Under physiological conditions, many vital functions of the body are controlled by transient release of bioactive substances at a specific time and site. Based on the circadian rhythm character of disease and chronotherapeutic conceptions, pulsatile delivery system has been designed to achieve optimal therapeutic effect and reduce the toxic and side-effect. In recent years, more and more studies are focused on the pulsatile multiparticulate drug delivery system. Pulsatile multiparticulate system possesses many benefits, such as no risk of dose dumping, predictable gastric emptying, flexible release patterns and increased bioavailability. Based on these premises, the aim of this review is to summarize the major design methods of pulsatile multiparticulate and the research progress.

Sunscreen products have been widely used with the increase in public awareness of sun protection. As sunscreens are structurally stable and resistant to degradation, the concern for their biotoxicity and impact on marine environment has been heightened increasingly, and several countries and regions have successively issued bans on relevant sunscreen products. However, these bans have not yet affected the key international rules and regulations related to sunscreen products. The risks of different sunscreens to marine ecosystems and potential risks to human themselves still need to be evaluated through further multidisciplinary researches.

OBJECTIVETo observe the difference in the analgesic effect on primary dysmenorrheal between acupuncture and sham acupuncture at Sanyinjiao (SP 6) during menstrual pain and evaluate the impact of psychological effect on acupuncture analgesia.
METHODSSixty subjects were randomized into an acupuncture group and a sham acupuncture group, 30 cases in each one. The conventional acupuncture and sham acupuncture were applied to Sanyinjiao (SP 6) on bilateral sides when menstrual pain began to attack and needles were retained for 30 min each time. Three menstrual cycles were required. The visual analogue scale (VAS) was adopted to determine the scores before and 0. 5 h, 1 h, 3 h, 6 h and 12 h after acupuncture during menstrual pain in each cycle separately.
RESULTSIn the acupuncture group, VAS score at each time point after acupuncture was reduced as compared with that at the previous one during menstrual pain in each menstrual cycle, indicating the significant difference (all P<0. 05). In the sham acupuncture group, the scores in 6 h and 12 h of acupuncture were reduced as compared with the previous one, indicating the significant difference (all P<0. 05). After acupuncture, VAS score at each time point in the acupuncture group was lower than that in the sham acupuncture group (all P<0. 05).
CONCLUSIONThe conventional acupuncture at Sanyinjiao (SP 6) achieves the significant analgesic effect on primary dysmenorrheal. The psychological placebo effect of sham acupuncture has no obvious impact on acupuncture analgesia.

Objective To evaluate the effect of hypoxic-ischemic time on reduction of hypoxic-ischemic brain injury by sevoflurane postconditioning in neonatal rats. Methods Two hundred and ten 7-day-old Sprague-Dawley rats (105 male,105 female),weighing 13-17 g,were randomly divided into 7groups (n=30 each) using a random number table:sham operation group (group Sham),hypoxia-ischemia group (group HI),and sevoflurane postconditioning at different hypoxic-ischemic time point groups (P0,P3,P6,P 12 and P24 groups).Immediately after ligation of the left common carotid artery, and at 3,6,12 and 24 h after ligation, the rats inhaled the mixed gas containing 2% sevoflurane for 30 min in P0,P3,P6,P13 and P24 groups, respectively. The fatality was recorded within 7 days after establishment of the model. At 7 days after establishment of the model, the rats were sacrificed, the brains were removed, and the right and left cerebral hemispheres were weighed separately, and the left/right cerebral hemisphere weight ratio was calculated. The hippocampal CA1 region and posterior cingulate gyrus were isolated, and the ratio of density of normal neurons in the left to the right was calculated. Results Compared with group Sham, the left cerebral hemisphere weight,left/right cerebral hemisphere weight ratio, and ratio of density of normal neurons were significantly decreased, and the fatality rate was increased in the other six groups (P<0.05).Compared with group HI,the left cerebral hemisphere weight,left/right cerebral hemisphere weight ratio, and ratio of density of normal ncurons were significantly increased in P0,P3 and P6 groups (P<0.05),and no significant change was found in the parameters mentioned above in P12 and P24 groups (P>0.05).Compared with group P6,the left cerebral hemisphere weight,left/right cerebral hemisphere weight ratio, and ratio of density of normal neurons were significantly increased in P0 and P3 groups (P< 0.05).There was no significant difference in the parameters mentioned above between group P0 and group P3 (P>0.05).Conclusion Sevoflurane postconditioning performed within 6 h of hypoxia-ischemia can reduce hypoxic-ischemic brain injury, and it provides no cerebral protection if exceeding 12 h.

An on-line solid phase extraction ( SPE ) coupled with HPLC-MS/MS method was developed to determine S-ammuxetine and R-ammuxetine in rat plasma. The sample preparation consisted of the following steps:A protein precipitation extraction used methanol and acetonitrile ( 50:50 , V/V ); an on-line SPE treatment to remove most matrixes in plasma;an enrichment and separation step used a C18 analytical column. S-and R-ammuxetine were determined by tandem mass spectrometry. The SPE column was a Retain PEP Javelin (10 mm × 2. 1 mm × 5 μm), while the chromatographic separation was achieved using a ZORBAX SB-C18 (50 mm × 2. 1 mm × 3. 5 μm) analytical column with an isocratic mobile phase composed of acetonitrile-water-formic acid (40:60:0. 1, V/V/V, 0. 3 mL/min). The selected reaction monitoring mode of the positive ion was performed and the precursor to the product ion transitions of m/z 292 . 1/154 . 0 and m/z 260. 4/116. 2 were used to measure S-ammuxetine, R-ammuxetine and internal standard (propranolol). The method was linear over a concentration range from 0 . 2 to 1000 μg/L with the correlation coefficients of 0 . 9903 and 0 . 9951 . The average intra-day precision values were 1 . 2% -12 . 0% for S-ammuxetine and 0. 4%-11. 2% for R-ammuxetine, respectively. The average recoveries were 94. 2%-101. 6% for S-ammuxetine and 94. 3% -109. 4% for R-ammuxetine. Compared to the literature, the sensitivity of this method increased dramatically. The present method has been successfully applied to the preclinical rat research of ammuxetine isomers following intragastric administration.

Objective:To evaluate the diagnosis significance of breast mammography assisted by hook-wire localization biopsy for BI-RADSⅣand above negative or non-palpable breast lesions. Methods:A total of 48 cases of mammary molybdenum target with BI-RADSⅣlevel and above but with clinical-touched negative or non-palpable breast lesions (including 4 bilateral lesions and 44 uni-lateral lesions;total of 52 lesions) were used in the mammography aided by hook-wire localization biopsy. Results:Among the 52 le-sions, 13 cases were malignant lesions (single), 6 cases were at Stage 0 (accounted for 46.15%), 5 cases were in Stage I (38.46%), and 2 cases were in StageⅡ(15.39%). The remaining 39 cases were benign lesions. Results showed that the detection rate for breast cancer was 25.0%. A total of 39 cases of benign lesions and 10 cases of malignant lesions were found among mammography BI-RADSⅣpa-tients. Zero cases of benign lesions and 3 cases of malignant change were found among BI-RADS Ⅴ patients. The positive rates of breast cancer among BI-RADSⅣandⅤpatients were 25.64%and 100%, respectively. Conclusion:Breast mammography assisted by hook-wire localization biopsy could precisely excise both BI-RADSⅣandⅤnegative/non-palpable breast lesions. This technique can also improve the quality of life and prognosis of patients. It is a safe, accurate, and low-cost diagnostic method. Thus, breast mammogra-phy assisted by hook-wire localization biopsy must be widely used in clinical applications in China.
