1.Effect of Chuankezhi Injection Combined with Chemotherapy on Quality of Life in Senile Patients with Non-small Cell Lung Cancer

Objective To observe the effect of Zusan Li acupoint injection with Chuankezhi injection ( CI) combined with chemotherapy on the quality of life in the stage III or IV senile patients with non-small cell lung cancer (NSCLC) . Methods We adopted research methods for the prospective, randomized, and controlled trial. A total of 62 qualified patients completed the treatment program, and were randomly divided into two groups. Thirty-two patients in the test group received CI combined with chemotherapy and syndrome differentiation treatment, and 30 patients in the control group were given chemotherapy and syndrome differentiation treatment. We used the WHO Quality of Life Older Module Scale ( WHOQOL-OLD) , Functional Assessment of Cancer Therapy-Lung ( FACT-L V4.0) and the Brief Fatigue Inventory ( BFI) to evaluate and analyze the quality of life before and after treatment. Results (1) In the WHOQOL-OLD evaluation, the total scores and the scores of dimensions of sensory abilities, autonomy, social participation, and past, present and future behavior were much improved in the test group after treatment ( P﹤0.01 compared with those before treatment) , while the control group only had statistically significant difference ( P﹤0.01) in the improvement of social participation and the total scores. The test group had higher scores difference in autonomy than the control group (P<0.01) . (2) In the FACT-L V4.0 evaluation, the total scores and the scores of domains of physiological condition, emotional status, functional status, and additional attention were much improved in the test group ( P﹤0.01 compared with those before treatment) , and the control group had an improvement in the total scores and scores of emotional status and additional attention after treatment ( P<0.01) . The test group had higher scores difference in physiological condition, functional status, and additional attention than the control group (P<0.01) . (3) In BFI evaluation, the degree of fatigue and fatigue influence were improved in both group after treatment ( P﹤0.01 compared with those before treatment) , and the improvement in the test group was superior to that in the control group ( P<0.05) . Conclusion Zusan Li acupoint injection with CI can improve the quality of life of senile NSCLC patients treated with chemotherapy in dimensions of sensory abilities, autonomy, social participation, and past, present and future behavior, and in domains of physiological conditions, functional status, additional concern, and it can also improve the degree of fatigue and fatigue influence.
2.Effectiveness and safety of Kang′ai injection combined with single agent chemotherapy in treatment of el-derly patients with advanced NSCLC

Objective To observe the effectiveness and safety of Kang′ai injection combined with docetaxel or gemcitabine chemotherapy in treatment of elderly patients with advanced non-small cell lung cancer(NSCLC). Methods A multi-center,randomized and parallel control study was carried out in 150 elderly patients with ad-vanced NSCLC treated in 3 hospitals during the period from June 2013 to August 2014.The patients were randomly divided into platinum-based group(platinum-based doublet chemotherapy)and Kang′ai group(Kang′ai injection combined with single agent chemotherapy). The clinical efficacy,quality of life and adverse reactions were ob-served and compared between the two groups. Results There was no significant difference in the disease control rate between the two groups group(79.66% vs 81.82%)(χ2=0.101,P=0.751)while the effective rate of Kang′ai group was significantly higher than that of platinum-based group(30.51% vs 14.29%)(χ2=5.240,P=0.022). The effective rate of clinical symptom relief in Kang′ai group after treatment was significantly higher than that in platinum-based group(69.49% vs 48.05%)(χ2=6.278,P=0.012),and the increase rate of PS score in Kang′ai group after the treatment was also significantly higher than that in platinum-based group(18.64% vs 3.90%)(χ2=7.868,P=0.005).There was no significant difference in quality of life between two groups(P>0.05).The quali-ty of life score of Kang′ai group was significantly higher than that of platinum-based group at the second course of treatment(P<0.05).The incidence of adverse reactions in Kang′ai group was significantly lower than that in the platinum-based group(47.46% vs 71.42%)(χ2= 8.070,P < 0.05). Conclusion Kang′ai injection combined with single agent chemotherapy can effectively relieve clinical symptoms and reduce the tumor size in elderly patients with advanced NSCLC,with high safety.
3.Analysis on TCM pulse diagnosis cited by Tanksuqnameh

By sorting out and analyzing the contents about pulse diagnosis in Tanksuqnameh, it was found that in terms of pulse theory, the book contains academic viewpoints of different periods and doctors about three portions and nine readings pulse method and nutritive qi operation law in Huang Di Nei Jing, the Cunkou pulse-taking method and the Cunguanchi theory in Nan Jing, the Renying and Qikou pulse-taking method and the three-region pulse-taking method in Mai Jing, and the seven superficies-indicating, eight interior-indicating and nine channels pulse of Mai Jue, etc.; in terms of pulse theory interpretation, multiple annotations from famous doctors are cited, and TCM basic theories and knowledge of astronomy and mathematics are applied, combined with the background of ancient Iranian medicine and local medical experience for explanation; in terms of pulse diagnosis techniques, pulse diagnosis techniques such as "three fingers determining three guan", "adjusting finger density", "floating and sinking pulse", "foot back pulse breaking life and death" were recorded in books of Nan Jing, Mai Jue, Lei Zheng Huo Ren Shu, etc.; in terms of influencing pulse factors, the male and female pulse, physical pulse, and four time pulse were recorded. Tanksuqnameh is a universal work of Persian Traditional Chinese Medicine, compiled by the author Rashid-ul-Din based on his research on Chinese culture and medical knowledge, combined with the unique customs, language and culture, and way of thinking in the Middle East region, to reorganize various pulse theories from before the Han Dynasty to the Tang and Song Dynasties in China. The publication of this book indicates that TCM pulse diagnosis had been promoted to the Middle East in the 14th century and provided valuable experience for local medical development. The author's sense of identification and research spirit towards different cultures provide a historical example for achieving cross-cultural communication between TCM and medicine in different regions. The preserved literature from the Tang and Song Dynasties in the book can provide clues for a deeper understanding of ancient pulse theory in TCM, but some details do not explain thoroughly or does not match the current clinical practice. Therefore, attention should be paid to distinguishing in research and application.
4.Cost-effectiveness analysis of enzalutamide in the treatment of metastatic prostate cancer

Objective To evaluate the cost-effectiveness of enzalutamide in the treatment of metastatic prostate cancer from the perspective of healthcare in China.Methods Based on the published phase Ⅲ randomized controlled trial(ENZAMET),the disease process of metastatic prostate cancer was classified into three states:progression-free survival,progression survival and death,and the model period was defined as 28 days,and the study period was lifelong,and a Markov model was established to evaluate the cost-effectiveness of the treatment of enzalutamide versus standard antiandrogen drugs in metastatic prostate cancer.Setting the willingness-to-pay(WTP)threshold at 3 times our 2022 gross domestic product(GDP)per capita and the robustness of the model analysis was verified by sensitivity analysis.Results Compared to the control group standard antiandrogen therapy,the incremental effect of enzalutamide was 0.92 quality-adjusted life years(QALYs),the incremental cost was 311 863.30 yuan,and the incremental cost-effectiveness ratio(ICER)was 338 981.85 yuan/QALY,which was higher than WTP threshold(257 094 yuan/QALY).The results of univariate sensitivity analyses showed that the total cost of the enzalutamide group,the PFS utility value,the cost of the PD status of enzalutamide group,and the unit price of enzalutamide had a greater impact on the model results.The results of the probabilistic sensitivity analysis suggested that the enzalutamide treatment regimen was not economical within the willingness-to-pay threshold of 3 times our 2022 GDP per capita.Conclusion Compared with the standard anti-androgen drugs,enzalutamide does not offer a cost-effectiveness advantage in the treatment of metastatic prostate cancer.
