1.Zhichuan decoction with glucocorticoid in treatment of bronchial asthma

Three hundred and ninety five patients with bronchial asthma were randomly divided into 4 groups:patients in group A received inhaled beclomethasone dipropionate(BDP)100μg and Zhichuan decoction;patients in group B were given inhaled BDP 200μg;patients in group C took Zhichuan decoction only;patients in group D were given inhaled BDP 100 μg.Methacholine airway reactivity,pulmonary ventilation function,serum acidophilia cationic protein and symptom score were determined before and after treatment.All above indicators were significantly improved in groups A and B,which suggests that Zhichuan decoction could reduce hormone dosage in control of airway chronic inflammation,reduction of airway reactivity and improvement of pulmonary ventilation function of patients with bronchial asthma.
2.Association study of the genetic polymorphism of D2S1338 and D19S433 loci and the initiative-aggressive behavior in male population

Objective To investigate the relationship of the initiative-aggressive behavior and D2S1338,D19S433 loci.Methods PCR and electrophoresis method were used to conduct genotype analysis on D2S1338 and D19S433 in the peripheral blood of 187 male initiative-aggressive violent offenders and 459 healthy men living in Jiangsu area.Results D2S1338 and D19S433 loci in initiative-aggressive behavior group and healthy group were found to coincide with Hardy-Weinberg equilibrium (P > 0.05).There were significant difference in locus D19S433 (P < 0.05)between initiative-aggressive behavior group and healthy group,but not on locus D2S1338 (P>0.05).Univariate analysis showed significant differences at allele 14.2 and genotype 14-14 on locus D19S433 between the two groups (P =0.0011,P =0.0008) with the OR values being 0.50 (95 % CI:0.33-0.76) and 3.49(95% CI:1.62-7.52),respectively.Conclusion Locus D19S433 may be related to with initiative-aggressive behavior with allele 14.2 being the resistant factor and genotype 14-14 being the susceptible factor.
3.Investigation on the disorders in calcium and phosphorus metabolism and abnormity in parathyroid function in the patients with maintenance hemodialysis

Objective To investigate disorders in calcium and phosphorus metabolism and abnormity in parathyroid function and their related factors in maintenance hemodialysis(MHD)patients.Methods We collected serum Hb,SCr,BUN,calcium,phosphorus,iPTH,high sensitivity C-reactive protein(hs-CRP),albumin and calculated albumin-corrected Ca and Kt/V from 198 patients with MHD in Jing'an District Central Hospital,Shanghai from Jan.2009 to Dec.2010.The calcium and phosphorus metabolism and parathyroid function were evaluated according to the guidelines of bone metabolism and controlling of bone disease in Kidney Disease Outcome Quality Initiative(KDOQI)recommended by National Kidney Foundation of the United States.198 patients were classified into the standard group(150-300 ng/L),lower than the standard group(<150 ng/L)and higher than the standard group(> 300 ng/L)according to serum iPTH levels.Results In the 198 cases,110 cases were normal in serum Ac-Ca(55.6%); 118 cases were normal in serum phosphorus (59.6%); 143 cases were normal in Ca × P(72.3%); 39 cases were normal in iPTH(19.7%).All four items up to the standard were 28 cases(14.1%)among 198 cases.However,132(66.7%)patients showed lower than 150 ng/L in serum iPTH.The average serum iPTH gradually decreased with age.The senior patients demonstrated the highest average serum Hs-CRP,the lowest average serum phosphorus and the lowest albumin.There were no statistical significance in sex,hemodialysis duration,diabetes,blood pressure,Hb,Bun,SCr,Ac-Ca and Kt/V among three groups.Conclusion The disorders in calcium and phosphorus metabolism and abnormity in parathyroid function is common in the MHD patients.We should pay attention to hypofunction of parathyroid gland in the patients with MHD,which may be related to aging,malnutrition and potential infection.These findings need further investigation.
4.Dynamic accumulations of bioactive components in different germplasm Isatis indigotica and comparative of its quality of medical material.

OBJECTIVETo study the dynamic accumulations of bioactive components in different germplasm Isatis indigotica and compare its quality of medical material, in order to provide a basis for breeding and high yield cultivation of I. indigotica.
METHODThe planting samples were collected during growth period, bioactive components in different germplasm Isatis indigotica were measured.
RESULTThe dynamic accumulations of bioactive components in different germplasm I. indigotica were consistently changed in a field experiment. The differences of bioactive components contents in medical material of I. indigotica were significant.
CONCLUSIONThe germplasm from Gansu Longxi showed a high yield and good quality characters in Fuyang area, and may be applied to production.

5.Contrast of growth characteristics of different germplasm and correlation between morphologic and individual yield of Isatis indigotica.

OBJECTIVETo study the growth characteristics of different germplasm and analyze the correlation between morphologic and individual yield of Isatis indigotica.
METHODThe planting samples were collected during the growth period, morphological characteristics and root yield per plant in 13 germplasm of I. indigotica from all over the country were determined by correlation and path analysis.
RESULTThe growth characteristics of various I. indigotica germplasm were consistent. The difference of morphological characteristics and root yield with various germplasm were significant. The root yield per plant was positively correlated to the main root length and the number of individual plant leaves.
CONCLUSIONThe number of leaves per plant and root length are selected for high-yield lines, meanwhile, leaf length and leaf diameter are important factors for breeding high-yield lines as well.

6.Effects of sowing date on morphologic characteristics, yield and quality of Radix isatidis.

OBJECTIVETo study the effects of sowing date on morphologic characteristics, yield and quality of Radix Isatidis, in order to provide a basis for selection of the suitable sowing date.
METHODThe experiment was carried out in a randomized complete block design arranged in split plot, the phonological period of Isatis indigotica was investigated and the morphological indexes, yield and quality at harvest were measured and determined by correlation and variance analysis.
RESULTThe different sowing dates showed a great effect on phonological period of I. indigotica. The biological characteristics, yield and active components of Radix Isatidis were negatively correlated with the sowing date, and the difference of morphological characteristics, yield and active compositions with sowing date was significant.
CONCLUSIONThe suitable sowing date of I. indigotica was during the period of time from late March to beginning of the April in Fuyang area, and considering the factors of rational rotation, the time cloud delayed until late May to the beginning of June.

Objective To study the histological classification,clinical stage,histological grade and prognosis of renal cell carcinoma by analyzing the records of the patients in Shanghai Renji hospital. Methods A consecutive series of 435 patients with renal cell carcinoma between 2003 and 2005derived from the renal cancer database were reviewed clinically and pathologically.The 1997 version of WHO histological classification for renal epithelial tumor,the 2002 version of AJCC clinical TNM staging system and the 1982 version of Fuhrmaffs system for nuclear grade were used.By survival analysis of 57 cases with advanced renal cell carcinoma using Kaplan-Meier method prognostic factors were confirmed using logrank test. Results Of a total 435 patients,cases were classified into 10(accounting for 2.4%of renal cell tumors)hereditary renal cancer in VHL disease,372(85.5%)clear cell renal cell carcinoma(CCRCC),13(3.0%)papillary renal cell carcinoma(PRCC),18(4.1%)chromophobe renal cell carcinoma(CRCC),4(0.9%)oncocytoma,4(0.9%)carcinoma of the collecting ducts of Bellini(CCDB),and 14(3.2%)renal cell carcinoma unclassified.There were 335(77%)patients undergone radical nephrectomy,74(17%)nephron sparing surgery and 26(6%)others,such as palliative nephrectomy.The patients with VHL disease come from 5 Chinese kindred and all had bilateral clear cell renal cell carcinomas and multifocal renal cysts.There were 7 paients of stage Ⅰ and 3 cases of stage Ⅱ and 6 cases of grade Ⅰ and 4 cases of grade Ⅱ.Genetic test revealed that all patients had VHL gene mutation.4 patients had recurrence while no evidence of local advance and distant metastasis were found during a mean of 28.6 months.Patients with chromophobe RCC are all of stage Ⅰ and 5 cases of grade Ⅰ and 13 cases of gradeⅡ.All patients are alive without recurrence or metastasis during a mean of 19.8 months.Collecting ducts RCC all presented with stage Ⅰ but grade Ⅲand with the median survival only 11.3 Months.Of clear cell and papillary RCC,260(67.6%),64(16.6%),32(8.3 %),29(7.5%)were stage Ⅰ,Ⅱ,Ⅲand Ⅳ,and of stage Ⅰ patients 147(38.2%),113(29.4%)were T1a and T1b respectively.124(32.2%),219(56.9%),40(10.4)and 2(0.5%)were grade Ⅰ,Ⅱ,Ⅲ,Ⅳ,respectively.Median survival of 57 advanced RCC is 16.0±1.3months,1-year survival is 55%,and 2-year survival is 31%,respectively.By using logrank test,clinical stage(<0.01),tumor size(<0.01),lymphadenopathy(<0.01),metastasis(<0.01)and tumor grade(<0.01)were anatomical and histological prognostic factors for advanced RCC. Coneluslons Different RCC subtypes have different clinical course.The RCC patients in VHL disease have VHL gene mutation and the tumors are often multifocal,bilateral,clear cell type with a low stage and grade which often recurrence but without metastasis.Chromophobe RCC may have a favorable prognosis but collecting duct RCC poor prognosis.In anatomical and histological level,clinical stage,tumor size,lymphadenopathy,metastasis and tumor grade are prognostic factors of survival for advanced RCC.
8.Association of malnutrition based on Global Leadership Initiative on Malnutrition criteria with the disease activity and adverse clinical outcomes in hospitalized ulcerative colitis patients

Objective:To investigate the association of malnutrition based on Global Leadership Initiative on Malnutrition (GLIM) criteria with the disease activity and clinical outcomes in hospitalized ulcerative colitis (UC) patients.Methods:Clinical data of 115 patients with UC hospitalized in the Affiliated Hospital of Qingdao University from September 2019 to March 2023 were prospectively analyzed. GLIM and European Society for Clinical Nutrition and Metabolism (ESPEN) 2015 criteria were used for the diagnosis of malnutrition, allowing the analysis of consistency between two diagnostic criteria for malnutrition. The relationship between malnutrition based on GLIM criteria and disease activity and clinical outcome was further investigated. The risk factors of adverse clinical outcomes in UC patients were analyzed using binary logistic regression.Results:GLIM and ESPEN 2015 diagnostic criteria showed high correlation and consistency (AUC=0.875, P<0.001; K=0.809, P<0.001). According to GLIM criteria, the prevalence of malnutrition among hospitalized UC patients was 32.17% (37 cases). Compared with non-malnourished UC patients, the modified Mayo score and C-reactive protein level of malnutrition patients were higher ( P<0.005), and the proportion of patients with severe disease activity was higher ( P=0.005). UC patients in the malnourished group had longer hospital stay ( P<0.001), higher hospitalization costs ( P<0.001), and higher rates of drug escalation/conversion therapy, re-admission and surgery at 12 weeks and 54 weeks ( P<0.05). Binary logistic regression analysis showed that a high Mayo score ( OR=3.606, P=0.016), a high modified Mayo score ( OR=1.346, P=0.009) and malnutrition ( OR=1.430, P=0.012) were independent risk factors for adverse clinical outcomes of hospitalized UC patients at 12 weeks. A high modified Mayo score ( OR=6.491, P=0.011) and malnutrition as per GLIM criteria ( OR=6.693, P=0.033) were independent risk factors for adverse clinical outcomes of hospitalized UC patients at 54 weeks. Conclusions:GLIM and ESPEN 2015 diagnostic criteria show high consistency in the diagnosis of malnutrition in hospitalized UC inpatients. Malnutrition may imply adverse clinical outcomes of hospitalized UC patients, which is an independent risk factor for the adverse clinical outcome of hospitalized UC patients.

Aging-elevated DNMT3A R882H-driven clonal hematopoiesis (CH) is a risk factor for myeloid malignancies remission and overall survival. Although some studies were conducted to investigate this phenomenon, the exact mechanism is still under debate. In this study, we observed that DNMT3A R878H bone marrow cells (human allele: DNMT3A R882H) displayed enhanced reconstitution capacity in aged bone marrow milieu and upon inflammatory insult. DNMT3A R878H protects hematopoietic stem and progenitor cells from the damage induced by chronic inflammation, especially TNFα insults. Mechanistically, we identified that RIPK1-RIPK3-MLKL-mediated necroptosis signaling was compromised in R878H cells in response to proliferation stress and TNFα insults. Briefly, we elucidated the molecular mechanism driving DNMT3A R878H-based clonal hematopoiesis, which raises clinical value for treating DNMT3A R882H-driven clonal hematopoiesis and myeloid malignancies with aging.
