1.Treatment of periamullary diverticula complicated with biliopancreatic disease

Objective To evaluate the curative methods of periampullary diverticula(PAD) complicated with biliopancreatic disease.Methods The clinical data of 32 cases of PAD complicated with biliopancreatic disease treated by surgical or endoscopic measures were retrospectively analyzed.Results Of the 32 cases,16 cases were treated with gastroectomy and Roux-en-Y gastrojejunostomy,and 8 with gastroectomy and Billroth Ⅱ gastrojejunostomy,8 with endoscopic sphincterotomy (EST).Postoperative complication occured in 4 cases,but no operative death.Twenty-seven cases were followed up for 5 months-8 years, and the results were excellent in 20 cases, good in 7 cases.Conclusions Gastroectomy and Roux-en-Y gastrojejunostomy is a better surgical method for PAD complicated with biliopancreatic disease.EST has become a new effective measure to deal with the diseases;It will be safe to use the first method for patients with intradiverticular papilla if it treated by EST.
2.Practical Value and Imaging Expression of Target Reconstruction Technique of HRCT in the Diagnosis of Pneumoconiosis

Objective:To study the practical value and imaging expression of target reconstruction technique of HRCT in the diagnosis of pneumoconiosis.Methods:Fifty-seven cases with different level type of pneumoconiosis were first examined by routine CT scan,followed by HRCT examination at top of aortic arch,carina of trachea,3 cm below bifurcation of bronchi,and 2 cm above thd right diaphragm.Target reconstruction have been finished using the raw data stored.Results:Target reconstruction photoimages were cleared to display the morphology of delicate lesions of pneumoconiosis:(1)Nodular shape(100%).(2)Interlobular septal thickening(98.24%).(3)Lobular structure abnormality(98.24%).(4)Subpleural nodes (84.21%).(5)Centrilobular emphysema(63.15%).(6)Subpleural line(64.91%).(7)Bronchiectasis(63.15%).(8)Ground-glass opacity(35.08%).(9)Honeycombing(15.78%),and to display earlier period change details.Conclusion:HRCT target reconstruction technique may make lesion dispaly more detail than comventional and high-resblution CT.It is reliable to display the delicate lesions of pneumoconiosis and provide more diagnostic information.
3.Solitary Pulmonary Nodule:Dynamic Spiral CT Scan of Thin Collimation with Pathology Study

Objective To evaluate the diagnosis and differentiation of solitary pulmonary nodule (SPN) using dynamic spiral CT scan of thin collimation.Methods The thin collimation no-enhanced CT scan and contrast enhanced scan in 30 seconds,1 minute,1 minutes,2 minutes,5 minutes,10 minutes,and 15 minutes after administration of media 100 ml were performed in 38 cases. Results The mean enhanced CT numbers of lung cancer and inflammatory pseudotumor were much higher than that of tuberculosis(TB) and hamartoma and statistically significant in different time of enhancement;20 HU was the threshold for a positive test,the sensitivity was 100% and the specificity was 96%.In time-attenuation curve analysis,lung cancer reached peak enhancement about 2 minutes,inflammatory pseudotumor in 5 minutes and keep longer enhanced time than that of lung cancer.No marked enhancement in SPN of TB and harmatoma,but ring-shaped enhancement can be seen in some of TB.More valuable imaging signs were found with thin collimation scan and more accurate to measure the CT numbers than traditional scan.Conclusion Dynamic spiral CT scan of thin collimation is a very valuable method for diagnosis and differentiation of SPN.
4.Analysis of the anxiety and depression in patients with gallbladder polyps, gallbladder stones and post-cholecystectomy

Objective:To investigate the anxiety and depression in patients with gallbladder polyps, gallbladder stones and post-cholecystectomy.Methods:From December 2020 to August 2021, consecutive patients who visited the Department of Gastroenterology, First Affiliated Hospital of Air Force Medical University, were enrolled and divided into normal gallbladder group, gallbladder polyp group, gallbladder stone group and post-cholecystectomy group, with 100 patients in each group. Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were used to evaluate patients′ anxiety or depression. Quality of life, sleep quality and severity of gastrointestinal symptom were assessed by short form 36 (SF-36), Pittsburgh sleep quality index (PSQI) scale and gastrointestinal symptom rating scale (GSRS) respectively. The incidence of anxiety or depression of patients in the 4 groups were compared, and the related risk factors of anxiety and depression were analyzed. Chi-square test and multiple logistic regression were used for statistical analysis.Results:Among 400 patients, 89 patients (22.2%) suffered from anxiety; and the incidences of anxiety of normal gallbladder group, gallbladder polyp group, gallbladder stone group and post-cholecystectomy group were 9.0% (9/100), 19.0% (19/100), 25.0% (25/100) and 36.0% (36/100) respectively. There was statistically significant difference in the incidence of anxiety among the 4 groups ( χ2=22.12, P<0.001). The incidence rates of anxiety of the gallbladder polyp group, gallbladder stone group and post-cholecystectomy group were all higher than that of the normal gallbladder group, and the differences were all statistically significant ( χ2=4.15, 9.07 and 20.90, P=0.042, 0.003 and <0.001). The incidence of anxiety of post-cholecystectomy group was higher than that of gallbladder polyp group, and the difference was statistically significant ( χ2=7.25, P=0.007). Among the 400 patients, 70 patients (17.5%) suffered from depression. The incidence rates of depression of the normal gallbladder group, gallbladder polyp group, gallbladder stone group and post-cholecystectomy group were 5.0% (5/100), 10.0% (10/100), 24.0% (24/100) and 31.0% (31/100) respectively. There was statistically significant difference in the incidence of depression among the 4 groups ( χ2=30.27, P<0.001). The incidences of depression of the gallbladder stone group and post-cholecystectomy group were both higher than that of the normal gallbladder group and gallbladder polyp group, and the differences were statistically significant ( χ2=14.56, 22.90, 6.94 and 13.53, P<0.001, <0.001, =0.008 and <0.001). The results of multivariate analysis showed that history of cholecystectomy, history of coronary heart disease, consumption of overnight food ≥3 times per week, PSQI score ≥ 8 and GSRS score ≥9 were independent risk factors for anxiety ( OR, 95%confidence interval: 4.02, 1.60 to 10.13; 10.01, 2.15 to 46.68; 4.15, 1.87 to 9.19; 4.69, 2.58 to 8.52; 3.02, 1.64 to 5.58; P=0.003, 0.003, <0.001, <0.001, and <0.001). Gallbladder stones history of cholecystectomy, consumption of overnight food ≥3 times per week, consumption of fresh fruit < 200 g/d, PSQI score ≥8 and GSRS score ≥9 were independent risk factors for depression ( OR, 95%confidence interval: 4.40, 1.38 to 14.06; 4.97, 1.54 to 16.12; 4.23, 1.61 to 11.08; 3.78, 1.32 to 10.85; 5.59, 2.78 to 11.27, 4.19, 1.94 to 9.04; P=0.012, 0.007, 0.003, 0.014, <0.001 and <0.001). Conclusions:The incidences of anxiety and depression in patients with gallbladder stones or post-cholecystectomy are higher than that of people with normal gallbladder. A history of cholecystectomy is the independent risk factor for anxiety and depression. Gallbladder stones is the independent risk factor of depression. Clinical attention should be paid to the mental state of patients with gallbladder polyps, gallbladder stones, especially post-cholecystectomy.
5.Influence of Helicobacter pylori infection on anxiety and depression in patients with chronic gastritis

Objective:To evaluate the influence of Helicobacter pylori ( H. pylori) infection on anxiety and depression in patients with chronic gastritis. Methods:From December 1 2020 to June 30 2021, 387 patients with chronic gastritis who visited the outpatient Department of Gastroenterology, the First Hospital Affiliated to Air Force Medical University were continuously recruited. According to the status of current H. pylori infection, the patients were divided into H. pylori uninfected group and H. pylori infected group. The general demographic information of patients was collected. Hamilton anxiety scale, Hamilton depression rating scale-24, Pittsburgh sleep quality index (PSQI) and gastrointestinal symptom rating scale (GSRS) were filled in. The detection rates of anxiety and depression were compared between the H. pylori uninfected group and the H. pylori infected group according to demographic characteristics. Chi-square test and multiple logistic regression analysis were used for statistical analysis. Results:Finally, 360 patients with chronic gastritis were enrolled, including 200 patients in H. pylori uninfected group and 160 patients in H. pylori infected group. The detection rates of anxiety and depression of the H. pylori infected group were both higher than those of the H. pylori uninfected group (48.1%, 77/160 vs. 30.0%, 60/200; 25.0%, 40/160 vs. 12.5%, 25/200), and the differences were statistically significant ( χ2=12.39 and 9.39, P<0.001 and=0.002). The detection rate of anxiety of male patients in the H. pylori infected group was higher than that in the H. pylori uninfected group (45.1%, 32/71 vs. 24.5%, 27/110); the detection rate of depression of female patients in the H. pylori infected group was higher than that in the H. pylori uninfected group (30.3%, 27/89 vs. 11.1%, 10/90), and the differences were statistically significant ( χ2=8.27 and 10.09, P=0.004 and 0.001). The detection rates of anxiety and depression of patients less than 48 years old in the H. pylori infected group were both higher than those in the H. pylori uninfected group (46.2%, 37/80 vs. 21.9%, 21/96; 20.0%, 16/80 vs. 7.3%, 7/96), and the differences were statistically significant ( χ2=11.73 and 6.20, P=0.001 and 0.013). The detection rates of anxiety and depression of the patients with high school education and below in the H. pylori infected group were higher than those in the H. pylori uninfected group (56.5%, 48/85 vs. 31.7%, 38/120; 32.9%, 28/85 vs. 14.2%, 17/120), and the differences were statistically significant ( χ2=12.57 and 10.24, P<0.001 and =0.001). The results of multivariate analysis showed that H. pylori infection, history of hypertension, PSQI score ≥8, GSRS score ≥7, chronic superficial gastritis and chronic atrophic gastritis were independent risk factors of anxiety in patients with chronic gastritis( P<0.001, =0.013, =0.001, <0.001, =0.036, =0.021), and the risk of anxiety of patients with H. pylori infection was 2.509 times as much as that in uninfected patients (95% confidence interval 1.512 to 4.163). H. pylori infection, PSQI score ≥8, GSRS score≥7, and having overnight dish ≥3 times per week all were independent risk factors of depression in patients with chronic gastritis( P=0.004, =0.002, <0.001, =0.001). The risk of depression in patients with H. pylori infection was 2.563 times as much as that in uninfected patients (95% confidence interval 1.356 to 4.846). Conclusion:H. pylori infection is correlated to anxiety and depression in patients with chronic gastritis, and it is an independent risk factor of anxiety and depression in patients with chronic gastritis.
6.CT quantitative study of coal miner's pneumoconiosis.

OBJECTIVETo study the value of CT quantitativeness in the diagnosis of coal miner's pneumoconiosis.
METHODS104 cases were examined by HRCT scan at top of aortic arc, carina of trachea, 3 cm below the bifurcation of bronchi, among them there were 87 patients with different stages of coal miner's pneumoconiosis, 17 cases of normal males as the control group. All images were determined by CT density histogram at specific region (- 1,024-0 HU). Calculated the percentage of each pixel included a varying number of CT value, and the ratio of density values in the specific region.
RESULTSThe ratio of density values in the region of -983 (-) -778 HU was 87.31% in normal control group, and 80.51%, 75.27% and 72.99% respectively in the I, II, III stages of coal miner's pneumoconiosis. There were statistically significant differences among the groups (P < 0.01).
CONCLUSIONCT quantitative histogram information was able to observe the fibrosis and its degree of coal miner's pneumoconiosis. It has a good diagnostic value for its reliability and objectiveness.

7.Clinical application of Guo's massage and tendon manipulation system in the treatment of chronic lateral ankle instability

Objective:To analyze the applicability of Guo's massage system for tendon management in the treatment of chronic lateral ankle instability (CLAI).Methods:Randomized controlled trial design. A total of 82 CLAI patients in our hospital from January 2021 to January 2022 were selected as observation subjects and divided into two groups using a random number table method, with 41 patients in each group. The control group received ankle rehabilitation exercise (3 times for 1 course, 6 courses), while the research group received Guo's massage and tendon manipulation therapy (3 times for 1 course, 6 courses). Before and after treatment, ankle joint range of motion and Y-balance tests were performed. The American Association of Foot and Ankle Surgeons (AOFAS) ankle posterior foot function score was used to evaluate ankle joint function, the VAS scale was used to evaluate patient pain, and the Foot Ankle Energy Scale (FAAM) was used to evaluate the degree of ankle joint function damage and evaluate clinical efficacy.Results:The total effective rate was 95.12%(39/41) in the research group and that in the control group was 78.05%(32/41) ( χ2=5.15, P=0.023). The range of motion of plantar flexion, range of dorsiflexion, varus range of motion, valgus range of motion, anterior extension distance, posterior inward extension distance, and posterior outward extension distance increased in the research group after treatment were significantly higher than those in the control group ( t=11.32, 14.02, 8.70, 18.09, P<0.01). Forward, rear Inner, and rear outer extension distances of the ankle Y-balance test in the research group were significantly higher than those of the control group. The AOFAS score in the research group was significantly higher than that of the control group ( t=5.94, P<0.01), VAS score was significantly lower than that of the control group ( t=13.38, P<0.01). The daily activity score and exercise score in FAAM in the research group were significantly higher than those in the control group ( t=9.14, 14.99, P<0.01). Conclusion:The use of Guo's massage system for tendon management can improve the ankle function of CLAI patients, reduce their pain level, and promote the recovery of the patients.
8. Research Progress of High-dose Dual Therapy in Eradication of Helicobacter pylori Infection

In the era of increasing of antibiotic resistance, Helicobacter pylori (Hp) eradication rates of traditional triple and quadruple therapy are gradually declining. High-dose dual therapy (HDDT) containing proton pump inhibitors and amoxicillin may be a new breakthrough in eradicating Hp infection. Current research shows that HDDT, whether used as a first-line regimen or a rescue regimen, has a high eradication rate and a low incidence of adverse reactions. It has the potential to become a new first-line eradication therapy or rescue therapy for Hp infection in clinical practice. This article reviewed the research progress of HDDT in eradication of Hp infection.
9.Effect of obesity on the efficacy of allogeneic hematopoietic stem cell transplantation

Objective:To investigate the effect of obesity on the efficacy of allogeneic hematopoietic stem cell transplantation.Methods:The clinical data of 81 patients who underwent allogeneic hematopoietic stem cell transplantation from August 2017 to September 2020 in the First Affiliated Hospital of Nanjing Medical University were retrospectively analyzed. According to the body mass index (BMI), the patients were divided into the obese group (BMI≥28 kg/m 2, 11 cases) and the non-obese group (BMI<28 kg/m 2, 70 cases). The clinicopathological characteristics, hematopoietic stem cell implantation, post-transplantation complications, survival and recurrence were compared between the two groups. Univariate and multivariate survival analyses were performed by using Cox proportional hazards regression model. Results:The median follow-up time of 81 patients was 280 d (8-1 218 d). The 1-year overall survival (OS) rate was 77.9%, and the 1-year progression-free survival (PFS) rate was 73.8%. The 1-year OS rates of the non-obese group and the obese group were 82.6% and 46.2% ( χ2 = 15.54, P<0.01), and the 1-year PFS rates were 82.1% and 36.4% ( χ2 = 15.56, P<0.01). The non-recurrence mortality (NRM) rates of the non-obese group and the obese group were 7.1% and 32.7% ( χ2 = 6.463, P = 0.01), and the cumulative recurrence rate was 11.5% and 42.9% ( χ2 = 8.146, P = 0.004). Between the non-obese group and the obese group, the median engraft time of neutrophils and platelets, acute graft-versus-host disease, chronic graft-versus-host disease, hemorrhagic cystitis, cytomegalovirus infection and Epstein-Barr virus infection had no statistical difference ( P > 0.05). The result of multivariate analysis showed that obesity was an independent adverse influencing factor for OS of patients with allogeneic hematopoietic stem cell transplantation ( HR = 3.814, 95% CI 1.343-10.827, P = 0.012). Conclusion:Obesity is an important unfavorable factor that affects patient's survival after allogeneic hematopoietic stem cell transplantation, and the improvement of the efficacy and survival of these patients is worthy of further study.
10.Clinical analysis of 81 cases of malignant lymphoma treated with autologous hematopoietic stem cell transplantation.

OBJECTIVETo investigated the curative effect of autologous hematopoietic stem cell transplantation (ASCT) for malignant lymphoma.
METHODSThe clinical data of 81 patients with malignant lymphoma received ASCT from April 1999 to October 2013 were retrospectively analyzed. Of 81 patients, 70 were non-Hodgkin's lymphoma (NHL) and 11 Hodgkin's lymphoma (HL). High dose of etoposide combined with G-CSF was used to mobilize peripheral hematopoietic stem cell. Preconditioning regimen was BEAM (carmustine + cytarabine + etoposide + melphalan).
RESULTSEnough peripheral blood stem cells were collected from all patients. All of the patients after transplantation achieved hematopoietic reconstitution, the median time of the absolute neutrophil count (ANC) recovery to >0.5×10⁹/L time was 10(7-16) d, and the median time of platelet count recovery to >20×10⁹/L was 10(6-17) d. With the follow-up of 23(2-139) months, progression free survival (PFS) was 72.7%, and overall survival (OS) was 88.6%. The median PFS and OS were not reached. Complete remission (CR) before ASCT was an independent prognostic factor of PFS. No transplant related death happened.
CONCLUSIONASCT was a safe and effective method for treatment of malignant lymphoma.
