1.Application of functional visual acuity in ophthalmology

Since functional visual acuity(FVA) was put foward,it is regraded as a kind of functional vision examination which can well reflect an individual' s performance in relation to certain daily activities involving visual tasks,the concept and the measurement is updating with the progress of technology.This examination was first applied in the patients with dry eye,and now it's applicable to detect and evaluate the refractive error,cataract,retinal pathological changes,or the eyedrops' use,and it can provide evidences for the early dignosis or the effects of thraphy of some ocular disease.In this paper,an introduction of the concept,methods and system of FVA,the clinical application of FVA in dry eye,refractive error,cataract,and in patients with fundus diseases and the research progress were reviewed.
2.Advances in Endocrine Disrupting Effects of Pentachlorophenol

Sodium pentachlorophenol (Na-PCP) has been used in China for years as a molluscacide to kill on-comelania, which is an intermediate host of schistosome. Large amounts of the powerful pesticide spayed over vast areas to control schistosomiasis contaminated the surface water sources, soil, sediment, animals and plants. Also it intruded into human body through food chain. Recently many studies that concern the endocrine disrupting effects of pentachlorophenol (PCP) were conducted. Animal studies showed that PCP/ Na-PCP could interfere in the normal function of thyroxine in many species, but without any effects on estrogen and testosterone. It was also found that PCP/ Na-PCP could effectively inhibit the activity of the human progesterone receptor (hPR) in yeast and interfere with the binding of physiological ligands to steroid receptors and binding proteins. Some epidemiologic data showed that PCP might interfere the normal endocrine function of female and paternal occupational exposure to PCP/Na-PCP could be associated with an increased risk of congenital anomalies or other adverse reproductive outcomes in offspring. All these findings indicate that pentachlorophenol could disrupt the normal function of endocrine system, but to con-firm that pentachlorophenol is one kind of endocrine disruptor and understand thoroughly the mechanism of PCP on endocrine system, more studies are needed.
3. Cost-effectiveness analysis of diagnosis and treatment of airway hyperresponsiveness in children after mycoplasma pneumonia

Objective To compare the cost-effectiveness of montelukast and salbutamal for the treatment of children with airway hyperresponsiveness (AHR) after mycoplasma pneumoniae(MPP). Methods A total of 374 children with pneumonia during Oct. 2007 to Oct. 2008 were observed, and 185 of them had mycoplasma infection (four-fold or greater rise by paired serum sample or higher than 1 : 160 by a single serum sample). Of the 185 patients 118 complicated with AHR were included for cost-effectiveness analysis. They were divided into three groups, namely, routine treatment (group A), routine treatment plus salbutamal (group B), routine treatment plus montelukast (group C). The incidence, duration and alleviating time of stimulating cough and hospital stay were observed in the three groups. The peak expiratory flow (PEF) was also determined. Cost- effectiveness analysis was performed using the costs of the first class second level hospital in Shanghai. Results The incidence of AHR was 63. 8% in children with mycoplasma infection pneumonia in our group. Montelukast and salbutamal shortened the course of stimulating cough compared with group A. Cost-effectiveness analysis revealed that the cost of salbutamal was less than that of montelukast, with the cost-effectiveness ratio being 0. 86(P<0. 05). Conclusion Salbutamal is more cost-effective than montelukast for MPP children without other complications, which is a valuable information for clinicians.

5.Application of qualitative interviews in inheritance research of famous old traditional Chinese medicine doctors: ideas and experience.

The inheritance of famous old traditional Chinese medicine (TCM) doctors plays an essential role in the fields of TCM research. Qualitative interviews allow for subjectivity and individuality within clinical experience as well as academic ideas of doctors, making it a potential appropriate research method for inheritance of famous old TCM doctors. We summarized current situations of inheritance research on famous old TCM doctors, and then discussed the feasibility of applying qualitative interviews in inheritance of famous old TCM doctors. By combining our experience in research on inheritance of famous old TCM doctors, we gave some advice on study design, interview implementation, data transcription and analyses , and report writing, providing a reference for further relevant research.

6.Efficacy of the Rehabilitation Strategy for Inpatients with Schizophrenia

Objective:To explore efficacy of the rehabilitative strategy in hospitalized schizophrenics.Method:Sixty-four schizophrenic patients were randomly divided into the intervention group and the control group.Both groups received the anti-psychotics therapy, but the teamwork education,medication management skills training module, symptom management skills training module were only given to the intervention group for ten weeks. And a one-year follow-up was carried out after their discharge from hospital. All subjects were evaluated with the Positive and Negative Syndrome Scale (PANSS), Nurses` Observation Scale for Inpatient Evaluation-30 (NOSIE-30), Insight and the Attitude to Treatment Scale, Social Disability Screening Schedule (SDSS), Morning Side Scale. We also recorded the relapse rate, reemployment rate, and re-hospitalization rate. Result:The clinical outcome of the intervention group was significantly superior to the control group on overall improvement according to PANSS (31.17?3.13 vs 52.34?13.02,7.34?1.08 vs 12.36?4.28,7.76?2.13 vs 13.26?4.50,16.13?1.82 vs 32.17?9.82 ),increase of active factor score of NOSIE-30 (-18.3?3.2 vs 14.4?6.7),and the reduction of inactive factor score of NOSIE-30(9.4?6.2 vs -22.3?7.4).The relapse rate(9.4% vs 54.8%),re-hospitalization rate(3.2% vs 42%),reemployment rate(37.5% vs 9.6%)were also better in the intervention group.Conclusion:The effect of rehabilitative strategy for schizophrenic patients not only improves the symptom, but also their social function.
7.Role of Video-assisted Thoracic Surgery in management of Penetrating Thoracoabdominal Injuries

Objective To evaluate the value of video-assisted thoracic surgery(VATS)for penetrating thoracoabdominal injuries.Methods Under general anaesthesia and double-lumen tracheal intubation,VATS was carried out to examine patients with chest injuries.Those who had mild injuries received VATS combined with mini-incision surgery.Electrocoagulation or suturing was employed to control intraoperative bleeding,and then blood clots were removed.Injured diaphragm muscles were repaired,and organs with hemorrhage or ruptures were sutured or resected.The seriously injured cases underwent open surgery and intra-abdominal exploration instead.Results A total of 18 patients received the operations.Among them,15 patients underwent VATS combined with mini-incision surgery(repair of the diaphragm was performed on 15,controlling intercostal arterial bleeding on 8,repair of the lung on 2,wedge resection of the lung on 2,and coagulated hemothorax removal on 2),the other 3 were converted to open surgery for repairing the diaphragm(3),heart(1),or esophagus(1),or lobectomy(2).Abdominal surgery via the thorax were performed on 13 cases,including repair of the liver in 1,lienectomy in 1,gastrorrhaphy in 1,repair of diaphragmatic hernia in 3,and intra-abdominal exploration in 7.Five patients received open surgery(pancreatic neoplasty in 1,repair of the liver in 1,lienectomy in 1,gastrorrhaphy in 2,and repair of the caul and mesenterium in 3).One patient who had multi-organ penetrating injuries died of hemorrhagic shock,circulatory failure,and DIC after open thoracic and abdominal surgeries.The mean operation time in this series was(125?44)minutes(ranged from 45 to 220),and the mean blood loss was(1089?582)ml(500 to 10 000 ml).Twenty-four hours after the operation,the mean volume of chest drainage was(234?75)ml(100 to 350 ml)in the first 24 hours after the operation.The chest drainage tube was withdrawn 2.5(2 to 5)days postoperation.The patients expelled gas in 1 to 4 days(mean,2 days).Totally,12 patients were followed up for 3 to 12 months(mean,6 months),during which none of them had trauma-related complications.Conclusion VATS combined with mini-incision surgery is safe and effective for patients with thoracoabdominal injuries,if the cases were carefully selected.

9.Cefepime-induced Neurotoxicity:Literature Analysis of 46 Cases

OBJECTIVE:To analyze the clinical characteristics of cefepime-induced neurotoxicity and to provide reference for rational use of cefepime.METHODS:Medical records of cefepime-induced neurotoxicity were collected from medical literatures from 1997 to 2009.Age and gender of 46 patients,primary diseases,drug usage,onset time of ADR,clinical symptom and prognosis were analyzed.RESULTS:Of 46 patients,25 cases were male and 21 female.Their average age was (59.56?18.84) years old (unknown age of 1 case).38 cases received cefepime alone and 8 cases cefepime combined with other drugs.All patients were administered by intravenous infusion.Of 46 patients,32 patients suffered from chronic renal failure,among which 10 patients received overdosage of medicine (unknown dosage of 1 case).Clinical symptoms of cefepime-induced neurotoxicity were encephalopathy,myoclonus and epilepsy.The occurrence time of neurotoxicity was (4.39?2.18)d (unknown occurrence time of 2 cases).The symptoms of 36 cases were relieved and cured after drug withdrawal and treatment with mean cure duration of (2.67?2.64)d.CONCLUSION:Great importance should be attached to the antibacterial effects of cefepime and cefepime-induced neurotoxicity.
