4.Acute myocardial infarction in pregnant women.
Annals of the Academy of Medicine, Singapore 2010;39(3):247-253
Acute myocardial infarction (AMI) in pregnant women is a rare but potentially lethal occurrence that should be carefully managed, especially in consideration of cardiac conditions being a rising cause of maternal deaths. Risk factors for AMI occurrence, in addition to typical cardiac-related risk factors, include medical conditions such as (pre) eclampsia, blood transfusions, thrombophilia and postpartum infections. Being older, multigravida or in the third trimester of pregnancy is also associated with an increased risk. The pathophysiological causes underlying AMI in pregnancy are diverse but generally associated with the coagulative and physiological changes related to the pregnancy. The selection of diagnostic modality and treatment options require careful consideration for pregnancy-related changes as well as risk of harm to the patient and fetus. This paper serves to review available literature regarding an extensive range of management issues that directly impact on maternal and fetal outcomes.

Objective To study the effect of heart function and histopathology after coronary vein bypass grafting. Methods 16 pigs were divided into experimental group and control group.Following 16 pig models of chronic myocardial isehemia of left ventri- cle were established, the left internal mammary artery (LIMA)-great cardiac vein (GCV) anastomasis in 8 pigs were completed 4 week later. After GCV occlusion at proximal, retrograde coronary venous perfusion was afforded. At 8 week, coronary angiography,echo- cardiography and myocardial radioisotope scanning were performed. After harvesting these hearts, histopathologic examination and electron microscope were undertaken. Results All anastomotic vessels of experimental pigs were smooth.LVEF in experimental group was significant higher than that in control group (P

Objective To explore the characteristics and clinical significance of lung function in children with mycoplasma pneumoniae pneumonia. Methods The pulmonary ventilation function of 60 cases of mycoplasma pneumoniae pneumonia was tested in the acute stage and 2 weeks after treatment by the pneumatometer made by JAEGER company in Germany. FVC, FEV1, PEF, FEF25,FEF50、FEF75 and MMEF75/25 was detected. Results In acute phase, lung function indexs (FVC, FEV1, PEF, FEF25, FEF50, FEF75, MMEF75/25) of 60 children with MPP were less than expected:(1.56 ± 0.53) L vs.(1.99 ± 0.69) L, (1.37 ± 0.47) L vs. (1.68 ± 0.57) L, (2.90 ± 0.86) L/s vs. (3.95 ± 1.08) L/s, (2.48 ± 0.67) L/s vs. (3.56 ± 0.89) L/s, (1.42 ± 0.41) L/s vs. (2.51 ± 0.64) L/s, (0.65 ± 0.20) L/s vs. (1.28 ± 0.33) L/s, (1.22 ± 0.77) L/s vs.(2.18 ± 0.61) L/s], and there were significant difference (P<0.01). In recovery period, the level of FVC, FEV1, PEF, FEF25, FEF50, FEF75, MMEF75/25 was significantly better than that in acute phase: (98.80 ± 9.34)% vs.(79.14 ± 6.28)%, (98.67 ± 8.28)% vs. (81.63 ± 6.56)%, (86.23 ± 6.86)% vs.(73.17 ± 6.21)%, (85.17 ± 7.86)% vs. (69.79 ± 8.16)%, (79.08 ± 7.99)% vs. (56.57 ± 8.77)%, (70.85 ± 7.48)% vs. (50.66 ± 9.86)%, (77.35 ± 6.81)% vs. (56.19 ± 9.61)%, P<0.01. Conclusions In the acute stage, the pulmonary function of children with MPP shows hybrid ventilation dysfunction. In the recovery period, pulmonary function index improves significantly, but there are still abnormal small airway indicators.

Object To optimize the process for the recovery of soyasaponins from bean product waste. Methods Conditions for the recovery were studied by orthogonal design guided by the yield of soya-saponins as determined by UV absorbance. Results The most influential factor in the recovery by refluxing was the concentration of alcohol; while in the recovery by percolation the influential factors were in the order of concentration of ethyl alcohol, amount of alcohol used and the rate of percolation. Conclusion The optimal recovery process was found to be a single reflux with eight times of 60% alcohol for 2.5 h.

Objective To discuss improving home environment and enhancing quality of life in violence psychopath through the health education pattern. Methods 120 violence psychopath who had been clinically recovered through hospital treatment were implemented a one-year health education through family follow-up or clinical subsequent visit by a group of doctors and nurses.Family environment scale (FES-CV) and general quality of life inventory (GQOLI) were used to assess the family environment and quality of life of the patients before and after the education. Results FES-CV scores improved significantly in terms of intimacy, emotional expression, contradictory, independence, achievement and controllability (P < 0.01 ).The scores of intimacy, emotional expression and independence were (5.58 ± 1.26) points,(5.89 ± 1.62) points and (4.24 ± 1.16)points before the health education and (6.92 ± 1.43 ) points,(6.54 ± 1.71 ) points and (5.75 ± 1.64) points after the health education. The differences were statistically significant (t =5.20,4.13,3.95,respectively;P <0.01 ).Each of the objective condition and the subjective satisfaction score of the GQOLI scale improved and the difference was statistically significant ( P <0.01 ),among which the physical function score of objective condition improved from (20.63 ± 4.52) points to (25.92 ± 6.43 ) points and there was significant differenoe between the groups( t =5.57,P < 0.01 ).Conclusions This research indicates that health education could improve home environment of violence psychopath, enhance their quality of life, promote social function restore and have a positive function on stabilizing the act of violence.
