1.Beard reconstruction by single-hair grafting technique

Objective To search for a good solution for aesthetic beard reconstruction and to explore the effect of the single-hair grafting in beard reconstruction. Methods Under the local anesthesia, a scalp strip (length range from 5~7 cm, width range from 1.0~1.5 cm) was harvested from the back of the head and then was divided into a series of single-hair micrografts. After the slits were made, the grafts were implanted into the holes, parallel to the original beard directions. The donor sites were directly closed. With this method, 18 sparse beard patients and 3 patients with upper lip scars were treated. Results The grafted hairs grew nearly in the direction of normal beard, which achieved 90 %~95 % survival in recipient areas. The patients were satisfied with the results, in which 19 cases received only 1 operation and 2 cases received another repeated operation. Conclusion Beard reconstruction by single-hair grafting technique is a good, safe and effective method.
2.Surgical experience on Roux-en-Y pancreaticojejunostomy after local pancreatic head resection for benign tumors of the pancreatic head.

OBJECTIVETo explore the value of Roux-en-Y pancreaticojejunostomy after local pancreatic head resection in treating benign tumors of pancreatic head (BTPH).
METHODSThe clinical data of 12 patients diagnosed as BTPH and treated by Roux-en-Y pancreaticojejunostomy after local pancreatic head resection in Department of General Surgery, Peking University Third Hospital from November 2006 to October 2013 were retrospectively analyzed.Of the 12 cases, 5 patients were male, 7 patients were female, the age of patients ranged from 21 to 64 years(average 42.3 years). Diameter of tumors was 3.0-4.8 cm.Diameter of pancreatic wound after resection was 5.1-7.9 cm, and main pancreatic duct injury happened in 1 case.
RESULTSTwo cases of mucinous cystadenoma, 2 insulinoma, 3 solid pseudopapillary tumor and 4 nonfunctional pancreatic neuroendocrine tumors were confirmed histopathologically.No mortality and pancreatic leakage occurred during the perioperative period.All the 12 patients had no sign of recurrence.Experienced good life quality without occurrence of diabetes during the follow-up period of 24-108 months(more than 60 months in 4 cases).
CONCLUSIONSRoux-en-Y pancreaticojejunostomy after local pancreatic head resection is a reasonable choice for benign tumors of the pancreatic head as long as the patient is properly selected.

3.Effect of high-intensity interval training on cognitive weakness in elderly patients with coronary heart disease

Objective:To investigate the effect of high-intensity interval training on cognitive weakness in elderly patients with coronary heart disease.Methods:A randomized controlled study was used. A total of 70 elderly patients with coronary heart disease after discharge from the Cardiology Department, Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from June 2020 to June 2021 were selected and divided into the control group (35 cases) and the observation group (35 cases) by random digits table method. The control group received conventional treatment, and the observation group received high-intensity interval training on the basis of conventional treatment. The changes of peak oxygen intake (PeakVO 2), anaerobic threshold oxygen uptake (ATVO 2), Fried phenotype score, Montreal Cognitive Assessment score, and SF-36 quality of life assessment score before and after 12 weeks of intervention were compared between the two groups. Results:During the intervention, 2 cases fell off in the control group and 2 cases in the observation group. After 12 weeks of intervention, PeakVO 2 and ATVO 2 in the observation group were (23.91 ± 5.88), (20.79 ± 5.19) ml·min -1·kg -1, respectively, higher than those in the control group(20.56 ± 5.81), (17.29 ± 5.36) ml·min -1·kg -1, and the differences were statistically significant ( t = 2.38, 2.69, both P<0.05). The scores of SF-36 quality of life assessment and Montreal Cognitive Assessment in the observation group were (85.33 ± 6.43), (22.64 ± 3.81) points, which were significantly higher than those in the control group (78.72 ± 8.14), (20.67 ± 3.04) points, and the differences were statistically significant ( t = 3.66, 2.32, both P<0.05); the Fried phenotype score in the observation group was (1.36 ± 0.99) points, which was significantly lower than that in the control group (3.03 ± 1.08) points, and the difference was statistically significant ( t = 6.54, P<0.05). Conclusions:High-intensity interval training can improve cardiopulmonary function, aerobic exercise ability, cognitive weakness and quality of life in elderly patients with coronary heart disease.
4.Advices on the prevention and control of nosocomial infection of COVID-19 within children′s hospitals

The epidemic of COVID-19 has become the topmost public health threat worldwide. The authors suggested that in addition to strengthening the organization and leadership of the abovementioned work, greater attention be paid to establishing and improving the prevention and control mechanism.Furthermore, special efforts should be given to the safety of the medical workers, by strengthening their infection monitoring and outbreak management. Medical workers in different work areas and positions should be placed under careful protection with due cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. Such special issues in the management of pediatric patients, as allocation of nurses, the screening and management of caregivers, the problem of breastfeeding, and the disinfection of children′s toys and diapers were discussed. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments, which aim to guide the prevention and control of COVID-19 nosocomial infection in the pediatric outpatient and ward.
5.Application of enhanced recovery after surgery in the treatment of children with congenital choledochal cyst.

OBJECTIVE:
To explore the feasibility of enhanced recovery after surgery (ERAS) in treatment of children with congenital choledochal cyst.
METHODS:
One hundred and thirty children with congenital choledochal cysts admitted in the Children's Hospital of Zhejiang University from June 2017 to June 2019 were divided into ERAS group (=65) and control group (=65) according to admission order. The intestinal tract condition during operation, time of operation, surgical results, time for eating after operation, abdominal drainage after operation, length of hospital stay after operation, total hospital expenses and complications were compared between two groups.
RESULTS:
Compared with the control group, the satisfaction of intestinal operation field, recovery of gastrointestinal function after operation,time required for the volume of peritoneal drainage fluid to be less than 50 mL,time of abdominal drainage tube removal, and length of hospital stay were all improved in ERAS group (<0.05 or <0.01).ERAS group had more peritoneal effusion after removal of abdominal drainage tube (<0.01), but the incidence of edema after operation was lower (<0.05). The satisfaction of parents in the two groups was similar, but the cooperation of parents in the ERAS group was improved (<0.05) and the total cost of hospitalization was reduced (<0.01).
CONCLUSIONS
ERAS has advantages over the traditional scheme and can be used in the clinical treatment of children with congenital choledochal cyst.

6. Advices on the prevention and control of nosocomial infection of novel coronavirus within children’s hospitals

The pneumonia caused by the novel coronavirus (2019-nCoV), which began in December 2019, has become the most serious public health problem, threatening people's health and life. This threat is posing a severe challenge on the diagnosis and treatment of 2019-nCoV infection, the prevention and control of hospital cross infection of medical staff. It is suggested that in addition to strengthening the organization and leadership of the abovementioned work, establishing and improving the prevention and control mechanism deserve greater attention. Furthermore, special attention should be given to the safety of the medical staff, strengthening their infection monitoring and outbreak management. Medical staff in different work areas and positions should be placed under careful protection, cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments. These measures are proposed to provide a guidance for the prevention and control of 2019-nCoV nosocomial infection in the pediatric outpatient and ward.
