1.Advances in Study on Intestinal Apical Sodium-dependent Bile Acid Transporter and Related Diseases

Bile acids play critical roles in the solubilization and absorption of lipids. The ileal apical sodium-dependent bile acid transporter( ASBT)located at the enterocyte brush border is responsible for the reuptake of bile acids and the maintenance of bile acid homeostasis. Recently,great success has been made in understanding the relationship between ASBT and intestinal inflammation,tumorigenesis,secretion,motility,sensation,gut microbiota,and gut-liver axis in addition to its expression regulation,which implicates ASBT as a contributor of some gastrointestinal diseases and a promising new therapeutic target for these diseases. In this review article,the advances in study on above-mentioned issues were summarized.
2.Non-genetic mechanisms of diabetic nephropathy

Diabetic nephropathy (DN) is one of the most common microvascular complications in diabetes mellitus patients and is characterized by thickened glomerular basement membrane,increased extracellular matrix formation,and podocyte loss.These phenomena lead to proteinuria and altered glomerular filtration rate,that is,the rate initially increases but progressively decreases.DN has become the leading cause of end-stage renal disease.Its prevalence shows a rapid growth trend and causes heavy social and economic burden in many countries.However,this disease is multifactorial,and its mechanism is poorly understood due to the complex pathogenesis of DN.In this review,we highlight the new molecular insights about the pathogenesis of DN from the aspects of immune inflammation response,epithelial-mesenchymal transition,apoptosis and mitochondrial damage,epigenetics,and podocyte-endothelial communication.This work offers groundwork for understanding the initiation and progression of DN,as well as provides ideas for developing new prevention and treatment measures.
3.The Karamay community alcohol abuse and self-rated health status survey

ObjectiveTo investigate the health status and incidence of alcohol abuse of Xinjiang community residents.MethodsThrough multi-stage random sampling,1992 community residents ( 15-65years old) were assessed using socio-demographic information questionnaire,alcohol use disorders identification test (AUDIT) and self-rated health measurement scale-prior test(SHRMS).Single factor analysis and rank sum test were used to identify the incidence of alcohol abuse,the distribution of different populations,and the self-rated health status of abusers.ResultsTotally 298 cases of alcohol abuse were identified,and accordingly,the prevalence rate of alcohol abuse for general population was 15.1% ( male 26.4%,female 4.8% ).Key factors for alcohol abuse included male,middle-aged,well-educated,at work,administrative work,high income levels and large number of household.The differences among groups were statistical significant.Presumably the workplace population was at high risk of alcohol abuse.Alcohol abusers were significantly worse than general population in self-rated health status(P < 0.01 ),especially in mental health and social health.ConclusionThe incidence of alcohol abuse is relatively high in Xinjiang rcgion,and the problem need more attention.Further survey and early intervention work for highrisk group are advised to achieve the purpose of prevention and control.

From the aspects of strengths,weaknesses,opportunities and threats,this paper analyzed external factors of the reorganizations and group building reform of public hospitals in Shanghai.Points made are as follows:Characteristics of individual hospitals should he respected instead of a generalized pattern for all;the role of the government to play in this regard should he guidance of an orderly competition,Strengthen Supervision,and promotion of the reform in property rights,These efforts will create a desirable environment for hospital groups;mid- and long-term goals for the reform should consolidate the independent legal entity status of such groups.
5.Expression of HIF-1? in rat's radiation-induced oral mucositis

AIM: To detect the mRNA and protein level of HIF-1 alpha in the tissues of rat's radiated mucosa. METHODS: The left buccal mucosa was irradiated and excised. The right buccal mucosa was excised to serve as own control tissue. The mRNA of HIF-1 alpha was determined by using the semi quantitative RT-PCR. SABC method was employed to immunostain and to elucidate the localization, intensity and distribution of HIF-1 alpha protein.RESULTS: A Sprague-Dawley rat's model of radiation-induced oral mucositis (ROM) was successfully established. The results of RT-PCR indicated that the left buccal mucosa expressed HIF-1 alpha mRNA while the right buccal mucosa did not or seldom expressed it. Immunohistochemical analysis of HIF-1 alpha demonstrated that the left side mucosa expressed HIF-1 alpha protein.CONCLUSION: The mucosa of ROM expresses the mRNA and protein of HIF-1 alpha. The expressions of HIF-1 alpha are correlated with the severity of ROM.
6.Gastrointestinal Microecology and Postoperative Diarrhea after Gastrointestinal Tumor Surgery

As a common postoperative complication after gastrointestinal tumor surgery, the diarrhea adversely affects the recovery and the life quality of the patients. The occurrence of diarrhea is closely related to the operations and the perioperative treatments. Meanwhile,the gastrointestinal microecological disorders are found to be involved. In this article,the author will make a systemic review on the related factors and their treatments of the postoperative diarrhea after gastrointestinal tumor surgery, and mainly discuss the microecological issues involved in these processes.
7. The diagnostic performance of version 2018 LI-RADS for hepatocellular carcinoma on Gd-EOB-DTPA enhanced MRI

Objective:
To explore the diagnostic performance of the most recent 2018 version of liver reporting and data system (LI-RADS) on gadolinium-ethoxybenzyl-diethylenetriamine pentaacetic acid (Gd-EOB-DTPA) enhanced MRI to diagnose hepatocellular carcinoma (HCC) in high-risk patients.
Methods:
From July 2015 to September 2018, 130 consecutive high-risk patients with 134 focal liver lesions were retrospectively enrolled in our center and underwent Gd-EOB-DTPA-enhanced MRI and subsequent hepatectomy within 1 month. Two independent radiologists blindly reviewed the preoperative MR images of all patients, and determined the presence of major features, ancillary features and the LI-RADS categories according to the version 2018 LI-RADS of each liver observation. The sensitivity, specificity, positive predictive value, negative predictive value, Youden index and accuracy of the 2018 version of LI-RADS were evaluated with postoperative histopathological results as references. The inter-observer agreement between the two radiologists was tested by Kappa analysis.

These measures of LR?4+LR?5 were 91.8% (85/97), 96.9% (94/97); 67.6% (25/37), 67.6% (25/37); 0.605 9, 0.644 6; 82.1% (110/134), 88.8% (119/134), respectively. Conclusion Version 2018 LI?RADS demonstrated high sensitivity and accuracy to diagnosis HCC in high?risk patients on Gd?EOB?DTPA enhanced MRI.
9.Qualitative studies on self-management experience of patients undergoing enterostomy: a Meta-synthesis

Objective:To systematically evaluate the qualitative research on self-management experience of enterostomy patients at home and abroad, comprehensively understand the self-management behavior of enterostomy patients, so as to assist patients in managing stomas and provide evidence-based basis for further promoting self-management of enterostomy patients.Methods:Qualitative studies on the self-management experience of patients undergoing enterostomy were systematically searched in Cochrane Library, Joanna Briggs Institute (JBI) Evidence-Based Healthcare Center in Australia, CINAHL, PubMed, Web of Science, Scopus, EBSCO, China Biology Medicine disc, China National Knowledge Infrastructure (CNKI), VIP and WanFang Data. The search period was from database establishment to November 30, 2021. The quality of article was evaluated using the quality evaluation criteria for qualitative study of the Australian JBI Evidence-Based Healthcare Center. The results were integrated using an aggregative integration method.Results:A total of 11 articles were included and 37 clear and comprehensive research results were extracted. The similar results were categorized into 8 new categories and merged into 3 integrated results, including many misconceptions and obstacles in self-management of daily life after discharge, desire for social support systems and continuous nursing guidance, and a lot of negative emotions were generated, showing different levels of psychological stress, a few patients could actively cope.Conclusions:Society, healthcare institutions, and patient families should pay sufficient attention to the self-management of patients undergoing enterostomy, and pay attention to the negative psychological reactions such as fear, inferiority, guilt, and social self-isolation reactions they may experience during the self-management. In response to the problems encountered by patients in their daily lives, as well as the confusion they face in terms of work and social limitations, we should provide guidance and support to help stoma patients improve their self-management experience.
10.An Improved Total RNA Extraction Method for White Jelly Mushroom Tremella fuciformis Rich in Polysaccharides.

An improved method for extracting high quality and quantity RNA from a jelly mushroom and a dimorphic fungus—Tremella fuciformis which is especially rich in polysaccharides, is described. RNA was extracted from T. fuciformis mycelium M1332 and its parental monokaryotic yeast-like cells Y13 and Y32. The A260/280 and A260/230 ratios were both approximately 2, and the RNA integrity number was larger than 8.9. The yields of RNA were between 108 and 213 µg/g fresh wt. Downstream molecular applications including reverse transcriptional PCR and quantitative real-time PCR were also performed. This protocol is reliable and may be widely applicable for total RNA extraction from other jelly mushrooms or filamentous fungi rich in polysaccharides.
