2.A Case of Generalized Annular Lichen Planus Controlled by Erythromycin.

A 69-year-old man presented with annular lichen planus involving both forearms, hand dorsa, wrists, inner sides of the thighs, knees and ankles. He was treated initially with systemic corticosteroids and etretinate, but rernission and recurr ence of the skin lesions were observed. During the follow-up, we found erythrasma on all his toewebs and both soles. After administration of erythromycin for the treatment of erythrasma, lesions of the lichen planus rapidly improved. Five months later, some lesions of lihen planus and erythrasma had recurred. After administration of erythromycin, the lesions of lichen planus improved again. No recurrence was observed for the following 8 months. We suspect that eradication of the chronic focus of infection and the anti-inflammatory effect, of erythromycin may lead to supression of abnormal immunological reactions and resolution of lichen planus.

3.A Case Report of the "Top of the Basilar" Syndrome with Peduncular Hallucinosis.

We report a case of the top of the basilar syndrome with peduncular hallucinosis in a 58 year old man who showed visual, oculomotor, and behaviorai symptoms and signs. Angiography revealed atherosclerotic stenosis at the junction of vertebral and basilar arteries. Mri showed high-signal lesions in various vertebrobasilar territories including thaiamus and midbrain.

4.A Case with the Alien Hand Sign.

We report a 60 year-old right-handed woman with left anterior cerebral artery territory infarction who showed the alien hand sign. The patient exhibited a severe disturbance of the right arm motor control characterized by forced grasping. Motor perseveration. And the presence of intermanual conflict that at times the right hand interfered with tasks performed by the left hand. Also, features of a transcortical motor aphasia signs of callosal interruption, urinary incontinence, and weakness of right leg were noted. MRI showed high signal areas in the left medial frontal cortex and anterior corpus callosum.

5.Percutaneous Transluminal angioplasty of membranous obstruction of IVC: a case report

Percutaneous tansluminal angioplasty(PTA) was done in 38-year-old female patient with membranous obstructionof IVC in the department of Radiology, Seoul National University Hospital. After PTA, pressure gradient between right atrium and IVC was relieved from 16mmHg to 6mmHg. The collateral circulation through hemiazygos vein andreflux into hepatic and right renal vein were disappeared. PTA is suggested to be an effective method in thetreatment of membranous obstruction of IVC.

6.Four Cases of Kartagener's Syndrome.

Kartagener's syndrome is an autosomaly inherited recessive condition characterized by situs inversus, bronchiectasis, and chronic sinusitis. And recently it was recognized as a subclass of dyskinetic cilia syndrome which caused by a defect in mucociliary transport owing to immotile or dyskinetic beating of cilia. Electron microsopy of cilia from sperm tails, nasal and bronchial epithelium of patients reveals the partial or complete absence of dynein arms. Our four patients were diagnosed as a Kartagener's syndrome by classic triad. We carried out electron microscopy of cilia of the nasal mucosa. And many other tests were done. One patient had squamous cell carcinoma of the lung, and another one patient revealed features of adult respiratory distress syndrome at admission. All patients improved with conservative therapy such as physiotherapy, bronchodilater, antibiotics except one patient who mechanical ventilation was required. A brief review of literature was made.

Facioscapulohumeral muscular dystrophy (FSHD) is a genetically trarlsmitted benign muscular dystrophy which has autosomal dominant inheritance pattern. It starts anytime within the first 30 years of life, and usually involves the face and shoulder girdle, and finally the pelvic muscles with very slow progression. Authors-report a fanily consisting of a father, two sons and one daughter, who had suffered from exertional dyspnea, weakness of facial muscle and winged scapulae, all wlth a slow progressive course. Two of these patients were biopsied arld confirmed light microscopically and electron microsopically.
