Thyroid cancer is not only an infrequent cancer, but also is the most common malignancy of the endocrine system. When thyroid cancer occurs in childhood and adolescence, it is an unusual but interesting malignancy. Thyroid cancer in childhood and adolescence has a slightly lower preference for female than for male subjects, as compared to adult patients. With regard to the pathogenesis the only factor definitely known to promote development of thyroid cancer in the pediatric age group is irradiation. The evidence demonstrates that thyroid carcinoma in the pediatric age group is a biologically self-reliant and more hostile nature than in adults but paradoxically the prognosis is good. Enhanced survival resulting from advances in therapy in patients with Hodgkin's disease is associated with long-term morbidity, including the potential for the development of a second solid malignancy. Second malignant neoplasms(SMNs) have become a principal consideration in evaluating the prolonged consequences of strategy in pediatric oncology. Thyroid carcinoma has proven to be a common SMN. We have experienced 2 cases of papillary thyroid cancer following radiation therapy for Hodgkin's lymphoma and a papillary thyroid cancer with pulmonary metastases.

Plesiomonas shigelloides was isolated from blood culture of a 53-year-old man with fever, who had treatment history of gastrointestinal malignancy. The patient showed neither clinical features nor hematological finding which suggest bacteremia. Identification of the isolate was delayed because of its similar characteristics with Aeromonas spp. and other gram-negative bacilli. The isolate was misinterpreted as susceptible to ampicillin by the first disk diffusion test. It may not always easy to identify P. shigelloides by conventional tests and to determine its antimicrobial susceptibility accurately, as laboratorians rarely have experience with the organism and as the organism may show unusual inhibition pattern when tested by disk diffusion method or Etest.

OBJECTIVE: The present study evaluated the characteristics and natural history of stroke patients. METHOD: Seven hundred and sixty seven consecutive patients admitted through acute care facility were reviewed. All medical records including age, case fatality, risk factors and radiological findings including CT or MRI were reviewed. To investigate the activities of daily living (ADL) of stroke patients, follow-up study was done in 303 patients through telephone interview or direct contact. Stroke was subdivided into cerebral infarct, intracerebral hemorrhage (ICH) and subarachnoid hemorrhage (SAH). RESULTS: The proportion of stroke subtypes were infarct (45.9%), ICH (34.7%) and SAH (19.4%). The recurrence rate of stroke was higher in the infarct patient. Higher percentage of fatality was noted in the SAH patient. Higher incidence of hemorrhage was still observed compared to western country. Hypertension is the major risk factors for all stroke subtypes. But among hypertensive individuals, only 29% patients controlled the hypertension. Fifty-eight percent of stroke patients regained independent ADL. CONCLUSION: The result of this study shows the clinical pictures of stroke patients. Further research was needed to investigate the trends of stroke and control of risk factors and attention must be paid to the patients who is dependent in ADL.

Paraffinoma is a well known complication of previous paraffin injection into the subcutaneous layer that presents as various conditions including firm mass formation, edema, induration, ulceration, and skin necrosis. Paraffinoma can mimic neoplasm on physical examination and imaging studies and may complicate ultrasonographic diagnoses due to typical posterior shadowing and high echogenicity. When paraffinomas involve around the thyroid gland, the diagnosis of thyroid tumors is very difficult. We present a case of thyroid cancer, the evaluation of which was complicated by the presence of cervical paraffinoma.

The purpose of this study was to identify the effect of individual emotional characteristics (empathetic concern and emotional contagion) on emotional labor (frequency of emotional display, intensity and variety of emotional display, surface acting, and deep acting) of school dietitians. Data was collected through a questionnaire survey of 309 school dietitians and then analyzed statistically by SPSS 18.0 package program. The results of this study were as follows: empathetic concern (3.93) occurred more often than emotional contagion (3.22) in school dietitians. The older (p < 0.001) and more experienced (p < 0.01) they were, the higher empathetic concern they had. It showed that married dieticians (p < 0.001), nutrition teachers (p < 0.01), and dieticians who were working in elementary schools (p < 0.01) had a higher rate of empathetic concern than single dieticians, non-nutrition teachers, and dieticians who were working in middle and high schools, respectively. Their level of emotional labor was the highest in deep acting (3.32), followed by surface acting (3.28), frequency (3.12), intensity and variety (3.09). According to multiple regression analysis, emotional contagion proved to be strongly significant and positively related to frequency of emotional display (beta = 0.257, p < 0.001). Both empathetic concern (beta = 0.117, p < 0.05) and emotional contagion (beta = 0.162, p < 0.01) were positively related to intensity and variety of emotional display, and empathetic concern (beta = 0.173, p < 0.01) had also an effect on deep acting. These results suggested that the emotional labor of school dietitians should be managed on the organizational viewpoint, not a personal matter.

We studied serum uric acid in 120 patients of essential hypertension visiting Department of Internal Medicine of Seoul Red Cross Hospital from October 1985 to November 1986 and analyzed its values in terms of various clinical and laboratory parameters. The results wereas follows; 1) The mean concentration of serum uric acid and incidence of hyperuricemia were 6.5+/-1.86mg%, 50.8% in hypertension group and those of normal control were 3.75+/-1.27mg%, 13.3%, respectively with significant high values in hypertension group. 2) The mean concentration of serum uric acid and the incidence of hyperuricemia were significantly correlated with the levels of diastolic blood pressure but there was only correlation between the incidence of hyperuricemia and the level of systolic pressure. 3) The mean concentration of serum uric acid and the incidence of hyperuricemia were significantly correlated with the level of BUN and serum creatinine, especially more in serum creatinine. 4) The mean concentration of serum uric acid and the incidence of hyperuricemia were not correlated with each level of serum cholesterol and triglyceride. 5) There was significant correlation between mean concentration of serum uric acid and abnormal urine finding but not in incidence of hyperuricemea.

A study on changes of serum HDL-cholesterol level in various diseased states was performed. Diseased groups were nephrotic syndrome, liver cirrhosis, hyperthyroidism, hypothyroidism and chronic alcoholism. Following results were obtained. 1) Compared to normal control group, levels of HDL-cholesterol in nephrotic syndrome were lower but the difference was significant statistically. 2) In liver cirrhosis, the result showed much lower levels than normal control group with high significance. 3) In both hyper-and hypothyroidism group, the levels of HDL-cholesterol were significantly lower than normal control group. 4) In chronic alcoholism, the levels were significantly higher than normal control group. 5) Except for the group of chronic alcoholism, the levels of HDL- cholesterol were higher in female groups than male groups, but was of no significance statistically.
