1.Microsurgical resection of large pituitary adenomas via transeyebrow supraorbital keyhole approach

Objective To study the microsurgical excision of large and giant pituitary adenomas via transeyebrow supraorbital keyhole approach and discuss the advantages of this approach Methods Forty four patients were treated with microneurosurgery via transeyebrow supraorbital keyhole approach Transverse incision was made about 5 cm,include external two third of eyebrow The skull window was about 3 cm in diameter The operative space was between subdual and epiarachnoid Under the microscope,border of tumor and its capsule could be found and dissected step by step Tumor was removed piece by piece and could be removed totally The follow up survey after 6 month, includes MRI examination,endocrine changes,visual power and visual field Result Total excision of pituitary adenomas was achieved in 40 patients;2 patients subtotal excision,and large part was removed in another 2 patients There was no mortality in this group The clinical symptoms were improved in all patients Conclusion The transeyebrow supraorbital keyhole technique is a minimally invasive technique and can be used in excision of large and giant pituitary adenomas,it can reduce the operative injury and morbidity
2.Treatment of epidermoid cyst with endoscope assisted keyhole approach microneurosurgery

Objective To explore the methods and techniques of the treatment for epidermoid cyst with endoscope assisted keyhole approach microneurosurgery Methods Sixteen patients with epidermoid cyst were treated with endoscope assisted keyhole approach microneurosurgery Different keyhole approaches were selected according to the position of the tumors With microneurosurgery,larger part of tumor was resected,then under neuroendoscope remains of tumors was found out and removed Result After the tumor were removed with microneurosurgery,the remains of tumors in 13 cases (81 25%) were still found under endoscope,and were resected with endoscope;the remains of tumors in 3 cases were not found Total excision of tumor was achieved in 14 patients (87 5%);Two patients subtotal excision Fifteen cases (93 75%) with clinical symptom had an improvement or recovery 2 weeks operation One case occurred nor bacterial meningitis and recovery 2 weeks after treatment Hydrocephelar and second intracranial hematoma were not found in all patients Following up was carried out for 10 patients after 3 12 months Nine patients got a good result One patient with trigeminal neuralgia Conclusion Endoscope assisted keyhole approach microneurosurgery can increase the total resection rate for epidermoid cyst,reduce the trauma of operation and reaction after operation [Key words] Epidermoid cyst; Intracranial; Neuroendoscope; Keyhole; Microneurosurgery
3.Transsphenoidal expanded to reach the tumor in central part of the skull base by microsurgical anatomy

Objective To study the microsurgical anatomy of the extended transsphenoidal approach to the central skull base, and provide anatomical parameters for the surgical approach. Methods Forty wet skulls were dissected in detail via mimic extended transsphenoidal approaches under an operating microscope and an extended exposed operating microscope. Important structures and connections were observed, measured precisely and photographed. Dry skulls were used to observe and measure the osseous structures related to the approaches. Results The extended transsphenoidal approach can expand ahead, astern and laterally to the suprasellar intradural space by resecting bone and displaying the basafrontal lobe, pre-chaismatic space, optic chiasma cistern, anterior cerebral artery complex, the branches and cavernous sinus segment of the internal carotid artery, pons, basilar artery and its branches, posterior cerebral artery, and superior cerebellar artery. Conclusion The extended transsphenoidal approach can provide the new pathway for treating lesions of the central skull base.
4.Research on the measuring scope of skull base midline structure tumors in the mothod of single-nostril microscopy combined with neural endoscopy

Objective By combined application of endoscopic and microscopic vivisection-pronged approach,forms a three-dimensional morphological observation through focusing on an observation of structure of various signs, imaging features, morphology and measuring in vivo data, which are concerned with expansion of transsphenoidal approach. This will provide expansion of transsphenoidal approach with an intuitive morphological imagines,detailed anatomical parameters,accurate imaging data,viviperception and data measurement. Methods 80 patients were recruited for investigation, preoperative measurement of the size of the patient' s nose,height of nasal columella,the plane angle among bridge of nose,inner canthal of eyes and nose columella,and the length of bridge of nose.in the operation,measure the distance from nasal columella to aperture of sphenoidal sinus; observe the shape of aperture of sphenoidal sinus; measure the longest and the shortest distance between aperture of sphenoidal sinus; observe the shape of septum of sphenoidal sinus,three uplift and the relationship among them,as well as the color and texture of the tumor.Measure sphenoid sinus wall,screening room,sellar floor,extent of slope resection,operating space,as well as observation & study of selection and repairing of skull base reconstruction materials. Results The shape according to the discretion of the column is divided into butterfly saddle nose, normal nose, eagle nose and acromegaly hypertrophy nose.The nostrils shape is kidney form,ball form,snow humanoid formand triangle form.The butterfly saddle biggest digging distance:between the cavernous sinus is( 18.9±2.51 ) mm,from saddle nodules to slope is(19.2 ±2.67) mm.After tumor resection,the transsphenoidal observation space is forward to former group of ethmoid sinus,back to the slope. Conclusion The measurement data might be benefit for selecting of speculum, evalution of placing depth on speculum and operation position and sellar floor opening range. It would also make it possible for a safer, minimally invasive and effective surgery by avoid damage to the internal carotid artery,cavernous sinus or cranial nerve.
5.The mechanism of alleviation of acute myocardial ischemia/reperfusion injury in rats by Fasudil

Objective To explore the potential mechanism of Fasudil hydrochloride pharmacologic postconditioning that alleviated acute myocardial ischemia/reperfusion injury (MI/RI) in rats,narrowed the scope of infacted myocardium,and inhibited cell apoptosis.Methods Ninety rats were randomly and averagely divided into 5 groups:sham group,ischemia/reperfusion (I/R) group,ischemic postconditioning (IPost) group,fasudil hydrochloride (FH) group,and FH and PI3K inhibitor (LY294002) (FH + I) group.The expressions of Rho associated coiled-coil forming protein kinase-1 (ROCK-1),Bcl-2,Bax,Caspase-3,Akt,and P-Akt in rat myocardial cells were determined.Results Compared to I/R group (ROCK1:2.94 ± 0.13),ROCK1 expression was not changed in IPost group (ROCK1:2.79 ± 0.11),FH group (ROCK1:2.83 ± 0.10),and FH + I group (ROCK1:2.85 ± 0.26).Compared to I/R group (Bcl-2:1.11 ± 0.12,P-Akt:1.09 ± 0.06,Bax:1.74 ± 0.06,and caspase-3:1.32 ± 0.12),the expressions of Bcl-2 and P-Akt in FH group (Bcl-2:1.76 ± 0.08,and P-Akt:1.73 ± 0.05) and IPost group (Bcl-2:1.74 ± 0.06,and P-Akt:1.37 ± 0.05) were all significantly higher than those in I/R group (P < 0.05),while the expressions of Bax and caspase-3 in FH group (Bax:0.98 ±0.14,and caspase-3:0.97 ±0.06) and IPost group (Bax:0.97 ±0.11,and caspase-3:1.07 ±0.08) were all significantly lower than those in I/R group (P <0.05).Compared to FH group,the expressions of Bcl-2 and P-Akt in FH + I group (Bcl2:1.05 ±0.12,and P-Akt:1.21 ±0.06) were decreased (P <0.05),while the levels of Bax and caspase-3 (Bax:1.61 ±0.11,and caspase-3:1.42 ± 0.11) were increased (P < 0.05).Conclusions The mechanism of FH postconditioning was associated with the inhibition of ROCK activity,and the activation of PI3K-Akt pathway.
6.New Approach of Fundus Image Segmentation Evaluation Based on Topology Structure.

In view of the evaluation of fundus image segmentation, a new evaluation method was proposed to make up insufficiency of the traditional evaluation method which only considers the overlap of pixels and neglects topology structure of the retinal vessel. Mathematical morphology and thinning algorithm were used to obtain the retinal vascular topology structure. Then three features of retinal vessel, including mutual information, correlation coefficient and ratio of nodes, were calculated. The features of the thinned images taken as topology structure of blood vessel were used to evaluate retinal image segmentation. The manually-labeled images and their eroded ones of STARE database were used in the experiment. The result showed that these features, including mutual information, correlation coefficient and ratio of nodes, could be used to evaluate the segmentation quality of retinal vessel on fundus image through topology structure, and the algorithm was simple. The method is of significance to the supplement of traditional segmentation evaluation of retinal vessel on fundus image.

7.Non-contrast-enhanced MR Angiography of Hand:Preliminary Clinical Experience

Purpose To assess the clinical application of non-contrast-enhanced MR angiography (NCE-MRA) using flow sensitive dephasing (FSD) prepared steady-state free precession (SSFP) for displaying hand arteries of patients with rheumatoid arthritis. Materials and Methods Twenty-two patients with rheumatoid arthritis were recruited in this study. All the patients undertook hand NCE-MRA and three-dimensional dynamic CE-MRA on a 1.5T MR scanner. The informed consent was obtained from each subject. Image quality was assessed independently by two experienced radiologists at three arterial segments (wrist arteries, palm arteries, andfinger arteries) with a four-point scale. Signal to noise ratio (SNR), contrast to noise ratio (CNR), and vessel sharpness were evaluated by a magnetic resonance physicist. The results and image quality were statistically compared between the two MRA techniques.Results Twenty-two patients of 24 hands successfully underwent NCE-MRA and CE-MRA scan. Among 72 vascular segments, 69 segments of NCE-MRA were diagnostic, which was higher than that of CE-MRA (96% vs 83%,P<0.05). Otherwise, the image quality, SNR, CNR and vessel sharpness of NCE-MRA were all superior to those of CE-MRA (P<0.05).Conclusion NCE-MRA using FSD-prepared SSFP allows clear depiction of the hand arterial tree, and the image quality is superior to that of dynamic CE-MRA. It is a potential tool for evaluating the disease of hand arteries.
8.The expression of CD62p and cytokines in acute hemorrhage patients with systemic inflammatory response syndrome and the influence on prognosis

Objective To study the expression of eytokines in acute hemorrhage patients with systenlic in-flammatory response syndrome(SIRS)and multiple organ dysfunction syndrome(MODS).Methods Platelet acti-vation markers CD62p and cytokines in acute hemorrhage patients with SIRS and MODS were detected by using flow cytometry,and compared with that of control group.Results The expression of CD62p and cytokines(TNF-α and IL-6)in acute hemorrhage patients,with or without SIRS and MODS wag significantly higher than that in control group(P<0.01),and higher in MODS group than in SIRS group,and in MODS death group than in MODS survival group(P<0.01).Conclusion The activation of platelet and the over-expression of cytokines participate in the on-set and development of acute hemorrhage,SIRS and MODS,and were related with the severity of disease and progno-sis.
9.Clinicopathological analysis of HBV recurrence in post-liver transplantation patients

Objective To investigate the clinicopathological characteristics of HBV recurrence after liver transplantation. Methods The retrospective analysis of the clinicopathological changes was performed on 17 patients who had HBV recurrence after liver transplantation in our medical department. Results HBV recurrence happened from 4 to 48 months. Twelve of them which were identified to be YMDD mutation switched to entecavir or added adefovir. Three of them receiving chemotherapy when liver cancer recurred switched to entecavir. Two of them with withdrawal of lamivudine were given lamivudine continuously. Liver function returned to the normal level and HBVDNA was < 102 U/ml after anti-hepatitis B virus. The histological changes in the transplanted livers included hepatocellular degeneration, necrosis and apoptosis, portal infiltrations and fibrosis.With time after recurrence, it was easier to see hepatitis B virus replication in liver cells, incidence of acute rejection, increases of liver fibrosis and the formation of fibrous septa, even pseudolobule.Conclusion In native HBV infection livers, fibrosis occurs more early and develops rapidly. The number of virus is closely related to liver necrosis and inflammation. Early discovery and change to quick and effective treatment of anti-hepatitis B virus in time can improve greatly the prognosis of the patients.
10. Clinical features of imbalance between Th1 and Th22 cells and its association with disease progression in patients with liver cirrhosis

Objective:
To investigate the clinical features of imbalance between Th1 and Th22 cells and its association with disease progression in patients with liver cirrhosis, and to explore immune therapeutic strategies for targeted therapy for liver cirrhosis.
Methods:
In vitro peripheral blood mononucleated cells (PBMCs) were collected by centrifugation. CD3-BV500 and CD8-PerCP-Cy5.5 staining was performed for these cells. IFNγ-PE-Cy7, IL-17a-APC, IL-22-PE, or the corresponding isotype control was added, and then PBMCs were fixed with 1% polyoxymethylene after being washed once by permeabilization-wash buffer. Flowjo software was used for the analysis of T lymphocyte subsets and cytokines. Th1 (CD4+IFNγ+), Th17 (CD4+IL-17a+), Th22 (CD4+IL-22+), Tc1 (CD8+IFNγ+), Tc17 (CD8+IL-17a+), and Tc22 (CD8+IL-22+) subsets were defined and the secretions of interferon-γ (IFN-γ), interleukin-17a (IL-17a), and interleukin-22 (IL-22) were measured for all subsets. LX-2 cells were cultured in a serum-free medium and different concentrations of recombinant human IL-22 protein (25, 50, 100 ng/ml) were added; 24 hours later, the activation marker α-smooth muscle actin (α-SMA) was used to measure LX-2 activation. Fetal bovine serum with a volume fraction of 10% was used as a positive control. Enzyme-linked immunosorbent assay (chemiluminescence) was used to measure the concentrations of hyaluronic acid, type III precollagen, and type IV collagen in supernatant. A one-way analysis of variance, the non-parametric Mann-Whitney U test, and the non-parametric Kruskal-wallis H test were used for statistical analysis based on data type.
Results:
Compared with the health control group, the liver cirrhosis groups with various causes had significant increases in peripheral Tc1, Th17, and Th22 cells. The percentage of Th17 cells in the liver cirrhosis group was 1.64 times that in the control group (4.25%±2.45% vs 2.59%±1.36%,
