Tp-Te and Tp-Ted could increase the incidence of ventricular arrhythmias.
6.Screening on the tumor related markers in the serum of the renal cell carcinoma patients by the SELDI-TOF-MS technique

Background and purpose:Surface enhanced laser desorption ionization time of flight mass spectrometry (SELDI-TOF-MS) is useful in helping to identify the molecular changes closely related to renal cell carcinoma. We explored the different expression of sera protein between human renal cell carcinoma patients and normal to screen renal cancer-specific biomarkers. Methods:The protein mass spectrometry of 28 cases with renal cell carcinoma and 28 normal persons were detected by WCX2 protein chip combining with SELDI-TOF-MS technique for screening the different proteins. Serum samples from 28 patients with clear renal cell carcinoma and 28 normal persons were used to detect biomarkers for clear renal cell carcinoma by SELDI-TOF-MS technique with WCX2 Proteinchip. Results:170 effective protein wave crests between 1.5?103-30?103(1.5-30 kD) were detected. Seven proteins were specifically detected in sera of patients with clear renal cell carcinoma, but not in normal donor. The proteins with MW 4.098,5.917,6.643 ?103 were down-regulated ,and four proteins with MW 5.572,6.344,6.529,8.518 ?103 were up-regulated. Conclusion:Detection of specific protein in human renal cell carcinoma sera is significant both for determination of clinical specific biomarkers and study of cancer development mechanism.
7.Pathologicd characteristics of osteoarthritis secondary to acetabular dysplasia and the short-term effects of total hip replacement

[Objective]To summarize the experiences of total hip replacement with cementless joint components in the treatment of osteoarthritis secondary to acetabular dysplasia in adults and to evaluate its early effect. [Method]From March 2000 to March 2007, thirty hips in 25 adult patients with osteoarthritis secondary to acetabular dysplasia undergoing total hip replacement with cementless joint components were included in this study. Of the patients, 5 were male and 20 were female.Their ages ranged from 35 to 55 years with an average age of 55 years. The patients mainly complained of pain and claudication. According to the Hartofilakidis classification , Semi-dislocation occurred in 21 hips and high-dislocation in 9 hips.Five patients underwent bilateral total hip replacements and twenty patients had unilateral total hip replacement.[Result]The duration of follow-up ranged from 8 months to 7 years ( average 3 years and 9 months ) . The average Harris score was increased from 36.09 to 88.21 points, They could take care of themselves and returned to their previous job. All the patients were pain-free and there was no sign of aseptic looseing or subsidence.[Conclusion]Total hip replacement with cementless joint components is an effective method for the treatment of osteoarthritis secondray to acetabular dysplasia in adults.The key point to the success for operation are appropriate cementless joint components, deepened medial wall of the acetabulum and improved the techniques of bone graft in the true acetabulum.
8.Prognostic evaluation of posterior wall fractures of the acetabulum by two scoring systems

[Objective]To evaluate the outcome inforation provided by two scoring systems after operative treatment of elementary posterior wall fractures of the acetabulum.[Method]Thirty-two patients with elementary posterior wall fracture of the acetabulum were included in the study.Functional outcomes were assessed with use of the eight health dimensions of the short form(SF)-36,and clinical outcomes were evaluated with use of the modified Merle d' Aubign? score.The SF-36 scores were compared with previously reported normal values and the correlation was analyzed between the modified Merle d' Aubign? score and score of each SF-36 health dimension.[Result]The mean modified Merle d' Aubign? score was 16.28 (range 13-18,SD 1.3),indicating overall good-to-excellent clinical results.However,all SF-36 health dimensions were significatly lower than the expected forms,with a significant statistical difference(P
9.Clinical application of template designed by three-dimensional reconstruction and rapid prototyping in locating acetabular prosthesis

[Objective]To provide a precise individualized design proposal for locating acetabular prosthesis by means of 3D reconstruction,reverse engineering software and rapid prototyping technology.[Method]3D CT scan pelvis image data were obtained from six patients,which displayed osteonecrosis of femoral head on one side.The data were transferred via a DICOM network into a computer workstation.A 3D model of hip was reconstructed using Amira 3.1 software,and saved in STL format.Then the 3D model was imported into Imageware 12.0 software.The normal hip joint centre of abnormal side was determined and the best orientation of the prosthesis(which was central axial ray of acetabula)was defined using reverse engineering.Locating template was designed according to the anatomic features of the acetabula and the central axial ray.Entity model was produced by rapid prototyping technology.The orientation of the prosthesis was located by matching the model to acetabular concavity at operation.[Result]By measuring postoperative standard hip anteroposterior film,the abduction angle of acetabular prosthesis was(46.00?2.49)? and the anteversion angle was(15.68?2.85)?.The expected result was obtained.[Conclusion]The navigation template designed and constructed preoperatively can provide precise location for acetabular prosthesis and avoid procedural errors at operation.
10.Association of anti-cyclic citrullinated peptide antibody with treatment target achierement and flare in patients with rheumatoid arthritis

Objective:Anti-cyclic citrullinated peptide (CCP) antibody is an important biomarker as-sociated with the diagnosis and prognosis of rheumatoid arthritis (RA). Different studies showed inconsistency in the relationship between anti-CCP antibody titers and RA disease activity. Therefore, we investigated the association between anti-CCP antibody with the possibility of achieving treatment target and flare.Methods:The enrolled RA patients must be anti-CCP antibody positive at baseline, and had at least one test result of anti-CCP antibody during follow-up at least one year after the baseline. The patients were divided into declined group and non-declined group according to the decrease of anti-CCP antibody titer over 10% or not during follow-up from the baseline. Single factor comparison, Pearson correlation, Spearman correlation and Kendall correlation analysis were used.Results:A total of 124 patients were included in this study. Sixty-five and 59 patients were in anti-CCP antibody declined and non-declined groups, respectively. At the end of the follow-up, the proportion of patients who achieved clinical remission or low disease activity were 78%(51/65) and 68% (40/59)in the declined and the non-declined groups, respectively ( P=0.181). The changes of Disease Activity Score with 28 joint (DAS28)-C-reaction protein (CRP), DAS28-erythrocyte sedimentation rate (ESR), tender joint count (TJC) and CRP in the declined group were significantly greater than those of the non-declined group ( P values <0.05). There was no positive correlation between anti-CCP antibody titer and several disease activity indicators at baseline ( r values <0.3, P values >0.05). The changes of anti-CCP antibody titers during the follow-up were also not correlated with changes in disease activity (but r values <0.3, P values <0.05). Meanwhile, both the baseline anti-CCP antibody titers and the changes of the anti-CCP antibody titers during follow-up were neither correlated with whether the patient achieved clinical remission or low disease activity at the end of the follow-up nor whether relapse happened. Conclusion:There is no significant correlation between anti-CCP antibody levels at baseline and disease activity, achievement of treatment target, or recurrence after treatment. The value of anti-CCP antibody in assessing disease activity, predicting treatment response, and predicting relapse needs to be confirmed in further large-scale prospective studies.
