Objective:To study the effect of caprylic/capric acid glycerides (Lab), propylene glycol (PG) and Azone on the transdermal behavior of ketoconazole and miconazole nitrate in compound ketoconazole gel, to screen appropriate penetration enhanc-ers. Methods:Using a RYJ-6A-type transdermal drug diffusion tester, the effects of Lab, PG and Azone at different concentrations on the transdermal behavior of ketoconazole and miconazole nitrate in compound ketoconazole gel were studied. Results:3% PG showed the most obvious penetration enhancement, which could increase the permeation of ketoconazole by 2. 004 times, and increase the pen-etration of miconazole nitrate by 1. 795 times, and the differences were statistically significant (P<0. 05). Conclusion:The penetra-tion effect of 3% PG is obvious, which can be applied in compound ketoconazole gel.

AIM: To study the protective effects of Shenqi Fuzheng Injection on radiation therapy of nasopharyngeral carcinoma. METHODS: 58 cases with nasopharyngeal carcioma were selected and divided into 2 groups randomly: 30 cases of treatment group (radiation therapy plus Shenqi Fuzheng Injection) and 28 cases of control group (radiation therapy only). Body weight, Karnofsky score, radioactive stomatitis and change of WBC were compared between the two groups. Objective This paper aims to assess the clinical efficacy of pemetrexed maintenance therapy in patients with advanced non-small cell lung cancer( NSCLC) through Meta analysis.Me thods Systematic lit-erature searches were performed in Cochrane、Pubmed,Web of science, Embase and ClinicalTrials databases.The related references had been traced.We made quality assessment of qualified randomized controlled trials( RCTs) of pemetrexed maintenance therapy compared with best supportive care( BSC) in advanced NSCLC.Besides,we u-tilized stata 12.0,Revman 5.3 and GRADEpro software to evaluate the overall quality of the evidence,according to the Cochrane collaboration to perform Meta-analysis.Resutl s Three RCTs were eligible and included 1257 patients.Meta-analysis results suggested that:compared to BSC,pemetrexed maintenance therapy had a statisti-cally significant benefit in improving progression-free survival(PFS)(HR =0.55,95% CI:0.48~0.64)and overall survival(HR=0.76,95%CI:0.65~0.88).The objective response(ORR)did not reach statistical signif-icance(RR=0.97,95%CI:0.86~1.10).Conclusion Compared with BSC,pemetrexed maintenance therapy statistically significantly improve PFS and OS,but has no demonstrable impact on ORR in patients with advanced NSCLC.

[Summary] Thedawn phenomenon is a term used to describe hyperglycemia or an increase in the amount of insulin needed to maintain normoglycemia, occurring in the absence of antecedent hypoglycemia, during the early morning hours, and mostly affecting children with type 1 diabetes. Currently, hyperglycemia before breakfast time is defined as dawn phenomenon, and postbreakfast abnormal hyperglycemia is referred as the extended dawn phenomenon. Dawn phenomenon is regarded as the result of the β cell dysfunction, which increased endogenous glucose production and the persistent insulin resistance. By using continuous glucose monitoring technology, dawn phenomenon can be effectively identified:The difference between the lowest glucose level during nocturnal time and the glucose value before breakfast over 1. 11 mmol/L. And the long-acting insulin analogs and insulin pump are effective treatments for dawn phenomenon.

[Objective]To determine the relation between the total count of the five free anthraquinone(A loe-emodin,Rhein,Emodin,Chrysophanol,Physcion) in Radix et Rhizoma Rhei and the decoction time.[Method]HPLC was used with the chromatographic conditions as follows:mobile phase:methanol-0.1% phosphonic acid solution = 85 :15,detection wave length:254nm,flow rate:1.0 ml/min,column temperature:room temperature.The count of the five free anthraquinone in the decoction taken in every 2 minutes after the solution boiled was determined.[Results]During the 22 minutes after the solution was boiled,the total count took on a ascending tendency,afterwards,it took on a descending tendency.[Conclusion]The biggest number of the total count was obtained 20min after the solution was boiled.

OBJECTIVE:To evaluate the radiosensitization effect of docetaxel versus sodium glyci-didazole.METHODS:A total of 105 patients with intermediate or advanced stage nasopharyngeal carcinoma were randomly assigned to receive docetaxel 40 mg once a week plus radiotherapy or sodium glycididazole 1 000 mg three times a week(Monday,Wednesday and Friday) plus radiotherapy for a total of 7 weeks,or radiotherapy only(control group).RESULTS:There were significant differences between docetaxel group or sodium glycididazole group and control group in sensitization efficiency and radiation dose on tumor regression(P0.05).CON-CLUSION:Lower dose docetaxel is better than sodium glycididazole in radiosensitization efficiency for advanced nasopharyngeal carcinoma,especially in controlling the metastasis,yet it has minimal side effect.

OBJECTIVE:To observe the clinic efficacy and safety of FOLFIRI as a second line treatment for patients with advanced colorectal cancer.METHODS:Twenty-eight patients with advanced colorectal cancer whose disease progressed after first treatment with FOLFOX4 were included to receive the second line treatment with FOLFIRI regimen.RESULTS:Of the total 28 cases,0 had complete response,6 partial response,16 stable disease,6 progressive disease,and the responsive rate was 21.4%.The majority of adverse reaction was nausea,vomiting,leucopenia and tardive diarrhea.CONCLUSION:As a second line therapy,FOLFIRI has a confirmed therapeutic response and tolerable toxicity in patients with advanced colorectal cancer.

Objective To investigate the effects of phenylephrine and norepinephrine on tissue oxygen metabolism in elderly patients undergoing fluid restriction. Methods Eighty elderly patients who had underwent elective colorectal surgery were selected, with ASA of Ⅰ-Ⅱ grade. The patients were divided into restricted fluid administration combined with phenylephrine group (group Ⅰ) and restricted fluid administration combined with norepinephrine group (groupⅡ) by random digits table with 40 cases each. The compound recipe sodium lactate was given at a rate of 5 ml/(kg · h). Small dose of phenylephrine was infused intravenously at 0.1-0.3μg/(kg · min) simultaneously during the surgery in group Ⅰ, or norepinephrine 0.01-0.03 μg/(kg · min) in group Ⅱ. Mean arterial pressure (MAP) was maintained ≥ 65 mmHg (1 mmHg = 0.133 kPa). The intraoperative blood loss was replaced with the equal volume of 6% hydroxyethyl starch 130/0.4. The blood gas analysis, hemoglobin and so on were determined before the surgery (T0), 1 h after beginning of surgery (T1), 2 h after beginning of surgery (T2) and at the time of leaving operation room (T3). The complications were recorded. Results There were no statistical differences in MAP, central venous pressure (CVP), heart rate, lactic acid, arterial partial pressure of oxygen, central venous oxygen pressure, central venous blood oxygen saturation, oxygen uptake rate, hemoglobin, albumin, blood urea nitrogen, creatinine and incidence of complication between 2 groups (P>0.05). The T1-3 hemoglobin and arterial oxygen content (CaO2), T1 and T3 venous oxygen content (CcvO2) and T2 atrerial venous oxygen content differences (Da-cvO2) in groupⅠwere significantly higher than those in groupⅡ, hemoglobin:(103.8 ± 7.9) g/L vs. (110.8 ± 8.6) g/L, (101.7 ± 7.7) g/L vs. (107.3 ± 7.8) g/L and (101.6 ± 6.9) g/L vs. (106.8 ± 5.6) g/L; CaO2: (140.6 ± 9.7) mmol/L vs. (149.6 ± 10.5) mmol/L, (137.5 ± 9.3) mmol/L vs. (144.9 ± 10.2) mmol/L and (137.2 ± 9.2) mmol/L vs. (143.1 ± 8.9) mmol/L;CcvO2:(119.8 ± 10.2) mmol/L vs. (126.4 ± 10.8) mmol/L and (102.3 ± 8.6) mmol/L vs. (108.5 ± 8.9) mmol/L;Da-cvO2:(19.6 ± 3.9) mmol/L vs. (23.0 ± 4.5) mmol/L, and there were statistical differences (P<0.05). Conclusions Phenylephrine has more effect on tissue oxygen metabolism in the elderly patients undergoing fluid-restricted abdominal surgery.

Objective To investigate the clinical pathological factors related to recurrence and metastasis of colorectal cancer patients treated with resection ,expect to improve the treatment effects .Methods The clinicopathologic and follow-up data of 212 patients with stage Ⅰ - Ⅲ colorectal cancer patients treated with resection from January 2009 to December 2013 were retrospec-tively analyzed by univariate and multivariate Logistic regression methods .Results Univariate analysis showed that T staging sys-tems ,gross type ,intestinal obstruction or perforation ,lymphatic and vascular invasion were correlated to recurrence of colorectal cancer after operation(P< 0 .05) ;age ,tumor size ,lymph node involvement ,T staging systems ,histological differentiation ,intestinal obstruction or perforation ,lymphatic and vascular invasion were correlated to metastasis of colorectal cancer after operation(P <0 .05) .Multivariate Logistic regression analysis showed that age ,tumor size and histological differentiation were independent factors for metastasis rate of colorectal cancer after operation(P< 0 .05) .There were no significant differences in Logistic regression analy-sis for recurrence(P> 0 .05) .Conclusion Age ,tumor size and histological differentiation are independent factors for metastasis rate of colorectal cancer after operation .

Objective To investigate and analyze the anxiety‐depression emotion in tumor patients in a tumor hospital to pro‐vide the scientific basis for mental intervention of the tumor patients .Methods Totally 100 tumor patients were surveyed by using the Beck Depression Inventory(Beck) ,Self‐Assessment Anxiety Scale(SAS) and self‐designed general condition information ques‐tionnaire .Results Among 100 tumor patients ,86 cases (86% ) had the depression emotion and 90 cases(90% ) had the anxiety e‐motion .In the comparison of anxiety‐depression emotion among different tumor sites ,disease entities ,genders ,age and educational levels ,except the depression emotion had the statistical difference between the patients with primary school degree and the patients with the cultural level higher than junior middle school (P<0 .05) ,the comparison in other indexes had no statistical differences (P>0 .05) .Conclusion The patients with tumor had higher occurrence rate of anxiety‐depression emotion .The mental intervention should be paid attention to improve their life quality .
