2.Preparation of rhubarb lactobacillin dispersible tablets

Objective: To obtain the optimum prescription of the rhubarb lactobacillin dispersible tablets.Methods: The kind and amount of the excipient were investigated by using the disintegrating time to establish the optimum prescription and preparation process.Results: The tablets were prepared by compressing moist granulation with the PVPP and CMS-Na as the disintegrant,MCC as diluent,sodium alginate as sweller,magnesium stearate as lubricant and 80% alcohol as adhesive agent.The preparation complied with the requirement of Pharmacopeia of the People's Republic of China(2005edition).Conclusion: This prescription of the tablet is reasonable and the preparation process is feasible.
3. Study on pharmacokinetics and pharmacodynamics of ginkgolides in rat model with cerebral ischemia injury

Objective To study the metabolic process of ginkgolides in rats with cerebral ischemic injury based on pharmacokinetic- pharmacodynamics (PK-PD) binding model. Methods The middle cerebral artery occlusion (MCAO) model was established by the suture method. After reperfusion, rats were randomly assigned to nasal administration, ig administration, and iv administration group.The orbital blood was taken at different time points of 0.25, 0.33, 0.5, 0.75, 1.0, 1.25, 1.5, 2.0, 4.0, 6.0, 9.0, and 12.0 h after the administration of the ginkgolides solution. The drug-time curve of ginkgolide B in plasma were drawn according to the concentration measured by LC-MS. The time-effect curve of superoxide dismutase (SOD) and malondialdehyde (MDA) were drawn based on the value measured by the kit. The pharmacokinetic parameters were calculated by DAS 2.0 software to fit the PK-PD binding model. Results The t1/2 of ginkgolide B of the rats in the administration group was smaller than that in the MCAO model group. The area under the curve (AUC) of nasal administration was significantly higher than intragastric administration and intravenous administration. Conclusion Ginkgolide B has a good protective and mitigating effect on ischemic stroke. The pharmacokinetics of nasal administration is better than iv and ig administration, which can provide reference for the development of nasal administration of ginkgolide B.
4.Preparation of Levofloxacin and Triamcinolone Acetonide Double-loaded Ophthalmic Gel

OBJECTIVE:To prepare Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel. METHODS:Us-ing levofloxacin hydrochloride and triamcinolone acetonide as main components,carbopol-940P as base material,HPMC K4M as tackifier,Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel was prepared. Using dissolution time as index, the contents of carbopol-940P and HPMC K4M were determined by single factor test,and dissolution time,viscosity and the con-tents of 2 main components were also determined. RESULTS:The concentrations of carbopol-940P and HPMC K4M were 0.4%and 1.2%,separately. The dissolution time was more than 24 h and viscosity was 1 142.67 Pa·s. The content of levofloxacin hydro-chloride was 97.3% of labelled amount (RSD=0.84%,n=3),and that of triamcinolone acetonide was 92.97% of labelled amount(RSD=1.32%,n=3). CONCLUSIONS:Levofloxacin and triamcinolone acetonide double-loaded ophthalmic gel has been prepared successfully.
5.ULTRASTRUCTURES AND THEIR RELATIONSHIPS OF THE NEUROHYPOPHYSIS IN THE RAT

Ultrastructures of the neurohypophysis in the rats have been studied by means of electron microscopy. Besides identifying the fact that the neurosecretory substances store in the nerve terminals and release into the blood, there are also axo-axonic synaptoid structures existed between neurosecretory terminals and neuro-glial synaptoid contacts between neurosecretory terminals and pituicytes. The characteristics of these contacts mentioned above are as follows: (1) the pre-and postsynaptic membranes are insignificantly thickened; (2) clustered microvesicles are close to and attach to the presynaptic membrane; (3) the synaptic cleft is about 20 nm wide and contains various amount of electron dense materials. The authors suggest that the two kinds of synaptoid structures and pituicytes are all involved in the regulation for the releasing of the neurosecretory substances.
6.Qualitative study of characteristics and practice of standardized training for resident doctors in psychiatric department

Objective:To understand the characteristics and practice of standardized training for resident in psychiatric department,providing a reference for other hospitals in the similar situation.Methods:Using qualitative method,in-depth interview and focus group interview were carried out with clinical teaching and teaching management experts of a tertiary hospital,to understand the characteristics,practice,and deficiencies of resident doctors standardized training for psychiatrists.Results:Focusing on clinical operation,psychotherapy,and communication capability were the major characteristics of resident doctors' standardized training for psychiatrists in this hospital.Though this hospital did positive trials on guarantee system improvement,outpatient and inpatient training combination,and training content expansion,it was not perfect enough in united training and training pattern.Conclusions:The psychiatry resident's standardized training for psychiatrists should establish complete standards of performance for united training,strengthen training mechanism and training mode,and make full use of modem distance education resources to further improve quality of the psychiatric residency training.
7.Meta Analysis of Chemotherapy with Best Supportive Care in the Survival of Inoperable Hepatocellular Carcinoma

Objective:To evaluate the effects of chemotherapy on survival of patients with inoperable hepatocellular carcinoma. Methods:MEDLINE and EMBASE searches were supplemented by information from trial registers. RCT (randomized clinical trial) for chemotherapy of old generation and best supportive care in inoperable hepatocellular carcinoma was selected. Best supportive care may include some non-cytotoxic drug,such as tamoxifen,octreotide and so on. A quantitative meta-analysis using updated information basing on inclusion and exclusion criterion from all available RCT was carried out by two reviewers. The meta-analysis was based on 6 months survival rate and 1 year survival rate. Result:There was no significant improvement for chemotherapy of old generation in 6 months survival rate and 1 year survival rate as compared with best supportive care. Conclusions:There is no convincing evidence that inoperable hepatocellular carcinoma patients will benefit from chemotherapy. A large number of randomized controlled clinical studies should be performed to confirm the efficacy of chemotherapy.
8.Inhibitory effects of Ginkgo biloba extract on renal inflammation in diabetic nephropathy model mice and its mechanism

OBJECTIVE To investigate the inhibitory effects of Ginkgo biloba extract (GBE) on renal inflammation in diabetic nephropathy (DN) model mice, and its potential mechanism. METHODS KK/Ay mice were fed with high fat and high sugar to induce DN model. They were divided into model group, positive control group [metformin 200 mg/(kg·d)], GBE low-dose and high-dose groups [100, 200 mg/(kg·d)], with 6 mice in each group. Six C57BL/6J mice were fed with a regular diet as the control group. Administration groups were given relevant liquid intragastrically, control group and model group were given constant volume of normal saline intragastrically, once a day, for 8 consecutive weeks. The body weight, fasting blood glucose, 24-hour food intake, 24-hour urine output, monocyte chemoattractant protein-1 (MCP-1), interleukin-12 (IL-12), IL-10, advanced glycation end products (AGEs), blood urea nitrogen (BUN) and serum creatinine (Scr) of mice were measured, and the ratio of bilateral kidneys to body weight was also calculated. The pathological injury and fibrotic changes of the renal cortex were observed, and the expressions of macrophage polarization marker proteins [type M1: inducible nitric oxide synthase (iNOS); type M2: arginase-1 (Arg-1)] and AGEs-the receptor of advanced glycation end products (RAGE)/Ras homolog gene pharm_chenjing@163.com family member A (RhoA)/Rho-associated coiled-coil forming protein kinase (ROCK) signaling pathway-related proteins were determined in renal cortex. RESULTS Compared with the model group, the symptoms such as renal cortical hyperplasia, vacuoles, infiltration of inflammatory cells, and renal cortical fibrosis had been improved in GBE low-dose and high-dose groups; body weight, serum level of IL-10, the expression of Arg-1 in the renal cortex were significantly higher than model group (P< 0.01); fasting blood glucose, 24-hour food intake, 24-hour urine output, serum levels of MCP-1, IL-12, BUN, Scr and AGEs, the ratio of bilateral kidneys to body weight, renal injury score, the proportion of renal interstitial fibrosis, the protein expressions of iNOS, RAGE, RhoA and ROCK1 (except for GBE low-dose group) in renal cortex were significantly lower than model group (P<0.01). CONCLUSIONS GBE could improve kidney damage and alleviate inflammatory response in DN model mice, the mechanism of which may be related to inhibiting the AGEs-RAGE/RhoA/ROCK signaling pathway and regulating macrophage polarization.
9.Correlation of mTOR and VEGF Gene with Nephropathy in Diabetic Rats

ObjectiveThe purpose of this study was to observe the correlation of mTOR and VEGF gene with nephropathy indicators in diabetic rats. Methods Forty-eight male Sprague-Dawley(SD)rats were divided into diabetes mellitus group(DM=28)and control group(NDM=20). Diabetic models were produced by injection of streptozotocin. In the courses of 12,16,20 and 24 weeks,the histology scores(HS)and glomerular basement membrane(GBM)thickness were collected. The protein and mRNA expressions of the gene of mTOR,VEGF and VEGFR2 were observed by immunohistochemistry and real-time quantitative polymerase chain reaction (RT-Q-PCR)by SYBR Green. And the standardized cycle of threshold(SCt)was regarded as the indicators of the mRNA expression. Results HS and GBM thickness were significantly higher in DM rats than those in NDM rats,especially in DM rats of the courses of 20 and 24 weeks(P < 0.01). IHC scores of VEGF and VEGFR2 were higher in total DM rats and were positively correlated with each other. There were positive correlations between HS with VEGF and VEGFR2 in total DM rats(P < 0.05). SCts of VEGF and VEGFR2 were significantly higher and were positively correlated with each other in total DM rats(P < 0.01). SCt of VEGF and GBM thickness showed positive correlation in total DM rats. SCt of VEGF was highest in the course of 12w DM rats. SCt of VEGFR2 gradually decreased following by the diabetic course,and was lowest in the course of 24w. There were no significantly differences in IHC scores and SCt of mTOR between DM and NDM rats. But the IHC scores of mTOR,VEGF and VEGFR2 were positively correlated with each other and with HS in total DM rats(P < 0.05). Conclusion HS and GBM thickness were higher in diabetic rats,especially in the course of 24w,which could reflect the injury of nephropathy. The protein and mRNA of VEGF and VEGFR2 were high expressed in kidney of DM rats and increased with the increasing of diabetic course. The mRNA expression of VEGF was positively correlated with GBM thickness of in diabetic nephropathy(DN). The protein expressions of mTOR,VEGF and VEGFR2 might have synergistic effects in DN of DM rats. But the results could not exclude the influences of different courses,sample size and experimental conditions.
10.Analysis on key aspects of cultural integration for cross-region specialists alliances of closed cooperation

Objective:To analyze key aspects in cultural integration for cross-region specialists alliances of closed cooperation, for promoting such integration and identifying efficient operation of the practice.Methods:Quantitative and qualitative methods were used. 192 employees of primary hospitals participated in a questionnaire survey about their personal information and hospital culture self-evaluation on 18th February, 2019. Data so collected were subject to descriptive analysis. On February 21, 2019, the deans of the hospitals were invited for in-depth interviews, and focus group discussions were arranged for 7 doctors from the center hospital who had provided medical support for the primary hospital for more than half and a year, and for 5 doctors and 2 head nurses from the obstetric and neonatal department of the primary hospital respectively. The purpose is to understand the direction, key aspects, achievements and challenges of cultural integration between the two hospitals. Content analysis method was used to study recordings and interview documentation.Results:Employees of the primary hospital had a high satisfaction with indicators of organizational citizenship behavior, and the quality of medical care and team orientation, with the self-rated original scoring for the elements of hospital culture being 3.755, 3.754 and 3.698 respectively. On the other hand, they found insufficiencies in the innovation, poor orientation and incentive mechanism, with the self-rated original scoring for the elements of hospital culture being 3.469, 3.391 and 3.297 respectively. The self-rated total scoring was lower among medical technicians and those with bachelor′s degree or above, which were 3.029 and 3.202 respectively. Hospital culture integration is designed to strengthen technical guidance and care for doctors and patients, and to strengthen cooperation and support in HR training, scientific research innovation and spiritual culture construction. The key to integration is acceptance. The current roadblocks for efficient operation of this model come from medical insurance policy, material resources policy, logistics support, informationization management, personnel training and support and performance management.Conclusions:The cultural integration of cross-region specialists alliances of closed cooperation should be realized through the interactions of values, systems, behaviors and material resources dimensions. Government should play a leading and coordinating role and improve supporting measures.
