Objective For an analysis and evaluation of the technical efficiency of tertiary hospitals in Tianjin. Methods Data Envelopment Analysis of different combinations of indicators and Spearman Rank Correlation Analysis were used for a study of the relative efficiency between 28 tertiary hospitals and the correlation between the indictors. Results The study showed that 12 hospitals reached DEA efficient, effective rate of 42.86%,and there is a correlation between the five sets of indicators. Conclusion Regulators of Tianjin hospitals should pay attention to efficiency calculations, make full use of medical resources, strengthen scientific management, and enhance the competitiveness of the hospital.

Objective To probe into the varying efficiency of public hospitals and recommend on efficiency improvement. Methods By means of DEA method including CCR model,BCC model and Malmquist index,to study the static and dynamic changes of operating efficiency of hospitals in Tianjin from 2008 to 2012.Results The overall efficiency of these 24 hospitals is not high, and the efficiency of the tertiary hospitals (0.900)is higher than the second-level hospitals(0.828).There exist problems that the size of the second-level hospitals is either too big or too small,a lot of health resources can be saved by managing the non-DEA efficient hospitals. The trends of the total factor productivity of 24 hospitals in Tianjin from 2008 to 2012 are increased. Thanks to the advanced technology and innovation, the overall efficiency of tertiary hospitals is improved.Through improving organizational management level, the overall efficiency of second-level hospitals is improved. Conclusion Hospital managers should introduce the high-level medical workers, enhance technological innovation, improve and standardize the hospital management and service system, optimize the allocation of medical resources and select the appropriate size of the hospital with the hospital's own situation to improve the operating efficiency of the public hospitals.

Thymus has an important significance to the pathogenesis and treatment for myasthenia gravis (MG). Thymoma accounts for 10 to 15% in the MG patients, which is short of localized damage feature so as to make misdiagnosis usually. Using citric acid extracted sacrolemma protein antigen from human skeletal muscle, CAE-Abs in the sera of 103 MG patients were detected by indirect hemoagglutination assay (IHA) and the positive rate was 78.9% (cases of 15/19); and then the CAE-Abs in the sera of 123 MG patients were detected by ELISA and the positive rate'was 82.6% (cases of 19/23). But the positive rate of CAE-Ab in the sera of MG without thymoma was only 0-4.0% (cases of 3/77). This results indicate that CAE-Ab has significant correlativity for MG with thymoma.

Study a method for the detemination of the content of polysaccharides in Gentiana farreri, and analysis of the content of polysaccharides from different producing areas. The results showed that using the anthrone-sulfuric acid method, simple operation, accurate result. Sample was measured at 620 nm absorbance after anthrone-sulfuric acid color, at this wavelength, solution absorption and glucose showed a good linear relationship; The linearity was in the range of 0.01-0.07 g x L(-1) (r = 0.996 7). The recovery rate was 99.41%, with RSD of 2.0%. Considering the experimental conditions, to determine the solid-liquid ratio 1:60, extracting time 50 min, concentration of ethanol 80%. The mass fraction of polysaccharides was the highest to reached 0.743% in G. farreri from Gansu Xiahe. This experiment has laid a good foundation for further study on G. farreri.

Objective To establish and optimize the evaluation indexes system of hospitals' technical efficiency in China. Methods Delphi method was used along with experts opinion to build the indexes system. Results The expert positive coefficient was over 80% in three rounds of consultation, and the coordination coefficients were 0.144, 0.302 and 0.533 respectively, proving the difference as statistically significant. In the end an evaluation indexes system of hospitals' technical efficiency had taken shape, comprising three level-1 indexes, 13 level-2 indexes and 114 level-3 indexes. Conclusions The evaluation indexes system of hospitals' technical efficiency established with Delphi method can contribute to optimizing resources allocation and promote efficiency of hospital service.

Objective To explore for the methede and effect of endoscopic treatment on biliary leakage and biliary duct damage. Methods All patients with biliary damage such as biliary leakage and biliary duct stricture were treated by endoscopic sphincoterotomy and endoscopic nasobiliary drainage (ENBD) during abdominal cavity drainage ENBD was removd when biliary leakage healed and abdominal cavity drainage ceased for 1~2 weeks were confirmed. Plastic stents were implanted to distend the biliary duct stricture for 2-3 months. Results Twenty-six patients with biliary leakage were cured 3-4 weeks after ENBD. Fourteen out of 17 patients implanted with plastic stent were recovered uneventfully after stent removed, and 4 patients also recovered after installation of double-stents for 3 months, while another case with calculus and stricture of left hepatic duct in spite of implantation of simple-stent suffered repeatedly from biliary tract infection and one case developed hepatic abscess after repeatedly infection for one year before he had the hepatic lobectomy. Conclution Endoscopic therapy is the first choice in treating biliary leakage or secondary duct stricture.

Objective To analyze, the surgical treatment for gastric cancer. Methods Clinical data of 1072 patients with gastric cancer were analyzed retrospectively. Results The main clinical manifestations included upper abdominal pain(671 cases), difficulty in swallow(311 cases), appetite loss(187 cases),wasting or weight loss(158 cases), nausea and vomiting(97 cases), distention or upset(178 cases) and hematochezia (116 cases). 922 of 1072 patients (86 %) underwent resection, including 783 radical operation and 139 pauiative operation, including 239 proximal gastrectomy,361 distal gastrectomy and 322 total gastrectomy (jejunal interposition in 132 cases and "P"-Roux-en-y reconstruction in 200 cases). 92 cases received combined organs resection while 150 cases received exploratory laparotomy among them. D1 lymphadenectomy was performed for 86 cases and D2 lymphadenectomy 592 cases, D3 99 cases, and D4 6 cases. Stage Ⅰ accounted for 102 cases and stage Ⅱ269 cases, ⅢA 378 cases, ⅢB 172 cases, and stage Ⅳ 151 cases. The 1, 3,5 year overall survival rate were 74.6 %, 61.7 % and 41.1% in all cases but 83.1%, 65.2 % and 45.8 % in the group under radical treatment of gastric carcinoma. Conclusion To increase the survival rate and improve the quality of life,we shall pay attention to surgical procedure, digestive tract reconstruction, prevention of positive stump and postoperative complications in gastric cancer.

Inosiplex is a compound formulation composed of inosine and p-acetaminobenzoic acid (PABA) salt of N,N-dimethylamino-2-propanol (DIP). This study was to investigate the clinical plasma pharmacokinetic properties of DIP and PABA after single and multiple oral doses of inosiplex tablets in healthy Chinese volunteers. The established LC/MS/MS method for plasma DIP determination had a linear range of 0.02-10 mg/mL, and the HPLC method for plasma PABA determination had a linear range of 0.05-40 mg/mL. Linear pharmacokinetic characteristics were found with single oral doses of 0.5, 1.0 and 2.0 g. No obvious accumulation effects were observed for DIP and PABA.
